    Analysis of postural stability and body composition of women after unilateral mastectomy

    Summary Introduction: The electrical bioimpedance method allows a non-invasive and precise assessment of the content of fat tissue, muscle mass or the amount of fluid in the body depending on the state of health or lifestyle. Changes in anthropometric features may affect postural stability, general physical fitness of women after mastectomy. In addition, changes in the distribution and amount of fat and lean body components may affect the ability to maintain balance. Aim of the study: Analysis of the relationship between postural stability and body composition of women after mastectomy based on posturographic examination. Material and methods: The study involved 40 women after mastectomy aged from 52 to 87 years. Postural Stability Test in static and dynamic mode on the Biodex Balance System platform was used to assess postural stability. The body bioimpedance method (BIA) was used to analyze the body composition. The research tool was the body composition analyzer Tanita MC 780 MA. The research was carried out in the Posturology Laboratory of the Institute of Physiotherapy at the Jan Kochanowski University in Kielce. Results and conclusions: Based on the research, a significant correlation was observed with a negative correlation in the Postural Stability Test, in static mode between fat mass and postural stability indexes. Also significant statistical results with a positive correlation were demonstrated in the same test between the total body water content and the postural stability indexes. Postural Stability Test in dynamic mode showed significant influence of lean and muscle mass in maintaining a stable posture. The standing posture of women after mastectomy was characterized by larger sublimations in the sagittal plane than the frontal plane (A / P> M // L)

    The aim of the study was to analyze the sagittal spinal curvatures in children with scoliosis and idiopathic scoliosis attitude and norm. The shape of the spine was assessed using optoelectronic Diers formetric III 4D. The research was Carried out in the Posturology Laboratory at the Faculty of Medicine and Health Sciences, UJK in Kielce (Poland). The biggest differences in the absolute values ​​of the variables tested in girls and boys with a group of scoliosis, attitudes and scoliotic standard body length was observed for SP-VP (mm). Univariate analysis of variance showed significant differences in the measurement of intra-lordosis angle among both boys and girls. This means that only the angles of lordosis differed significantly between the groups scoliosis, attitudes and scoliotic standard. Both girls and boys lordosis angle of scoliosis in the group was significantly lower than in the attitudes scolitic group and with the standard. In children with scoliosis it occurred in the first place to shallow lumbar lordosis. The size of the physiological thoracic kyphosis and lumbar lordosis should be taken into account in the selection of methods of treatment of scoliosis. In the treatment of scoliosis, use patterns shoulders and the pelvis, to restore the normal range thoracic kyphosis and lumbar lordosis

    Efficacy and safety of insulin pump treatment in adult T1DM patients : influence of age and social environment

    Introduction and objective. Continuous subcutaneous insulin infusion (CSII) via personal insulin pump is a valuable therapeutic tool in T1DM patients. However, adherence to recommended CSII- related behaviours may be of concern to young adults with intensive, variable daily activities (students, young professionals). The aim of this observational study was to estimate treatment outcomes in young adult patients with T1DM, and compare them with older individuals. Materials and methods. Overall, 140 adults with T1DM on CSII were examined, divided into 2 subgroups: 77 patients younger than 26 years of age (mean 20.6 years) and 63 older subjects (mean 39.0). We compared the glycaemic control in both groups of T1DM subjects and analyzed treatment attitudes to identify potentially modifiable behaviours influencing the efficacy of the treatment. Results. The younger individuals were characterized by significantly worse treatment outcomes, compared to the older ones: the mean HbA1c levels were 7.6 ± 1.3% and 6.9±1.3% (p=0.00001), while the mean glucose levels based on glucometer downloads were 161±33.6 mg/dL and 136±21.8 mg/dL (p=0.00001), respectively. The frequency of self-monitoring of blood glucose (SMBG) was lower in younger individuals (5.3±2.1 vs. 7.0±2.8 daily, p=0.0005, respectively); they were also less frequently used advanced pump functions, e.g. the bolus calculator (48% vs. 67% users, p=0.0014, respectively). Conclusions. The efficacy of CSII treatment observed in young T1DM adults was worse than in older patients. The reason for this phenomenon remains unclear, it may be due simply to age-dependend behaviours, to social environment, or both

    SARS-CoV-2 infects an <I>in vitro</I> model of the human developing pancreas through endocytosis

    Recent studies showed that SARS-CoV-2 can infect adult human pancreas and trigger pancreatic damage. Here, using human fetal pancreas samples and 3D differentiation of human pluripotent cells into pancreatic endocrine cells, we determined that SARS-CoV-2 receptors ACE2, TMPRSS2, and NRP1 are expressed in precursors of insulin-producing pancreatic ÎČ-cells, rendering them permissive to SARS-CoV-2 infection. We also show that SARS-CoV-2 enters and undergoes efficient replication in human multipotent pancreatic and endocrine progenitors in vitro. Moreover, we investigated mechanisms by which SARS-CoV-2 enters pancreatic cells, and found that ACE2 mediates the entry, while NRP1 and TMPRSS2 do not. Surprisingly, we found that in pancreatic progenitors, SARS-CoV-2 enters cells via cathepsin-dependent endocytosis, which is a different route than in respiratory tract. Therefore, pancreatic spheroids might serve as a model to study candidate drugs for endocytosis-mediated viral entry inhibition and to investigate whether SARS-CoV-2 infection may affect pancreas development, possibly causing lifelong health consequences

    W niniejszej pracy przedstawiono wƂasne, jednooƛrodkowe doƛwiadczenia dotyczące stosowania interferonu w pierwszej linii leczenia systemowego ziarniniaka grzybiastego. Badaniem objęto grupę chorych leczonych w Centrum Onkologii w Bydgoszczy, u ktĂłrych po wyczerpaniu moĆŒliwoƛci leczenia miejscowego oraz w stopniu zaawansowania Ib–III podjęto terapię systemową. Maksymalny czas prowadzenia terapii sięgaƂ 3 lat. Interferon alfa jest leczeniem dobrze tolerowanym, skutkującym poprawąjakoƛci ĆŒycia u 80% chorych. Lek ten powoduje znaczną bądĆș caƂkowitą remisję trwającą do kilkunastu miesięcy. Terapia ziarniniaka grzybiastego jest leczeniem choroby przewlekƂej, co wpƂywa na zasady prowadzenia terapii.This paper reports our own experience on interferon-alpha as the first-line systemic treatment of mycosis fungoides in the group of patients with advanced stages of disease after failure of skin-directedtherapies. Therapy resulted in improved quality of life and long-lasting, high response rate up to 80%.     Guidelines of the Polish Respiratory Society for diagnosis and treatment of idiopathic pulmonary fibrosis

    Introduction: This document presents the guidelines of the Polish Respiratory Society (PTChP, Polskie Towarzystwo Chorób PƂuc) for diagnosis and treatment of idiopathic pulmonary fibrosis (IPF), developed by a group of Polish experts.Material and methods: The recommendations were developed in the form of answers to previously formulated questions concer-ning everyday diagnostic and therapeutic challenges. They were developed based on a current literature review using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology.Results: We formulated 28 recommendations for diagnosis (8), pharmacological treatment (12) as well as non-pharma-cological and palliative therapy (8). The experts suggest that surgical lung biopsy (SLB) not be performed in patients with the probable usual interstitial pneumonia (UIP) pattern, with an appropriate clinical context and unanimous opinion of a  multidisciplinary team. The experts recommend using antifibrotic agents in IPF patients and suggest their use irrespective of the degree of functional impairment. As regards non-pharmacological and palliative treatment, strong re-commendations were formulated regarding pulmonary rehabilitation, oxygen therapy (in patients with chronic respiratory failure), preventive vaccinations as well as referring IPF patients to transplant centres.     The Bcl I single nucleotide polymorphism of the human glucocorticoid receptor gene h-GR/NR3C1 promoter in patients with bronchial asthma: pilot study

    Bcl I in the promoter polymorphism observed within h-GR/NR3C1 gene may play an important role in the development of bronchial asthma and resistance to GCs in the severe bronchial asthma. The aim of the investigation was to study the correlation between this h-GR/NR3C1 gene polymorphism and occurrence of asthma in the population of Polish asthmatics. Peripheral blood was obtained from 70 healthy volunteers and 59 asthma patients. Structuralized anamnesis, spirometry and allergy skin prick tests were performed in all participants. Genotyping was carried out with PCR–RFLP method. In healthy, non-atopic population variants of Bcl I: GG, GC, CC were found with frequency 0.129/0.471/0.400, respectively. In asthma patients Bcl I: GG, GC, CC occurred with respective frequencies of 0.410/0.462/0.128. Chi-square analysis revealed a significantly different (P < 0.05) distribution between cases and controls for the Bcl I polymorphism. The Bcl I polymorphism of h-GR/NR3C1 gene is significantly associated with bronchial asthma, susceptibility to the development of severe form and resistance to GCs in Polish population

    Measurement of the top quark forward-backward production asymmetry and the anomalous chromoelectric and chromomagnetic moments in pp collisions at √s = 13 TeV

    Abstract The parton-level top quark (t) forward-backward asymmetry and the anomalous chromoelectric (d̂ t) and chromomagnetic (Ό̂ t) moments have been measured using LHC pp collisions at a center-of-mass energy of 13 TeV, collected in the CMS detector in a data sample corresponding to an integrated luminosity of 35.9 fb−1. The linearized variable AFB(1) is used to approximate the asymmetry. Candidate t t ÂŻ events decaying to a muon or electron and jets in final states with low and high Lorentz boosts are selected and reconstructed using a fit of the kinematic distributions of the decay products to those expected for t t ÂŻ final states. The values found for the parameters are AFB(1)=0.048−0.087+0.095(stat)−0.029+0.020(syst),Ό̂t=−0.024−0.009+0.013(stat)−0.011+0.016(syst), and a limit is placed on the magnitude of | d̂ t| &lt; 0.03 at 95% confidence level.     MUSiC : a model-unspecific search for new physics in proton-proton collisions at root s=13TeV

    Results of the Model Unspecific Search in CMS (MUSiC), using proton-proton collision data recorded at the LHC at a centre-of-mass energy of 13 TeV, corresponding to an integrated luminosity of 35.9 fb(-1), are presented. The MUSiC analysis searches for anomalies that could be signatures of physics beyond the standard model. The analysis is based on the comparison of observed data with the standard model prediction, as determined from simulation, in several hundred final states and multiple kinematic distributions. Events containing at least one electron or muon are classified based on their final state topology, and an automated search algorithm surveys the observed data for deviations from the prediction. The sensitivity of the search is validated using multiple methods. No significant deviations from the predictions have been observed. For a wide range of final state topologies, agreement is found between the data and the standard model simulation. This analysis complements dedicated search analyses by significantly expanding the range of final states covered using a model independent approach with the largest data set to date to probe phase space regions beyond the reach of previous general searches.Peer reviewe
