Second generation governance indicators
This paper summarizes progress made in a DfID-funded World Bank initiative to test and develop policy-relevant, politically acceptable, quantitative indicators of governance. There are two major components involved in the process of generating indicators that are practical means of reform. Political acceptability is key in developing neutral quantitative benchmarks of good governance that can be embraced by reformers. In addition to political acceptability, measuring governance must be comprehensive and institutionally specific so that reformers know which institutions to reform and how to do so. Are steady magnetospheric convection events prolonged substorms?
Magnetospheric modes, including substorms, sawtooth events, and steady magnetospheric convection events, have in the past been described as different responses of the magnetosphere to coupling with the solar wind. Using previously determined event lists for sawtooth events, steady magnetospheric convection events, and substorms, we produce a statistical study of these event types to examine their similarities and behavior in terms of solar wind parameters, auroral brightness, open magnetospheric flux, and geomagnetic indices. A superposed epoch analysis shows that individual sawteeth show the same signatures as substorms but occur during more extreme cases of solar wind driving as well as geomagnetic activity. We also explore the limitations of current methods of identifying steady magnetospheric convection events and explain why some of those events are flagged inappropriately. We show that 58% of the steady magnetospheric convection events considered, as identified by criteria defined in previous studies are part of a prolonged version of substorms due to continued dayside driving during expansion phase. The remaining 42% are episodes of enhanced magnetospheric convection, occurring after extended periods of dayside driving
Characteristics of cancer patients using homeopathy compared with those in conventional care: a cross-sectional study
Background: There are only few studies on cancer patients who are treated in complementary and alternative medicine clinics and comparing them with patients in conventional care. We will present the comparison of characteristics of two patient cohorts: one was treated in a homeopathic cancer care clinic and one was treated in a conventional oncology care (CC) outpatient clinic. Patients and methods: Six-hundred and forty-seven patients were included in this cross-sectional cohort study and had to fill in questionnaires [health-related quality of life (QoL) (Functional Assessment of Cancer Therapy—General Scale), depression and anxiety (Hospital Anxiety and Depression Scale), fatigue (Multidimensional Fatigue Inventory) and expectancies toward treatment]. Clinical data were extracted from medical records. This study presents the comparison of both cohorts. Results: Patients in the homeopathy cohort are younger, better educated and more often employed than patients in the CC cohort. The most pronounced differences indicate longer disease histories and different diagnostic and clinical pretreatment variables. Despite the clinical differences, QoL as well as anxiety, depression and fatigue was similar in both the groups. Conclusions: Homeopathic treatment is sought by cancer patients at a different phase during the course of the disease, which has particular implications for research. Testing nowcasts of the ionospheric convection from the expanding and contracting polar cap model
The expanding/contracting polar cap (ECPC) model, or the time-dependent Dungey cycle, provides a theoretical framework for understanding solar wind-magnetosphere-ionosphere coupling. The ECPC describes the relationship between magnetopause reconnection and substorm growth phase, magnetotail reconnection and substorm expansion phase, associated changes in auroral morphology, and ionospheric convective motions. Despite the many successes of the model, there has yet to be a rigorous test of the predictions or nowcasts made regarding ionospheric convection, which remains a final hurdle for the validation of the ECPC. In this study we undertake a comparison of ionospheric convection, as measured in situ by ion drift meters on board DMSP (Defense Meteorological Satellite Program) satellites and from the ground by SuperDARN (Super Dual Auroral Radar Network), with motions nowcasted by a theoretical model. The model is coupled to measurements of changes in the size of the polar cap made using global auroral imagery from the IMAGE FUV (Imager for Magnetopause to Aurora Global Exploration Far Ultraviolet) instrument, as well as the dayside reconnection rate, estimated using the OMNI data set. The results show that we can largely nowcast the magnitudes of ionospheric convection flows using the context of our understanding of magnetic reconnection at the magnetopause and in the magnetotail
Subitizing with Variational Autoencoders
Numerosity, the number of objects in a set, is a basic property of a given
visual scene. Many animals develop the perceptual ability to subitize: the
near-instantaneous identification of the numerosity in small sets of visual
items. In computer vision, it has been shown that numerosity emerges as a
statistical property in neural networks during unsupervised learning from
simple synthetic images. In this work, we focus on more complex natural images
using unsupervised hierarchical neural networks. Specifically, we show that
variational autoencoders are able to spontaneously perform subitizing after
training without supervision on a large amount images from the Salient Object
Subitizing dataset. While our method is unable to outperform supervised
convolutional networks for subitizing, we observe that the networks learn to
encode numerosity as basic visual property. Moreover, we find that the learned
representations are likely invariant to object area; an observation in
alignment with studies on biological neural networks in cognitive neuroscience

Miscommunication in Doctor-Patient Communication
The effectiveness of medical treatment depends on the quality of the patient–clinician relationship. It has been proposed that this depends on the extent to which the patient and clinician build a shared understanding of illness and treatment. Here, we use the tools of conversation analysis (CA) to explore this idea in the context of psychiatric consultations. The CA “repair” framework provides an analysis of the processes people use to deal with problems in speaking, hearing, and understanding. These problems are especially critical in the treatment of psychosis where patients and health care professionals need to communicate about the disputed meaning of hallucinations and delusion. Patients do not feel understood, they are frequently non‐adherent with treatment, and many have poor outcomes. We present an overview of two studies focusing on the role of repair as a mechanism for producing and clarifying meaning in psychiatrist–patient communication and its association with treatment outcomes. The first study shows patient clarification or repair of psychiatrists’ talk is associated with better patient adherence to treatment. The second study shows that training which emphasizes the importance of building an understanding of patients’ psychotic experiences increases psychiatrists’ self‐repair. We propose that psychiatrists are working harder to make their talk understandable and acceptable to the patient by taking the patient's perspective into account. We conclude that these findings provide evidence that repair is an important mechanism for building shared understanding in doctor–patient communication and contributes to better therapeutic relationships and treatment adherence. The conversation analytic account of repair is currently the most sophisticated empirical model for analyzing how people construct shared meaning and understanding. Repair appears to reflect greater commitment to and engagement in communication and improve both the quality and outcomes of communication. Reducing potential miscommunication between psychiatrists and their patients with psychosis is a low‐cost means of enhancing treatment from both the psychiatrist and patient perspective. Given that misunderstanding and miscommunication are particularly problematic in psychosis, this is critical for improving the longer term outcomes of treatment for these patients who often have poor relationships with psychiatrists and health care services more widely
