    Development of a direct fuel injector for a two-stroke gasoline engine

    A prototype pressure-swirl injector was designed to suit an arbitrary two�stroke, gasoline direct injection engine requirement. A commercial computational fluid dynamics software FLUENT was used as a tool to analyze the performance of the conceptual design. A prototype injector was fabricated once the conceptual design met the requirements. The fabricated prototype injector was then subjected to a series of specification tests such as leak test, static flow rate test and discharge coefficient test. A dedicated test rig was set up to evaluate the performance of the prototype injector. The spray images at specified time frame were illuminated by Nd:Y AG laser sheet and captured using a high-speed digital camera. The stored images were analyzed to give data of spray angle, and droplet Sauter mean diameter at fuel-air pressure differential ranging from 1.0 to 5.0 MPa, with a step size of 1.0 MPa. From the tests, the prototype injector static flow rate, half spray cone angle and the droplet Sauter mean diameter at fuel injection pressure of 5.0 MPa was found to comply with the outlined requirements, which are approximately 475 cc/min, 32° and 19 11m respectively. Finally, the experimental data was compared with the calculated data. It was found that the measured data of static flow rate, discharge coefficient, and droplet Sauter mean diameter were higher than the computed data at fuel-air pressure differential between 1.0 and 3.0 MPa. In contrast, the calculated initial spray angle was overestimated by 3% at all tested fuel-air pressure differentials

    Performance and emission evaluations of a prototype stepped-piston engine using carburetor and direct fuel-injection systems

    Two-stroke engines have been used for sometimes in automotive and stationary applications since early 20th century. The advantages of two-stroke engines are obvious, i.e., lighter, simpler and less expensive to manufacture. Technically, two-stroke engines have the potential to pack almost twice the power into the same space because there are twice as many power strokes per revolution. The combination of lightweight and twice the power gives two-stroke engines a great power-to-weight ratio compared to many four stroke engine designs. However due to the short-circuiting process of the fuel before combustion, this has resulted in deterioration in overall performances especially poor combustion efficiency and high white smoke emission problem. Coupled with the improvement in the four-stroke engine technology, the former has overcome the latter in being the choice for mobile platform applications. Due to high fuel cost and the need to explore the use of other fuel sources, notably gaseous fuels, a number of enthusiasts and engine developers have revisited the two-stroke engine design. Fuels such as hydrogen and methane are said to be ideal for use with the incorporation of the some new features (Goldsborough and Blaringan, 2003). An engine design and development program was initiated at Universiti Teknologi Malaysia (UTM) in year 2003 to develop local R&D capabilities in small power-train engineering. The exercise evolved around the development of an air-cooled single cylinder of stepped-piston engine concept. The term “stepped piston” refers to the conventional piston having compounded with a larger diameter section at the rear section of its geometry. The changes to the original design were made as the research group feels that there are rooms for improvements. In addition to this, the modifications will infuse other innovative scope of work from design to product testing activities (Hooper, 1985).This program, eventually leads to the incorporation of features, is expected to enhance performance of the prototype and subsequently exhaust emission. This is in anticipation of producing a working prototype for multiple applications namely stationary and automotive. The gasoline stepped-piston engine is a relatively new design concept for small mobile power plants. It is an engine, operating on a two-stroke cycle but is infused with four-stroke engine features. It has a build-in supercharger mechanism (by virtue of the extended flange) that improves the scavenging process thus improve combustion efficiency. Due to these operating characteristics, the engine has all the attributes of a low emission, high-efficiency power plant that eliminates many of the major weaknesses associated with the Otto four-stroke engine and with modern two-stroke engines

    Simulation study on the performance of a counter-rotating savonius vertical axis wind turbine

    Wind power is an energy source that is becoming an alternative to burning fossil fuels that may harm the environment during operations due to the emission of harmful gases. In this study, simulation and performance investigations of a counter-rotating vertical axis wind turbine (VAWT) based on the Savonius S-type rotor have been analysed through a computational simulation approach. The foremost motive of this study is to widen the operating wind speed range of the counter-rotating concept in a VAWT while enhancing the conversion efficiency of a single-rotor VAWT system. The 3D simulations were performed based on the K-omega shear stress transport (SST) turbulence model. The results have shown that the counter-rotating model possesses better performance characteristics in terms of torque, power and their corresponding coefficients compared to a single-rotor design of a wind turbine. A maximum output of more than two times was obtained from the new CRWT system compared to that of a single-rotor wind turbine (SRWT). Moreover, the output of the top rotor was higher than the bottom rotor due to the increased higher rotational speed of the top rotor

    CNG-Diesel Dual Fuel Controlling Concept for Common Rail Diesel

    Compressed Natural Gas (CNG) is gaining interest as a clean fossil fuel alternative in a diesel dual fuel system. The dual fuel system is proven to provide benefits in terms of fuel consumption and exhaust emission. This article briefly describes a concept of controlling strategy of a CNG-diesel dual fuel system for a common rail diesel engine. A lower diesel common rail pressure was emulated to reduce the diesel fuel quantity, then substitute it with an equivalent CNG fuel quantity. The tuning process is vital to ensure a comparable performance. It requires measurement of lambda values and tuning of both diesel and CNG set values in their respective look-up tables for the whole engine operation. Test results showed that the lambda values are between 1.5 and 3.0, depending on the load demand indicated by the accelerator pedal positions. This concept is relatively easy to be implemented, but it may cause poor combustion and emission quality due to poor diesel fuel atomization at lower injection pressure. However, an optimum performance and emission could be achieved by scrutinizing the diesel fuel reduction and CNG fuel substitution

    Population Health Metrics Research Consortium gold standard verbal autopsy validation study: design, implementation, and development of analysis datasets

    Background: Verbal autopsy methods are critically important for evaluating the leading causes of death in populations without adequate vital registration systems. With a myriad of analytical and data collection approaches, it is essential to create a high quality validation dataset from different populations to evaluate comparative method performance and make recommendations for future verbal autopsy implementation. This study was undertaken to compile a set of strictly defined gold standard deaths for which verbal autopsies were collected to validate the accuracy of different methods of verbal autopsy cause of death assignment.Methods: Data collection was implemented in six sites in four countries: Andhra Pradesh, India; Bohol, Philippines; Dar es Salaam, Tanzania; Mexico City, Mexico; Pemba Island, Tanzania; and Uttar Pradesh, India. The Population Health Metrics Research Consortium (PHMRC) developed stringent diagnostic criteria including laboratory, pathology, and medical imaging findings to identify gold standard deaths in health facilities as well as an enhanced verbal autopsy instrument based on World Health Organization (WHO) standards. A cause list was constructed based on the WHO Global Burden of Disease estimates of the leading causes of death, potential to identify unique signs and symptoms, and the likely existence of sufficient medical technology to ascertain gold standard cases. Blinded verbal autopsies were collected on all gold standard deaths.Results: Over 12,000 verbal autopsies on deaths with gold standard diagnoses were collected (7,836 adults, 2,075 children, 1,629 neonates, and 1,002 stillbirths). Difficulties in finding sufficient cases to meet gold standard criteria as well as problems with misclassification for certain causes meant that the target list of causes for analysis was reduced to 34 for adults, 21 for children, and 10 for neonates, excluding stillbirths. To ensure strict independence for the validation of methods and assessment of comparative performance, 500 test-train datasets were created from the universe of cases, covering a range of cause-specific compositions.Conclusions: This unique, robust validation dataset will allow scholars to evaluate the performance of different verbal autopsy analytic methods as well as instrument design. This dataset can be used to inform the implementation of verbal autopsies to more reliably ascertain cause of death in national health information systems

    Infected pancreatic necrosis: outcomes and clinical predictors of mortality. A post hoc analysis of the MANCTRA-1 international study

    : The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135-15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359-5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138-5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184-5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598-9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090-6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286-5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912-7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138-0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143-0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.     Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference) and obesity (BMI >2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents, the increases in double burden were driven by increases in obesity, and decreases in double burden by declining underweight or thinness. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesit

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030

    Evaluation of appendicitis risk prediction models in adults with suspected appendicitis

    Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent). Conclusion Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy.
