A través del treball que realitzem dia a dia, hem vist com les TIC han passat de ser una necessitat formativa a convertir-se en un context d'aprenentatge diari entre les persones que participen del nostre projecte. Des que a l'any 1999 vam decidir transformar les nostres activitats per integrar-nos plenament a la societat de la informació hem canviat metodologies, prioritats i maneres de treballar. Mostrar quines han estat aquestes transformacions i quins resultat són els que s'han assolit és el principal repte d'aquest article

    The Achieved Dreams of a Neighborhood

    This article explains the social transformation process initiated at the end of the 1970s within the neighborhood of La Verneda-Sant Martı´ in Barcelona. This process started with the foundation of an adult education center that was organized as a Learning Community (the first one in the world). From the beginning, it was administered for and by the community. It became a space of debate where the demands and dreams of the neighbors converged about transforming their neighborhood along with the recommendations of the international scientific community. Twenty years later, the dreams came true: There have been substantial improvements throughout the urban space, infrastructures, housing, urban thoroughfares, and public highways. The INCLUD-ED European project, using the communicative methodology of research, has thoroughly studied the transformation carried out in the La Verneda-Sant Martı´ Adult School and its neighborhood. INCLUD-ED has identified successful practices within diverse social areas that are transferable to other contexts and contribute to overcoming inequalities and improving the most underprivileged neighborhoods

    A les cèl·lules, una mul tud de biomolècules canvien i interaccionen dinàmicament formant un entramat extraordinàriament complex que lliga metabolisme, proteoma i genoma entre si i amb l'entorn. Entendre com el conjunt de funcionalitats i comportaments cel·lulars emergeixen de tota aquesta complexitat d'interaccions en constant canvi i evolució és un dels grans reptes per al coneixement en el segle. La ciència de les xarxes complexes ens permet abordar l'estudi de la biologia cel·lular a gran escala més enllà dels constituents individuals. Des d'aquest enfocament, en aquest article repassarem breument alguns dels resultats més recents en la investigació dels sistemes cel·lulars, incloent-hi la redundància i plasticitat del metabolisme, mètodes capaços de detectar errors en la determinació experimental d'interaccions entre proteïnes, i l'explicació de fenòmens evolutius en termes del mapa genotip-fenotip

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    Role of age and comorbidities in mortality of patients with infective endocarditis

    [Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality. [Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk. [Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality. [Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group

    Spatiotemporal Characteristics of the Largest HIV-1 CRF02_AG Outbreak in Spain: Evidence for Onward Transmissions

    Background and Aim: The circulating recombinant form 02_AG (CRF02_AG) is the predominant clade among the human immunodeficiency virus type-1 (HIV-1) non-Bs with a prevalence of 5.97% (95% Confidence Interval-CI: 5.41–6.57%) across Spain. Our aim was to estimate the levels of regional clustering for CRF02_AG and the spatiotemporal characteristics of the largest CRF02_AG subepidemic in Spain.Methods: We studied 396 CRF02_AG sequences obtained from HIV-1 diagnosed patients during 2000–2014 from 10 autonomous communities of Spain. Phylogenetic analysis was performed on the 391 CRF02_AG sequences along with all globally sampled CRF02_AG sequences (N = 3,302) as references. Phylodynamic and phylogeographic analysis was performed to the largest CRF02_AG monophyletic cluster by a Bayesian method in BEAST v1.8.0 and by reconstructing ancestral states using the criterion of parsimony in Mesquite v3.4, respectively.Results: The HIV-1 CRF02_AG prevalence differed across Spanish autonomous communities we sampled from (p &lt; 0.001). Phylogenetic analysis revealed that 52.7% of the CRF02_AG sequences formed 56 monophyletic clusters, with a range of 2–79 sequences. The CRF02_AG regional dispersal differed across Spain (p = 0.003), as suggested by monophyletic clustering. For the largest monophyletic cluster (subepidemic) (N = 79), 49.4% of the clustered sequences originated from Madrid, while most sequences (51.9%) had been obtained from men having sex with men (MSM). Molecular clock analysis suggested that the origin (tMRCA) of the CRF02_AG subepidemic was in 2002 (median estimate; 95% Highest Posterior Density-HPD interval: 1999–2004). Additionally, we found significant clustering within the CRF02_AG subepidemic according to the ethnic origin.Conclusion: CRF02_AG has been introduced as a result of multiple introductions in Spain, following regional dispersal in several cases. We showed that CRF02_AG transmissions were mostly due to regional dispersal in Spain. The hot-spot for the largest CRF02_AG regional subepidemic in Spain was in Madrid associated with MSM transmission risk group. The existence of subepidemics suggest that several spillovers occurred from Madrid to other areas. CRF02_AG sequences from Hispanics were clustered in a separate subclade suggesting no linkage between the local and Hispanic subepidemics

A través del trabajo que venimos realizando día a día, hemos visto como las TIC han pasado de ser una necesidad formativa a convertirse en un contexto de aprendizaje cotidiano entre las personas que participan de nuestro proyecto. Desde que en 1999 decidimos transformar nuestras actividades para integrarnos plenamente en la sociedad de la información hemos cambiado metodologías, prioridades y formas de trabajar. Mostrar cuáles han sido esas transformaciones y qué resultados son los que se han alcanzado es el principal reto del artículo

    Educación física y promoción de la salud: estrategias de intervención en la escuela

    Este trabajo pretende profundizar, desde una perspectiva multidisciplinar, en las posibilidades de estrategias e intervenciones que ofrece la Educación Física en el contexto educativo, concretamente en la educación primaria, y que contribuyen al establecimiento de hábitos saludables en el alumnado y que pueden extenderse más allá del propio alumnado. La identificación de tales intervenciones se ha realizado a partir de una revisión profunda de las bases de datos que recogen las publicaciones de más impacto internacional a través de la Web of Knowledge. Asimismo, se presenta la potencial aplicabilidad de estas prácticas, por compartir algunas de las características que las definen o por ser propuestas ya compartidas, en centros educativos que se han transformado en Comunidades de Aprendizaj

    Sociedad de la información y educación: El aprendizaje de las tecnologías de la información y la Comunicación bajo una perspectiva dialógica

    [spa] Esta tesis doctoral analiza los elementos del aprendizaje dialógico que, aplicados a los procesos de alfabetización mediática, consiguen superar la fractura digital y generar una mayor participación y transformación social en personas adultas participantes. Para la consecución de este objetivo, se ha realizado un estudio de caso, bajo el paradigma de la metodología comunicativa, y desarrollado en el Punto Ómnia de la Asociación de personas participantes Ágora- Escuela de personas adultas La Verneda- Sant Martí. Este Punto Ómnia es un proyecto de acceso a las tecnologías gestionado desde el 1999 por personas adultas en procesos de educación básica y donde se lleva a cabo una alfabetización mediática basada en los principios del aprendizaje dialógico. Éste está logrando que personas adultas no sólo aprendan las tecnologías sino también se empoderen socialmente, consiguiendo una mayor participación y transformación social. Los principales resultados del estudio de caso muestran que su éxito está fundamentado en dos pilares principales. Por un lado, un proceso de alfabetización mediática que parte del diálogo igualitario como base de las interacciones de aprendizaje entre personas adultas para conseguir unos resultados académicos de excelencia. Por otro lado, la existencia de mecanismos de participación basados en pretensiones de validez y que facilitan la inclusión de las voces de las propias personas adultas en la gestión del Punto. La estructura de la tesis doctoral está organizada en cinco bloques. El primero presenta el estado de la cuestión donde, por un lado, se define los cuatro elementos básicos que delimitan esta tesis doctoral: Sociedad de la Información, tecnologías de la información y la comunicación, educación de personas adultas y fractura digital y, por otro lado, introduce el concepto y las dimensiones de la alfabetización mediática. En el segundo se exponen las preguntas de investigación, los objetivos e hipótesis de la presente tesis doctoral. Además, se explica la metodología y la estrategia analítica seguida así como se detalla el trabajo de campo realizado. El tercer bloque revisa las principales contribuciones realizadas desde el discurso postmoderno y la perspectiva transformadora a la apropiación de las tecnologías en la actual Sociedad de la Información. El cuarto bloque, describe el contexto de aprendizaje del estudio de caso y recoge los resultados del análisis del trabajo de campo. Finalmente, el quinto bloque sistematiza las conclusiones que corroboran la hipótesis principal y las sub-hipótesis planteadas.[eng] This doctoral dissertation analyses the elements of the dialogical learning that, applied to the media literacy processes, achieve to overcome the digital divide and to generate greater social participation and transformation in adult participants. For the consecution of this objective, a case study has been carried out, under the paradigm of the communicative methodology, and developed at the Omnia Point of the Association of Participants Ágora – La Verneda-Sant Martí Adult School. Omnia Point is a technology access project managed since 1999 by adult people in processes of basic education, where media literacy is carried out based on the principles of dialogic learning. This methodology allows adult people not only to learn technologies, but also to socially empower, hence achieving a greater social participation and transformation. The main results of the case of study show that its success is founded on two mainstays: on the one hand, a process of media literacy with the egalitarian dialogue as base for the learning interactions among the adult people to achieve results of academic excellence; on the other hand, the existence of participation mechanisms based on pretensions of validity and which facilitate the inclusion of the voices of the adult people themselves in the management of the Omnia Point. The structure of the doctoral dissertation is organized in five sections. The first one presents the state of the art: the four basic elements of this doctoral dissertation are defined –information society, information and communication technology, adult education and digital divide–, and the concept and dimensions of media literacy are introduced. In the second section, the questions of research, objectives and hypothesis of this doctoral dissertation are expounded. Besides, the methodology and analytical strategy followed are explained, as well as the field work performed. The third section reviews the main contributions from the postmodern discourse and the transforming perspective to the appropriation of technologies in the current information society. The fourth section describes the case of study context of learning and gathers the results of the field work analysis. Finally, the fifth section systematizes the conclusions corroborating the main hypothesis and sub-hypothesis considered
