    Urban sprawl and land use/land-cover transition probabilities in peri-urban Kumasi, Ghana

    This paper examines Land Use and Land Cover (LULC) transition probabilities and its implications for Kumasi Metropolis using remote sensing image analysis technique. Methods used for the study include sub-setting of satellite images for the metropolis using the metropolitan shapefile boundary and classification of the images using maximum likelihood image classification algorithm. A Markov Model was applied to predict probabilities of LULC changes in 15 years (2016 - 2031). Study results show the probability of urban lands changing to agricultural land as low and so is the probability of farmland transitioning to urban land use. Vegetation however shows a high probability of change to built-up area while the likelihood of change from water to other land cover types is not a possibility. The study recommends enforcement of relevant land use policies backed by vigorous public education to make sustainable urban land use in the Metropolis a reality. Also, vertical rather than horizontal construction of buildings could stem the sprawling city

    Optimism/pessimism and health-related quality of life during pregnancy across three continents: a matched cohort study in China, Ghana, and the United States

    <p>Abstract</p> <p>Background</p> <p>Little is known about how optimism/pessimism and health-related quality of life compare across cultures.</p> <p>Methods</p> <p>Three samples of pregnant women in their final trimester were recruited from China, Ghana, and the United States (U.S.). Participants completed a survey that included the Life Orientation Test - Revised (LOT-R, an optimism/pessimism measure), the Short Form 12 (SF-12, a quality of life measure), and questions addressing health and demographic factors. A three-country set was created for analysis by matching women on age, gestational age at enrollment, and number of previous pregnancies. Anovas with post-hoc pairwise comparisons were used to compare results across the cohorts. Multivariate regression analysis was used to create a model to identify those variables most strongly associated with optimism/pessimism.</p> <p>Results</p> <p>LOT-R scores varied significantly across cultures in these samples, with Ghanaian pregnant women being the most optimistic and least pessimistic and Chinese pregnant women being the least optimistic overall and the least pessimistic in subscale analysis. Four key variables predicted approximately 20% of the variance in overall optimism scores: country of origin (p = .006), working for money (p = .05); level of education (p = .002), and ever being treated for emotional issues with medication (p < .001). Quality of life scores also varied by country in these samples, with the most pronounced difference occurring in the vitality measure. U.S. pregnant women reported far lower vitality scores than both Chinese and Ghanaian pregnant women in our sample.</p> <p>Conclusion</p> <p>This research raises important questions regarding what it is about country of origin that so strongly influences optimism/pessimism among pregnant women. Further research is warranted exploring underlying conceptualization of optimism/pessimism and health related quality of life across countries.</p

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    A tailored within-community specimen collection strategy increased uptake of cervical cancer screening in a cross-sectional study in Ghana

    Abstract Background The implementation of cervical cancer screening strategies has reported different rates of success in different countries due to population specific factors that limit women’s participation. We report observations and the development of a community-based specimen collection strategy which resulted from interactions with women in the study communities, following an initial low response to a hospital based cervical cancer screening strategy. Method Women were recruited by a house survey and invited to report at a hospital either within a week or after a week for self and health-personnel specimen collections. However, due to the very low response and subsequent interactions with the women of the communities, another strategy was developed that required recruited women report at a central location within their respective communities for specimen collections at times that did not interfere with their daily routines. Results For specimen collection, of the 156 participants who opted to report after a week at the hospital, 60 (38.5%) reported. Of the 118 participants who opted to report within 1 week at the hospital, 55 (46.6%) reported. Of the 103 participants were invited to report at a specified location within the community, 98 (95.1%) reported. An overall response rate of 60.4% was attained. Almost 89.7% (226 of 253) of the women performed both self and health personnel sample collection. Conclusion The community-based strategy with self-specimen collection and HPV testing holds great potential for increasing women’s participation in cervical cancer screening in Ghana as compared to the hospital based strategy

    Prevalence of Porcine Rotavirus Antigen and Associated Risk Factors in Pig-Raising Communities and Institutional Piggeries in Zaria, Kaduna State, Nigeria

    Porcine rotaviruses are potential reservoirs for genetic exchange with human rotaviruses. A cross-sectional study was carried out to determine the prevalence of porcine Rotavirus antigen and associated risk factors in pig-raising communities and institutional piggeries in Zaria, Kaduna State, Nigeria. A total of 376 faecal samples from pigs of all ages were collected from backyard and institutional piggeries by convenience sampling. The faecal samples collected were analysed using commercially available ELISA kit: BioK 343/2, for the antigenic diagnosis of rotavirus in porcine faeces. The overall prevalence of rotavirus antigen in pigs was 9.8 % (37/376). Piglets (10.4 %) had a higher prevalence than adults (9.1 %), while males (10.1 %) were more infected than females (9.6 %). Breed-specific prevalences revealed 5.9 %, 12 % and 15.5 % for local, exotic and cross-breeds, respectively. There was a significant association between breed (P < 0.05) (Odds Ratio OR = 2.927; 95 % Confidence Interval CI on OR = 1.288—6.653) and rotavirus infection. Management system revealed 14 % and 8.2 % prevalence for intensive and semi-intensive systems, respectively. There is evidence of Rotavirus infection (9.8 %) in pigs in Zaria, and the breed is a risk factor. This study provides the first data on the prevalence of rotavirus and risk factors of rotavirus infection among pigs in Zaria and environs, Kaduna state, Nigeria. There is a need to enlighten the public on the zoonotic implication and economic impacts of rotavirus infections

    Methodological development of tools to measure how women are treated during facility-based childbirth in four countries: labor observation and community survey

    BACKGROUND: Efforts to improve maternal health are increasingly focused on improving the quality of care provided to women at health facilities, including the promotion of respectful care and eliminating mistreatment of women during childbirth. A WHO-led multi-country research project aims to develop and validate two tools (labor observation and community survey) to measure how women are treated during facility-based childbirth. This paper describes the development process for these measurement tools, and how they were implemented in a multi-country study (Ghana, Guinea, Myanmar and Nigeria). METHODS: An iterative mixed-methods approach was used to develop two measurement tools. Methodological development was conducted in four steps: (1) initial tool development; (2) validity testing, item adjustment and piloting of paper-based tools; (3) conversion to digital, tablet-based tools; and (4) data collection and analysis. These steps included systematic reviews, primary qualitative research, mapping of existing tools, item consolidation, peer review by key stakeholders and piloting. RESULTS: The development, structure, administration format, and implementation of the labor observation and community survey tools are described. For the labor observations, a total of 2016 women participated: 408 in Nigeria, 682 in Guinea, and 926 in Ghana. For the community survey, a total of 2672 women participated: 561 in Nigeria, 644 in Guinea, 836 in Ghana, and 631 in Myanmar. Of the 2016 women who participated in the labor observations, 1536 women (76.2%) also participated in the community survey and have linked data: 779 in Ghana, 425 in Guinea, and 332 in Nigeria. CONCLUSIONS: An important step to improve the quality of maternity care is to understand the magnitude and burden of mistreatment across contexts. Researchers and healthcare providers in maternal health are encouraged to use and implement these tools, to inform the development of more women-centered, respectful maternity healthcare services. By measuring the prevalence of mistreatment of women during childbirth, we will be able to design and implement programs and policies to transform maternity services
