A propósito del Bicentenario: ambigüedades en la Independencia de Arequipa
This research aims to assess the contingencies and ambiguities of independence in Arequipa, where the elite and commoners were not set in a large economic gap, rather it was diffuse in economic terms, but not so within a hegemony and subordination based on concepts of honor and honesty to the rich for the poor, in this network are two key steps in entering Arequipa Pumacahua —1814— and the oath of independence —1825—, mentions historiography arequipeño patriotic capacity from some characters, which we believe false as both the mob and the elite did not want independence, only some reforms sought to improve their situation in the urban world, where if there is pressure for a deeper transformation is in the rural area. Themethodology we used is the regional history, plus archival sources used belong to repositories Arequipa Regional Archive, Archive Archbishop of Arequipa and the Municipal Archives. This research presents some results about the ambivalence of elite-mob groups multiethnic, and geographic features that allow distances appreciate a more particular national activity, coupled with the high Peru commercially. Lets see that finally the independence of Arequipa more Sucre was mandated by a statement published in the newspaper and Spring of Arequipa, a landmark of the population patriotism, it does not inhibit that there were some intellectuals, priests and people who did agree with a radical change.Esta investigación tiene como finalidad apreciar las contingencias y ambigüedades de la independencia en Arequipa, donde la diferencia entre la elite y la plebe no estaba establecida en la gran brecha económica, antes bien era difusa; no así en relación de una hegemonía y subordinación que estaban basadas en conceptos de honor para el rico y honradez para el pobre. En ese contexto se dan dos etapas fundamentales: el ingreso de Pumacahua en Arequipa —1814— y la jura de la independencia —1825—; la historiografía menciona la capacidad patriótica del arequipeño a partir de algunos personajes, no obstante, tanto la plebe como la elite no quisieron la independencia, solo pretendían algunas reformas que mejoraran su situación en el mundo urbano. Fue en la zona rural donde se puso en evidencia una auténtica preocupación por lograr transformaciones más profundas. Para este estudio, se usó la historia regional como metodología y las fuentes de archivo utilizadas pertenecen a los repositorios del Archivo Regional de Arequipa, Archivo Arzobispal de Arequipa y el Archivo Municipal. La presente investigación muestra algunos resultados en torno a la ambivalencia de los grupos elite-plebe pluriétnicos. Asimismo, la fisonomía geográfica y las distancias permiten apreciar una actividad antes que nacional, particular: esto respondía a la unión comercial que en aquellos años existió con el Alto Perú. Finalmente, la independencia de Arequipa fue más un mandato de Sucre 2 que el resultado de un patriotismo señero de la población; ello no impide reconocer el mérito de algunos intelectuales, sacerdotes y población que estuvieron de acuerdo con un cambio radical
Comparison of Academic Procrastination in University Health and Social Science Students
Academic procrastination in university students of social and health sciences was compared according to socio-academic variables, such as: age, gender, occupation, area and year of study. The research was descriptive-comparative, quantitative, non-experimental; 1000 university social and health sciences students intentionally selected according to quotas participated. The information was collected with a duly validated instrument about academic procrastination. It was observed that: the majority of students perceive high academic procrastination (62%), low academic self-regulation (41%) and high procrastination of activities (74%). Concluding that the trend of academic procrastination is mostly present in students of social sciences, in male students, in the first years or academic cycles and in those who are studying while working. Therefore, most students who procrastinate avoid prioritizing the development of academic activities for others that are of particular interest such as: excessive use of technology, social networks, dependence on cell phones and work
Propiedades psicométricas de la escala de salud mental positiva en Arequipa (Perú)
La salud mental positiva es un constructo complejo que está relacionado con el funcionamiento óptimo de la persona. Comprende un conjunto de cualidades orientadas al desarrollo del potencial del individuo. La Escala de Salud Mental Positiva es uno de los instrumentos más utilizados para evaluarlo, sin embargo, los antecedentes señalan inconsistencias respecto a su estructura interna. El objetivo del presente estudio fue analizar las propiedades psicométricas de la Escala de Salud Mental Positiva en Arequipa-Perú. Participaron 3933 personas, 50.3% fueron mujeres y 49.7% fueron varones, incluyendo desde adolescentes hasta adultos mayores. La evaluación mediante AFC de la estructura original evidenció índices de ajuste pobres, por lo que se tuvo que evaluar la dimensionalidad y proponer una nueva estructura. Para lo cual, se dividió a la muestra (n1 = 1966 y n2 = 1967). En la primera, se aplicó un AFE y en la segunda se valida mediante un AFC. Se hallaron tres factores y se concluye que tiene un buen ajuste (χ2(431) = 2473.378; CFI = .959; TLI = .956, RMSEA = .049; SRMR = .051). La consistencia interna mostró valores mayores a .81. Finalmente, se evaluó la equivalencia de la medición según el sexo, hallando que el instrumento presenta invarianza de la medición.Positive mental health is a complex construct that is related to the optimal functioning of the person. Three factors were found and it is concluded that it has a good fit (χ2 (431) = 2,473.378; CFI = .959; TLI = .956, RMSEA = .049; SRMR = .051). The internal consistency showed values greater than .81. Finally, the equivalence of the measurement according to sex was evaluated, finding that the instrument presents measurement invariance
Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)
Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters.
Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs).
Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001).
Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio
Omecamtiv mecarbil in chronic heart failure with reduced ejection fraction, GALACTIC‐HF: baseline characteristics and comparison with contemporary clinical trials
Aims:
The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.
Methods and Results:
Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure < 100 mmHg (n = 1127), estimated glomerular filtration rate < 30 mL/min/1.73 m2 (n = 528), and treated with sacubitril‐valsartan at baseline (n = 1594).
Conclusions:
GALACTIC‐HF enrolled a well‐treated, high‐risk population from both inpatient and outpatient settings, which will provide a definitive evaluation of the efficacy and safety of this novel therapy, as well as informing its potential future implementation

Colombian surgical outcomes study insights on perioperative mortality rate, a main indicator of the lancet commission on global surgery – a prospective cohort studyResearch in context
Summary: Background: Surgical care holds significant importance in healthcare, especially in low and middle-income countries, as at least 50% of the 4.2 million deaths within the initial 30 days following surgery take place in these countries. The Lancet Commission on Global Surgery proposed six indicators to enhance surgical care. In Colombia, studies have been made using secondary data. However, strategies to reduce perioperative mortality have not been implemented. This study aims to describe the fourth indicator, perioperative mortality rate (POMR), with primary data in Colombia. Methods: A multicentre prospective cohort study was conducted across 54 centres (hospitals) in Colombia. Each centre selected a 7-day recruitment period between 05/2022 and 01/2023. Inclusion criteria involved patients over 18 years of age undergoing surgical procedures in operating rooms. Data quality was ensured through a verification guideline and statistical analysis using mixed-effects multilevel modelling with a case mix analysis of mortality by procedure-related, patient-related, and hospital-related conditions. Findings: 3807 patients were included with a median age of 48 (IQR 32–64), 80.3% were classified as ASA I or II, and 27% of the procedures had a low-surgical complexity. Leading procedures were Orthopedics (19.2%) and Gynaecology/Obstetrics (17.7%). According to the Clavien–Dindo scale, postoperative complications were distributed in major complications (11.7%, 10.68–12.76) and any complication (31.6%, 30.09–33.07). POMR stood at 1.9% (1.48–2.37), with elective and emergency surgery mortalities at 0.7% (0.40–1.23) and 3% (2.3–3.89) respectively. Interpretation: The POMR was higher than the ratio reported in previous national studies, even when patients had a low–risk profile and low-complexity procedures. The present research represents significant public health progress with valuable insights for national decision-makers to improve the quality of surgical care. NEOTROPICAL CARNIVORES: a data set on carnivore distribution in the Neotropics
Mammalian carnivores are considered a key group in maintaining ecological health and can indicate potential ecological integrity in landscapes where they occur. Carnivores also hold high conservation value and their habitat requirements can guide management and conservation plans. The order Carnivora has 84 species from 8 families in the Neotropical region: Canidae; Felidae; Mephitidae; Mustelidae; Otariidae; Phocidae; Procyonidae; and Ursidae. Herein, we include published and unpublished data on native terrestrial Neotropical carnivores (Canidae; Felidae; Mephitidae; Mustelidae; Procyonidae; and Ursidae). NEOTROPICAL CARNIVORES is a publicly available data set that includes 99,605 data entries from 35,511 unique georeferenced coordinates. Detection/non-detection and quantitative data were obtained from 1818 to 2018 by researchers, governmental agencies, non-governmental organizations, and private consultants. Data were collected using several methods including camera trapping, museum collections, roadkill, line transect, and opportunistic records. Literature (peer-reviewed and grey literature) from Portuguese, Spanish and English were incorporated in this compilation. Most of the data set consists of detection data entries (n = 79,343; 79.7%) but also includes non-detection data (n = 20,262; 20.3%). Of those, 43.3% also include count data (n = 43,151). The information available in NEOTROPICAL CARNIVORES will contribute to macroecological, ecological, and conservation questions in multiple spatio-temporal perspectives. As carnivores play key roles in trophic interactions, a better understanding of their distribution and habitat requirements are essential to establish conservation management plans and safeguard the future ecological health of Neotropical ecosystems. Our data paper, combined with other large-scale data sets, has great potential to clarify species distribution and related ecological processes within the Neotropics. There are no copyright restrictions and no restriction for using data from this data paper, as long as the data paper is cited as the source of the information used. We also request that users inform us of how they intend to use the data
