    Malignant tumor of the peripheral nerve sheath in a left Whippet canine kidney

    Background: Malignant tumors of the peripheral nerve sheath (MTPNS`s) are considered rare tumors that can affect soft tissues. In dogs, the occurrence is more common in the nerves of the brachial plexus, but they can affect the lumbosacral plexus and cranial nerves. Rarely, they can affect spinal nerves and nerve roots and the urinary tract, especially in kidneys. The present report aims to describe a clinical case of a 10-year-old sterilized female whippet, who had a history of persistent hematuria for months, with subsequent diagnosis of MTPNS as the cause of hematuria.Case: The patient came for evaluation with a history of persistent hematuria. Evaluation of abnormal elements and sedimentation, showed the description of numerous red blood cells and the presence of proteinuria. The abdominal ultrasound revealed a left kidney with enlarged dimensions, irregular contour, loss of corticomedullary definition. The urinary vesicle showed an increase in cellularity. On physical examination, the patient had vital parameters within the normal range. A Snap 4DX® Plus exam was requested, which showed a reaction for Dirofilaria immitis. With this result, it was initially suspected that renal vasculitis. After starting the treatment, the patient started to present normal colored urine. However, after the end of this period, the patient returned to hematuria. After six months of treatment and without justification for the permanence of hematuria, urethrocystoscopy was indicated, which revealed a urinary vesicle with a hemorrhagic focus. A urinary bladder wall biopsy was performed, which showed no changes. Four months after the urethrocystoscopic exam, the patient had her first azotemic crisis. This time that the left renal neoformation observed on ultrasound examination. With the discovery of the origin of the problem, a therapeutic approach could be instituted, consisting of the left nephrectomy. The histopathological analysis showed malignant spindle cell neoplasia. Immunohistochemical analysis in peroxidase revealed positivity for the markers, concluding the diagnosis as a tumor of the peripheral nerve sheath. Seven months after surgery, a new ultrasound examination of the abdominal region, as well as tests for the assessment of renal function, all of which were within normal parameters.Discussion: Most of the MTPNS originate in the peripheral nerves and in the cranial nerve sheath, but it can rarely originate in the abdominal cavity, as in the case reported here, in which the origin was renal. Such neoplasms are not common in dogs, with an incidence of 0.5%. The age of the affected dogs is, on average, nine years for the appearance of the neoplasia, with females with the highest frequency of occurrence. Both data corroborate the patient's age at the time of diagnosis. This is a tumor that is considered to be highly malignant and progressive, which did not occur in the patient in question, since she had a history of persistent hematuria for a long period, with no manifestation of the presence of the tumor on imaging. Still regarding the clinical manifestation of the patient's persistent hematuria, the same occurrence can be observed in a human patient who also reported a history of persistent hematuria and, like the patient in the report, not associated with any other clinical signs. It is important to differentiate this type of neoplasia from the others, because in many cases, the histological patterns are similar, making the performance of immunohistochemistry essential for the diagnosis. Renal MTPNS is rare in humans, and in dogs it is no different. This fact makes it difficult to describe the prognosis. Thus, efforts should be considered for the early diagnosis of peripheral nerve neoplasms, in view of their low response to available halopathic treatments and taking into account the rare aspect and evolution of this disease

    Introdução: O diabetes insipidus nefrogênico é uma patologia que resulta da resistência à ação do hormônio antidiurético. A principal ação desse hormônio é regular a capacidade do rim de reabsorver água, o que explica as manifestações clínicas da doença que envolvem polidipsia, noctúria e poliúria. O diagnóstico é realizado, principalmente, a partir da privação hídrica e da análise da consequente concentração da urina. O tratamento, de maneira geral, consiste na administração de fármacos que auxiliam na diminuição do volume urinário associado a uma dieta pobre em solutos a fim de controlar os sintomas. Objetivos: O objetivo desse estudo é revisar sobre o diabetes insipidus nefrogênico, compreendendo epidemiologia, fisiopatologia, manifestações clinicas, diagnóstico e tratamento. Métodos: Os bancos de dados Pubmed, Diretrizes e UpToDate foram pesquisados eletronicamente utilizando os descritores diabetes insipidus nefrogênico; diabetes insipidus nos idiomas inglês e português. Discussão e Conclusão: Trata-se de uma doença de manifestações clínicas sistêmicas, uma vez que a fisiopatologia envolve o equilíbrio osmótico do corpo. Distúrbios eletrolíticos podem se fazer presente em pacientes não tratados, embora o tratamento não exclua o risco de  pela excreção hídrica excessiva, especificamente a hipernatremia. Trata-se de uma doença de difícil controle pois mesmo com as medicações e mudanças na alimentação o paciente ainda apresentará parte dos sintomas

    O estudo descreve o treinamento concorrente (TC) no desenvolvimento da força e motricidade global, a partir da sua prática por um paciente com autismo clássico, pois o sujeito com autismo pode desenvolver déficits na motricidade global, além de apresentar problemas de coordenação neuromuscular. O objetivo foi analisar através do treinamento os níveis de força e motricidade global de um paciente autista clássico no espaço terapêutico Amira Figueiras (ARIMA). O estudo foi desenvolvido por meio de uma analise documental dos prontuários e relatórios semestrais disponibilizados pelo Espaço. Foram analisados os avanços do mesmo de modo gradativo ao longo das sessões terapêuticas de treinamento, apresentando seus resultados por meio de gráfico e quadro, onde é possível identificar grandes níveis de evolução no desenvolvimento das capacidades objetivadas na pesquisa. Por fim, é preciso compreender que as dificuldades encontradas perante as observações da escassez de conteúdo bibliográfico e a prática do treinamento concorrente para este grupo o tornam um tema delicado, porém para que resultados eficazes sejam alcançados é necessária a mediação adequada do professor de Educação Física capacitado para o desenvolvimento desta prática

    Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.     Os aspectos semiológicos do acidente vascular encefálico: uma abordagem neurológica

    O Acidente Vascular Encefálico (AVC) é um evento neurológico súbito, com um foco de isquemia ou hemorragia. Ambos, qualificados pelo déficit neurológico focal abrupto. Ressaltando, que estes déficits podem ocorrer, sendo a ocorrência espontânea, perduração de 15 minutos, autoresolutiva é denominada como Ataque Isquêmico Transitório (AIT), no entanto, toda insuficiência neural que não melhorar pós esse período deve ser manejado como AVC. O artigo objetivou descrever os principais aspectos clínicos do AVC. O AVC é uma emergência para a saúde pública, em razão de ser um potencial em gerar morbimortalidades para os portadores e prejuízos para os sistemas de saúde. O AVC do tipo isquêmico representa a maioria das ocorrências, o quadro clínico do paciente é correspondente ao tecido neural afetado, inicialmente a tomografia computadorizada sem contraste é o primeiro exame, por ser crucial para descartar a etiologia hemorrágica, a condução terapêutica se baseia em medidas neuroprotetoras através da estabilização da glicemia, temperatura e sódio, adequar os níveis pressóricos, mediante o prazo estipulado impor terapia antitrombótica. A manifestação hemorrágica, pode ocorrer por torção de aneurisma sacular originando o sangramento subaracnóideo ou por hipertensão gerando o sangramento intraparenquimatoso. A partir da análise das informações coletadas, elucida-se que o diagnóstico precoce e o período transcorrido até o manejo terapêutico são cruciais para o desfecho clínico do portador, ou seja, é possível a normalização ou ocorrer sequelas neurais e óbito

    Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.
