    Marital interaction theory: some implications for research

    This project examines specific basic areas of marital interaction theory. Individual chapters are developed around the following themes: historical development of the theory of interaction; a theoretical discussion of selected concepts; personality as a determinant of interaction; communication in marriage; marital interaction patterns; methodology pertaining to research findings; and finally some implications for research which became apparent as this project developed. The project lays the conceptual groundwork for a series of empirical studies that will use the interactional approach as a theoretical base. The immediate purpose of this project therefore is to describe the important dimensions of the interactional approach as these pertain to marital interaction. Interaction is defined as a reciprocal relationship in which each person may produce effects upon the other. Here the essence of marriage is interaction; thus marital interaction is critical in its effect on the spouses and the continuity of the marriage. Some of the basic concepts of George Mead are described and it is suggested that they are essential elements of the interactional approach. The major assumptions of this approach are that a) human behavior cannot be derived or inferred from nonhuman forms, b) the social act is the primary analytic unit for an understanding of society and the personality, c) the human infant is potentially social, and d) the human being is actor as well as reactor. Personality is discussed as a determinant of the quality and quantity of marital interaction. The issue of personal competence is explored, particularly in respect to verbal ability. Communication, the process of influence, is defined for marital interaction and the consequent formation of patterns. Identification and classification of marital interaction patterns are limited to a description of selected examples from family research and the observations of clinicians. Patterns tend to be defined in psychological terms although communicative behavior is stressed. The absence of common criteria and terminology is noted. Description of dysfunctional patterns predominates. Two methods frequently used in recent studies of marital interaction are direct observation and analysis of reports from marital partners. It is suggested that a combination of these techniques could increase the quantity of information about marital interaction. Research questions focus on the problem of adult socialization, its implications for the establishment of marital interaction patterns, and the need to determine the relation between the interaction process and the functional or dysfunctional quality of the resultant pattern. An inseparable part of the aforementioned research areas is the function of communication. It is suggested that investigation of verbal communication might yield significant data for the understanding of the interaction process as it affects the personalities of the spouses and the formation of patterns

    A New Distance to The Antennae Galaxies (NGC 4038/39) Based on the Type Ia Supernova 2007sr

    Traditionally, the distance to NGC 4038/39 has been derived from the systemic recession velocity, yielding about 20 Mpc for H_0 = 72 km/s/Mpc. Recently, this widely adopted distance has been challenged based on photometry of the presumed tip of the red giant branch (TRGB), which seems to yield a shorter distance of 13.3+-1.0 Mpc and, with it, nearly 1 mag lower luminosities and smaller radii for objects in this prototypical merger. Here we present a new distance estimate based on observations of the Type Ia supernova (SN) 2007sr in the southern tail, made at Las Campanas Observatory as part of the Carnegie Supernova Project. The resulting distance of D(SN Ia) = 22.3+-2.8 Mpc [(m-M)_0 = 31.74+-0.27 mag] is in good agreement with a refined distance estimate based on the recession velocity and the large-scale flow model developed by Tonry and collaborators, D(flow) = 22.5+-2.8 Mpc. We point out three serious problems that a short distance of 13.3 Mpc would entail, and trace the claimed short distance to a likely misidentification of the TRGB. Reanalyzing Hubble Space Telescope (HST) data in the Archive with an improved method, we find a TRGB fainter by 0.9 mag and derive from it a preliminary new TRGB distance of D(TRGB) = 20.0+-1.6 Mpc. Finally, assessing our three distance estimates we recommend using a conservative, rounded value of D = 22+-3 Mpc as the best currently available distance to The Antennae.Comment: 8 pages, 5 figures, 1 table (emulateapj; uses amsmath package).     Cross-cutting principles for planetary health education

    Since the 2015 launch of the Rockefeller Foundation Lancet Commission on planetary health,1 an enormous groundswell of interest in planetary health education has emerged across many disciplines, institutions, and geographical regions. Advancing these global efforts in planetary health education will equip the next generation of scholars to address crucial questions in this emerging field and support the development of a community of practice. To provide a foundation for the growing interest and efforts in this field, the Planetary Health Alliance has facilitated the first attempt to create a set of principles for planetary health education that intersect education at all levels, across all scales, and in all regions of the world—ie, a set of cross-cutting principles

    Multiple novel prostate cancer susceptibility signals identified by fine-mapping of known risk loci among Europeans

    Genome-wide association studies (GWAS) have identified numerous common prostate cancer (PrCa) susceptibility loci. We have fine-mapped 64 GWAS regions known at the conclusion of the iCOGS study using large-scale genotyping and imputation in 25 723 PrCa cases and 26 274 controls of European ancestry. We detected evidence for multiple independent signals at 16 regions, 12 of which contained additional newly identified significant associations. A single signal comprising a spectrum of correlated variation was observed at 39 regions; 35 of which are now described by a novel more significantly associated lead SNP, while the originally reported variant remained as the lead SNP only in 4 regions. We also confirmed two association signals in Europeans that had been previously reported only in East-Asian GWAS. Based on statistical evidence and linkage disequilibrium (LD) structure, we have curated and narrowed down the list of the most likely candidate causal variants for each region. Functional annotation using data from ENCODE filtered for PrCa cell lines and eQTL analysis demonstrated significant enrichment for overlap with bio-features within this set. By incorporating the novel risk variants identified here alongside the refined data for existing association signals, we estimate that these loci now explain ∼38.9% of the familial relative risk of PrCa, an 8.9% improvement over the previously reported GWAS tag SNPs. This suggests that a significant fraction of the heritability of PrCa may have been hidden during the discovery phase of GWAS, in particular due to the presence of multiple independent signals within the same regio

    Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     Independent and combined effects of improved water, sanitation, and hygiene, and improved complementary feeding, on child stunting and anaemia in rural Zimbabwe: a cluster-randomised trial.

    BACKGROUND: Child stunting reduces survival and impairs neurodevelopment. We tested the independent and combined effects of improved water, sanitation, and hygiene (WASH), and improved infant and young child feeding (IYCF) on stunting and anaemia in in Zimbabwe. METHODS: We did a cluster-randomised, community-based, 2 × 2 factorial trial in two rural districts in Zimbabwe. Clusters were defined as the catchment area of between one and four village health workers employed by the Zimbabwe Ministry of Health and Child Care. Women were eligible for inclusion if they permanently lived in clusters and were confirmed pregnant. Clusters were randomly assigned (1:1:1:1) to standard of care (52 clusters), IYCF (20 g of a small-quantity lipid-based nutrient supplement per day from age 6 to 18 months plus complementary feeding counselling; 53 clusters), WASH (construction of a ventilated improved pit latrine, provision of two handwashing stations, liquid soap, chlorine, and play space plus hygiene counselling; 53 clusters), or IYCF plus WASH (53 clusters). A constrained randomisation technique was used to achieve balance across the groups for 14 variables related to geography, demography, water access, and community-level sanitation coverage. Masking of participants and fieldworkers was not possible. The primary outcomes were infant length-for-age Z score and haemoglobin concentrations at 18 months of age among children born to mothers who were HIV negative during pregnancy. These outcomes were analysed in the intention-to-treat population. We estimated the effects of the interventions by comparing the two IYCF groups with the two non-IYCF groups and the two WASH groups with the two non-WASH groups, except for outcomes that had an important statistical interaction between the interventions. This trial is registered with ClinicalTrials.gov, number NCT01824940. FINDINGS: Between Nov 22, 2012, and March 27, 2015, 5280 pregnant women were enrolled from 211 clusters. 3686 children born to HIV-negative mothers were assessed at age 18 months (884 in the standard of care group from 52 clusters, 893 in the IYCF group from 53 clusters, 918 in the WASH group from 53 clusters, and 991 in the IYCF plus WASH group from 51 clusters). In the IYCF intervention groups, the mean length-for-age Z score was 0·16 (95% CI 0·08-0·23) higher and the mean haemoglobin concentration was 2·03 g/L (1·28-2·79) higher than those in the non-IYCF intervention groups. The IYCF intervention reduced the number of stunted children from 620 (35%) of 1792 to 514 (27%) of 1879, and the number of children with anaemia from 245 (13·9%) of 1759 to 193 (10·5%) of 1845. The WASH intervention had no effect on either primary outcome. Neither intervention reduced the prevalence of diarrhoea at 12 or 18 months. No trial-related serious adverse events, and only three trial-related adverse events, were reported. INTERPRETATION: Household-level elementary WASH interventions implemented in rural areas in low-income countries are unlikely to reduce stunting or anaemia and might not reduce diarrhoea. Implementation of these WASH interventions in combination with IYCF interventions is unlikely to reduce stunting or anaemia more than implementation of IYCF alone.     Flipped classroom for academic and career advising: an innovative technique for medical student advising

    Introduction: Career advising for medical students can be challenging for both the student and the adviser. Our objective was to design, implement, and evaluate a "flipped classroom" style advising session. Methods: We performed a single-center cross-sectional study at an academic medical center, where a novel flipped classroom style student advising model was implemented and evaluated. In this model, students were provided a document to review and fill out prior to their one-on-one advising session. Results: Ninety-four percent (95% CI, 88%-100%) of the medical students surveyed felt that the advising session was more effective as a result of the outline provided and completed before the session and that the pre-advising document helped them gain a better understanding of the content to be discussed at the session.
