    Primary de novo intraosseous carcinoma : report of a new case

    Primary de novo intraosseous carcinoma of the jaws has been rarely reported. We present a new case of this unusual tumour and discuss its histopathological and clinical aspects. The subject was a 76-year-old man who was seen due to complaints of pain and the presence of gingival changes in the left mandible. A panoramic radiograph and computed tomography revealed a large mandibular radiolucency. A segmentary mandibulectomy was performed and histopathologic examination proved that the tumour was an intraosseous squamous cell carcinoma.     Oroantral communications : a retrospective analysis

    Objetive. To analyze characteristics, clinical evolution and surgical techniques of oroantral communication (OAC). Study Design. We included all patients operated at the University Central Hospital (Oviedo, Spain) between 1996 and 2007. The variables assessed were age, sex, medical history, OAC size, sinus disease, surgical technique, duration of hospitalization and post-surgical evolution. Results. We analyzed 12 patients (7 men and 5 women) with an average age of 47.5 years. The most frequent cause of oroantral communication was the extraction of the first upper molar. The average size of fistula was 0.9 cm. Buccal flap repair was used in 7 patients, palatal rotation-advancement flap in 4 patients and buccal fat pad in only one patient. Suture dehiscence was observed in one patient treated with a palatal flap, but no additional surgery was required. Three OAC recurred; all of them following a buccal flap procedure. All recurrences spontaneously closed between one and four months following the procedure. Conclusion. OACs are rare complications and treatment should be individualized to avoid further complication

    Use of cultured human epithelium for coverage : a defect of radial forearm free flap donor site

    The radial forearm free flap has been popular in many areas of reconstructive surgery. Despite the many attributes of this flap in maxillofacial reconstruction, one of the disadvantages has been the morbidity of the donor site. Allogeneic cultured epidermis has been successfully applied on large second degree burns and on chronic leg ulcers. Autologous human keratinocytes and fibroblast equivalents can be cultured in-vitro from a small skin sample in order to produce a sufficient amount of epithelial autografts to cover the large defects of third-degree burn wounds. Interestingly, transplanted cultured epidermis retains characteristics of the original donor site. We report a case of a patient who underwent skin replacement by cultured epithelial autograft after wound breakdown occurred in the forearm donor site during the early postoperative period. This method could represent an auspicious alternative to conventional grafting methods for forearm free flap reconstruction. To the best of our knowledge, skin replacement by cultured epithelial autografts in this region has not been extensively described in the literature

    Spontaneous fracture of the mandibular genial tubercles : a case report

    Fracture of the mandibular genial tubercles is an uncommon pathology affecting edentulous patients with severe maxillary atrophy. Usually occurs spontaneously which complicates the diagnosis. Their importance lies in the functional alterations, which occur as a consequence of the disinsertion of the genihyoid and genioglossus muscles. The treatment of fracture of the genial tubercles is controversial, including no surgical intervention, excision of the avulsed bone fragments, and muscular repositioning. There have been only 11 cases reported in the literature of this fracture, most of them spontaneous. We present a difficult diagnosis situation of spontaneous fracture of the genial tubercles in an 86-year-old edentulous female with a painful sublingual and submental hematoma and anterior cervical echimosis. Computerized Tomography should be made to confirm the diagnosis. Surgical treatment was not necessary, and follow-up at 6 months revealed complete symptomatic recovery, and full return of function

    DESIGN AND INTEGRATION OF A HUMAN-ROBOT PHYSICAL INTERACTION PLATFORM WITH PURPOSES OF MEDICAL DIAGNOSIS AND REHABILITATION OF UPPER LIMB

    In this paper a human-robot physical interaction system with purposes of diagnosis and rehabilitation of upper limb is proposed.Anunderactuatedhaptic device with six degrees of freedom is used, with low inertia and low joint friction.Adaptive control technique is used for passive haptic guidance and active exploration, in order to compensate the dynamic uncertainty of the human operator in the loop.To validate the experimental platform, a procedure is established with three steps: i) knowledge of the haptic interface (interaction with the kinematic virtual environment), ii) navigation in a virtual pipe with changes in the geometric characteristics (verification of position, velocity, collisions and runtime), and iii) haptic guidance in a structured path based on a clinical protocol (study of convergence and energy).Environmental conditions such as temperature, humidity, lighting and noise are characterizedwith purposes to define experimental conditions.In this work, we assess based on the NASA-TLX protocol,the workload perception of simple temporal-spatial tasks

    Isla Grande (Archipiélago de Nuestra Señora del Rosario, Colombia) tiene varias par ticularidades que justifican su investigación. Destaca su riqueza ambiental y de arre cifes coralinos, por lo que ha sido considerado como uno de los parques naturales nacionales más importantes en el Caribe colombiano. A esto se une que sus habitan tes son afrodescendientes y que la propiedad del territorio es de carácter colectivo. Ello conlleva un particular proceso de organización local autónomo para establecer un modelo de gestión territorial y de desarrollo local. El turismo se ha convertido en una oportunidad, pero también constituye una ame naza para el medio natural. De esta manera, se ha querido planificar y gestionar esta actividad sobre la base del ecoturismo y el etnomarketing. En esta investigación hay un compromiso de la Universidad de Cartagena de Indias con la sostenibilidad ambiental de este singular territorio. Se ha trabajado con en cuestas y entrevistas a nativos, turistas y empresarios. También, ha sido importante la observación directa y la realización de talleres participativos para conocer y diag nosticar la actividad turística. Ello ha permitido, entre otras investigaciones, formular un plan de marketing en el que se promocionen y se den a conocer las potencialida des ecoturísticas de la isla, los saberes ancestrales de los nativos, entre otros aspec tos, y contribuir, de la mano de los actores locales, a su sostenibilidad ambiental.Isla Grande (Archipelago of Nuestra Señora del Rosario, Colombia) has several pecu liarities that justify its investigation. Its environmental wealth and coral, reefs stand out, which is why it has been considered one of the most important national natural parks in the Colombian Caribbean. To this is added that its inhabitants are Afro-de scendants and that the ownership of the territory is collective. This entails a particu lar autonomous local organization process to establish a territorial management and local development model. Tourism has become an opportunity, but it also constitutes a threat to the natural environment. In this way, it has been wanted to plan and manage this activity on the basis of ecotourism and ethnomarketing. In this research there is a commitment from the University of Cartagena de Indias with the environmental sustainability of this unique territory. It has worked with sur veys and interviews with natives, tourists and businessmen. Direct observation and participatory workshops have also been important to understand and diagnose tourist activity. This has allowed, among other investigations, to formulate a market ing plan in which the island’s ecotourism potential, the ancestral knowledge of the natives, among other aspects, is promoted and made known, and to contribute hand in hand with the actors premises with their environmental sustainability
