Sustitución ureteral bilateral con asa ileal

This series describes the technique of bilateral ureteral replacement with ileum. Aim: To report 4 cases of bilateral ileal ureteral replacement performed at our institution, its indications, the surgical technique, complications, and a review of the literature. Material and Method: We report 4 cases of extensive bilateral ureteral injury of different etiologies, whose treatment with curative intent was to replace the damaged ureter with a isoperistaltic small bowel segment. Results: The surgical technique used was standard in all cases. There were no intraoperative complications and only one patient had hyperchloremic acidosis. No patient has shown loss of renal function in the long-term follow up. Conclusion: Is an effective therapeutic, safe and reproducible technique to replacement of major bilateral ureteral injuries, independent of the original cause.La sustitución ureteral por un asa de íleon desfuncionalizada fue descrita hace más de 2 siglos y continúa siendo una alternativa terapéutica en la actualidad. Esta serie describe la técnica de sustitución ureteral bilateral con íleon. Objetivo: Presentar 4 casos de sustitución ureteral ileal bilateral realizados en nuestro centro, indicaciones de la técnica quirúrgica, complicaciones y revisión de la literatura. Material y Método: Presentamos 4 casos de lesión ureteral bilateral extensa, de etiologías diferentes, cuyo tratamiento con intención curativa fue la sustitución de la lesión ureteral por un segmento intestinal isoperis-táltico. Resultados: La técnica quirúrgica empleada fue estándar en todos los casos. No hubo complicaciones intraoperatorias, y sólo una paciente presentó descompensación metabólica por acidosis hiperclorémica. En ningún paciente se ha demostrado pérdida en la función renal. Conclusión: El segmento intestinal de íleon desfuncionalizado es una alternativa terapéutica eficaz, segura y reproducible, para el reemplazo de importantes lesiones ureterales bilaterales, independiente de la causa original.http://www.scielo.cl/scielo.php?script=sci_arttext&pid=S0718-40262012000500012&nrm=is
Resultados del tratamiento quirúrgico del dolor lumbar con instrumentación diapasón
Se presenta un estudio sobre 23 pacientes con patología lumbosacra tratados
quirúrgicamente mediante artrodesis e instrumentación vertebral transpedicular tipo diapasón.
El promedio de seguimiento ha sido de 25 meses (18-38). El dolor lumbar estaba relacionado en
10 casos con estenosis de canal, en 6 con espondilolistesis y en 7 con hernia discal y artrosis secundaria.
Todos fueron instrumentados por vía posterior mediante fijación transpedicular y artrodesados
posterolateralmente con injerto autólogo. Se consiguió una artrodesis sólida en un
96%. No se han presentado complicaciones preoperatorias. Ocho pacientes presentaron complicaciones
en el postoperatorio inmediato. Observamos un desplazamiento de barra y un desanclaje
del tornillo fuera del pedículo. Siguiendo los criterios de valoración de Henderson se ha conseguido
un 96% de excelentes y buenos resultados.A series of 23 patient with lumbosacral pathology, surgically treated by posterior
arthrodesis and vertebral diapason transpedicular fixation was retrospectively review. The average
follow-up period was 25 months (18-38). The diagnosis was spinal stenosis in 10 cases, spondylolisthesis
in 6, and disc herniation with secondary osteoartrhitis in 7 cases. Apart from transpedicular
fixation all cases underwent posterolateral fusion with autogenous bone graft. A solid arthrodesis
was achieved in a 96% of patients. There were no per-operative complication. Eight cases showed
complications in the immediate postoperative period. We observed displacement of a bar and
detachement of a screw out off the pedicle. According to Henderson's assessment criteria we obtained
96% of excellent and good results
Neuronal and astrocytic tetraploidy is increased in drug-resistant epilepsy
Aims: Epilepsy is one of the most prevalent neurological diseases. A third of patients with epilepsy remain drug-resistant. The exact aetiology of drug-resistant epilepsy (DRE) is still unknown. Neuronal tetraploidy has been associated with neuropathology. The aim of this study was to assess the presence of tetraploid neurons and astrocytes in DRE. Methods. For that purpose, cortex, hippocampus and amygdala samples were obtained from patients subjected to surgical resection of the epileptogenic zone. Post-mortem brain tissue of subjects without previous records of neurological, neurodegenerative or psychiatric diseases was used as control. Results: The percentage of tetraploid cells was measured by immunostaining of neurons (NeuN) or astrocytes (S100β) followed by flow cytometry analysis. The results were confirmed by image cytometry (ImageStream X Amnis System Cytometer) and with an alternative astrocyte biomarker (NDRG2). Statistical comparison was performed using univariate tests. A total of 22 patients and 10 controls were included. Tetraploid neurons and astrocytes were found both in healthy individuals and DRE patients in the three brain areas analysed: cortex, hippocampus and amygdala. DRE patients presented a higher number of tetraploid neurons (p = 0.020) and astrocytes (p = 0.002) in the hippocampus than controls. These results were validated by image cytometry. Conclusions: We demonstrated the presence of both tetraploid neurons and astrocytes in healthy subjects as well as increased levels of both cell populations in DRE patients. Herein, we describe for the first time the presence of tetraploid astrocytes in healthy subjects. Furthermore, these results provide new insights into epilepsy, opening new avenues for future treatment

Anisotropy and chemical composition of ultra-high energy cosmic rays using arrival directions measured by the Pierre Auger Observatory
The Pierre Auger Collaboration has reported evidence for anisotropy in the
distribution of arrival directions of the cosmic rays with energies
eV. These show a correlation with the distribution
of nearby extragalactic objects, including an apparent excess around the
direction of Centaurus A. If the particles responsible for these excesses at
are heavy nuclei with charge , the proton component of the
sources should lead to excesses in the same regions at energies . We here
report the lack of anisotropies in these directions at energies above
(for illustrative values of ). If the anisotropies
above are due to nuclei with charge , and under reasonable
assumptions about the acceleration process, these observations imply stringent
constraints on the allowed proton fraction at the lower energies
Highlights from the Pierre Auger Observatory
The Pierre Auger Observatory is the world's largest cosmic ray observatory.
Our current exposure reaches nearly 40,000 km str and provides us with an
unprecedented quality data set. The performance and stability of the detectors
and their enhancements are described. Data analyses have led to a number of
major breakthroughs. Among these we discuss the energy spectrum and the
searches for large-scale anisotropies. We present analyses of our X
data and show how it can be interpreted in terms of mass composition. We also
describe some new analyses that extract mass sensitive parameters from the 100%
duty cycle SD data. A coherent interpretation of all these recent results opens
new directions. The consequences regarding the cosmic ray composition and the

A search for point sources of EeV photons
Measurements of air showers made using the hybrid technique developed with
the fluorescence and surface detectors of the Pierre Auger Observatory allow a
sensitive search for point sources of EeV photons anywhere in the exposed sky.
A multivariate analysis reduces the background of hadronic cosmic rays. The
search is sensitive to a declination band from -85{\deg} to +20{\deg}, in an
energy range from 10^17.3 eV to 10^18.5 eV. No photon point source has been
detected. An upper limit on the photon flux has been derived for every
direction. The mean value of the energy flux limit that results from this,
assuming a photon spectral index of -2, is 0.06 eV cm^-2 s^-1, and no celestial
direction exceeds 0.25 eV cm^-2 s^-1. These upper limits constrain scenarios in
which EeV cosmic ray protons are emitted by non-transient sources in the
