Renal disaster relief in Europe: the experience at L'Aquila, Italy, in April 2009
On 6 April 2009, an earthquake struck the city of L'Aquila and the surrounding Abruzzo mountains. The disaster left 66 000 people homeless, while 1500 were wounded and 298 died. Although Europe as a whole is not so often affected by massive earthquakes, Italy is an exception with 12 earthquakes with an intensity >6.0 on the Richter scale during the last 100 years. This article offers preliminary information on the L'Aquila earthquake. For the time being, nine AKI patients who needed dialysis treatment are known. In all of them, kidney function recovered. This positive result can be attributed to the efficient and intensive rescue efforts coupled to the availability of disaster plans that had been developed in advance. This article stresses the importance of (i) advance planning of disaster rescue; (ii) the inclusion in these plans of approaches for kidney problems and their complications; (iii) the formulation of recommendations supporting (para-) medical professionals in their preventive, therapeutic and logistic approach to massive incidences of crush
Tailoring Three-Point Functions and Integrability III. Classical Tunneling
We compute three-point functions between one large classical operator and two
large BPS operators at weak coupling. We consider operators made out of the
scalars of N=4 SYM, dual to strings moving in the sphere. The three-point
function exponentiates and can be thought of as a classical tunneling process
in which the classical string-like operator decays into two classical BPS
states. From an Integrability/Condensed Matter point of view, we simplified
inner products of spin chain Bethe states in a classical limit corresponding to
long wavelength excitations above the ferromagnetic vacuum. As a by-product we
solved a new long-range Ising model in the thermodynamic limit.Comment: 37 pages, 10 figure

Publisher Copyright: Copyright Š 2017 Massachusetts Medical Society.BACKGROUND: Findings from clinical trials of proprotein convertase subtilisinâkexin type 9 (PCSK9) inhibitors have led to concern that these drugs or the low levels of low-density lipoprotein (LDL) cholesterol that result from their use are associated with cognitive deficits. METHODS: In a subgroup of patients from a randomized, placebo-controlled trial of evolocumab added to statin therapy, we prospectively assessed cognitive function using the Cambridge Neuropsychological Test Automated Battery. The primary end point was the score on the spatial working memory strategy index of executive function (scores range from 4 to 28, with lower scores indicating a more efficient use of strategy and planning). Secondary end points were the scores for working memory (scores range from 0 to 279, with lower scores indicating fewer errors), episodic memory (scores range from 0 to 70, with lower scores indicating fewer errors), and psychomotor speed (scores range from 100 to 5100 msec, with faster times representing better performance). Assessments of cognitive function were performed at baseline, week 24, yearly, and at the end of the trial. The primary analysis was a noninferiority comparison of the mean change from baseline in the score on the spatial working memory strategy index of executive function between the patients who received evolocumab and those who received placebo; the noninferiority margin was set at 20% of the standard deviation of the score in the placebo group. RESULTS: A total of 1204 patients were followed for a median of 19 months; the mean (ÂąSD) change from baseline over time in the raw score for the spatial working memory strategy index of executive function (primary end point) was â0.21Âą2.62 in the evolocumab group and â0.29Âą2.81 in the placebo group (P<0.001 for noninferiority; P=0.85 for superiority). There were no significant between-group differences in the secondary end points of scores for working memory (change in raw score, â0.52 in the evolocumab group and â0.93 in the placebo group), episodic memory (change in raw score, â1.53 and â1.53, respectively), or psychomotor speed (change in raw score, 5.2 msec and 0.9 msec, respectively). In an exploratory analysis, there were no associations between LDL cholesterol levels and cognitive changes. CONCLUSIONS: In a randomized trial involving patients who received either evolocumab or placebo in addition to statin therapy, no significant between-group difference in cognitive function was observed over a median of 19 months.publishersversionPeer reviewe
Tailoring Three-Point Functions and Integrability II. Weak/strong coupling match
We compute three-point functions of single trace operators in planar N=4 SYM.
We consider the limit where one of the operators is much smaller than the other
two. We find a precise match between weak and strong coupling in the
Frolov-Tseytlin classical limit for a very general class of classical
solutions. To achieve this match we clarify the issue of back-reaction and
identify precisely which three-point functions are captured by a classical

Knowledge levels of medical faculty students and residents about ionizing radiation
This study aimed to evaluate the knowledge levels of medical school students and
residents about ionising radiation. The study is designed as descriptive research,
and it was conducted with 369 medical school students and residents. A survey
form was used in the research. A Chi-square test was used to compare categorical
variables. In the study, 369 people were reached within the scope of the research.
A total of 60.7% of the research participants were clinical medicine students (4th,
5th, 6th grade) and 39.3% were residents. A total of 42.0% of the participants of
the study were male, 58.0% were women. It was found that 17.9% of the clinical
medical students and 18.6% of the residents had sufficient knowledge of ionising
radiation (p=0.002). A total of 87.0% of the participants in the study answered
correctly that magnetic resonance imaging (MRI) does not contain radiation and
93.5% answered correctly that ultrasonography (USG) does not contain radiation.
74.8% of the participants stated that having knowledge about ionising radiation
would contribute to âprotecting sensitive groups from ionising radiationâ in medical
practice. This ratio is 77.2% in clinical medicine students and 71.0% in residents.
The study found that knowledge levels of medical faculty students and residents
about ionising radiation were insufficient. Medical students and residents are
recommended to be trained on radiation and the radiological requests of residents
to be evaluated
Malnutrition and inflammation in acute kidney injury due to earthquake-related crush syndrome
<p>Abstract</p> <p>Background</p> <p>Malnutrition and inflammation are common and serious complications in patients with acute kidney injury (AKI). However, the profile of these complications in patients with AKI caused by crush syndrome (CS) remains unclear. This study describes the clinical characteristics of malnutrition and inflammation in patients with AKI and CS due to the Wenchuan earthquake.</p> <p>Methods</p> <p>One thousand and twelve victims and eighteen healthy adults were recruited to the study. They were divided into five groups: Group A was composed of victims without CS and AKI (904 cases); Group B was composed of patients with CS and AKI who haven't received renal replacement therapy (RRT) (57 cases); and Group C was composed of patients with CS and AKI receiving RRT (25 cases); Group D was composed of earthquake victims with AKI but without CS (26 cases); and Group E was composed of 18 healthy adult controls. The C-reactive protein (CRP), prealbumin, transferrin, interleukin-6 and TNF-Îą were measured and compared between Group E and 18 patients from Group C.</p> <p>Results</p> <p>The results indicate that participants in Group C had the highest level of serum creatinine, blood urea nitrogen and uric acid. Approximately 92% of patients with CS who had RRT were suffering from hypoalbuminemia. The interleukin-6 and CRP levels were significantly higher in patients with CS AKI receiving RRT than in the control group. Patients in Group C received the highest dosages of albumin, plasma or red blood cell transfusions. One patient in Group C died during treatment.</p> <p>Conclusions</p> <p>Malnutrition and inflammation was common in patients with earthquake-related CS and had a negative impact on the prognosis of these subjects. The results of this study indicate that the use of RRT, intensive nutritional supplementation and transfusion alleviated the degree of malnutrition and inflammation in hemodialysis patients with crush syndrome.</p
Statin use is associated with fewer periodontal lesions: A retrospective study
<p>Abstract</p> <p>Background</p> <p>Inflammatory processes are considered to participate in the development of cardiovascular disease (CVD). Statins have been used successfully in the prevention and treatment of coronary heart disease. Chronic periodontitis has been suggested to contribute to CVD. The aim of this study was to examine the association of statin use and clinical markers of chronic periodontitis.</p> <p>Methods</p> <p>Periodontal probing pocket depth (PPD) values were collected from dental records of 100 consecutive adult patients referred to a university dental clinic for treatment of advanced chronic periodontitis. A novel index, Periodontal Inflammatory Burden Index (PIBI), was derived from the PPD values to estimate systemic effects of periodontitis.</p> <p>Results</p> <p>Periodontitis patients taking statins had a 37% lower number of pathological periodontal pockets than those without statin medication (P = 0.00043). PIBI, which combines and unifies the data on PPD, was 40% smaller in statin using patients than in patients without statin (P = 0.00069). PIBI of subjects on simvastatin and atorvastatin both differed significantly from patients without statin and were on the same level. The subjects' number of teeth had no effect on the results</p> <p>Conclusion</p> <p>Patients on statin medication exhibit fewer signs of periodontal inflammatory injury than subjects without the statin regimen. PIBI provides a tool for monitoring inflammatory load of chronic periodontitis. The apparent beneficial effects of statins may in part be mediated by their pleiotropic anti-inflammatory effect on periodontal tissue.</p
Performance of CMS muon reconstruction in pp collision events at sqrt(s) = 7 TeV
The performance of muon reconstruction, identification, and triggering in CMS
has been studied using 40 inverse picobarns of data collected in pp collisions
at sqrt(s) = 7 TeV at the LHC in 2010. A few benchmark sets of selection
criteria covering a wide range of physics analysis needs have been examined.
For all considered selections, the efficiency to reconstruct and identify a
muon with a transverse momentum pT larger than a few GeV is above 95% over the
whole region of pseudorapidity covered by the CMS muon system, abs(eta) < 2.4,
while the probability to misidentify a hadron as a muon is well below 1%. The
efficiency to trigger on single muons with pT above a few GeV is higher than
90% over the full eta range, and typically substantially better. The overall
momentum scale is measured to a precision of 0.2% with muons from Z decays. The
transverse momentum resolution varies from 1% to 6% depending on pseudorapidity
for muons with pT below 100 GeV and, using cosmic rays, it is shown to be
better than 10% in the central region up to pT = 1 TeV. Observed distributions
of all quantities are well reproduced by the Monte Carlo simulation.Comment: Replaced with published version. Added journal reference and DO
