    Efficient injection from large telescopes into single-mode fibres: Enabling the era of ultra-precision astronomy

    Photonic technologies offer numerous advantages for astronomical instruments such as spectrographs and interferometers owing to their small footprints and diverse range of functionalities. Operating at the diffraction-limit, it is notoriously difficult to efficiently couple such devices directly with large telescopes. We demonstrate that with careful control of both the non-ideal pupil geometry of a telescope and residual wavefront errors, efficient coupling with single-mode devices can indeed be realised. A fibre injection was built within the Subaru Coronagraphic Extreme Adaptive Optics (SCExAO) instrument. Light was coupled into a single-mode fibre operating in the near-IR (J-H bands) which was downstream of the extreme adaptive optics system and the pupil apodising optics. A coupling efficiency of 86% of the theoretical maximum limit was achieved at 1550 nm for a diffraction-limited beam in the laboratory, and was linearly correlated with Strehl ratio. The coupling efficiency was constant to within <30% in the range 1250-1600 nm. Preliminary on-sky data with a Strehl ratio of 60% in the H-band produced a coupling efficiency into a single-mode fibre of ~50%, consistent with expectations. The coupling was >40% for 84% of the time and >50% for 41% of the time. The laboratory results allow us to forecast that extreme adaptive optics levels of correction (Strehl ratio >90% in H-band) would allow coupling of >67% (of the order of coupling to multimode fibres currently). For Strehl ratios <20%, few-port photonic lanterns become a superior choice but the signal-to-noise must be considered. These results illustrate a clear path to efficient on-sky coupling into a single-mode fibre, which could be used to realise modal-noise-free radial velocity machines, very-long-baseline optical/near-IR interferometers and/or simply exploit photonic technologies in future instrument design.Comment: 15 pages, 16 figures, 1 table, published in A&

Standard palliative (20Gy/5#) or dose escalated treatment (36Gy/6#) will be delivered using advanced radiotherapy techniques and pain responses will be compared at week 5.     Implementation of foetal fibronectin testing: Admissions, maternal interventions and costs at 1 year

    Foetal fibronectin testing (fFN) has a high negative predictive value for preterm delivery, but it has a cost implication. This two-stage prospective study evaluated the real patient costs and clinical impact of introducing the fFN test in women presenting acutely with threatened preterm labour in a tertiary UK obstetric hospital. Introduction of the fFN test for women with threatened preterm labour reduced antenatal admissions and in utero transfers, and reduced steroid treatment and tocolysis, even at 1 year after implementation. The total number of bed days for women with threatened preterm labour who did not deliver during admission fell from 132 (mean 8.8 days) to 25 days (mean 3.6 days). The mean cost of admission per woman before introduction of the fFN test was £1032 (95% CI £880 to £1184); after it was £339 (95% CI £261 to £417). In this small single centre study, the introduction of the test produced a cost saving of £693 per woman (95% CI, £464 to £922) which over 12 months potentially saves £74844 (95% CI £50,112 to £99,576). Further studies are needed to formally evaluate the cost-effectiveness of the fFN test and its impact on clinical decision-making in large populations

    CHIMPS: the <sup>13</sup>CO/C<sup>18</sup>O (<i>J</i> = 3 → 2) Heterodyne Inner Milky Way Plane Survey

    We present the 13CO/C18O (J = 3 → 2) Heterodyne Inner Milky Way Plane Survey (CHIMPS) which has been carried out using the Heterodyne Array Receiver Program on the 15 m James Clerk Maxwell Telescope (JCMT) in Hawaii. The high-resolution spectral survey currently covers |b| ≤ 0.5° and 28° ≲ l ≲ 46°, with an angular resolution of 15 arcsec in 0.5 km s-1 velocity channels. The spectra have a median rms of ˜0.6 K at this resolution, and for optically thin gas at an excitation temperature of 10 K, this sensitivity corresponds to column densities of NH2 ˜ 3 × 1020 cm-2 and NH2 ˜ 4 × 1021 cm-2 for 13CO and C18O, respectively. The molecular gas that CHIMPS traces is at higher column densities and is also more optically thin than in other publicly available CO surveys due to its rarer isotopologues, and thus more representative of the three-dimensional structure of the clouds. The critical density of the J = 3 → 2 transition of CO is ≳104 cm-3 at temperatures of ≤20 K, and so the higher density gas associated with star formation is well traced. These data complement other existing Galactic plane surveys, especially the JCMT Galactic Plane Survey which has similar spatial resolution and column density sensitivity, and the Herschel infrared Galactic Plane Survey. In this paper, we discuss the observations, data reduction and characteristics of the survey, presenting integrated-emission maps for the region covered. Position-velocity diagrams allow comparison with Galactic structure models of the Milky Way, and while we find good agreement with a particular four-arm model, there are some significant deviations

    The regulatory landscape in the UK is changing again. From 1 April 2019 NHS England and NHS Improvement became what is effectively a single organisation with far reaching responsibility for the oversight of the system. The structural features of this change, which will eventually require legislative reform, have been widely debated, not least by those affected by plans for a collaborative approach to improvement in the NHS.12 But there has been less discussion about the style and approach to regulation that might be best suited to drive improvement in the NHS as set out in the long term plan.3 We contend that a major change is required in the way the system interacts with service providers if we are to be successful in developing a new service model for the 21st century

    Transition Property For Cube-Free Words

    We study cube-free words over arbitrary non-unary finite alphabets and prove the following structural property: for every pair (u,v)(u,v) of dd-ary cube-free words, if uu can be infinitely extended to the right and vv can be infinitely extended to the left respecting the cube-freeness property, then there exists a "transition" word ww over the same alphabet such that uwvuwv is cube free. The crucial case is the case of the binary alphabet, analyzed in the central part of the paper. The obtained "transition property", together with the developed technique, allowed us to solve cube-free versions of three old open problems by Restivo and Salemi. Besides, it has some further implications for combinatorics on words; e.g., it implies the existence of infinite cube-free words of very big subword (factor) complexity.Comment: 14 pages, 5 figure

    Urothelial cells may indicate underlying bacteriuria in pregnancy at term: a comparative study

    BACKGROUND: Urinary tract infection is common in pregnancy. Urine is sampled from by mid-stream collection (MSU). If epithelial cells are detected, contamination by vulvo-vagial skin and skin bacteria is assumed. Outside pregnancy, catheter specimen urine (CSU) is considered less susceptible to contamination. We compared MSU and CSU methods in term pregnancy to test these assumptions. METHODS: Healthy pregnant women at term gestation (n = 32, median gestation 38 + 6 weeks, IQR 37 + 6–39 + 2) undergoing elective caesarean section provided a MSU and CSU for paired comparison that were each analysed for bacterial growth and bladder distress by fresh microscopy, sediment culture and immunofluorescent staining. Participants completed a detailed questionnaire on lower urinary tract symptoms. Epithelial cells found in urine were tested for urothelial origin by immunofluorescent staining of Uroplakin III (UP3), a urothelial cell surface glycoprotein. Urothelial cells with closely associated bacteria, or “clue cells”, were also counted. Wilcoxons signed rank test was used for paired analysis. RESULTS: Women reported multiple lower urinary tract symptoms (median 3, IQR 0–8). MSU had higher white blood cell counts (median 67 vs 46, z = 2.75, p = 0.005) and epithelial cell counts (median 41 vs 22, z = 2.57, p = 0.009) on fresh microscopy. The proportion of UP3+ cells was not different (0.920 vs 0.935, z = 0.08, p = 0.95), however MSU had a higher proportion of clue cells (0.978 vs 0.772, z = 3.17, p = 0.001). MSU had more bacterial growth on sediment culture compared to CSU specimens (median 8088 total cfu/ml vs 0, z = 4.86, p = 0.001). Despite this, routine laboratory cultures reported a negative screening culture for 40.6% of MSU specimens. CONCLUSION: Our findings have implications for the correct interpretation of MSU findings in term pregnancy. We observed that MSU samples had greater bacterial growth and variety when compared to CSU samples. The majority of epithelial cells in both MSU and CSU samples were urothelial in origin, implying no difference in contamination. MSU samples had a higher proportion of clue cells to UP3+ cells, indicating a greater sensitivity to bacterial invasion. Urinary epithelial cells should not be disregarded as contamination, instead alerting us to underlying bacterial activity

    The Rise and Fall of Debris Disks: MIPS Observations of h and chi Persei and the Evolution of Mid-IR Emission from Planet Formation

    We describe Spitzer/MIPS observations of the double cluster, h and χ\chi Persei, covering a \sim 0.6 square-degree area surrounding the cores of both clusters. The data are combined with IRAC and 2MASS data to investigate \sim 616 sources from 1.25-24 μm\mu m. We use the long-baseline KsK_{s}-[24] color to identify two populations with IR excess indicative of circumstellar material: Be stars with 24 μm\mu m excess from optically-thin free free emission and 17 fainter sources (J\sim 14-15) with [24] excess consistent with a circumstellar disk. The frequency of IR excess for the fainter sources increases from 4.5 μm\mu m through 24 μm\mu m. The IR excess is likely due to debris from the planet formation process. The wavelength-dependent behavior is consistent with an inside-out clearing of circumstellar disks. A comparison of the 24 μm\mu m excess population in h and χ\chi Per sources with results for other clusters shows that 24 μm\mu m emission from debris disks 'rises' from 5 to 10 Myr, peaks at \sim 10-15 Myr, and then 'falls' from \sim 15/20 Myr to 1 Gyr.Comment: 48 pages, 15 figures, accepted for publication in The Astrophysical Journa
