The Lazarus project: A pragmatic approach to binary black hole evolutions
We present a detailed description of techniques developed to combine 3D
numerical simulations and, subsequently, a single black hole close-limit
approximation. This method has made it possible to compute the first complete
waveforms covering the post-orbital dynamics of a binary black hole system with
the numerical simulation covering the essential non-linear interaction before
the close limit becomes applicable for the late time dynamics. To determine
when close-limit perturbation theory is applicable we apply a combination of
invariant a priori estimates and a posteriori consistency checks of the
robustness of our results against exchange of linear and non-linear treatments
near the interface. Once the numerically modeled binary system reaches a regime
that can be treated as perturbations of the Kerr spacetime, we must
approximately relate the numerical coordinates to the perturbative background
coordinates. We also perform a rotation of a numerically defined tetrad to
asymptotically reproduce the tetrad required in the perturbative treatment. We
can then produce numerical Cauchy data for the close-limit evolution in the
form of the Weyl scalar and its time derivative
with both objects being first order coordinate and tetrad invariant. The
Teukolsky equation in Boyer-Lindquist coordinates is adopted to further
continue the evolution. To illustrate the application of these techniques we
evolve a single Kerr hole and compute the spurious radiation as a measure of
the error of the whole procedure. We also briefly discuss the extension of the
project to make use of improved full numerical evolutions and outline the
approach to a full understanding of astrophysical black hole binary systems

High resolution AMI Large Array imaging of spinning dust sources: spatially correlated 8 micron emission and evidence of a stellar wind in L675
We present 25 arcsecond resolution radio images of five Lynds Dark Nebulae
(L675, L944, L1103, L1111 & L1246) at 16 GHz made with the Arcminute
Microkelvin Imager (AMI) Large Array. These objects were previously observed
with the AMI Small Array to have an excess of emission at microwave frequencies
relative to lower frequency radio data. In L675 we find a flat spectrum compact
radio counterpart to the 850 micron emission seen with SCUBA and suggest that
it is cm-wave emission from a previously unknown deeply embedded young
protostar. In the case of L1246 the cm-wave emission is spatially correlated
with 8 micron emission seen with Spitzer. Since the MIR emission is present
only in Spitzer band 4 we suggest that it arises from a population of PAH
molecules, which also give rise to the cm-wave emission through spinning dust

The Birth of High Mass Stars: Accretion and/or Mergers?
The observational consequences of the merger scenario for massive star
formation are explored and contrasted with the gradual accumulation of mass by
accretion. Protostellar mergers may produce high luminosity infrared flares
lasting years to centuries followed by a luminosity decline on the
Kelvin-Helmholtz time-scale of the merger product. Mergers may be surrounded by
thick tori of expanding debris, impulsive wide-angle outflows, and shock
induced maser and radio continuum emission. Collision products are expected to
have fast stellar rotation and a large multiplicity fraction. Close encounters
or mergers will produce circumstellar debris disks with an orientation that
differs form that of a pre-existing disk. The extremely rare merger of two
stars close to the upper-mass end of the IMF may be a possible pathway to
hypernova generated gamma-ray bursters. While accretional growth can lead to
the formation of massive stars in isolation or in loose clusters, mergers can
only occur in high-density cluster environments. It is proposed that the
outflow emerging from the OMC1 core in the Orion molecular cloud was produced
by a protostellar merger that released between to ergs less
than a thousand years ago
Cost–utility analysis of antibiotic treatment in patients with chronic low back pain and Modic changes: results from a randomised, placebo-controlled trial in Norway (the AIM study)
Objective
To evaluate the cost–utility of 100 days of antibiotics in patients with chronic low back pain (LBP) and type I or II Modic changes included in the Antibiotic treatment In patients with chronic low back pain and Modic changes (AIM) study.
Design
A cost–utility analysis from a societal and healthcare perspective alongside a double-blinded, parallel group, placebo, multicentre trial.
Setting
Hospital outpatient clinics at six hospitals in Norway. The main results from the AIM study showed a small effect in back-related disability in favour of the antibiotics group, and slightly larger in those with type I Modic changes, but this effect was below the pre-defined threshold for clinically relevant effect.
Participants
180 patients with chronic LBP, previous disc herniation and Modic changes type I (n=118) or type II (n=62) were randomised to antibiotic treatment (n=89) or placebo-control (n=91).
Interventions
Oral treatment with either 750 mg amoxicillin or placebo three times daily for 100 days.
Main outcome measures
Quality-adjusted life years (QALYs) by EuroQoL-5D over 12 months and costs for healthcare and productivity loss measured in Euro (€1=NOK 10), in the intention-to-treat population. Cost–utility was expressed in incremental cost-effectiveness ratio (ICER).
Results
Mean (SD) total cost was €21 046 (20 105) in the amoxicillin group and €19 076 (19 356) in the placebo group, mean difference €1970 (95% CI; −3835 to 7774). Cost per QALY gained was €24 625. In those with type I Modic changes, the amoxicillin group had higher healthcare consumption than the placebo group, resulting in €39 425 per QALY gained. Given these ICERs and a willingness-to-pay threshold of €27 500 (NOK 275 000), the probability of amoxicillin being cost-effective was 51%. Even when the willingness-to-pay threshold increased to €55 000, the probability of amoxicillin being cost-effective was never higher than 53%.
Conclusions
Amoxicillin treatment showed no evidence of being cost-effective for people with chronic LBP and Modic changes during 1-year follow-up.publishedVersio
Progressive star formation in the young galactic super star cluster NGC 3603
Early release science observations of the cluster NGC3603 with the WFC3 on
the refurbished HST allow us to study its recent star formation history. Our
analysis focuses on stars with Halpha excess emission, a robust indicator of
their pre-main sequence (PMS) accreting status. The comparison with theoretical
PMS isochrones shows that 2/3 of the objects with Halpha excess emission have
ages from 1 to 10 Myr, with a median value of 3 Myr, while a surprising 1/3 of
them are older than 10 Myr. The study of the spatial distribution of these PMS
stars allows us to confirm their cluster membership and to statistically
separate them from field stars. This result establishes unambiguously for the
first time that star formation in and around the cluster has been ongoing for
at least 10-20 Myr, at an apparently increasing rate.Comment: 10 pages, 8 figures, accepted for publication in The Astrophysical

Numerical Relativity: A review
Computer simulations are enabling researchers to investigate systems which
are extremely difficult to handle analytically. In the particular case of
General Relativity, numerical models have proved extremely valuable for
investigations of strong field scenarios and been crucial to reveal unexpected
phenomena. Considerable efforts are being spent to simulate astrophysically
relevant simulations, understand different aspects of the theory and even
provide insights in the search for a quantum theory of gravity. In the present
article I review the present status of the field of Numerical Relativity,
describe the techniques most commonly used and discuss open problems and (some)

Migraine aura or transient ischemic attacks? A five-year follow-up case-control study of women with transient central nervous system disorders in pregnancy
<p>Abstract</p> <p>Background</p> <p>Migraine aura may be difficult to differentiate from transient ischemic attacks and other transient neurological disorders in pregnant women. The aims of the present study were to investigate and diagnose all pregnant women with transient neurological disorders of suspected central nervous system origin, and to compare this group with a control group of pregnant women with regard to vascular risk factors and prognosis.</p> <p>Methods</p> <p>During a 28 month period, 41 patients were detected with transient neurological symptoms during pregnancy. These were studied in detail with thorough clinical and laboratory investigations in order to make a certain diagnosis and to evaluate whether the episodes might be of a vascular nature. For comparison, the same investigations were performed in 41 pregnant controls. To assess the prognosis, both patients and controls were followed with questionnaires every year for five years.</p> <p>Results</p> <p>Migraine with aura was the most common cause of symptoms during pregnancy, occurring in 34 patients, while 2 were diagnosed with stroke, 2 with carpal tunnel syndrome, 1 with partial epilepsy, 1 with multiple sclerosis and 1 with presyncope. Patients had more headache before pregnancy than controls, but the average levels of vascular risk factors were similar. None of the patients or the controls reported cerebrovascular episodes during the five-year follow-up.</p> <p>Conclusion</p> <p>The diagnosis of migraine aura was difficult because for many patients it was their first ever attack and headache tended to be absent or of non-migraineous type. The aura features were more complex, with several aura symptoms and a higher prevalence of sensory and dysphasic aura than usual. Gradually developing aura symptoms, or different aura symptoms occurring in succession as described in the International Classification of Headache Disorders, seem to be useful for differentiating aura from other transient disorders. A meticulous history and clinical neurological examination are more useful than routine supplementary investigations for cerebrovascular disease. The five-year follow-up clearly indicates that migraine with aura in pregnancy usually has a good prognosis with regard to cerebrovascular events.</p
