    Risk, precaution and science: towards a more constructive policy debate. Talking point on the precautionary principle

    Few issues in contemporary risk policy are as momentous or contentious as the precautionary principle. Since it first emerged in German environmental policy, it has been championed by environmentalists and consumer protection groups, and resisted by the industries they oppose (Raffensperger & Tickner, 1999). Various versions of the principle now proliferate across different national and international jurisdictions and policy areas (Fisher, 2002). From a guiding theme in European Commission (EC) environmental policy, it has become a general principle of EC law (CEC, 2000; Vos & Wendler, 2006). Its influence has extended from the regulation of environmental, technological and health risks to the wider governance of science, innovation and trade (O'Riordan & Cameron, 1994)

    Milligram mass metrology using an electrostatic force balance

    Although mass is typically defined within the International System of Units (SI) at the kilogram level, the pending SI redefinition provides an opportunity to realize mass at any scale using electrical metrology. We propose the use of an electromechanical balance to realize mass at the milligram level using SI electrical units. An integrated concentric-cylinder vacuum gap capacitor allows us to leverage the highly precise references available for capacitance, voltage and length to generate an electrostatic reference force. Weighing experiments performed on 1 mg and 20 mg artifacts show the same or lower uncertainty than similar experiments performed by subdividing the kilogram. The measurement is currently limited by the stability of the materials that compose the mass artifacts and the changes in adsorbed layers on the artifact surfaces as they are transferred from vacuum to air

    The histone chaperones Vps75 and Nap1 form ring-like, tetrameric structures in solution

    NAP-1 fold histone chaperones play an important role in escorting histones to and from sites of nucleosome assembly and disassembly. The two NAP-1 fold histone chaperones in budding yeast, Vps75 and Nap1, have previously been crystalized in a characteristic homodimeric conformation. In this study, a combination of small angle X-ray scattering, multi angle light scattering and pulsed electron–electron double resonance approaches were used to show that both Vps75 and Nap1 adopt ring-shaped tetrameric conformations in solution. This suggests that the formation of homotetramers is a common feature of NAP-1 fold histone chaperones. The tetramerisation of NAP-1 fold histone chaperones may act to shield acidic surfaces in the absence of histone cargo thus providing a ‘self-chaperoning’ type mechanism

    Predicting asthma-related crisis events using routine electronic healthcare data

    Background There is no published algorithm predicting asthma crisis events (accident and emergency [A&E] attendance, hospitalisation, or death) using routinely available electronic health record (EHR) data. Aim To develop an algorithm to identify individuals at high risk of an asthma crisis event. Design and setting Database analysis from primary care EHRs of people with asthma across England and Scotland. Method Multivariable logistic regression was applied to a dataset of 61 861 people with asthma from England and Scotland using the Clinical Practice Research Datalink. External validation was performed using the Secure Anonymised Information Linkage Databank of 174 240 patients from Wales. Outcomes were ≄1 hospitalisation (development dataset) and asthma-related hospitalisation, A&E attendance, or death (validation dataset) within a 12-month period. Results Risk factors for asthma-related crisis events included previous hospitalisation, older age, underweight, smoking, and blood eosinophilia. The prediction algorithm had acceptable predictive ability with a receiver operating characteristic of 0.71 (95% confidence interval [CI] = 0.70 to 0.72) in the validation dataset. Using a cut-point based on the 7% of the population at greatest risk results in a positive predictive value of 5.7% (95% CI = 5.3% to 6.1%) and a negative predictive value of 98.9% (95% CI = 98.9% to 99.0%), with sensitivity of 28.5% (95% CI = 26.7% to 30.3%) and specificity of 93.3% (95% CI = 93.2% to 93.4%); those individuals had an event risk of 6.0% compared with 1.1% for the remaining population. In total, 18 people would need to be followed to identify one admission. Conclusion This externally validated algorithm has acceptable predictive ability for identifying patients at high risk of asthma-related crisis events and excluding those not at high risk

    A possible jet precession in the periodic quasar B0605-085

    The quasar B0605-085 (OH 010) shows a hint for probable periodical variability in the radio total flux-density light curves. We study the possible periodicity of B0605-085 in the total flux-density, spectra and opacity changes in order to compare it with jet kinematics on parsec scales. We have analyzed archival total flux-density variability at ten frequencies (408 MHz, 4.8 GHz, 6.7 GHz, 8 GHz, 10.7 GHz, 14.5 GHz, 22 GHz, 37 GHz, 90 GHz, and 230 GHz) together with the archival high-resolution very long baseline interferometry data at 15 GHz from the MOJAVE monitoring campaign. Using the Fourier transform and discrete autocorrelation methods we have searched for periods in the total flux-density light curves. In addition, spectral evolution and changes of the opacity have been analyzed. We found a period in multi-frequency total flux-density light curves of 7.9+-0.5 yrs. Moreover, a quasi-stationary jet component C1 follows a prominent helical path on a similar time scale of 8 years. We have also found that the average instantaneous speeds of the jet components show a clear helical pattern along the jet with a characteristic scale of 3 mas. Taking into account average speeds of jet components, this scale corresponds to a time scale of about 7.7 years. Jet precession can explain the helical path of the quasi-stationary jet component C1 and the periodical modulation of the total flux-density light curves. We have fitted a precession model to the trajectory of the jet component C1, with a viewing angle phi=2.6+-2.2 degrees, aperture angle of the precession cone Omega=23.9+-1.9 degrees and fixed precession period (in the observers frame) P = 7.9 yrs.Comment: 14 pages, 16 figures, 5 tables, accepted for publication in A&

    A qualitative study of professional and client perspectives on information flows and decision aid use

    <p>Abstract</p> <p>Background</p> <p>This paper explores the meanings given by a diverse range of stakeholders to a decision aid aimed at helping carers of people in early to moderate stages of dementia (PWD) to select community based respite services. Decision aids aim to empower clients to share decision making with health professionals. However, the match between health professionals' perspectives on decision support needs and their clients' perspective is an important and often unstudied aspect of decision aid use.</p> <p>Methods</p> <p>A secondary analysis was undertaken of qualitative data collected as part of a larger study. The data included twelve interviews with carers of people with dementia, three interviews with expert advisors, and three focus groups with health professionals. A theoretical analysis was conducted, drawing on theories of 'positioning' and professional identity.</p> <p>Results</p> <p>Health professionals are seen to hold varying attitudes and beliefs about carers' decision support needs, and these appeared to be grounded in the professional identity of each group. These attitudes and beliefs shaped their attitudes towards decision aids, the information they believed should be offered to dementia carers, and the timing of its offering. Some groups understood carers as needing to be protected from realistic information and consequently saw a need to filter information to carer clients.</p> <p>Conclusion</p> <p>Health professionals' beliefs may cause them to restrict information flows, which can limit carers' ability to make decisions, and limit health services' ability to improve partnering and shared decision making. In an era where information is freely available to those with the resources to access it, we question whether health professionals should filter information.</p

    Climate Services for Resilient Development in South Asia and Bangladesh: Semi-Annual and Inception Period Report April 2017

    Developing countries are at considerable risk from climate variability and climate change, both of which threaten poverty reduction and development efforts. The Climate Services for Resilient Development (CSRD) partnership is led by the United States Government has developed a consortium of global leaders in science, technology and development finance to assist at-risk nations to adapt to these problems. CSRD is aligned with the the Global Framework for Climate Services and works in Bangladesh, Ethiopia, and Colombia to creating and provide timely and useful climate data, information, tools, and services. Within South Asia, efforts to develop agricultural climate services under CSRD are led by the International Maize and Wheat Improvement Center (CIMMYT). CSRD in turn works to support Investment Options Paper (IOP) for Climate Services for Resilient Development in Bangladesh, compiled by the Asian Development Bank (ADB) in 2016. CSRD’s core objectives are to prepare farmers, extension services, and agricultural policy makers with actionable climate information and crop management advisories to reduce agricultural production risks and to increase the resilience of smallholder farming communities. This report summarizes CSRD activities, achievements, and challenges during the project’s inception phase (from the end of November 2017 through April of 2017)
