Right-heart infective endocarditis: apropos of 10 cases
The prevalence and characteristics of right heart endocarditis in Africa are not well known. The aim of this study was to describe the epidemiological, clinical and laboratory profiles of patients with right-heart infective endocarditis. This was a 10-year retrospective study conducted in 2 cardiology departments in Dakar, Senegal. All patients who met the diagnosis of right heart infective endocarditis according to the Duke's criteria were included. We studied the epidemiological, clinical as well as their laboratory profiles. There were 10 cases of right-heart infective endocarditis representing 3.04% of cases of infective endocarditis. There was a valvulopathy in 3 patients, an atrial septal defect in 1 patient, parturiency in 2 patients and the presence of a pacemaker in one patient. Anaemia was present in 9 patients whilst leukocytosis in 6 patients. The port of entry was found to be oral in three cases, ENT in one case and urogenital in two cases. Apart from one patient with vegetations in the tricuspid and pulmonary valves, the rest had localized vegetation only at the tricuspid valve. However, blood culture was positive in only three patients. There was a favorable outcome after antibiotic treatment in 4 patients with others having complications; three cases of renal impairment, two cases of heart failure and one case of pulmonary embolism. There was one mortality. Right heart infective endocarditis is rare but associated with potentially fatal complications.Pan African Medical Journal 2015; 2
Qualite de vie apres un accident vasculaire cerebral au Senegal: a propos de 50 cas
Introduction: L’accident vasculaire cérébral (AVC) constitue une cause majeure de mortalité et de handicap chez les survivants. La qualité de vie dépend de la nature du handicap et de sa perception par le patient, son entourage et la communauté.Objectif: Notre objectif était d’évaluer le handicap après un AVC et d’apprécier son retentissement sur la qualité de vie des patients victimes.Patients et Méthodes: Nous avons mené une étude prospective, longitudinale de février 2008 à mai 2009, à la clinique neurologique de FANN. Les patients étaient vus à la phase initiale de l’AVC et 6 mois après et soumis à un questionnaire comportant plusieurs items relatifs à la nature de l’accident vasculaire cérébral, le vécu psychoaffectif et socioprofessionnel, l’index de Barthel a été utilisé chez tous les malades.Résultats: Nous avons colligé 50 patients âgés de 15 à 82 ans, avec une sex-ratio de 1,27. L’accident était ischémique dans 70%, hémorragique dans 30% des cas. 66% des patients avaient un index de Barthel entre (60-100), 6% étaient entre (0-20). Le score était meilleur chez les jeunes (15ans-34ans) qui avait tous un index entre (60-100). Par ailleurs 50% des patients de la tranche supérieure à 75ans étaient dans l’intervalle (60-100). Les AVCH avaient des Meilleurs résultats avec 73,3% à un index entre (60-100). seul 12% ont été réinsérés, sur le plan professionnel. 54,05%, n’ont pas noté de changement dans leur vie conjugale. Sur le plan familial, 56% ne notaient aucun changement. Une grande partie de notre série 44% acceptaient bien leurs  déficits,. 70% ne faisaient plus d’activités de temps libre.Conclusion: La qualité de vie est un concept multidimensionnel qui incorpore outre la santé physique les aspects mentaux et sociaux de la maladie.Mots clés: AVC, qualité de vie, SénégalEnglish Title: Quality of life after stroke in Senegal: about 50 casesEnglish AbstractIntroduction: Stroke is a major cause of mortality and disability in survivors. The quality of life depends on the nature of disability and its perception by the patient, his or her environment and community. The aim of this study was to assess disability after stroke and its impact on quality of life of patients (victims).Patients And Methods: We conducted a prospective, longitudinal study from February 2008 to May 2009, at the Neurological Clinic of Fann. Patients were seen at the initial phase of stroke and 6 months later and answered a questionnary containing 13 items like nature of stroke disability psychological emotional and socioprofessional filds. The index of barthel was used at all the patient’s.Results: We collected, were like this 50 patients aged 15-82 years with a sex ratio of 1.27. The accident was ischemic in 70% of cases, hemorrhagic in 30% of cases. 66% had a Barthel index between (60-100), 6% were between (0-20). The score was significantly better in young series (15-34), which all have a Barthel index between (60-100), more over 50% of patients in the portion above 75 years were in the range (60 - 100). Hemorragic stroke had the best results with 73.3% a Barthel index between (60-100). On a professional level, only 12% patients were rehabilitated. 54.05% didn’t notice any change in their marital life. On the home front, 56% noted any change. A large part of our series 44% accepted although their deficits. 70% didn’t have any more free time activities.Keywords: quality of life, stroke, Senega
Analyse d'incertitude en situation accidentelle : transfert de radionucléides dans l'environnement et évaluation de l'exposition humaine par voie alimentaire
Risk assessment, in case of nuclear emergency, is confronted to uncertainties on the transfer of radioactive substances in terrestrial ecosystems and to human population through the food chain, which could affect the reliability of decisions. The extent of the repercussions of Chernobyl and Fukushima accidents highlighted the difficulty of managing the consequences of such disasters and specifically to accommodate the different sources of uncertainty within decision-making processes. The objective of this research is to develop a methodology to account for uncertainties within environmental and food risk assessment models to improve decision support tools used for accidental situations. Different hierarchical Bayesian models aiming at capturing, within a unique modelling framework, uncertainty and variability about radioecological parameters of great importance for accidental situation were developed. Models parameters were estimated by Bayesian inference applied on databases obtained by an extended literature review. The impact on the risk assessment models of uncertainties about these radioecological parameters was then assessed by stochastic simulations and sensitivity analyses applied on two case-studies: a hypothetical accident simulating a standardized deposition of radionuclides and the accident of Fukushima nuclear power plant. The works developed in this project contribute to enhance knowledge on key processes governing environmental transfer of radionuclides and to improve the parameterization of the radioecological risk assessment models with respect to the research lines outlined by the scientific community in radioecology.L’évaluation des risques, en situation d’urgence nucléaire, est entachée d’incertitudes sur le transfert de substances radioactives dans les écosystèmes terrestres et vers l’homme à travers la chaîne alimentaire pouvant altérer les décisions. Quantification of Conceptual Model Uncertainty in the Modeling of Wet Deposited Atmospheric Pollutants
International audienceConceptual model uncertainty and parameter uncertainty are dominant contributors to the total uncertainty of a radioecological model output. In the present study the focus is on conceptual model uncertainty, which is often not acknowledged. Conceptual model uncertainty is assessed by subtracting from the total uncertainty of the model output the propagated parameter uncertainty, obtained by means of Bayesian inference analysis. The conceptual model uncertainty is quantified for two process-based models, which describe the interception of wet deposited pollutants under equilibrium and kinetic conditions, respectively. The natural variability due the chemical valence of the elements considered is accounted for in both models. Quantitative evidence has been obtained that the conceptual model uncertainty can contribute to the total uncertainty budget of the models for interception of wet deposited pollutants at least as much as, if not more than, parameter uncertainty
New approach for the assessment of cluster diets
Dietary risk assessment is a major public health concern, positioned in the context of establishing overall food safety policy. It requires some understanding of population food choices although geographical location and social-cultural environment are variable. Several years ago, a cluster analysis based on FAO consumption data, ranging from 1990 to 1994, was at the origin of the 13, so called, GEMS/Food cluster diets. This analysis required the initial identification of 19 food markers based on geographical and cultural differences. This paper proposes a new modelling of FAO food consumption database in order to define new cluster diets based on updated consumption data from 2002 to 2007 and better adapted statistical methods. Two statistical methods were combined to extract, consumption systems that generate a substructure from the initial food consumption database and then by deriving a clustering of countries according to their consumption system profiles. The clustering resulted in 17 cluster diets composed of 2 up to 30 countries. The few discrepancies between these new clusters and former ones may be due to more recent data, and to the fact that the new approach is based on another mathematical modelling which does not require any initial identification of food markers

Unavoidable Risks: Local Perspectives on Water Contact Behavior and Implications for Schistosomiasis Control in an Agricultural Region of Northern Senegal
Human schistosomiasis is a snail-borne parasitic disease affecting more than 200 million people worldwide. Direct contact with snail-infested freshwater is the primary route of exposure. Water management infrastructure, including dams and irrigation schemes, expands snail habitat, increasing the risk across the landscape. The Diama Dam, built on the lower basin of the Senegal River to prevent saltwater intrusion and promote year-round agriculture in the drought-prone Sahel, is a paradigmatic case. Since dam completion in 1986, the rural population-whose livelihoods rely mostly on agriculture-has suffered high rates of schistosome infection. The region remains one of the most hyperendemic regions in the world. Because of the convergence between livelihoods and environmental conditions favorable to transmission, schistosomiasis is considered an illustrative case of a disease-driven poverty trap (DDPT). The literature to date on the topic, however, remains largely theoretical. With qualitative data generated from 12 focus groups in four villages, we conducted team-based theme analysis to investigate how perception of schistosomiasis risk and reported preventive behaviors may suggest the presence of a DDPT. Our analysis reveals three key findings: 1) rural villagers understand schistosomiasis risk (i.e., where and when infections occur), 2) accordingly, they adopt some preventive behaviors, but ultimately, 3) exposure persists, because of circumstances characteristic of rural livelihoods. These findings highlight the capacity of local populations to participate actively in schistosomiasis control programs and the limitations of widespread drug treatment campaigns. Interventions that target the environmental reservoir of disease may provide opportunities to reduce exposure while maintaining resource-dependent livelihoods
