    Strategies to Promote Market-Oriented Smallholder Agriculture in Developing Countries: A Case of Kenya

    Smallholder Agriculture is key to livelihoods of many rural households in developing and transition economies. In Kenya, small farms account for over 75% of total agricultural production and nearly 50% of the marketed output. Despite favourable trends in global development drivers such as rising population, per capita incomes and emerging urban dietary preferences, most smallholder farmers remain poor. This study sought to characterize agricultural commercialization trends, identify and prioritize constraints to participation in markets, analyse determinants of percentage of output sold, and explore strategies to promote market-oriented production. A participatory Rapid Rural Appraisal approach, household survey and a Truncated Regression model were used. A sample of 224 farmers: 76 of them growing maize, 77 involved in horticulture (kales and tomatoes) and 71 practising dairy, were interviewed in one peri-urban and one rural district (Kiambu and Kisii, respectively). Results show that in rural areas, lower levels of output are sold and fewer farmers participate in markets compared to the peri-urban areas. Opportunities for profitable commercial agriculture are observed in growing demand, emerging food preferences and intensive farming. At village-level, market participation is hampered by poor quality and high cost of inputs, high transportation costs, high market charges and unreliable market information. At the household-level, the determinants of percentage of output sold are producer prices, market information arrangement, output, distance to the market, share of non-farm income and gender.     Changing the Principal Supervisor Role to Better Support Principals: Evidence from the Principal Supervisor Initiative

    The principal supervisor job has traditionally revolved around administration, operations and compliance, but as principals have increasingly been called on in recent years to concentrate on supporting high-quality teaching, the idea of a complementary makeover of the supervisor job has gained attention. In 2014, with funding from The Wallace Foundation, six large school districts around the country embarked on a four-year, $24-million initiative to redesign the supervisor position so it focused primarily on supporting principals in their role as instructional leaders.This report, which looks at the final year and the effects of the Principal Supervisor Initiative, concludes that the effort succeeded in changing the job so that it centered on developing and evaluating principals to help them promote effective teaching and learning in their schools. Over the course of the initiative, principals' ratings of their supervisors' effectiveness rose from 3.88 to 4.10 on a scale of 1-to-5, a statistically significant increase. Principals reported greater frequency of supervisor practices to develop school leadership—such as helping principals with data analysis, providing them with useful feedback and working with them to assess teacher effectiveness

    A scoping review of research funding for small-scale farmers in water scarce regions

    Water scarcity is a global issue that disproportionately affects small-scale farmers in low- and middle-income countries (LMICs). Through geospatial analysis, we estimated that less than 37% of small-scale farms probably have irrigation in water scarce regions across LMICs, compared with 42% of non-small-scale farms. Through a literature synthesis assisted by machine learning, we then systematically mapped the existing research for on-farm interventions that improve the incomes or yields of small-scale farmers in water scarce regions. We mapped over 888 on-farm interventions used to combat water scarcity from 560 publications and showed a research bias towards yields rather than livelihoods. We found gaps in evidence for many commonly proposed solutions, including livestock management, digital technology and solutions to protect natural resources at the farm-level, such as buffer strips. Our findings can be used to set a funding agenda for research on the geographies that are most at risk of water scarcity and the interventions that most lack evidence

    A Pilot Study Investigating the Utilization of Crest Pads for Treatment of Toe Callus and Ulceration (Presentation)

    Patients with lesser-toe deformities are at increased risk of developing calluses and ulcers on the distal ends of the affected digits because of the increased pressures applied to these areas. The number of diabetic patients in the United States continues to increase, along with associated comorbidities such as peripheral vascular disease and peripheral neuropathy. These conditions predispose patients to developing foot ulcerations, especially if foot deformities are present. Crest pads are a simple-to-make, inexpensive option to treat calluses and ulcerations on the distal ends of digits; however, there is no research available that support their use. Crest pads consist of rolled gauze covered in moleskin, with a large opening that fits over several toes and lies on the dorsal aspect of the foot, with the padded portion resting under the toes. Over several days of use, the pad molds to the plantar aspect of the toes, offloading pressure from the distal end of the affected digit(s). The sample was obtained through a retrospective chart review of patients identified as having had at least one nail care visit and at least one follow-up visit at a vascular surgery practice between August 2011 and December 2014. Potential subjects with toe deformities who presented with callus or ulcer on the distal end of a digit were considered for inclusion, if they received a crest pad as part of their treatment plan. The scholarly project was a preintervention or postintervention design with subjects acting as their own controls. McNemar\u27s test was used to analyze the results which were statistically significant (P \u3c .0001 at first callus follow-up and P = .0002 at second callus follow-up) for callus, hemorrhagic callus, and/or ulcer improvement following the crest pad intervention. The results of this scholarly project support the use of crest pads in patients with lesser-toe deformities to treat distal toe calluses and/or ulcerations

    Acute fatal presentation of ornithine transcarbamylase deficiency in a previously healthy male

    Ornithine transcarbamylase (OTC) deficiency is an X-linked urea cycle defect. While hemizygous males typically present with hyperammonemic coma in infancy, reports of rare late-onset presentations exist, with poor outcomes in males up to 58 years old. Relatives with mutations identical to affected patients often remain asymptomatic, and it is likely that environmental and genetic factors influence disease penetrance and expression. Here, we present our investigation of a patient with late-onset presentation, and we emphasize the potential role of environmental and genetic factors on disease expression. The patient was a previously healthy 62-year-old man who developed mental slowing, refractory seizures, and coma over an 8-day period. Interestingly, the patient had recently used home gardening fertilizers and pesticides. Evaluations for drug and alcohol use, infections, and liver disease were negative. Despite aggressive therapy, blood NH3 concentration peaked at 2,050 μM and the patient died from cerebral edema and cerebellar herniation. Analysis of the OTC gene showed a Pro-225-Thr (P225T) change in exon 7, a mutation that has been previously implicated in OTC deficiency. This case illustrates that OTC deficiency can cause acute, severe hyperammonemia in a previously healthy adult and that the P225T mutation can be associated with late-onset OTC deficiency. We speculate that exposure to organic chemicals might have contributed to the onset of symptoms in this patient. This case also emphasizes that persistent hyperammonemia may cause irreversible neurologic damage and that after the diagnosis of hyperammonemia is established in an acutely ill patient, certain diagnostic tests should be performed to differentiate between urea cycle disorders and other causes of hyperammonemic encephalopathy

    SKA2 regulated hyperactive secretory autophagy drives neuroinflammation-induced neurodegeneration

    High levels of proinflammatory cytokines induce neurotoxicity and catalyze inflammation-driven neurodegeneration, but the specific release mechanisms from microglia remain elusive. Here we show that secretory autophagy (SA), a non-lytic modality of autophagy for secretion of vesicular cargo, regulates neuroinflammation-mediated neurodegeneration via SKA2 and FKBP5 signaling. SKA2 inhibits SA-dependent IL-1β release by counteracting FKBP5 function. Hippocampal Ska2 knockdown in male mice hyperactivates SA resulting in neuroinflammation, subsequent neurodegeneration and complete hippocampal atrophy within six weeks. The hyperactivation of SA increases IL-1β release, contributing to an inflammatory feed-forward vicious cycle including NLRP3-inflammasome activation and Gasdermin D-mediated neurotoxicity, which ultimately drives neurodegeneration. Results from protein expression and co-immunoprecipitation analyses of male and female postmortem human brains demonstrate that SA is hyperactivated in Alzheimer's disease. Overall, our findings suggest that SKA2-regulated, hyperactive SA facilitates neuroinflammation and is linked to Alzheimer's disease, providing mechanistic insight into the biology of neuroinflammation

    Functionalized Poly(3-hexylthiophene)s via Lithium–Bromine Exchange

    Poly(3-hexylthiophene) (P3HT) is one of the most extensively investigated conjugated polymers and has been employed as the active material in many devices including field-effect transistors, organic photovoltaics and sensors. As a result, methods to further tune the properties of P3HT are desirable for specific applications. Herein, we report a facile postpolymerization modification strategy to functionalize the 4-position of commercially available P3HT in two simple steps–bromination of the 4-position of P3HT (Br–P3HT) followed by lithium−bromine exchange and quenching with an electrophile. We achieved near quantitative lithium–bromine exchange with Br–P3HT, which requires over 100 thienyl lithiates to be present on a single polymer chain. The lithiated-P3HT is readily combined with functional electrophiles, resulting in P3HT derivatives with ketones, secondary alcohols, trimethylsilyl (TMS) group, fluorine, or an azide at the 4-position. We demonstrated that the azide-modified P3HT could undergo Cu-catalyzed or Cu-free click chemistry, significantly expanding the complexity of the structures that can be appended to P3HT using this method.National Science Foundation (U.S.) (ECCS-0939514
