    Softer perspectives on enhancing the patient experience using IS/IT

    Purpose – This paper aims to argue that the implementation of the Choose and Book system has failed due to the inability of project sponsors to appreciate the complex and far-reaching softer implications of the implementation, especially in a complex organisation such as the NHS, which has multifarious stakeholders. Design/methodology/approach – The authors use practice-oriented research to try and isolate key parameters. These parameters are compared with existing conventional thinking in a number of focused areas. Findings – Like many previous NHS initiatives, the focus of this system is in its obvious link to patients. However we find that although this project has cultural, social and organisational implications, programme managers and champions of the Connecting for Health programme emphasised the technical domains to IS/IT adoption. Research limitations/implications – This paper has been written in advance of a fully implemented Choose and Book system. Practical implications – The paper requests that more attention be paid to the softer side of IS/IT delivery, implementation, introduction and adoption. Originality/value – The paper shows that patient experience within the UK healthcare sector is still well below what is desired

    Intelligent systems for monitoring and preventing in healthcare information systems

    Nowadays the interoperability in Healthcare Information Systems (HIS) is a fundamental requirement. The Agency for Integration, Diffusion and Archive of Medical Information (AIDA) is an interoperability healthcare platform that ensures these demands and it is implemented in Centro Hospitalar do Porto (CHP), a major healthcare unit in Portugal. Therefore, the overall performance of CHP HIS depends on the success of AIDA functioning. This paper presents monitoring and prevention systems implemented in the CHP, which aim to improve the system integrity and high availability. These systems allow the monitoring and the detection of situations conducive to failure in the AIDA main components: database, machines and intelligent agents. Through the monitoring systems, it was found that the database most critical period is between 11:00 and 12:00 and the resources are well balanced. The prevention systems detected abnormal situations that were reported to the administrators that took preventive actions, avoiding damage to AIDA workflow

    Information and Communication Technology Applications and use in Medical Records and Information Management in Selected Hospitals in Ijebu Ode Local Government Area, Ogun State.

    This study investigated the information and communication technology application and use in medical records and information management in selected hospitals in Ijebu-Ode Local Government Area of Ogun State. The Objectives of the study found out the level of application and compliance of ICT and also investigated the challenges facing the application. The survey research design was adopted while total enumeration sampling technique was used in the selection of samples. Questionnaire was used to gather data for this study while mean and standard deviation frequency counts and percentages was used in data analysis. Findings shows that the information and communication tools applicable in the selected hospitals are computers (100%), softwares such as Microsoft word, excel etc (100%), surgical ICT tools (100%), internet (90%), management software (90%), infection detecting technologies (90%), Health Information system programme (HISP) (90%), ultrasound imaging device (80%), telemedicine (76%), national health care management information system (NHC/MIS) (75%) and scanner (60%). The challenges facing the use of ICT tools in medical records and information management in the hospitals are inadequate training, insufficient knowledge on use of IT tools, failure of equipment/old IT tools, security/privacy issues, inadequate ICT facilities, lack of physical access to the tools and affordability. The research therefore recommended among others that there should be more training, awareness, exposure or workshops for staff in the hospital on the application and use of ICT tools which are seldom or never used in the hospitals to improve the health care services rendered in the hospitals

    The impact of EU Digital Services Act and Digital Markets Act on health information systems

    The European Parliament has approved new legislation, the Digital Services Act (DSA) and Digital Markets Act (DMA) to improve the functioning of the internal market of intermediary services in the European Union (EU) where there is a risk that the major so-called gatekeeper companies can exercise unfair control of core platform services. The purpose of this study was to investigate, what health information systems could be in the scopes of these acts and what requirements the acts may have for the production, the sale and the use of health information systems. The act texts were examined bearing in mind what types of health information systems exist and what their user bases are. Those health information systems that can belong or do not belong to the groups of systems regulated by the DMA and DSA were identified. The most relevant requirements for these systems were also identified from these acts. The result of the study is that these acts have only minor consequences for the healthcare information systems sector as they are not often intermediary (hosting) services in the meaning of the DSA or gatekeepers in the meaning of the DMA. The emerging digital healthcare platforms are most affected by the new DSA and secondly such peer support patient portals where patients can supply content for others to see. Apparently, no digital healthcare platform has yet reached such a size or a dominant role within the EU that it would fall under the scope of the DMA. The two above mentioned healthcare related intermediary services have due diligence obligations to remove illegal contents from their services and to treat their business and consumer customers fairly. The obligations include clear and fair terms and conditions, the provision of a single point of contact for users and authorities, content moderation, complaint handling, marking advertising clearly, annual reporting, and responding to the contacts from the authorities. The obligations increase when the size of the enterprise increases. It is still too early to produce healthcare information systems specific guidance to support the implementation of these two acts as the acts themselves and potential upcoming general guidance documents can serve the health information systems community sufficiently well

    ANALYZING MEDICAL TRANSACTION DATA BY USING ASSOCIATION RULE MINING WITH MULTIPLE MINIMUM SUPPORTS

    The quick development of IS has a huge impact on the healthcare industry. almost all the existing hospitals, clinics and other healthcare-related institutes have adopted a functionally powerful and highly integrated Hospital Information System (HIS) for management of clinic or medical-related affairs. The medical data stored in the HIS are collected from many different medical subsystems, However, problems of failed data sharing and inconsistent data content often occur among these subsystems, resulting in many hospitals collect a large amount of medical data, but not the ability to process and analyse these data properly, letting the valuable data in the HIS all go to waste. In this study, we made a practical visit to a certain hospital in Taiwan and collected radioimmunoassay (RIA) data from the Laboratory Information System (LIS) and the Departmental Registration System (DRS) of this hospital. Further, we proposed a method of the association rule mining in combination with the concept of multiple minimum supports to analyse and find valuable association rules from the RIA data. The analytical results found the method we proposed can indeed find association rules that would not be able to be found with the traditional association mining methods. It is very helpful in improving doctor-patient relationship and upgrading health care quality

    An Evaluation of Understandability of Patient Journey Models in Mental Health.

    BACKGROUND: There is a significant trend toward implementing health information technology to reduce administrative costs and improve patient care. Unfortunately, little awareness exists of the challenges of integrating information systems with existing clinical practice. The systematic integration of clinical processes with information system and health information technology can benefit the patients, staff, and the delivery of care. OBJECTIVES: This paper presents a comparison of the degree of understandability of patient journey models. In particular, the authors demonstrate the value of a relatively new patient journey modeling technique called the Patient Journey Modeling Architecture (PaJMa) when compared with traditional manufacturing based process modeling tools. The paper also presents results from a small pilot case study that compared the usability of 5 modeling approaches in a mental health care environment. METHOD: Five business process modeling techniques were used to represent a selected patient journey. A mix of both qualitative and quantitative methods was used to evaluate these models. Techniques included a focus group and survey to measure usability of the various models. RESULTS: The preliminary evaluation of the usability of the 5 modeling techniques has shown increased staff understanding of the representation of their processes and activities when presented with the models. Improved individual role identification throughout the models was also observed. The extended version of the PaJMa methodology provided the most clarity of information flows for clinicians. CONCLUSIONS: The extended version of PaJMa provided a significant improvement in the ease of interpretation for clinicians and increased the engagement with the modeling process. The use of color and its effectiveness in distinguishing the representation of roles was a key feature of the framework not present in other modeling approaches. Future research should focus on extending the pilot case study to a more diversified group of clinicians and health care support workers

    An intelligent decision support tool for early diagnosis of functional pituitary adenomas

    In this work, a web based integrated Medical Decision Support System (MDSS) tool for mainly early diagnosis of functional pituitary adenomas (i.e., somatotrophinoma, corticotrophinoma and prolactinoma) is developed. In the MDSS tool, hormone diseases are described by means of well-classified set of attributes generated from the typical sign and symptoms of disorders.The proposed tool is based on a stationary linear stochastic system model which specifically predicts the selected hormone diseases employing certain system parameters. The MDSS tool is user friendly which includes questions and answers at the opening session of the self-test. Questions and answers session will be completed by “yes” or “no” type of simple-responses. Based on our clinical results, MDSS tool yields more than 99% correct decisions on the selected hormone diseases. It is expected that effective use of the proposed MDSS tool will save substantial amount of valuable time of an expert endocrinologists and minimizes the cost of diagnosis. Furthermore, it will provide the opportunity for early diagnosis for the patient and the expert medical doctor to take the necessary preventive measures.Publisher's Versio
