    The Effects of Supramalleolar Orthoses on the Biomechanics of the Knee, Foot, and Ankle during Gait: A Single-Subject Design

    Gait deviations are a common problem associated with disorders of movement and posture such as cerebral palsy (CP). Inhibitive casts and ankle-foot orthoses have been used to treat gait deviations in children with CP, but they have not satisfied the needs of children who are able to achieve active dorsiflexion and plantarflexion but lack stability at the subtalar joint. Supramalleolar orthoses (SMOs) were developed to address that need. SMOs, along with physical therapy, have been used to treat children with CP, but little research has been conducted to determine the actual effects of the SMO on lower extremity biomechanics during gait. The purpose of this study was to describe the biomechanical changes that occur at the knee, foot, and ankle as a single-subject walked barefoot, with shoes, and with shoes and SMOs. It was hoped that while walking with shoes and SMOs, the subject\u27s gait pattern could be documented as more similar to normal arnbulation patterns. The subject walked on a treadmill while he was videotaped, and measurements of joint range of motion and bony alignment were taken from video photographs. From this data, it was determined that although utilization of SMOs did not produce a normal gait pattern, they did effectively control medial and lateral movement at the subtalar joint while normalizing tone

Our goal was to assist open education resources designers to develop empathy toward their learners and place themselves in their learners’ shoes.     Cold Regime Interannual Variability of Primary and Secondary Producer Community Composition in the Southeastern Bering Sea

    Variability of hydrographic conditions and primary and secondary productivity between cold and warm climatic regimes in the Bering Sea has been the subject of much study in recent years, while interannual variability within a single regime and across multiple trophic levels has been less well-documented. Measurements from an instrumented mooring on the southeastern shelf of the Bering Sea were analyzed for the spring-to-summer transitions within the cold regime years of 2009–2012 to investigate the interannual variability of hydrographic conditions, primary producer biomass, and acoustically-derived secondary producer and consumer abundance and community structure. Hydrographic conditions in 2012 were significantly different than in 2009, 2010, and 2011, driven largely by increased ice extent and thickness, later ice retreat, and earlier stratification of the water column. Primary producer biomass was more tightly coupled to hydrographic conditions in 2012 than in 2009 or 2011, and shallow and mid-column phytoplankton blooms tended to occur independent of one another. There was a high degree of variability in the relationships between different classes of secondary producers and hydrographic conditions, evidence of significant intra-consumer interactions, and trade-offs between different consumer size classes in each year. Phytoplankton blooms stimulated different populations of secondary producers in each year, and summer consumer populations appeared to determine dominant populations in the subsequent spring. Overall, primary producers and secondary producers were more tightly coupled to each other and to hydrographic conditions in the coldest year compared to the warmer years. The highly variable nature of the interactions between the atmospherically-driven hydrographic environment, primary and secondary producers, and within food webs underscores the need to revisit how climatic regimes within the Bering Sea are defined and predicted to function given changing climate scenarios

    Impact of race on efficacy and safety during treatment with olanzapine in schizophrenia, schizophreniform or schizoaffective disorder

    <p>Abstract</p> <p>Background</p> <p>To examine potential differences in efficacy and safety of treatment with olanzapine in patients with schizophrenia of white and black descent.</p> <p>Methods</p> <p>A post-hoc, pooled analysis of 6 randomized, double-blind trials in the treatment of schizophrenia, schizophreniform disorder, or schizoaffective disorder compared white (N = 605) and black (N = 375) patients treated with olanzapine (5 to 20 mg/day) for 24 to 28 weeks. Efficacy measurements included the Positive and Negative Syndrome Scale (PANSS) total score; and positive, negative, and general psychopathology scores; and the Clinical Global Impression of Severity (CGI-S) scores at 6 months. Safety measures included differences in the frequencies of adverse events along with measures of extrapyramidal symptoms, weight, glucose, and lipid changes over time.</p> <p>Results</p> <p>51% of black patients and 45% of white patients experienced early study discontinuation (P = .133). Of those who discontinued, significantly more white patients experienced psychiatric worsening (P = .002) while significantly more black patients discontinued for reasons other than efficacy or tolerability (P = .014). Discontinuation for intolerability was not different between groups (P = .320). For the estimated change in PANSS total score over 6 months, there was no significant difference in efficacy between white and black patients (P = .928), nor on the estimated PANSS positive (P = .435), negative (P = .756) or general psychopathology (P = .165) scores. Overall, there was no significant difference in the change in CGI-S score between groups from baseline to endpoint (P = .979). Weight change was not significantly different in white and black patients over 6 months (P = .127). However, mean weight change was significantly greater in black versus white patients at Weeks 12 and 20 only (P = .028 and P = .026, respectively). Additionally, a significantly greater percentage of black patients experienced clinically significant weight gain (≥7%) at anytime compared to white patients (36.1% vs. 30.4%, P = .021). Changes across metabolic parameters (combined fasting and random lipids and glucose) were also not significantly different between groups, with the exception of a greater categorical change in total cholesterol from borderline to high among white subjects and a categorical change from normal to low in high density lipoprotein (HDL) cholesterol among white males.</p> <p>Conclusions</p> <p>The findings did not demonstrate overall substantive differences in efficacy or safety between white and black patients diagnosed with schizophrenia or related disorders treated with olanzapine. However, a significantly greater percentage of black patients (36.1%) experienced clinically significant weight gain compared to white patients (30.4%).</p

    Spectroscopic Survey of G and K Dwarfs in the Hipparcos Catalog. I. Comparison between the Hipparcos and Photometric Parallaxes

    The tension between the Hipparcos parallax of the Pleiades and other independent distance estimates continues even after the new reduction of the Hipparcos astrometric data and the development of a new geometric distance measurement for the cluster. A short Pleiades distance from the Hipparcos parallax predicts a number of stars in the solar neighborhood that are sub-luminous at a given photospheric abundance. We test this hypothesis using the spectroscopic abundances for a subset of stars in the Hipparcos catalog, which occupy the same region as the Pleiades in the color–magnitude diagram. We derive stellar parameters for 170 nearby G- and K-type field dwarfs in the Hipparcos catalog based on high-resolution spectra obtained using KPNO 4 m echelle spectrograph. Our analysis shows that, when the Hipparcos parallaxes are adopted, most of our sample stars follow empirical color–magnitude relations. A small fraction of stars are too faint compared to main-sequence fitting relations by ΔM_V ≳ 0.3 mag, but the differences are marginal at a 2σ level, partly due to relatively large parallax errors. On the other hand, we find that the photometric distances of stars showing signatures of youth as determined from lithium absorption line strengths and R_(HK)^' chromospheric activity indices are consistent with the Hipparcos parallaxes. Our result is contradictory to a suggestion that the Pleiades distance from main-sequence fitting is significantly altered by stellar activity and/or the young age of its stars, and provides an additional supporting evidence for the long-distance scale of the Pleiades

    Aging and Endothelial Progenitor Cell Telomere Length in Healthy Men

    BACKGROUND: Telomere length declines with age in mature endothelial cells and is thought to contribute to endothelial dysfunction and atherogenesis. Bone marrow-derived circulating endothelial progenitor cells (EPCs) are critical to vascular health as they contribute to both reendothelialization and neovascularization. We tested the hypothesis that EPC telomere length decreases with age in healthy adult humans.METHODS: Peripheral blood samples were collected from 40 healthy, non-obese, sedentary men: 12 young (age 21-34 years), 12 middle-aged (43-55 years) and 16 older (57-68 years). Putative EPCs were isolated from peripheral blood mononuclear cells and telomere length was determined using genomic DNA preparation and Southern hybridization techniques.RESULTS: EPC telomere length (base pairs) was approximately 20% (p=0.01) lower in the older (8492+523 bp) compared to the middle-aged (10,565+572 bp) and young (10,205+501 bp) men. Of note, there was no difference in EPC telomere length between the middle-aged and young men.CONCLUSIONS: These results demonstrate that EPC telomere length declines with age in healthy, sedentary men. Interestingly, telomere length was well preserved in the middle-aged compared to young men, suggesting that EPC telomere shortening occurs after the age of 55 years

    A Test of Pre-Main-Sequence Lithium Depletion Models

    Despite the extensive study of lithium depletion during pre-main-sequence contraction, studies of individual stars show discrepancies between ages determined from the HR diagram and ages determined from lithium depletion (Song et al. 2002, White & Hillenbrand 2005) indicating open questions in the pre-main-sequence evolutionary models. To further test these models, we present high resolution spectra for members of the Beta Pictoris Moving Group (BPMG), which is young and nearby. We measure equivalent widths of the 6707.8 Angstrom Li I line in these stars and use them to determine lithium abundances. We combine the lithium abundance with the predictions of pre-main-sequence evolutionary models in order to calculate a lithium depletion age for each star. We compare this age to the age predicted by the HR diagram of the same model. We find that the evolutionary models under-predict the amount of lithium depletion for the BPMG given its nominal HR diagram age of ~12 Myr (Zuckerman et al. 2001), particularly for the mid-M stars, which have no observable Li I line. This results in systematically older ages calculated from lithium depletion isochrones than from the HR diagram. We suggest that this discrepancy may be related to the discrepancy between measured M-dwarf radii and the smaller radii predicted by evolutionary models.Comment: Accepted by ApJ; 21 pages, 5 figure
