    Dentro de la amplia nómina de autores que, tras la aparición de las Novelas ejemplares (1613) cervantinas, convierten la novela corta en el género narrativo de mayor éxito editorial del Barroco, la andaluza Mariana de Carvajal ha sido tradicionalmente infravalorada por una crítica que ha estimado que su tardía colección, Navidades de Madrid (1663), carece del nervio narrativo de otros autores, presenta una acusada inclinación al costumbrismo y viene, en fin, a representar la decadencia y la domesticación del género. Pero Carvajal aprovecha un molde agotado a esas alturas para reflejar sus inquietudes cotidianas y las del sector social al que ella representa: una nobleza segundona, pseudoburguesa o venida a menos que debe sobrevivir inmersa en la crisis social barroca. Así considerados, sus relatos aparecen en ocasiones como un antecedente de algo nuevo: la novela burguesa y de costumbres. Esta es la perspectiva que ofrece esta obra, un estudio que se centra en la categoría narrativa que más información contiene sobre el contexto social y literario: el personaje

    This paper describes the design process, hydraulic stability tests and construction of two single-layer Cubipod armored breakwaters in the Port of Punta Langosteira (A Coruña, Spain), located on the Atlantic coast of Spain, the first single-layer armors of randomly placed massive concrete armor units. The environmental, geotechnical, economic and logistic conditions favored randomly-placed Cubipods in single-layer armoring. 3D hydraulic stability tests of single-layer Cubipod armored breakwaters proved useful to validate the final design with 15-tonne and 25-tonne Cubipod units

    [EN] The software used for making educational tools allows analyze the engineering phenomena in a more complete and explained way, by means of simulation and experimentation. It is necessary to know the equations about the behaviour of the object under study for the generation of these tools, and to solve them through the right software, obtaining the output values from the input values. With this software it is possible to realize new studies only modifying the parameters of the model or the input values. In this work the alternating current electric circuits are analyzed. Joined with the simulation, this technology also allows the experimentation through the development of practices by WEB that can be done with remote access. These tools can be used both for teaching in the classroom and laboratories. and for doing exercises and in their own study time of the students[ES] La utilización de software para la elaboración de herramientas didácticas permite, mediante simulación y experimentación, analizar los fenómenos de Ingeniería de una forma más completa e ilustrativa. Para la generación de estas herramientas será necesario conocer las ecuaciones que rigen el comportamiento del fenómeno objeto de estudio, y resolverlas mediante el software adecuado, obteniendo las variables de salida a partir de las variables de entrada. Con el software es posible realizar nuevos estudios modificando los parámetros del modelo o las variables de entrada. En este trabajo se analizan los Circuitos Eléctricos de Corriente Alterna. Junto a la simulación, la tecnología permite también la experimentación, mediante el desarrollo de prácticas vía WEB que pueden ser realizadas con acceso remoto.     Hepatitis B surface antigen loss after discontinuing nucleos(t)ide analogue for treatment of chronic hepatitis B patients is persistent in White patients

    [Objective]: The objective of this study was to determine the long-term clinical outcome and persistence of hepatitis B surface antigen (HBsAg) loss after discontinuation of treatment. [Background]: The prognosis of patients with chronic hepatitis B (CHB) treated with nucleos(t)ide analogues (NAs) who discontinue treatment after loss of HBsAg remains largely unknown, particularly in White patients. [Patients and methods]: We analysed a cohort of patients with CHB who discontinued NA treatment after loss of HBsAg. A total of 69 patients with hepatitis-B-e antigen-positive or hepatitis-B-e antigen-negative CHB with undetectable HBsAg during NA treatment were included after discontinuation of treatment, and followed up for a median period of 37.8 months (interquartile range: 23.8–54.6 months). [Results]: At the end of follow-up, none of the patients showed spontaneous reappearance of HBsAg and only one patient had detectable hepatitis B virus DNA (22 IU/ml). Another patient negative for HBsAg and anti-HBs developed hepatitis B virus reactivation without elevated transaminases after treatment with corticosteroids and vincristine for dendritic cell neoplasm, 38 months after withdrawal of the antiviral treatment. Regarding clinical outcome, a patient with cirrhosis developed hepatocellular carcinoma, 6.6 years after discontinuing treatment. None of the patients had hepatic decompensation or underwent liver transplantation. [Conclusion]: HBsAg clearance after discontinuing NAs in patients with CHB is persistent and associated with good prognosis. The risk for developing hepatocellular carcinoma persists among patients with cirrhosis

    A Case of Bicuspid Aorta with Stanford Type a Aortic Dissection

    Bicuspid aortic valve disease (BAV) is a congenital disease that frequently produces complications during adulthood, and is considered not only a problem of valvulogenesis, but also a more complex genetic alteration that involves the development of the heart and the aorta, which entails a high morbidity and mortality in these patients. In 50% of adults with this pathology, non-valvular anomalies are observed, with dilation of the ascending aorta being the most common. Genes such as NOTCH1, UFDL1, ACTA2, eNOS, among others, have been identified as responsible for the appearance of aneurysms, which increases the risk in these patients of complications such as aortic dissection and its subsequent rupture. Painless aortic dissection may be seen in less than 5% of patients and may have an atypical clinical presentation. A case and review of the literature is presented in which we can observe the great complexity of this pathology, in the case of a patient with BAV, annulo-ectatic dilation of the aortic root and ascending aorta who developed heart failure, syncope and presented as a complication Stanford type A aortic dissection, requiring highly specialized surgical treatment

    Cannabinoid agonists showing BuChE inhibition as potential therapeutic agents for Alzheimer's disease

    Designing drugs with a specific multi-target profile is a promising approach against multifactorial illnesses as Alzheimer's disease. In this work, new indazole ethers that possess dual activity as both cannabinoid agonists CB2 and inhibitors of BuChE have been designed by computational methods. On the basis of this knowledge, the synthesis, pharmacological evaluation and docking studies of a new class of indazoles has been performed. Pharmacological evaluation includes radioligand binding assays with [3H]-CP55940 for CB1R and CB2R and functional activity for cannabinoid receptors on isolated tissue. Additionally, in vitro inhibitory assays of AChE/BuChE and the corresponding competition studies have been carried out. The results of pharmacological tests have revealed that three of these derivatives behave as CB2 cannabinoid agonists and simultaneously show BuChE inhibition. In particular, compounds 3 and 24 have emerged as promising candidates as novel cannabinoids that inhibit BuChE by a non-competitive or mixed mechanism, respectively.     Definite and indeterminate nonalcoholic steatohepatitis share similar clinical features and prognosis: A longitudinal study of 1893 biopsy-proven nonalcoholic fatty liver disease subjects

    [Background and Aim] Histological score systems may not fully capture the essential nonalcoholic steatohepatitis (NASH) features, which is one of the leading causes of screening failure in clinical trials. We assessed the NASH distribution and its components across the fibrosis stages and their impact on the prognosis and their relationship with the concept of metabolic-associated fatty liver disease (MAFLD).[Methods] Spanish multicenter study including 1893 biopsy-proven nonalcoholic fatty liver disease (NAFLD) patients from HEPAmet registry. NASH was diagnosed by NAS score ≥4 (including steatosis, ballooning and lobular inflammation) and fibrosis by Kleiner score. The presence of MAFLD was determined. Progression to cirrhosis, first episode of decompensated cirrhosis and death were collected during the follow-up (4.7 ± 3.8 years).[Results] Fibrosis was F0 34.3% (649/1893), F1 27% (511/1893), F2 16.5% (312/1893), F3 15% (284/1893) and F4 7.2% (137/1893). NASH diagnosis 51.9% (982/1893), and its individual components (severe steatosis, ballooning and lobular inflammation), increased from F0 (33.6%) to F2 (68.6%), and decreased significantly in F4 patients (51.8%) (P = .0001). More than 70% of non-NASH patients showed some inflammatory activity (ballooning or lobular inflammation), showing a similar MAFLD rate than NASH (96.2% [945/982] vs. 95.2% [535/562]) and significantly higher than nonalcoholic fatty liver (NAFL) subjects (89.1% [311/349]) (P < .0001). Progression to cirrhosis was similar between NASH (9.5% [51/539]) and indeterminate NASH (7.9% [25/316]), and higher than steatosis (5% [14/263]) (logRank 8.417; P = .015). Death and decompensated cirrhosis were similar between these.[Conclusions] The prevalence of steatohepatitis decreased in advanced liver disease. However, most of these patients showed some inflammatory activity histologically and had metabolic disturbances.
