Realizing EU ETS Monitoring and Compliance in Poland
The success of the European Union Emissions Trading Scheme (EU ETS) as a whole depends on proper national monitoring and compliance mechanisms in each of the 31 participating States. This article will focus on the legal and practical realization of an EU ETS monitoring and compliance system in Poland - being one of the EU member States where the gradual realization of the EU ETS system is meeting strong political resistance. Keywords: EU Emissions Trading System, Poland, monitoring, compliance, legal implementation
Cardiovascular safety of tocilizumab versus etanercept in rheumatoid arthritis: a randomized controlled trial
Objective:
To compare the risk for major adverse cardiovascular events (MACE) in RA patients treated with tocilizumab versus the tumor necrosis factor inhibitor etanercept.
Methods:
This randomized, open‐label, parallel‐group trial enrolled patients with active seropositive RA (N=3080), inadequate responses to conventional synthetic disease‐modifying antirheumatic drugs, and at least one cardiovascular risk factor. Patients were randomly assigned 1:1 to open‐label tocilizumab 8 mg/kg/month or etanercept 50 mg/week and followed up for an average of 3.2 years. The primary end point was comparison of time‐to‐first MACE. The trial was powered to exclude a 1.8 or higher relative hazard of MACE for tocilizumab versus etanercept.
Results:
By week 4, serum low‐density lipoprotein cholesterol, high‐density lipoprotein cholesterol, and triglyceride levels were 11.1%, 5.7%, and 13.6% higher, respectively, for patients allocated to tocilizumab compared with etanercept (all P<.001). During follow‐up, 83 MACE occurred in the tocilizumab group compared with 78 in the etanercept group. The estimated hazard of MACE for tocilizumab relative to etanercept was 1.05 (95% confidence interval=0.77, 1.43). Result were similar in sensitivity analyses and the on‐treatment analysis. Adverse events that occurred more frequently in the tocilizumab group included serious infection and gastrointestinal perforation.
Conclusion:
The trial, which provides insights into the cardiovascular safety of tocilizumab versus etanercept, excluded a relative risk for MACE of 1.43 or higher. This result should be interpreted in the context of the clinical efficacy and the non‐cardiovascular safety of tocilizumab
Turizm Sektöründe Antraktlı Çalışma Algısı: Antalya'daki Beş Yıldızlı Otel Çalışanları Açısından Bir Değerlendirme Entracte Work Perception in Tourism Sector: An Evaluation in Terms Of Five Star Hotels Employees in Antalya
Businesses have turned to flexible applications in working life in order to be able
to adapt and survive in increasing global competition. In terms of labour, permanent
and secure employment has now been replaced by short-term, temporary, irregular,
precarious and low-paid forms of employment. In some businesses, working and rest
periods of workers has flexibilized in accordance with these employment patterns and the intensity of work. Employers are extend the length of time employees stay in
workplace by arranging work and rest periods according to their intensity of work and
in a manner not in compliance with legal regulations. It is observed that in practice,
this form of work, called "entracte work” is frequently applied in lots of businesses
especially accommodation enterprises in the service sector. The purpose of the study is
to determine the working conditions of workers who are involved in the practice of
entracte work which is becoming widespread in the tourism sector and to detect the
perceptions of workers towards entracte work. In this context, survey was conducted
with 394 workers working with entracte practice in Antalya. As a result of factor
analysis, a 4-dimensional structure has been reached about the employee’s perception
of entracte work. According to the findings, it has been determined that employees are
generally not satisfied with this working practice
Targeting inflammation to reduce cardiovascular disease risk: a realistic clinical prospect?
Data from basic science experiments is overwhelmingly supportive of the causal role of immune-inflammatory response(s) at the core of atherosclerosis, and therefore the theoretical potential to manipulate the inflammatory response to prevent cardiovascular events. However, extrapolation to humans requires care and we still lack definitive evidence to show that interfering in immune-inflammatory processes may safely lessen clinical atherosclerosis. In this review, we discuss key therapeutic targets in the treatment of vascular inflammation, placing basic research in to a wider clinical perspective, as well as identifying outstanding questions
