    Evaluation of Venous Congestion by Point-of-Care Ultrasound: State of the Art

    The assessment of venous congestion is one of the grea-test medical challenges of this century: from more invasive procedures to the arising of ultrasound evaluation, there has been a constant quest for reliable and non-invasive bedside tools to determine and monitor hemodynamic status. Venous hypertension is an important pathophysiological mechanism of organ congestion, leading to its injury in various clinical settings.     Influence of pH and temperature in the performance of Zn phosphate conversion coatings

    Financiado para publicación en acceso aberto: Universidade de Vigo/CISUGThe influence of variations in temperature (from 50 ◩C to 70 ◩C) and pH (2.4 and 2.9) in the Zn phosphate conversion treatment of a high-strength steel was assessed in this study. The effect of the temperature changes depends on the pH of the bath. The results evidenced an enhanced corrosion resistance with the increased temperature for baths at pH 2.4, whereas no relevant differences were concluded for baths at pH 2.9. This latter pH condition provided a better surface coverage, even with lower coating masses, than the former value. The microstructural analysis showed the development of films composed of smaller crystals for the treatments at pH 2.9. Impedance spectroscopy was established as a powerful tool with great potential for characterising phosphate layers. The accuracy of this methodology greatly depends on the equivalent circuit used for the simulation. A model based on the transmission line allows the reliable estimation of the percentage of exposed area (coating efficiency), the identification of changes in the film conductivity, and the qualitative analysis of its structureXunta de Galicia | Ref.     Intravitreal docosahexaenoic acid in a rabbit model: preclinical safety assessment

    Purpose The purpose of the present study was to evaluate the retinal toxicity of a single dose of intravitreal docosahexaenoic acid (DHA) in rabbit eyes over a short-term period. Methods Sixteen New Zealand albino rabbits were selected for this pre-clinical study. Six concentrations of DHA (Brudy Laboratories, Barcelona, Spain) were prepared: 10 mg/50 ”l, 5 mg/50 ”l, 2'5 mg/50 ”l, 50 ”g/50 ”l, 25 ”g/50 ”l, and 5 ”g/50 ”l. Each concentration was injected intravitreally in the right eye of two rabbits. As a control, the vehicle solution was injected in one eye of four animals. Retinal safety was studied by slit-lamp examination, and electroretinography. All the rabbits were euthanized one week after the intravitreal injection of DHA and the eyeballs were processed to morphologic and morphometric histological examination by light microscopy. At the same time aqueous and vitreous humor samples were taken to quantify the concentration of omega-3 acids by gas chromatography. Statistical analysis was performed by SPSS 21.0. Results Slit-lamp examination revealed an important inflammatory reaction on the anterior chamber of the rabbits injected with the higher concentrations of DHA (10 mg/50 ”l, 5 mg/50 ”l, 2'5 mg/50 ”) Lower concentrations showed no inflammation. Electroretinography and histological studies showed no significant difference between control and DHA-injected groups except for the group injected with 50 ”g/50 ”l. Conclusions Our results indicate that administration of intravitreal DHA is safe in the albino rabbit model up to the maximum tolerated dose of 25 ”g/50 ”l. Further studies should be performed in order to evaluate the effect of intravitreal injection of DHA as a treatment, alone or in combination, of different retinal diseases

    Vida sem Escola e a saĂșde mental dos estudantes de escolas pĂșblicas durante a pandemia de Covid-19

    The present study analyzes the effects of the Covid-19 pandemic on students' mental health during part of the suspension of the in-person classes. The study is a cross-sectional survey completed between October and December 2020. An online self-report questionnaire was answered by students between 13 and 20 years of age, from the 9th grade to high school, who followed remote school activities in 21 state and municipal public schools located in the suburbs of the municipalities of SĂŁo Paulo and Guarulhos. Two linear regression models were used in the analysis, considering as dependent variables the depression scores by the Child Depression Inventory and anxiety by the SCARED (Screen for Child Anxiety Related Emotional Disorders). The time of exposure to the screens, the inversion of sleep, and the female gender, simultaneously with the difficulties of remote education and other social markers (such as color/race and cases of Covid-19 at home), are associated with symptoms of depression and anxiety during the first wave of the Covid-19 in the metropolitan area of SĂŁo Paulo. The findings reinforce the importance of school routine in the lives of these young people and the challenges posed to schools to promote students' mental health in the post-pandemic reality.O presente estudo analisa os efeitos da pandemia de Covid-19 sobre a saĂșde mental dos estudantes durante parte do perĂ­odo de suspensĂŁo das aulas presenciais. Trata-se de estudo transversal, aplicado entre outubro e dezembro de 2020, baseado em questionĂĄrio online de autorrelato respondido por estudantes entre 13 e 20 anos, do 9Âș ano do Ensino Fundamental e do Ensino MĂ©dio, que acompanhavam as atividades escolares remotas em 21 escolas pĂșblicas estaduais e municipais, localizadas nas periferias dos municĂ­pios de SĂŁo Paulo e Guarulhos.     Vitamin D levels and perinatal depressive symptoms in women at risk: a secondary analysis of the mothers, omega-3, and mental health study

    Abstract Background Vitamin D insufficiency may be associated with depressive symptoms in non-pregnant adults. We performed this study to evaluate whether low maternal vitamin D levels are associated with depressive symptoms in pregnancy. Methods This study was a secondary analysis of a randomized trial designed to assess whether prenatal omega-3 fatty acid supplementation would prevent depressive symptoms. Pregnant women from Michigan who were at risk for depression based on Edinburgh Postnatal Depression Scale Score or history of depression were enrolled. Participants completed the Beck Depression Inventory (BDI) and Mini International Neuropsychiatric Interview at 12–20 weeks, 26–28 weeks, 34–36 weeks, and 6–8 weeks postpartum. Vitamin D levels were measured at 12–20 weeks (N = 117) and 34–36 weeks (N = 112). Complete datasets were available on 105 subjects. Using regression analyses, we evaluated the relationship between vitamin D levels with BDI scores as well as with MINI diagnoses of major depressive disorder and generalized anxiety disorder. Our primary outcome measure was the association of maternal vitamin D levels with BDI scores during early and late pregnancy and postpartum. Results We found that vitamin D levels at 12–20 weeks were inversely associated with BDI scores both at 12—20 and at 34–36 weeks’ gestation (P < 0.05, both). For every one unit increase in vitamin D in early pregnancy, the average decrease in the mean BDI score was .14 units. Vitamin D levels were not associated with diagnoses of major depressive disorder or generalized anxiety disorder. Conclusions In women at risk for depression, early pregnancy low vitamin D levels are associated with higher depressive symptom scores in early and late pregnancy. Future investigations should study whether vitamin D supplementation in early pregnancy may prevent perinatal depressive symptoms.     Predictors of Recovery from Prenatal Depressive Symptoms from Pregnancy Through Postpartum

    Abstract Background: Identifying predictors of the course of depressive symptoms from pregnancy through postpartum is important to inform clinical interventions. Methods: This longitudinal study investigated predictors of recovery from prenatal elevated depressive symptoms in the postpartum period. Forty-one pregnant women completed demographic, interpersonal, and psychosocial self-report assessment measures at 32 weeks of gestation and again 12 weeks postpartum. Results: Of those with elevated depressive symptoms, defined as a Beck Depression Inventory-II (BDI-II) score ≄10, at the prenatal baseline, 39% (n=16) recovered to nonelevated symptom levels postpartum, whereas 61% (n=25) experienced sustained elevated symptoms. Women who recovered evidenced significantly lower baseline depression severity and more frequent engagement in physical activity and cohabitated with a romantic partner. In multiparous women (n=25), history of past postpartum depression (PPD) differentiated between those with transient and those with persisting symptoms, although history of lifetime depression did not. None of the additional demographic, interpersonal, or psychosocial variables investigated differentiated between groups. Logistic regression analysis showed prenatal depression severity and exercise frequency as predictors of recovery postpartum. Conclusions: Results suggest most women will not experience spontaneous recovery. Women with prenatal heightened symptom severity and previous experiences with PPD are acutely vulnerable to experience sustained symptoms. In contrast, having a cohabitating partner and engagement in prenatal exercise predicted symptom improvement. Physical exercise may be an important clinical recommendation, as it may improve mood. Given the small sample size, these results are preliminary. The present review summarizes the epidemiological studies evaluating the association between smoking and AMD, the mechanisms through which smoking induces damage to the chorioretinal tissues, and the relevance of advising patients to quit smoking for their visual health

    The mothers, Omega-3 and mental health study

    <p>Abstract</p> <p>Background</p> <p>Major depressive disorder (MDD) during pregnancy and postpartum depression are associated with significant maternal and neonatal morbidity. While antidepressants are readily used in pregnancy, studies have raised concerns regarding neurobehavioral outcomes in exposed infants. Omega-3 fatty acid supplementation, most frequently from fish oil, has emerged as a possible treatment or prevention strategy for MDD in non-pregnant individuals, and may have beneficial effects in pregnant women. Although published observational studies in the psychiatric literature suggest that maternal docosahexaenoic acid (DHA) deficiency may lead to the development of MDD in pregnancy and postpartum, there are more intervention trials suggesting clinical benefit for supplementation with eicosapentaenoic acid (EPA) in MDD.</p> <p>Methods/Design</p> <p>The Mothers, Omega-3 and Mental Health study is a double blind, placebo-controlled, randomized controlled trial to assess whether omega-3 fatty acid supplementation may prevent antenatal and postpartum depressive symptoms among pregnant women at risk for depression. We plan to recruit 126 pregnant women at less than 20 weeks gestation from prenatal clinics at two health systems in Ann Arbor, Michigan and the surrounding communities. We will follow them prospectively over the course of their pregnancies and up to 6 weeks postpartum. Enrolled participants will be randomized to one of three groups: a) EPA-rich fish oil supplement (1060 mg EPA plus 274 mg DHA) b) DHA-rich fish oil supplement (900 mg DHA plus 180 mg EPA; or c) a placebo. The primary outcome for this study is the Beck Depression Inventory (BDI) score at 6 weeks postpartum. We will need to randomize 126 women to have 80% power to detect a 50% reduction in participants' mean BDI scores with EPA or DHA supplementation compared with placebo. We will also gather information on secondary outcome measures which will include: omega-3 fatty acid concentrations in maternal plasma and cord blood, pro-inflammatory cytokine levels (IL-1ÎČ, IL-6, and TNF-α) in maternal and cord blood, need for and dosage of antidepressant medications, and obstetrical outcomes.     Health related quality of life measure in systemic pediatric rheumatic diseases and its translation to different languages: an international collaboration

    Background: Rheumatic diseases in children are associated with significant morbidity and poor health-related quality of life (HRQOL). There is no health-related quality of life (HRQOL) scale available specifically for children with less common rheumatic diseases. These diseases share several features with systemic lupus erythematosus (SLE) such as their chronic episodic nature, multi-systemic involvement, and the need for immunosuppressive medications. HRQOL scale developed for pediatric SLE will likely be applicable to children with systemic inflammatory diseases.Findings: We adapted Simple Measure of Impact of Lupus Erythematosus in Youngsters (SMILEY (c)) to Simple Measure of Impact of Illness in Youngsters (SMILY (c)-Illness) and had it reviewed by pediatric rheumatologists for its appropriateness and cultural suitability. We tested SMILY (c)-Illness in patients with inflammatory rheumatic diseases and then translated it into 28 languages. Nineteen children (79% female, n= 15) and 17 parents participated. the mean age was 12 +/- 4 years, with median disease duration of 21 months (1-172 months). We translated SMILY (c)-Illness into the following 28 languages: Danish, Dutch, French (France), English (UK), German (Germany), German (Austria), German (Switzerland), Hebrew, Italian, Portuguese (Brazil), Slovene, Spanish (USA and Puerto Rico), Spanish (Spain), Spanish (Argentina), Spanish (Mexico), Spanish (Venezuela), Turkish, Afrikaans, Arabic (Saudi Arabia), Arabic (Egypt), Czech, Greek, Hindi, Hungarian, Japanese, Romanian, Serbian and Xhosa.Conclusion: SMILY (c)-Illness is a brief, easy to administer and score HRQOL scale for children with systemic rheumatic diseases. It is suitable for use across different age groups and literacy levels.
