    Foveal Densifometry in Retinitis Pigmentosa

    Cone density and psychophysical thresholds were investigated in patients with retinitis pigmentosa. Our aim was to assess possible disturbances of foveal cones, especially in patients with good visual acuity. Using the continuously recording densitometer described by van Norren and van der Kraats, we examined ten patients (19 eyes). With the same apparatus it was possible to determine cone final threshold. In all patients double density values were lower than in an age-matched and sex-matched control group of an equal number, and the half time of regeneration was increased in eight eyes. In 12 eyes of eight patients the foveal final threshold was raised above normal. Pseudoprotanomaly was found in seven eyes in four males. Possible explanations for these findings are discussed. Since reduced double density was found in all patients with retinitis pigmentosa, we conclude that disturbed foveal cones are probably a common and rather early feature in this disease. Typical observations in the disease are the characteristic fundus picture of bone-spicule pigmentation, a lowered or undetectable scotopic b-wave on electrophysiologic examination, an annular scotoma on perimetric examination, and a raised final threshold on examination with the Goldmann-Weekers adaptometer. The disease affects primarily the receptor layer, particularly the cells of the midperipheral retina; in a later stage the cones in the fovea may also become involved. It is well known that the central field of vision may be retained for a long time and that the visual acuity can remain relativily unimpaired, even in the far advanced stages of the disease. When the technique of retinal densitometry came into use, it was to be expected that attention would be especially focused on measuring the properties of the rods, and several investigators demonstrated that a reduction in the density of rhodopsin correlated well with the loss of visual sensitivity. 1 -2 Perlman and Auerbach 3 distinguished two groups of patients: one group with a recessive inheritance, in which the relation between the log threshold elevation and the fraction of rhodopsin was probably linear, and another group with principally dominant inheritance in which absolute threshold elevation was linearly related to the fraction of rhodopsin present. It was our aim to investigate more thoroughly whether densitometric examination could demonstrate an impaired function in foveal cones in patients with retinitis pigmentosa. For this purpose we used the retinal densitometer described by van Norren and van der Kraats, 4 an instrument that enables reliable and relatively easy measurements in patients. To assess the functional state of the fovea, we measured psychophysical cone dark adaptation to all patients with the same apparatus. Our findings show that in all patients tested by densitometry, the double density values were significantly less than normal, and that in several of these subjects the half-time of pigment regeneration was slower than in the normal. Materials and Methods Subjects For the patient group only those patients were selected who had a confirmed retinitis pigmentosa, established by the following criteria.     Chemotherapy-induced fatigue is a multidimensional symptom. Oxidative stress has been proposed as a working mechanism for anthracycline-induced cardiotoxicity. In this study, doxorubicin (DOX) was tested on skeletal muscle function. Doxorubicin induced impaired ex vivo skeletal muscle relaxation followed in time by contraction impediment, which could be explained by DOX-induced changes in Ca2+ responses of myotubes in vitro. The Ca2+ responses in skeletal muscle, however, could not be explained by oxidative stress

    Near-infrared reflectance imaging of neovascular age-related macular degeneration

    Contains fulltext : 81007.pdf (publisher's version ) (Closed access)PURPOSE: To evaluate various types of neovascular age-related macular degeneration (AMD) by near-infrared fundus reflectance (NIR) as compared to fundus fluorescein angiography (FFA) and to test NIR for assessment of leakage due to choroidal neovascularization (CNV). PATIENTS AND METHODS: Thirty-three patients with neovascular AMD (cases) and 20 age-matched patients with non-exudative AMD and healthy subjects (controls) were examined with a confocal scanning laser ophthalmoscope (Heidelberg Retina Angiograph 2). NIR images of neovascular AMD were qualitatively compared to the corresponding FFA and to age-matched controls. CNV membranes and exudation areas were manually segmented on FFA and NIR and analyzed quantitatively. Results : Of all cases included, five eyes had classic CNV, six had minimal classic lesions, 15 occult CNV's and seven eyes had retinal angiomatous proliferation (RAP). A dark halo on NIR was found in all cases and showed high correspondence to leakage on FFA (r (2) = 0.93; p < 0,0005). In classic CNV and minimal classic CNV, the classic part of the lesion on FFA revealed strong correlation to a dark core surrounded by a bright reflecting ring on NIR (r (2) = 0.88; p < 0.0005). Occult parts on FFA of minimal classic CNV and occult CNV lesions appeared as poorly demarcated, jagged areas of increased NIR. RAP was characterized by speckled NIR located at the intraretinal neovascular complex. CONCLUSIONS: NIR imaging in neovascular AMD revealed characteristic alterations depending on the type of CNV. These changes may reflect histological differences of the lesions. Leakage caused local darkening of NIR, presumably originating from increased light-scattering and absorbance by fluid accumulation and sub-cellular structure alterations

    Beneficial immune modulatory effects of a specific nutritional combination in a murine model for cancer cachexia

    The majority of patients with advanced cancer are recognised by impaired immune competence influenced by several factors, including the type and stage of the tumour and the presence of cachexia. Recently, a specific nutritional combination containing fish oil, specific oligosaccharide mixture, high protein content and leucine has been developed aimed to support the immune system of cancer patients in order to reduce the frequency and severity of (infectious) complications. In a recently modified animal model cachexia is induced by inoculation of C26 tumour cells in mice. In a pre-cachectic state, no effect was observed on contact hypersensitivity, a validated in vivo method to measure Th1-mediated immune function, after adding the individual nutritional ingredients to the diet of tumour-bearing mice. However, the complete mixture resulted in significantly improved Th1 immunity. Moreover, in a cachectic state, the complete mixture reduced plasma levels of pro-inflammatory cytokines and beneficially affected ex vivo immune function. Accordingly, the combination of the nutritional ingredients is required to obtain a synergistic effect, leading to a reduced inflammatory state and improved immune competence. From this, it can be concluded that the specific nutritional combination has potential as immune-supporting nutritional intervention to reduce the risk of (infectious) complications in cancer patients

    Comparison of Blue Light-Filtering IOLs and UV Light-Filtering IOLs for Cataract Surgery: A Meta-Analysis

    Background: A number of published randomized controlled trials have been conducted to evaluate visual performance of blue light-filtering intraocular lenses (IOL) and UV light-filtering intraocular lenses (IOL) after cataract phacoemulsification surgery. However, results have not always been consistent. Therefore, we carried out a meta-analysis to compare the effectiveness of blue light-filtering IOLs versus UV light-filtering IOLs in cataract surgery. Methods and Findings: Comprehensive searches of PubMed, Embase, Cochrane Library and the Chinese BioMedical literature databases were performed using web-based search engines. Fifteen trials (1690 eyes) were included for systematic review, and 11 of 15 studies were included in this meta-analysis. The results showed that there were no significant differences in postoperative mean best corrected visual acuity, contrast sensitivity, overall color vision, or in the blue light spectrum under photopic light conditions between blue light-filtering IOLs and UV light-filtering IOLs [WMD = 20.01, 95%CI (20.03, 0.01), P = 0.46; WMD = 0.07, 95%CI (20.04, 0.19), P = 0.20; SMD = 0.14, 95%CI (20.33, 0.60), P = 0.566; SMD = 0.20, 95%CI (20.04, 0.43), P = 0.099]. However, color vision with blue light-filtering IOLs was significantly reduced in the blue light spectrum under mesopic light conditions [SMD = 0.74, 95%CI (0.29, 1.18), P = 0.001]. Conclusion: This meta-analysis demonstrates that postoperative visual performance with blue light-filtering IOLs is approximately equal to that of UV light-filtering IOLs after cataract surgery, but color vision with blue light-filtering IOL

    Human S-cone electroretinograms obtained by silent substitution stimulation

    YesWe used triple silent substitution stimuli to characterize human S-cone electroretinograms (ERGs) in normal trichromats. Short-wavelength-cone (S-cone) ERGs were found to have different morphological features and temporal frequency response characteristics compared to ERGs derived from L-cones, M-cones, and rod photoreceptors in normal participants. Furthermore, in two cases of retinal pathology, blue cone monochromatism (BCM) and enhanced S-cone syndrome (ESCS), S-cone ERGs elicited by our stimuli were preserved and enhanced, respectively.
