    Il monastero cistercense di S. Martino al Monte nel Duecento attraverso i documenti pontifici: prime riflessioni sulle relazioni con la curia romana

    This essay aims to reconstruct the history of the Cistercian monastery of S. Martino al Monte, located near Viterbo (Lazio), and focuses on its relations with the papacy and the papal curia during the 13th century. The analysis of the monastery’s documentation, especially papal records, has revealed that the first half of the century was a fruitful time for the Cistercian community. The monastery strengthened its relations with the Roman curia thanks to the personality of some eminent abbots who were involved in the workings of the curia, and to the support of some cardinals who promoted the Cistercian coenobium, subsidized its building renovation, and sponsored the abbey's interests in the curia. The monastery also enlarged its patrimonial endowment and reorganized its administrative and accounting system to improve the management of its possessions.El artículo pretende reconstruir la historia del monasterio cisterciense de S. Martino al Monte, situado cerca de Viterbo (Lazio), en sus relaciones con los papas y la curia papal durante el siglo XIII. El análisis de la documentación monástica, especialmente la papal, ha revelado una época provechosa para la comunidad cisterciense en la primera mitad del siglo. El monasterio reforzó sus relaciones con la curia romana gracias a la personalidad de eminentes abades integrados en los mecanismos curiales y al apoyo de algunos cardenales que promovieron el cenobio, subvencionaron su renovación edilicia y patrocinaron los intereses de la abadía en la curia. También se enriqueció en cuanto a su dotación patrimonial y organizó un sistema ordenado de gestión administrativa y contable de sus bienes

    Introduction: The nursing staff of urgency / emergency services often assist victims with firearms and stab wounds, therefore, these professionals have great opportunities to adequately preserve the forensic traces present in the victim's body and collaborate with the criminal investigation. Objective: The objective of this study was to analyze the performance and knowledge of nurses and nursing technicians in the preservation of forensic traces in shooting and stabbings victims treated at a hospital that is reference when it comes to emergencies in northeastern Brazil, highlighting the difficulties health professionals regarding the identification, collection and preservation of forensic remains in the victims. Methods: This is a descriptive study with a quantitative approach, conducted from 2017 to 2018. Results: Most nurses and nursing technicians perform less than 50% of the total actions proposed in the questionnaire (81,8% and 85,2%), as well as less than half of the procedures (87,9% and 90.9%). When comparing the answers of the three question groups (documentation, preservation and collection), it was noticed that the procedures related to documentation are the most performed and known by nurses when compared to nursing technicians. Conclusion: Although nurses and nursing technicians realize the importance of measures for the preservation of traces in shot and stabbed victims, few of them perform the necessary procedures for the preservation of forensic remains. In addition, the research findings highlight that the poor knowledge about the procedures to be performed was directly proportional to their execution.Introdução: A equipe de enfermagem dos serviços de urgência /emergência frequentemente atende vítimas por arma de fogo e branca, dessa forma, esses profissionais possuem grandes oportunidades para preservar adequadamente os vestígios forenses presentes no corpo da vítima e colaborar com a investigação criminal. Objetivo: objetivou-se analisar a atuação e conhecimento de enfermeiros e técnicos de enfermagem na preservação de vestígios forenses da vítima de violência por arma de fogo e arma branca atendida em um hospital de referência para urgências/emergências no Nordeste do Brasil, elencando as dificuldades dos profissionais de saúde quanto à identificação, coleta e preservação dos vestígios forenses nas vítimas. Métodos: trata-se de um estudo descritivo, com abordagem quantitativa, realizado no período de 2017 a 2018. Resultados: A maior parte dos enfermeiros e dos técnicos de enfermagem executam menos de 50% do total das ações propostas no questionário (81,8% e 85,2%), bem como desconhecem menos da metade dos procedimentos (87,9% e 90,9%). Ao comparar as respostas dos três grupos de perguntas (documentação, preservação e coleta), percebeu-se que os procedimentos relacionados a documentação são os mais e executados e conhecidos pelos enfermeiros quando comparado aos técnicos de enfermagem. Conclusão: Embora enfermeiros e técnicos de enfermagem percebam a importância de medidas para a preservação de vestígios de vítimas de violência por arma de fogo e arma branca, poucos deles executam os procedimentos necessários para a preservação dos vestígios forenses. Além disso, os achados da pesquisa destacam que o conhecimento deficiente sobre os procedimentos a serem realizados foi diretamente proporcional à sua execução

    Cabbage and fermented vegetables : From death rate heterogeneity in countries to candidates for mitigation strategies of severe COVID-19

    Large differences in COVID-19 death rates exist between countries and between regions of the same country. Some very low death rate countries such as Eastern Asia, Central Europe, or the Balkans have a common feature of eating large quantities of fermented foods. Although biases exist when examining ecological studies, fermented vegetables or cabbage have been associated with low death rates in European countries. SARS-CoV-2 binds to its receptor, the angiotensin-converting enzyme 2 (ACE2). As a result of SARS-CoV-2 binding, ACE2 downregulation enhances the angiotensin II receptor type 1 (AT(1)R) axis associated with oxidative stress. This leads to insulin resistance as well as lung and endothelial damage, two severe outcomes of COVID-19. The nuclear factor (erythroid-derived 2)-like 2 (Nrf2) is the most potent antioxidant in humans and can block in particular the AT(1)R axis. Cabbage contains precursors of sulforaphane, the most active natural activator of Nrf2. Fermented vegetables contain many lactobacilli, which are also potent Nrf2 activators. Three examples are: kimchi in Korea, westernized foods, and the slum paradox. It is proposed that fermented cabbage is a proof-of-concept of dietary manipulations that may enhance Nrf2-associated antioxidant effects, helpful in mitigating COVID-19 severity.Peer reviewe

    Nrf2-interacting nutrients and COVID-19 : time for research to develop adaptation strategies

    There are large between- and within-country variations in COVID-19 death rates. Some very low death rate settings such as Eastern Asia, Central Europe, the Balkans and Africa have a common feature of eating large quantities of fermented foods whose intake is associated with the activation of the Nrf2 (Nuclear factor (erythroid-derived 2)-like 2) anti-oxidant transcription factor. There are many Nrf2-interacting nutrients (berberine, curcumin, epigallocatechin gallate, genistein, quercetin, resveratrol, sulforaphane) that all act similarly to reduce insulin resistance, endothelial damage, lung injury and cytokine storm. They also act on the same mechanisms (mTOR: Mammalian target of rapamycin, PPAR gamma:Peroxisome proliferator-activated receptor, NF kappa B: Nuclear factor kappa B, ERK: Extracellular signal-regulated kinases and eIF2 alpha:Elongation initiation factor 2 alpha). They may as a result be important in mitigating the severity of COVID-19, acting through the endoplasmic reticulum stress or ACE-Angiotensin-II-AT(1)R axis (AT(1)R) pathway. Many Nrf2-interacting nutrients are also interacting with TRPA1 and/or TRPV1. Interestingly, geographical areas with very low COVID-19 mortality are those with the lowest prevalence of obesity (Sub-Saharan Africa and Asia). It is tempting to propose that Nrf2-interacting foods and nutrients can re-balance insulin resistance and have a significant effect on COVID-19 severity. It is therefore possible that the intake of these foods may restore an optimal natural balance for the Nrf2 pathway and may be of interest in the mitigation of COVID-19 severity

    Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     The paper traces the scientific motivations that led the authors to project and then to realize the monographic issue of "Ricerche Storiche” which title is "La didattica della storia di genere: esperienze, metodologie, percorsi" (n. 2, 2019). The article widens to retrace, albeit briefly, the history of women's education from the Middle Ages to the late Modern Age, highlighting the thematic focus more interesting, the articulations and the historical evidences, in an attempt to focus attention on the laborious and fragmented acquisition of cultural tools by women in the ancient Italian society. The authors highlight the numerous training agencies, so to speak, of the past, enhancing the individual and collective paths of schooling that developed along non-institutional paths.Nel saggio si ripercorrono le motivazioni scientifiche che hanno indotto le autrici a progettare e poi a realizzare il numero monografico di “Ricerche Storiche” dedicato a “La didattica della storia di genere: esperienze, metodologie, percorsi” (n. 2, 2019). Il contributo, tuttavia, si amplia a ripercorrere, per quanto in breve, la storia dell’istruzione delle donne dal Medioevo alla tarda Età Moderna, sottolineandone i nuclei tematici di maggiore interesse, gli snodi e le evidenze storiche, nel tentativo di porre l’attenzione sulla faticosa e frammentaria acquisizione degli strumenti culturali da parte delle donne nella società italiana delle epoche passate. Le autrici evidenziano le numerose agenzie formative, per così dire, del passato, valorizzando i percorsi individuali e collettivi della scolarizzazione che si snodarono lungo tracciati non istituzionali
