Android Platform Physics Game
Android je nejrychleji se rozvíjející a nejlépe prodávaný operační systém pro mobilní zařízení. Tato práce pojednává o historii a možnostech tohoto systému, ale také o vývoji aplikací pro zařízení, fungující na této platformě. Krátce se také věnuje principům fyzikální simulace v počítačových hrách. Součástí práce je i podrobný popis návrhu a implementace hry pro platformu Android, která využívá zmíněnou fyzikální simulaci.Android is the fastest growing and best selling operating system for mobile devices. This paper briefly describes the history of this system, its possibilities and application programming basics for this platform. It contains a short introduction to physics simulation used in computer games. The paper also includes a detailed description of design and implementation of a mobile game for Android system, based on physics simulation.
Vertebro-/Kyphoplasty History, Development, Results
Many investigations prove the significant analgetic effect of vertebral augmentation. The reasons for the decrease in pain are found in the stabilization of fracture fragments as well as the toxic-thermic effect of polymethylmethacrylate (PMMA), used in the majority of cases. The techniques, primarily in use since 1984, can be divided in vertebro- and kyphoplasty. Vertebroplasty is the direct injection of PMMA into the trabecular vertebral body, while kyphoplasty uses an inflatable bone tamp to create a cavity which is filled with highly viscous cement allowing a certain degree of vertebral height restoration. Both techniques are used percutaneously. Indications for augmentation are painful osteoporotic vertebral fractures, metastatic osteolyses, and painful or destabilizing vertebral hemangiomas. An immunohistochemical study of the tissue bridging adult spondylolytic defects—the presence and significance of fibrocartilaginous entheses
Introduction Spondylolytic spondylolisthesis is an osseous discontinuity of the vertebral arch that predominantly affects the fifth lumbar vertebra. Biomechanical factors are closely related to the condition. An immunohistochemical investigation of lysis-zone tissue obtained from patients with isthmic spondylolisthesis was performed to determine the molecular composition of the lysis-zone tissue and enable interpretation of the mechanical demands to which the tissue is subject. Methods: During surgery, the tissue filling the spondylytic defects was removed from 13 patients. Twelve spondylolistheses were at the L5/S1 level with slippage being less than Meyerding grade II. Samples were methanol fixed, decalcified and cryosectioned. Sections were labelled with a panel of monoclonal antibodies directed against collagens, glycosaminoglycans and proteoglycans. Results: The lysis-zone tissue had an ordered collagenous structure with distinct fibrocartilaginous entheses at both ends. Typically, these had zones of calcified and uncalcified fibrocartilage labelling strongly for type II collagen and aggrecan. Labelling was also detected around bony spurs that extended from the enthesis into the lysis-zone. The entheses also labelled for types I, III and VI collagens, chondroitin four and six sulfate, keratan and dermatan sulfate, link protein, versican and tenascin. Conclusions: Although the gap filled by the lysis tissue is a pathological feature, the tissue itself has hallmarks of a normal ligament—i.e. fibrocartilaginous entheses at either end of an ordered collagenous fibre structure. The fibrocartilage is believed to dissipate stress concentration at the hard/soft tissue boundary. The widespread occurrence of molecules typical of cartilage in the attachment of the lysis tissue, suggests that compressive and shear forces are present to which the enthesis is adapted, in addition to the expected tensile forces across the spondylolysis. Komunikowanie polityczne w Tychach w okresie transformacji ustrojowej
Rozwój komunikowania politycznego jako jednej z form komunikowania
społecznego wynika z konieczności porozumiewania się w systemach
demokratycznych głównych aktorów politycznych - rządów, partii politycznych,
grup interesów ze swoim elektoratem i sympatykami. Komunikowanie polityczne
jako samodzielny obszar studiów akademickich zaistniało dopiero w latach
czterdziestych i pięćdziesiątych XX wieku. Prekursorami nauki o komunikowaniu
politycznym byli Amerykanie, którzy pozostawili trwały dorobek i liczną literaturę. Komunikowanie polityczne cieszy się również dużym zainteresowaniem
w Polsce. W niniejszej pracy doktorskiej dokonano szerszego omówienia procesu
lokalnego komunikowania politycznego. Opisu dokonano na podstawie
dostępnych opracowań teoretycznych oraz na podstawie analizy wyników
samodzielnie przeprowadzonych badań empirycznych. Dla lepszego uchwycenia
występujących zjawisk i procesów społecznych, jako obszar badawczy zostało
wybrane miasto Tychy stanowiące przykład typowego pod względem ładu
architektonicznego, struktury demograficznej i zachodzących procesów
społecznych miasta w systemie satelitów aglomeracji górnośląskiej. Głównym
celem pracy było:
1. scharakteryzowanie lokalnego procesu komunikowania politycznego
aktorów sceny politycznej miasta Tychy z mieszkańcami w okresie
badawczym obejmującym okres transformacji ustrojowej (od 1989
do 2006 r.);
2. przeprowadzenie analizy wyników wyborczych (samorządowych);
3. opisanie postrzegania komunikowania politycznego przez lokalne
organizacje polityczne;
4. określenie modeli działalności lokalnych organizacji politycznych;
5. uchwycenie miejsca, jakie zajmują media lokalne w procesie
komunikowania politycznego;
6. prezentacja zainteresowania sprawami politycznymi lokalnego
audytorium komunikowania politycznego.
Problem badawczy został ujęty w formie deskryptywnej i obejmował
zagadnienie skuteczności technik komunikowania politycznego stosowanych
w okresie między wyborami na poziomie lokalnym, na przykładzie miasta Tychy.
Dla zidentyfikowanego problemu badawczego została sformułowana hipoteza
badawcza: ”Im bardziej zorganizowane komunikowanie polityczne
z wyborcami w okresie między wyborami - tym lepszy wynik wyborczy”.
W celu zweryfikowania hipotezy poprzez badania dostarczono danych, które
opisywały:
• znajomość technik komunikowania politycznego wśród lokalnych
organizacji politycznych;
• ilość podjętych działań i ich ocenę dokonaną przez wyborców;
• wpływ tych działań na preferencje wyborcze;
• zakres i wpływ działań podejmowanych przez liderów i ich wpływ na
preferencje wyborcze;
• wpływ czynników zewnętrznych (ogólnokrajowej sytuacji społecznoekonomicznej)
na planowanie działań politycznych i na preferencje
wyborcze;
• zainteresowanie mieszkańców sprawami politycznymi;
• opinie mieszkańców na temat zdarzeń politycznych;
• identyfikację źródeł informacji o sprawach społeczno-politycznych;
• motywy działań politycznych;
• sugestie mieszkańców na temat działań politycznych, jakie powinny być
realizowane przez aktorów politycznych.
Niezbędne informacje, które pozwoliły na weryfikację hipotezy zostały
zebrane poprzez badania empiryczne. Zaplanowane badania zostały podzielone
na dwa etapy. W pierwszym etapie badaniu zostali poddani aktorzy kolektywni
lokalnej sceny politycznej (11 organizacji politycznych), a w drugim - próba
reprezentacyjna mieszkańców miasta (500 osób). Dane zostały zebrane poprzez
zastosowanie takich technik badawczych, jak badania ankietowe i wywiad
skategoryzowany. Były to badania sondażowe zmierzające do ustalenia opinii,
postaw, zachowań, motywów i oczekiwań wyborców. Przyjęta metoda badawcza
pozwoliła w sposób uporządkowany osiągnąć cel badawczy, zapewniając
maksymalny stopień efektywności. Zebrane materiały poddano weryfikacji,
selekcji, klasyfikacji, kategoryzacji, skalowaniu, a następnie analizie ilościowej
i jakościowej. Badania społeczne zostały przeprowadzone w miesiącach od marca
do maja 2005 roku.
Przeprowadzone badania obejmowały także potrzeby praktyki związane
z istniejącymi możliwościami zastosowania osiągnięć naukowych w zakresie
komunikowania politycznego, sprawdzając wycinkowo prawdziwość ich ustaleń
i ukazując nowe problemy badawcze. W końcowym efekcie uzyskane wyniki
pozwoliły na wysuniecie dalszych hipotez, przyczyniając się do rozwoju teorii
poprzez wzbogacenie wiedzy o badanych zjawiskach. Zrealizowane badania
społeczne mogą także stanowić podstawę dla dalszych, pogłębionych badań
rzeczywistości społeczno-politycznej oraz podstawę do dalszej analizy procesów
społecznych, a poprzez ich wysoki stopień oryginalności mogą być przydatne
w znalezieniu ogólnej prawidłowości determinującej określone zachowania
elementów lokalnego komunikowania politycznego.
Niniejsza praca składa się z 4 rozdziałów. Pierwszy rozdział –
wprowadzający - poświęcony jest szerokiej perspektywie komunikowania
masowego i politycznego w aspekcie teoretycznym. Zaprezentowano w nim
genezę komunikowania politycznego oraz podejścia badawcze reprezentowane
przez różne szkoły. Wśród nich znalazły się między innymi takie podejścia
badawcze, jak: socjologiczne, medioznawcze, politologiczne, interakcyjne,
dialogowe, prawoznawcze, ekonomiczne, psychologiczne, pedagogiczne,
językoznawcze. Rozdział ten wyjaśnia specyfikę, zakres i charakter studiów nad
komunikowaniem politycznym. Jest także przeglądem definicji i ujęć teoretycznych
procesu i systemu. W tym miejscu zostały przybliżone teorie komunikowania
politycznego takie, jak: socjologiczne ujęcie M. Rusha, definicja systemowa
J. Blumlera i M. Gurevitcha, medioznawcza perspektywa R. Negrine,
amerykańska wizja R. E. Dentona i G. Woodwarda, trójelementowa koncepcja
B. McNaira, złoty trójkąt komunikowania Perloffa, syntetyczna definicja B. Dobek-
Ostrowskiej. Kolejne podrozdziały poświęcone zostały między innymi
komunikowaniu politycznemu w przestrzeni lokalnej i marketingowi politycznemu -
jako specyficznej formie komunikowania politycznego zyskującej coraz większą
popularność na polskiej scenie politycznej. Rozdział zamykają obszerne opisy
technik oddziaływania politycznego oraz analiza uczestników procesu
komunikowania politycznego - aktorów politycznych.
W rozdziale drugim został podjęty ważny problem dla praktycznego
wymiaru komunikowania. W centrum uwagi znalazła się analiza sceny politycznej
miasta Tychy w okresie transformacji ustrojowej. Zaprezentowane zostały
szczegółowe analizy wyników wyborów samorządowych od 1989 do 2006 roku.
Rozdział ten ukazuje dynamikę lokalnej sceny politycznej w perspektywie
wieloletniej oraz wskazuje na jej najważniejszych aktorów politycznych.
Zaprezentowano tutaj także historię miasta, najważniejsze etapy jego budowy
i czynniki rozwojowe.
Rozdział trzeci prezentuje wyniki badań ankietowych przeprowadzonych
wśród głównych aktorów politycznych. Opisuje ich działalność oraz działania
komunikacyjne, przyjęte modele organizacyjne oraz stosowane przez nich formy
komunikowania politycznego. Analiza tego ogniwa komunikowania politycznego
obejmuje także swym zakresem komunikację wewnętrzną i zewnętrzną oraz
zagadnienia z zakresu planowania i zarządzania kampanią wyborczą. Wśród
przeprowadzonych badań nie zabrakło również analizy mediów lokalnych
w zakresie lokalnego komunikowania politycznego. W rozdziale trzecim
zamieszczono także analizy prasoznawcze najważniejszych lokalnych tytułów -
prasy samorządowej „Twoje Miasto”, Górnośląskiego Tygodnika Regionalnego
„ECHO”, dodatku „Dziennika Zachodniego”, tygodnika społeczno-politycznego
„Tydzień w Tychach”, tygodnika „Nowe Echo”.
Pracę zamyka rozdział czwarty wyjaśniający zachowania audytorium
lokalnego komunikowania politycznego na podstawie przeprowadzonych badań
empirycznych wśród mieszkańców miasta Tychy. Zebrane wyniki pozwoliły między
innymi na dokonanie analiz źródeł przekazów komunikatów politycznych;
czytelnictwa prasy lokalnej, samorządowej i regionalnej; zainteresowania
lokalnymi sprawami politycznymi. W tym miejscu została poddana opisowi lokalna
aktywność polityczna i ocena komunikacji wyborczej oraz organów samorządu
terytorialnego. Rozdział zamyka problem aktywności medialnej obywateli na
przykładzie analizy prasoznawczej tygodnika „Echo”, wypowiedzi w prasie
samorządowej „Twoje Miasto”, internetowego czatu i forum dyskusyjnego
miejskiego portalu, rubryki czytelniczej w „Tydzień w Tychach” oraz listów
i sygnałów w „Nowym Echu”. Przytoczone wyniki badań zobrazowane zostały
licznymi zestawieniami tabelarycznymi i wykresami.
Oddając niniejszą pracę, chciałbym podziękować mojemu Promotorowi

Monitoring and alleviating the effects of pressure related injuries for spinal surgery–a need for improvement?
The development of pressure injuries (PIs) is a common complication arising during surgery. Monitoring and alleviating PIs is currently not possible during lengthy spinal and orthopaedic surgeries, especially if a patient requires x-rays. We interviewed surgical staff to gain an understanding of attitudes regarding current methods and approaches for the management of PIs to allow the development of new devices based on staff feedback. Interviewees provided insights regarding the possible design of a new device used to monitor and alleviate pressure injuries in a surgical environment. From the interviews we derived key themes, including current devices/systems, current risk assessment/scales, and design considerations. As a result, we identified critical design requirements and an appropriate product design specification.
Background: Complications can arise when perioperative staff are monitoring a patient’s health status during surgery, including the development of pressure injuries (PIs). It is important to reduce the incidence rates of developing PIs intraoperatively, and thereby mitigate both increased costs and negative impacts to patients and care providers. Current pressure relief supports offer limited monitoring abilities throughout the perioperative environment.
Aim: The aim of this study is to establish clear design requirements and a product design specification that enables a new closed-loop product solution to be developed with real-time pressure monitoring during surgery.
Method: We conducted semi-structured interviews with nine different clinical staff members involved in the perioperative environment. We conducted the interviews over a two-week period with two major teaching hospitals within the East Midlands, UK. Questions focused on healthcare professionals’ perceptions of current methods and their approaches to managing PIs. We recorded and transcribed participants’ data using NVivo software.
Conclusion: The interview data identified a need for an improved method to manage and monitor PIs. We established that the incorporation of live feedback to reduce the prevalence of PIs is a critical design priority. Based on the interviews, we developed design criteria and a product design specification (PDS). Interview transcripts suggest a new device design should focus on a novel radiolucent pressure redistribution solution capable of reducing PIs intraoperatively while incorporating live feedback
Correlation of Incisura Anatomy With Syndesmotic Malreduction
Background: The anatomy of the syndesmosis is variable, yet little is known on the correlation between differences in anatomy and syndesmosis reduction results. The aim of this study was to analyze the correlation between syndesmotic anatomy and the modes of syndesmotic malreduction. Methods: Bilateral postreduction ankle computed tomography (CT) scans of 72 patients treated for fractures with syndesmotic disruption were analyzed. Incisura depth, fibular engagement into the incisura, and incisura rotation were correlated with degree of syndesmotic malreduction in coronal and sagittal planes as well as rotational malreduction. Results: Clinically relevant malreduction in the coronal plane, sagittal plane, and rotation affected 8.3%, 27.8%, and 19.4% of syndesmoses, respectively. The syndesmoses with a deep incisura and the fibula not engaged into the tibial incisura were at risk of overcompression, anteverted incisuras at risk of anterior fibular translation, and retroverted incisuras at risk of posterior fibular translation. Conclusions: Certain morphologic configurations of the tibial incisura increased the risk of specific syndesmotic malreduction patterns. Level of Evidence: Level III, comparative study
Transcostovertebral kyphoplasty of the mid and high thoracic spine
While Kyphoplasty is increasingly becoming a recognised minimally invasive treatment option for osteoporotic vertebral fractures and neoplastic vertebral collapse, the experience in the treatment of vertebrae of the mid (T5-8)- and high (T1-4) thoracic levels is limited. The slender pedicle morphology restricts the transpedicular approach at these levels, necessitating extrapedicular placement techniques. Fifty five vertebrae of 32 consecutive patients were treated with kyphoplasty at levels ranging from T2-T8 for vertebral fractures (27 patients) or osteolytic collapse (5 patients). All procedures were performed through the transcostovertebral approach under fluoroscopic guidance. The radioanatomical landmarks of this minimally invasive approach were consistently identified and strictly adhered to. One fracture required open instrumentation due to posterior column injury in addition to kyphoplasty. Identification of specific radioanatomical landmarks allowed precise tool introduction in all cases without intraspinal or paravertebral malplacement. Average operating time for patients with osteoporotic fractures was 30min per level (range 13-60min) and 52min per level (range 35-95min) in neoplastic cases. Biopsy yield in patients with known or suspected malignancies was 100%. Epidural cement leakage was detected in one patient with pedicular osteolysis. Perforation of the lateral vertebral cortex during balloon inflation occurred in another patient. Both intraoperative complications were without clinical significance. Kyphoplasty in mid- to -high thoracic levels is possible via the transcostovertebral route under fluoroscopic guidance. Strict adherence to a stepwise protocol of tool introduction following defined radioanatomical landmarks is mandatory for the safe completion of this minimally invasive techniqu
Fluoroscopic radiation exposure of the kyphoplasty patient
Kyphoplasty (KP) is a minimally invasive technique for the percutaneous stabilisation of vertebral fractures. As such, this technique is highly dependent upon intraoperative fluoroscopic visualisation. In order to assess the range of radiation doses that patients are typically subjected to, 60 consecutive procedures using simultaneous bilateral fluoroscopy were analysed with respect to exposure time (ET). In a subset of 16 of these patients, a theoretical entrance skin dose (ESD) and effective dose was additionally calculated from intraoperatively measured dose area product. Average fluoroscopy time for single level cases reached 2.2min (range 0.6-4.3) in the lateral plane and 1.6min (range 0.5-3.0) in the anterior-posterior plane. For multiple level cases the corresponding ET per level was 1.7min (range 0.6-2.9) per level in the lateral and 1.1min (range 0.5-2.0) in the anterior-posterior plane. ESD was estimated as an average 0.32Gy (range 0.05-0.86) in the anterior-posterior and 0.68Gy (range 0.10-1.43) in the lateral plane. Effective dose (cumulative from both planes) averaged 4.28mSv (range 0.47-10.14). Safety margins for the development of early transient erythema are respected within the presented fluoroscopy times. Longer ET in the lateral plane may however breach the 2Gy threshold. Use of large c-arms and judiciously operating the exposure is recommended. Athletic population with spondylolysis: review of outcomes following surgical repair or conservative management
Objective
The study aims to critically review the outcomes associated with the surgical repair or conservative management of spondylolysis in athletes.
Methods
The English literature listed in MEDLINE/PubMed was reviewed to identify related articles using the term “spondylolysis AND athlete.” The criteria for studies to be included were management of spondylolysis in athletes, English text, and no year, follow-up, or study design restrictions. The references of the retrieved articles were also evaluated. The primary outcome was time to return to sport. This search yielded 180 citations, and 25 publications were included in the review.
Results
Treatment methods were dichotomized as operative and nonoperative. In the nonoperative group, 390 athletes were included. A combination of bracing with physical therapy and restriction of activities was used. Conservative measures allowed athletes to return to sport in 3.7 months (weighted mean). One hundred seventy-four patients were treated surgically. The most common technique was Buck's, using a compression screw (91/174). All authors reported satisfactory outcomes. Time to return to play was 7.9 months (weighted mean). There were insufficient studies with suitably homogenous subgroups to conduct a meta-analysis.
Conclusion
There is no gold standard approach for the management of spondylolysis in the athletic population. The existing literature suggests initial therapy should be a course of conservative management with thoracolumbosacral orthosis brace, physiotherapy, and activity modification. If conservative management fails, surgical intervention should be considered. Two-sided clinical studies are needed to determine an optimal pathway for the management of athletes with spondylolysis
