    Minimally invasive and full sternotomy in aortic valve replacement: a comparative early operative outcomes

    Introduction: aortic valve replacement is usually performed through a median full sternotomy (MFS) in our department. Minimally invasive aortic valve replacement (MIAVR) has been recently adopted as a new approach. According to the literature, the superiority of MIAVR is controversial. In this study we report early post-operative outcomes in MIAVR compared with MFS access with reference to blood Loss, wound infections, post-operative recovery, morbidity and mortality. Methods: this study was a prospective data collection from 36 consecutive patients undergoing isolated valve replacement. Two population study was identified, MIAVR group (group I n=18) and MFS group (group II n=18). Patients´ data were collected and analyzed using IBM SPSS statistics 21 software and Khi2 test has been used to compare the variables. The study variables are presented as numbers, percentage, median with interquartile range. Pre-operative planning was performed so that to obtain similar characteristics. Results: in group I, upper mini-sternotomy was used in 12 patients and right mini-thoracotomy in 6 patients. There was no difference in term of mortality and morbidity. MIAVR was associated with longer CPB time (93.25 (58-161) vs 131 (75-215) mins, P=0.047) with no significant difference in term of ACC time (81 (33-162) vs 58.8 (59-102) mins P=0.158). MIAVR´ Patients had likely lower incidence of red blood cells transfusion (16.7 vs 52.3%) without significant difference about post-operative haemoglobin (P = 0,330). Patients in group I had shorter ventilation time (2.35 (1-12) vs 9.3 (1-48) hours P < 0.01), shorter ICU stay (2.44 (1-8) vs 4.25 (1-9) days, P = 0,024). The length of hospital stay was shorter, 6.5 (5-9) days in group I vs 7.4 (6-11), P=0.0274. Length of chest tube stay was shorter in group I (mean 1.53 vs 2.4 days, P=0,033). Wound infections were not found in both groups. Conclusion: minimally invasive aortic valve replacement is associated with less blood loss, faster post-operative recovery faster post-operative recovery but increase operation time

    Temperature, water activity and gas composition effects on the growth and aflatoxin production by Aspergillus flavus on paddy

    The main aim of this study was to evaluate the combined effects of temperature with water activity (aw) and CO2 with aw on the growth and aflatoxin production by Aspergillus flavus Link on paddy. The effects of temperature (20–30 °C) and aw (0.92–0.98) on the relationship between colony diameter and aflatoxin production, and the influence of aw (0.92–0.98) and CO2 (20–80%) on the growth and toxin production were studied using full factorial design. Colony diameters were regularly measured and aflatoxins were periodically analyzed using HPLC with fluorescence detector. The growth and aflatoxin formation increased with aw at the temperatures studied, and toxin production was positively correlated with the incubation time and colony diameter. Except at 0.92 aw, as much as 80% CO2 failed to inhibit the growth of fungi completely. However, at all aw levels studied the growth parameters as estimated by Baranyi function and aflatoxin were affected by the increment in CO2 where growth rates and aflatoxin were negatively correlated with CO2 while the lag phase durations were positively correlated with CO2. Under 0.98 aw, the atmosphere enriched with 20% and 80% CO2 lead to at least 59% and 88% reduction in growth and 47% and 97% in the toxin production, respectively. At 0.95 aw, the lag phases of both isolates in average increased by a factor of 1.7–12 when the CO2 levels in the headspace were between 20 and 80% compared to the control. The growth rate and lag phase durations under the modified atmospheres were successfully described using a polynomial equation (R2 > 0.97). The results of the study could form a basis of indicative guidelines on the possible control of A. flavus and aflatoxin in paddy during temporary storage prior to drying

In some real applications in decision making, similarity may bring counterintuitive outcomes from the decision maker’s standpoint. Therefore, in this research, we propose some novel similarity measures for bipolar and interval-valued bipolar neutrosophic set such as the cosine similarity measures and weighted cosine similarity measures. The propositions of these similarity measures are examined, and two multi-attribute decision making techniques are presented based on proposed measures. For verifying the feasibility of proposed measures, two numerical examples are presented in comparison with the related methods for demonstrating the practicality of the proposed method. Finally, we applied the proposed measures of similarity for diagnosing bipolar disorder diseases

However, in the setting of post-transplantation immune suppression it may be expressed differently.</p> <p>Case presentation</p> <p>A 35-year-old Sudanese woman, with a history of renal transplantation two and half years prior to presentation, was on a single immune suppression therapy in the form of cyclosporine-A since the transplantation. During a regular follow-up visit, she was noticed to have gingival hypertrophy and bilateral breast and axillary swellings. She underwent successful surgical resection of the bilateral fibroadenomas.</p> <p>Conclusions</p> <p>Cyclosporine-A therapy post renal transplantation is associated with an increased incidence of benign breast changes as fibroadenoma. Regular follow-up and appropriate selection of immunosuppressant therapy are essential in the post transplantation management of these patients.</p

    Use of randomly ampliÆed polymorphic DNA analysis to differentiate isolates of Vibrio para-haemolyticus from cockles (Anadara granosa)

    A total of 35 Kanagawa-negative strains of Vibrio parahaemolyticus isolated from cockles (Anadara granosa) were investigated by randomly amplified polymorphic DNA fingerprinting with three primers and their plasmid profiles. Eighteen strains carried small plasmid(s) of 2.4 to 7.3kb that enabled the V. parahaemolyticus to be grouped into eight plasmid patterns. The three primers generated polymorphisms in all 35 strains of V. parahaemolyticus tested, producing bands ranging from 0.25 to 3.9kb. The RAPD profiles revealed a high level of DNA sequence diversity within the Vibrio parahaemolyticus strains tested, and that cockles in the study area are populated by genetically polymorphic strains of V. parahaemolyticus

    String theory on NS-NS AdS_3 x S^3 admits an exactly marginal deformation which breaks the SL(2,R)_R x SL(2,R)_L isometry of AdS_3 down to SL(2,R)_R x U(1)_L. The holographic dual is an exotic and only partially understood type of two-dimensional CFT with a reduced unbroken global conformal symmetry group. In this paper we study the deformed theory on the string worldsheet. It is found to be related by a spectral flow which is nonlocal in spacetime to the undeformed worldsheet theory.     Synthesis of graft copolymers based on hyaluronan and poly(3-hydroxyalkanoates)

    This work reports the synthesis and characterisation of new amphiphilic hyaluronan (HA) grafted with poly(3-hydroxyalkanoates) (PHAs) conjugates. Hydrolytic depolymerisation of PHAs was used for the synthesis of defined oligo(3-hydroxyalkanoates)-containing carboxylic terminal moieties. A kinetic study of the depolymerisation was followed to prepare oligomers of required molecular weight. PHAs were coupled with hydroxyl groups of HA mediated by N, N'-carbonyldiimidazole (CDI) or HSTU Tetramethyl-O-(N-succinimidyl) uronium hexafluorophosphate. For the first time, the covalent bonding of oligo derivatives of P(3-hydroxybutyrate), P(3-hydroxyoctanoate), P(3-hydroxyoctanoate-co-3-hydroxydecanoate) and P(3-hydroxyoctanoate-co-3-hydroxydecanoate-co-3-hydroxydodecanoate) and HA was achieved by “grafting to” strategy. Achieved grafting degree was a function of hydrophobicity of PHA, Mw and polarity of the solvent. The most suitable reaction conditions were observed for oligo (3-hydroxybutyrate) grafted to HA (grafting degree of 14%). Graft copolymers were characterized by FT-IR, NMR, DSC and SEC-MALLS. Graft copolymers can be physically loaded with hydrophobic drugs and may serve as drug delivery system
