    Symptomatic seizures in preterm newborns: a review on clinical features and prognosis

    Neonatal seizures are the most common neurological event in newborns, showing higher prevalence in preterm than in full-term infants. In the majority of cases they represent acute symptomatic phenomena, the main etiologies being intraventricular haemorrhage, hypoxic-ischemic encephalopathy, central nervous system infections and transient metabolic derangements.Current definition of neonatal seizures requires detection of paroxysmal EEG-changes, and in preterm newborns the incidence of electrographic-only seizures seems to be particularly high, further stressing the crucial role of electroencephalogram monitoring in this population. Imaging work-up includes an integration of serial cranial ultrasound and brain magnetic resonance at term-equivalent age. Unfavourable outcomes following seizures in preterm infants include death, neurodevelopmental impairment, epilepsy, cerebral palsy, hearing and visual impairment. As experimental evidence suggests a detrimental role of seizures per se in determining subsequent outcome, they should be promptly treated with the aim to reduce seizure burden and long-term disabilities. However, neonatal seizures show low response to conventional anticonvulsant drugs, and this is even more evident in preterm newborns, due to intrinsic developmental factors. As a consequence, as literature does not provide any specific guidelines, due to the lack of robust evidence, off-label medications are often administered in clinical practice

    A probabilistic approach for the evaluation of the stabilizing forces of fully grouted bolts

    The essential task of the ground reinforcement techniques is to keep the rock as stable as possible. In particular passive rock bolt should resist the rock movements along its entire length and through the resulting reaction forces, to improve the load-bearing capacity of the rock. Among different calculation techniques, the calculations based on Block Reinforcement Procedure (BRP) was used in this paper, also adopting some simplified equations available in the scientific literature. However, parameters influencing the interaction are difficult to evaluate. Therefore, the problem of the reliable definition of the parameters that most influence the behavior of the bolts and the evaluation of the stabilizing forces of the potentially unstable block of rock remains. A new probabilistic approach is presented in this article, able to appropriately manage the uncertainties present on the fundamental parameters of the bolt-rock interaction and on the mechanical characteristics of the sliding surface of the block. Through the use of a Monte Carlo procedure, in fact, it was possible to obtain different samples of the safety factors of the rock block, one for each diameter of the steel bars used for its stabilization. Finally, the probabilistic management of the safety factor samples allowed the correct design of the steel bars, by evaluating the probability that the safety factor of the block with regard to potential slipping has a value lower than a pre-established limit. The probabilistic approach developed was applied to a real problem of stabilization of a potentially unstable rock block due to planar sliding, present on a municipal road in North Italy

    A simplified mathematical approach for the evaluation of the stabilizing forces applied by a passive cemented bolt to a sliding rock block

    Passive bolting is used to stabilise unstable rock blocks in surface and underground structures due to the various advantages it offers. Despite its use, the design phase still presents aspects of considerable complexity because the fact that the load of the bolt and therefore, its static action depends on its interaction with the block and the stable rock. In the present work, a mathematical model was developed which is capable of directly calculating the stabilisation forces as a function of the characteristic parameters of the bolt and of its interaction with the rock. This discussion is based on a simplified hypothesis of bolt behaviour, which provides negligible errors, and on the observation that the critical point is positioned at the intersection of the bolt with one of the lateral surfaces that separate it from the portion of stable rock. The formulation of the stabilisation forces obtained made it possible to evaluate the static contribution of each single bolt to the stability of the rock block, by varying the diameter of the steel bar and then designing the bolting operation to achieve acceptable stability conditions for the rock block. The application of stabilising equations to a real case, for which the results of load tests on bolt tests were available, allowed us to outline steps to be taken in the bolt design process

    Evaluation of the safety factors of shotcrete linings during the creep stage

    Sprayed concrete linings (SCLs) used in tunnels generally develop secondary deformations over time, even in the presence of constant stress levels within the lining. These deformations influence the loading process on the lining and therefore also the stress levels within the support structure. The behaviour of a SCL in a tunnel was investigated under different operating conditions in order to evaluate the effect of secondary deformations on the evolution of stability conditions (safety margins with respect to the possible concrete failure) over time, after completion of the tunnel construction. A parametric analysis was performed to study eight different types of tunnels, with variable geometry and rock quality, and eight different types of sprayed concrete. The 64 cases of the parametric analysis covered the vast range of variability of influential parameters and yielded useful information concerning the effects of secondary deformations over time on the static behaviour of the lining and on the safety factor with reference to the possible failure of the sprayed concrete

    Biogeochemical processes in sediments of the Manfredonia Gulf (Southern Adriatic Sea): early diagenesis of carbon and nutrient and benthic exchange

    In order to understand the mechanisms responsible of the recycle of carbon and nutrients at the sediment-water interface and to understand the role of sediments in nutrients mass balance in coastal water, cores were collected (pore waters and solid phases) and benthic fluxes (oxygen, dissolved nutrients, dissolved iron and manganese, alkalinity and TCO2) were measured in two stations in the Gulf of Manfredonia (Southern Adriatic Sea). Stations were chosen to include a site, in the offshore part of the gulf, under the influence of western Adriatic current and another site, in the inner part of the gulf, under influence of gyres occurring inside the gulf. Both stations were placed in areas characterized by high sedimentation rate. Fluxes at sediment water interface show higher values in S2 site during the summer. Bio-irrigation seems to be the main transport mechanism characterizing both sites, with more evident effects during summer in S1 site

    Intercostal neurolysis for the treatment of postsurgical thoracic pain: A case series

    Introduction: We investigated the possible role of intercostal surgical neurolysis in relieving chronic neuropathic pain refractory to other nonsurgical treatments in patients with postsurgical thoracic pain. Methods: We retrospectively collected clinical data on patients referred to the Neurosurgery Unit of Policlinic Hospital of Milan. Ten patients (age range, 20-68 years) suffering from neuropathic pain for at least 2 months after thoracic surgery underwent intercostal neurolysis. Results: Compared with preneurolysis, pain intensity decreased 1 month postneurolysis and remained stable 2 months postneurolysis (median score [interquartile range]: 8 [6-9] preneurolysis, 4 [3-5] 1 month after, and 3 [2-5] 2 months after, P < 0.001). Antiepileptic drugs for pain control decreased after neurolysis. Discussion: Surgical intercostal neurolysis may be a promising therapeutic option in patients with chronic neuropathic pain associated with neurological deficits

    Workaholism, intensive smartphone use, and the sleep-wake cycle: A multiple mediation analysis

    Recent contributions have reported sleep disorders as one of the health impairment outcomes of workaholism. A possible factor affecting the sleep-wake cycle might be the intensive use of smartphones. The current study aimed to explore the role of intensive smartphone use in the relationship between workaholism and the sleep-wake cycle. Two serial multiple mediation models were tested on a sample of 418 employees, who filled self-report questionnaires measuring workaholism, use of smartphones, sleep quality and daytime sleepiness, using conditional process analysis for testing direct and indirect effects. Results supported our hypotheses regarding two serial multiple mediation models-that intensive smartphone use and poor sleep quality mediated the relationship between workaholism and daytime sleepiness, and that smartphone use and daytime sleepiness mediated the relationship between workaholism and poor quality of sleep. Although the use of a cross-sectional design and the snowball technique for collecting data can be considered as possible limitations, the current study is one of the first to document the potential detrimental role of the intensive smartphone use on the workaholism-sleep disorders relationship
