    Outcome selection, measurement and reporting for new surgical procedures and devices:a systematic review of IDEAL/IDEAL-D studies to inform development of a core outcome set

    BackgroundOutcome selection, measurement and reporting for the evaluation of new surgical procedures and devices is inconsistent and lacks standardization. A core outcome set may promote the safe and transparent evaluation of surgical innovations. This systematic review examined outcome selection, measurement and reporting in studies conducted within the IDEAL (Idea, Development, Exploration, Assessment and Long‐term monitoring) framework to examine current practice and inform the development of a core outcome set for early‐phase studies of surgical procedures/devices.MethodsWeb of Science and Scopus citation searches were performed to identify author‐reported IDEAL/IDEAL‐D studies for any surgical procedure/device. Outcomes were extracted verbatim, including contextual information regarding outcome selection and measurement. Outcomes were categorized to inform a conceptual framework of outcome domains relevant to evaluating innovation.ResultsSome 48 studies were identified. Outcome selection, measurement and reporting varied widely across studies in different IDEAL stages. From 1737 outcomes extracted, 22 domains specific to evaluating innovation were conceptualized under seven broad categories: procedure completion success/failure; modifications; unanticipated events; surgeons' experiences; patients' experiences; resource use specific to the innovative procedure/device; and other innovation‐specific outcomes. Most innovation‐specific outcomes were measured and reported in only a small number of studies.ConclusionThis review highlighted the need for guidance and standardization in outcome selection and reporting in the evaluation of new surgical procedures/devices. Novel outcome domains specific to innovation have been identified to establish a core outcome set for future evaluations of surgical innovations

    Capabilities of Global Ocean Programmes to Inform Climate Services

    AbstractClimate services are identified as a means of providing the information that is needed to support decision makers in assessing the impacts of climate change on the oceans. We discuss the current observation programs to support these services, and their capacity to provide the information needed to monitor and address key science questions. An analysis of the current oceanographic observation programs is shown to be undersubscribed from their original plans. There are vulnerabilities in the current observing programs, particularly in relation to satellite measurements. The interaction of climate services with the research community, with policy makers and stakeholders and operational centres is outlined and leads to four recommendations. The key recommendations are for the more pervasisve development of climate services and for a modest increment in the observing program informed by the recommendations of the OceanObs’09 conference

    Recent progress in antiretroviral therapy has greatly extended the survival of people living with human immunodeficiency virus (PLWH).1 Research found that several geriatric syndromes, such as falls, frailty, and polypharmacy, are more common among aging PLWH than among the general population.2–4 Currently, most PLWH receive all of their care from their infectious diseases (ID) providers. Therefore, they are important stakeholders in future efforts to care for older PLWH (aged ≄50 y). However, there is neither a clear consensus on the optimal care model to meet this population’s needs nor a body of evidence defining ID providers’ perspectives on the best models for incorporating geriatrics care into current practices

    Chemical dissociation of human awareness: focus on non-competitive NMDA receptor antagonists

    Since the mid-1950s the pharmaceutical industry has developed a number of chemicals, including phencyclidine, ketamine and related arylcyclohexylamines (PCE and TCP), dizocilpine (MK-801), N-allylnormetazocine [ NANM, (±)SKF-10,047], etoxadrol, dioxadrol and its enantiomers dexoxadrol and levoxadrol, which produce a constellation of unusual behavioral effects in animals and man. The compounds best studied in humans are phencyclidine and ketamine. They produce a remarkable dose-dependent dissociation of awareness. All of these substances are now known to be non-competitive antagonists of NMDA receptors of glutamic acid. They act in the NMDA receptor ion channel.     Football charity matches and tournaments played a significant part in the development of the sport in Britain, overlapping the era of friendly games and the advent of competitive leagues. The football community prided itself on its contributions to charity, raising more money than any other sport before 1914, and stakeholders within the game – associations, clubs, players and patrons – gained considerable kudos for this perceived altruism. However, this paper will demonstrate that amounts donated, though welcome, were relatively minor sources of revenue for both institutions and individuals, and that the charity match became less important to clubs in a professional, and increasingly commercial, era

    Researchers in sport have claimed that reflective practice is important for competent practice. Evidence supporting this claim is sparse, highly theoretical and located within a variety of domains. The aim of this study was to assimilate and analyse the last 12 years of reflective practice literature within the sport domain in order to identify new areas of inquiry, emerging trends with regard to findings or methodology, and to identify implications for future research and practice. A sample of 68 papers published between 2001 and 2012 was examined, and investigated for the research locations, data collection methods utilised, and the professions and communities involved. The paper concludes with some suggestions for future research

    Tensor polarization in elastic electron-deuteron scattering in the momentum transfer range 3.8≀Q≀4.6 fm-1

    The tensor polarization of the recoil deuteron in elastic electron-deuteron scattering has been measured at the Bates Linear Accelerator Center at three values of four-momentum transfer Q=3.78, 4.22, and 4.62 fm-1, corresponding to incident electron energies of 653, 755, and 853 MeV. The scattered electrons and the recoil deuterons were detected in coincidence. The recoil deuterons were transported to a liquid hydrogen target to undergo a second scattering. The angular distribution of the d→-p scattering was measured using a polarimeter. The polarimeter was calibrated in an auxiliary experiment using a polarized deuteron beam at the Laboratoire National Saturne. A Monte Carlo procedure was used to generate interpolated calibration data because the energy spread in the deuteron energies in the Bates experiment spanned the range of deuteron energies in the calibration experiment. The extracted values of t20 are compared to predictions of different theoretical models of the electromagnetic form factors of the deuteron: nonrelativistic and relativistic nucleon-meson dynamics, Skyrme model, quark models, and perturbative quantum chromodynamics. Along with the world data the structure functions A(Q) and B(Q) are used to separate the charge monopole and charge quadrupole form factors of the deuteron. A node in the charge monopole form factor is observed at Q=4.39±0.16 fm-1

    Shell evolution approaching the N=20 island of inversion : Structure of 26Na

    The levels in 26Na with single particle character have been observed for the first time using the d(25Na, pÎł) reaction at 5 MeV/nucleon. The measured excitation energies and the deduced spectroscopic factors are in good overall agreement with (0+1)hω shell model calculations performed in a complete spsdfp basis and incorporating a reduction in the N=20 gap. Notably, the 1p3/2 neutron configuration was found to play an enhanced role in the structure of the low-lying negative parity states in 26Na, compared to the isotone 28Al. Thus, the lowering of the 1p3/2 orbital relative to the 0f7/2 occurring in the neighbouring Z=10 and 12 nuclei - 25,27Ne and 27,29Mg - is seen also to occur at Z=11 and further strengthens the constraints on the modelling of the transition into the island of inversion
