    Cardiovascular risk rate in hypertensive patients attended in primary health care units: the influence of pharmaceutical care

    Cardiovascular complications are relevant due to their frequency and severity on the hypertension scenario. Studies refer Pharmaceutical Care (PC) as capable of decreasing cardiovascular risk rate (%CVR) on hypertensive patients. This study aimed to investigate, through a randomized clinical assay, the influence of PC service on the %CVR of hypertensive patients assisted in a health primary care unit from Fortaleza-Ceará. Two study groups were formed: i. Intervention Group (IG), which received orientation about taking medicines, actions aiming to prevent/solve medicine interactions and adverse effects and non-pharmacological interventions for 9 months and, ii. Control Group (CG), which received traditional assistance of the unit and was monitored during the same period. It was observed a statistically significant reduction on %CVR (10.76 to 7.86; p=0.04) and systolic blood pressure levels (SBP) (137.69 to 131.54; pAs complicações cardiovasculares apresentam relevância devido à sua freqüência e gravidade no contexto da hipertensão. Estudos referem que a prestação do Cuidado Farmacêutico (CF) é capaz de reduzir a taxa de risco cardiovascular (%RCV) em hipertensos. Esse trabalho objetivou investigar, com um ensaio clínico randomizado, a influência da prestação do CF na %RCV em hipertensos atendidos em uma unidade de atenção primária à saúde de Fortaleza-Ceará. Formarm-se dois grupos de estudo: i. Grupo Intervenção (GI), que recebeu orientações sobre tomada dos medicamentos, ações visando prevenir/resolver interações medicamentosas e reações adversas e intervenções não-farmacológicas por 9 meses e ii.     Prevalência de fragilidade e fatores associados em idosos comunitários de Belo Horizonte, Minas Gerais, Brasil: dados do estudo FIBRA

    A fragilidade apresenta elevada variabilidade na prevalência e é pouco investigada em países em desenvolvimento. O objetivo deste estudo foi identificar a prevalência e fatores associados à fragilidade em idosos comunitários de Belo Horizonte, Minas Gerais, Brasil. Aspectos clínicos, funcionais e sociodemográficos foram avaliados. Foi utilizado o modelo de regressão ordinal, para verificar condições associadas à fragilidade e determinar a razão de chance (α = 0,05). A prevalência de pré-fragilidade foi 46,3% e de fragilidade de 8,7%. Os idosos pré-frágeis e frágeis apresentaram, respectivamente, maiores e crescentes razão de chances para dependência em atividades instrumentais de vida diária; restrição em atividades avançadas de vida diária; utilização de dispositivos auxiliares da marcha; comorbidades; quedas; sintomas depressivos; menor autoeficácia para prevenir quedas; hospitalização e idade avançada. Foi identificado elevado percentual de estados de fragilidade associados a maiores chances para condições adversas de saúde, especialmente as relacionadas à incapacidade

    O tropicalismo arranca em 1967, através do corpo: a música de Caetano Veloso e Gilberto Gil, os vestíveis de Hélio Oiticica, as propostas teatrais de José Celso Martinez Corrêa e os cenários de Hélio Eichbauer. Hoje as coisas são um pouco mais complexas. Em tempo de redes sociais, os aspirantes ao poder fazem uso da sua imediatez para suscitarem reações epidérmicas, superficiais, populistas e de grande instantaneidade. A boçalidade triunfa nas caixas de comentários, e com mais alguns perfis falsificados podem manipular-se plebiscitos, movimentos secessionistas, ou, e também censurar-se exposições de arte. Nesta variação do fascismo, a epiderme eletrificada das redes sociais estrutura-se como uma poderosa arena onde se aparenta uma falsa democracia. Talvez a arte continue a ser um reduto para reflexão, mas vemos que a censura se manifesta hoje de modo talvez mais eficaz, silenciando artistas e professores, através da pressão mediatizada, da emoção do momento.     Como é característica fundadora do projeto CSO – criadores sobre outras obras – os artistas são aqui convidados a comunicar o seu conhecimento sobre outros artistas, dentro de um descentramento que toma os idiomas ibéricos como uma plataforma territorial de emergência, e de resistência, também. É uma área cultural periférica, alternativa, que se afirma em crescente grandeza. Dentro deste dispositivo comunicativo, a revista Croma delimita mais ainda o seu tema, nos artistas que de algum modo fazem incorporar a implicação social, a interação e a criação e formação de novos públicos como um dos componentes estruturantes da sua obra. Surgem na Croma obras intervenientes, que provocam, e que convocam, que estabelecem pontes, ou que as ameaçam. São obras em que a componente relacional (N. Bourriaud) ou formativa e integradora (P. Freire) assumem uma atualidade constante e renovada.     Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.     Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     ATLANTIC EPIPHYTES: a data set of vascular and non-vascular epiphyte plants and lichens from the Atlantic Forest

    Epiphytes are hyper-diverse and one of the frequently undervalued life forms in plant surveys and biodiversity inventories. Epiphytes of the Atlantic Forest, one of the most endangered ecosystems in the world, have high endemism and radiated recently in the Pliocene. We aimed to (1) compile an extensive Atlantic Forest data set on vascular, non-vascular plants (including hemiepiphytes), and lichen epiphyte species occurrence and abundance; (2) describe the epiphyte distribution in the Atlantic Forest, in order to indicate future sampling efforts. Our work presents the first epiphyte data set with information on abundance and occurrence of epiphyte phorophyte species. All data compiled here come from three main sources provided by the authors: published sources (comprising peer-reviewed articles, books, and theses), unpublished data, and herbarium data. We compiled a data set composed of 2,095 species, from 89,270 holo/hemiepiphyte records, in the Atlantic Forest of Brazil, Argentina, Paraguay, and Uruguay, recorded from 1824 to early 2018. Most of the records were from qualitative data (occurrence only, 88%), well distributed throughout the Atlantic Forest. For quantitative records, the most common sampling method was individual trees (71%), followed by plot sampling (19%), and transect sampling (10%). Angiosperms (81%) were the most frequently registered group, and Bromeliaceae and Orchidaceae were the families with the greatest number of records (27,272 and 21,945, respectively). Ferns and Lycophytes presented fewer records than Angiosperms, and Polypodiaceae were the most recorded family, and more concentrated in the Southern and Southeastern regions. Data on non-vascular plants and lichens were scarce, with a few disjunct records concentrated in the Northeastern region of the Atlantic Forest. For all non-vascular plant records, Lejeuneaceae, a family of liverworts, was the most recorded family. We hope that our effort to organize scattered epiphyte data help advance the knowledge of epiphyte ecology, as well as our understanding of macroecological and biogeographical patterns in the Atlantic Forest.
