    Complex Comorbidity of Substance Use Disorders with Anxiety Disorders: Diagnosis and Treatment

    Substance use disorders is a worldwide public health problem that commonly ocur together with our psychiatric and medical disorders. Along with etiologic origins, prognosis of anxiety disorders intercepts with substance use disorders. Due to overlapping symptoms and complaints, it is always difficult for clinicians to recognise these disorders separately. In addition, selecting the best treatment approach is challenging because of the relative risk for developing anxiety disorders in substance use patients or vice versa. In this chapter, authors are focused on adding new aspects to the clinicians for evaluating, treating and following patients with comorbid substance use disorder and anxiety disorder

    Pre-treatment of the beta3-adrenergic receptor agonist BRL37344 reduces in vivo myocardial ischemia/reperfusion injury by improving AMPK and SIRT1 activity and by suppressing mTOR and p70S6K signaling pathways

    This study aimed to investigate the role and signaling pathways of β3-AR in myocardial ischemia/reperfusion (I/R) injury, which is one of the leading causes of death worldwide. 47 male rats were randomly divided into two main groups to evaluate infarct size and molecular parameters. Rats in both groups were randomly divided into 4 groups. Control (sham), I/R (30 min ischemia/120 min reperfusion), BRL37344 (BRL) (A) (5 µg/kg single-dose pre-treatment (preT) before I/R) and BRL (B) (5 µg/kg/day preT for 10 days before I/R). Infarct size was determined with triphenyltetrazolium chloride staining and analyzed with ImageJ program. The levels of AMPK, SIRT1, mTOR, and p70SK6 responsible for cellular energy and autophagy were evaluated by western blot. Infarct size increased in the I/R group (44.84 ± 1.47%) and reduced in the single-dose and 10-day BRL-treated groups (32.22 ± 1.57%, 29.65 ± 0.55%; respectively). AMPK and SIRT1 levels were decreased by I/R but improved in the treatment groups. While mTOR and p70S6K levels increased in the I/R group, they decreased with BRL preT. BRL preT protects the heart against I/R injury. These beneficial effects are mediated in part by activation of AMPK and SIRT1, inhibition of mTOR and p70S6K, and consequently protected autophagy.

    Learning Fourier-Constrained Diffusion Bridges for MRI Reconstruction

    Recent years have witnessed a surge in deep generative models for accelerated MRI reconstruction. Diffusion priors in particular have gained traction with their superior representational fidelity and diversity. Instead of the target transformation from undersampled to fully-sampled data, common diffusion priors are trained to learn a multi-step transformation from Gaussian noise onto fully-sampled data. During inference, data-fidelity projections are injected in between reverse diffusion steps to reach a compromise solution within the span of both the diffusion prior and the imaging operator. Unfortunately, suboptimal solutions can arise as the normality assumption of the diffusion prior causes divergence between learned and target transformations. To address this limitation, here we introduce the first diffusion bridge for accelerated MRI reconstruction. The proposed Fourier-constrained diffusion bridge (FDB) leverages a generalized process to transform between undersampled and fully-sampled data via random noise addition and random frequency removal as degradation operators. Unlike common diffusion priors that use an asymptotic endpoint based on Gaussian noise, FDB captures a transformation between finite endpoints where the initial endpoint is based on moderate degradation of fully-sampled data. Demonstrations on brain MRI indicate that FDB outperforms state-of-the-art reconstruction methods including conventional diffusion priors

    THE EFFECT OF COVID-19 OUTBREAK ON THE MENTAL STATUS OF HEALTH CARE PROFESSIONALS IN THE PEDIATRIC INTENSIVE CARE UNIT

    Background: Coronavirus disease 2019 (COVID-19) may affect the mental status of health care professionals. The purpose of our study is to evaluate the mental health effects of the COVID-19 epidemic on health care professionals in the pediatric intensive care units (PICUs). Subjects and methods: Our study was conducted prospectively between 01.04.20 and 10.04.20. The created questionnaire was applied to health care professionals through online platforms. Thus it was involved in 5 different institutions that participated from different regions of Turkey. With the questionnaire, we applied; the participants\u27 age, gender, the general status of contamination and the level of COVID-19 knowledge were questioned. Besides; Beck Anxiety Scale, Acute Stress Scale (PCL-5), STAI-1 and STAI-2 (State and Trait Anxiety Inventory 1-2) scales were used to determine the anxiety levels. Results: A survey of 210 participants, 86 (41%) doctors, 124 (59%) nurses, were included in our study. When we evaluate the Beck anxiety levels, the majority of the participants (44%) were normal, while about one third had mild anxiety. When we evaluated the acute stress scale, all participants had a certain amount of stress levels. The majority (80 people each (38%)) experienced mild and moderate acute stress. Being female and having chronic disease poses a high risk for anxiety (OR, 0.330; 95% CI, 0.087-1.250, p <0.05 and OR, 0.246; 95% CI, 0.068-1.116, p<0.05), preoccupation (OR, 0.603; 95% CI 0.261-1.395, p<0.05 and OR, 0.433; 95% CI, 0.122-1.538, p<0.05) and acute stress (OR, 0.294; 95% CI, 0.033-2.649, p<0.05 and OR, 0.317; 95 % CI 0.060-1.679, p<0.05). Professional definition, marital status and having a child do not pose any risk factors. Conclusion: Our study has shown that the COVID-19 outbreak affects the mental status of health care professionals working at PICU at various levels

    Serum Calcium Levels Are Associated with Novel Cardiometabolic Risk Factors in the Population-Based CoLaus Study

    BACKGROUND: Associations of serum calcium levels with the metabolic syndrome and other novel cardio-metabolic risk factors not classically included in the metabolic syndrome, such as those involved in oxidative stress, are largely unexplored. We analyzed the association of albumin-corrected serum calcium levels with conventional and non-conventional cardio-metabolic risk factors in a general adult population. METHODOLOGY/PRINCIPAL FINDINGS: The CoLaus study is a population-based study including Caucasians from Lausanne, Switzerland. The metabolic syndrome was defined using the Adult Treatment Panel III criteria. Non-conventional cardio-metabolic risk factors considered included: fat mass, leptin, LDL particle size, apolipoprotein B, fasting insulin, adiponectin, ultrasensitive CRP, serum uric acid, homocysteine, and gamma-glutamyltransferase. We used adjusted standardized multivariable regression to compare the association of each cardio-metabolic risk factor with albumin-corrected serum calcium. We assessed associations of albumin-corrected serum calcium with the cumulative number of non-conventional cardio-metabolic risk factors. We analyzed 4,231 subjects aged 35 to 75 years. Corrected serum calcium increased with both the number of the metabolic syndrome components and the number of non-conventional cardio-metabolic risk factors, independently of the metabolic syndrome and BMI. Among conventional and non-conventional cardio-metabolic risk factors, the strongest positive associations were found for factors related to oxidative stress (uric acid, homocysteine and gamma-glutamyltransferase). Adiponectin had the strongest negative association with corrected serum calcium. CONCLUSIONS/SIGNIFICANCE: Serum calcium was associated with the metabolic syndrome and with non-conventional cardio-metabolic risk factors independently of the metabolic syndrome. Associations with uric acid, homocysteine and gamma-glutamyltransferase were the strongest. These novel findings suggest that serum calcium levels may be associated with cardiovascular risk via oxidative stress
