    Environmental equity in southeast Louisiana: oil, people, policy, and the geography of industrial hazards

    This dissertation explores issues and concepts underlying the creation of Louisiana’s rural-industrial petrochemical complex as they relate to environmental equity and industrial development’s deleterious consequences. Cumulative hazards models examine the distribution of technological hazards associated with petroleum extraction and processing and explores how this varies among different socioeconomic groups in three coastal Louisiana parishes. Considerable onshore oil extraction occurs in Jefferson Parish. Lafourche Parish is the primary land-based supply center for the majority of the offshore oil activity in the Gulf of Mexico. The refineries of Saint Bernard Parish represent the endpoint of the flow of raw product and the staging point from which the refined product is transported to the wider market. Transportation infrastructure and a web of oil and gas pipelines connect these three parishes to each other and the areas beyond. The hazards models found a range of potential impacts affecting a wide swath of communities. Chalmette, for example, faces the greatest immediate risk from Saint Bernard Parish’s two large refineries and has the lowest proportion of minority residents within the study area. Conversely, Houma Indians residing on the wetland fringe stand at risk from the petroleum industry, both directly, through potential residential exposure, and indirectly, through potentially impacted hunting and fishing grounds. These results highlight the importance of using a fine grained localized environmental equity model. Historic process-based analyses examined the development of environmental inequity in communities identified by the hazards models. The historical development of environmental inequity is an extremely dynamic process. Generally, industry arrived first followed by population. After the initial siting, industry and population grew concomitantly. Industry growth continued even after the communities became predominantly minority or low-income. This research examined regional migration patterns over the last decade to explore why people move into potentially hazardous areas. Statistical analysis reveals that historic segregation patterns are a primary factor in most population movement. Additionally, populations are moving into neighborhoods proximate to their place of employment. Finally, this dissertation describes how the results achieved compare to those of other environmental equity studies and identifies six tenets of a practical, attainable, place-specific model of environmental equity

    “You turn the tap on, the water's there, and you just think everything's fine”: a mixed methods approach to understanding public perceptions of groundwater management in Baton Rouge, Louisiana, USA

    In Louisiana's Capital Area Groundwater Conservation District (CAGWCD), extensive groundwater withdrawals from the Southern Hills Aquifer System have begun to accelerate the infiltration of saltwater into the aquifer's freshwater sands. This accelerated saltwater intrusion has the potential to reduce the amount of groundwater available for public consumption and other industrial and agricultural uses throughout the region. In response to this threat, the Capital Area Ground Water Conservation Commission has begun development of a long-term strategic plan to achieve and maintain sustainable and resilient groundwater withdrawals from the aquifer system. The development of the strategic plan includes an assessment of public attitudes regarding groundwater and groundwater management in the CAGWCD. This paper presents the results of mixed methods public participatory research to evaluate current and historical views and attitudes around groundwater quality, quantity, and cost in the CAGWCD. The mixed methods approach used in this research employed a sequential explanatory design model consisting of two phases. The first phase involved the implementation of an internet-based survey, followed by a qualitative phase aimed at explaining and enhancing the quantitative results. The qualitative phase employed a combination of one-on-one interviews and focus groups. The research found that the primary governance obstacle that decision-makers may face in managing groundwater is a broad lack of public awareness of groundwater and groundwater issues in the CAGWCD. Despite the criticality of over-pumping and saltwater intrusion into the aquifer system, survey research and subsequent interviews and focus groups have shown that the public is largely unaware of these issues. This research also found a general lack of trust in both industry and government to manage groundwater issues and highlighted the need for groundwater management efforts to be led by unbiased, trusted institutions

    Anticipating and Adapting to the Future Impacts of Climate Change on the Health, Security and Welfare of Low Elevation Coastal Zone (LECZ) Communities in Southeastern USA

    Low elevation coastal zones (LECZ) are extensive throughout the southeastern United States. LECZ communities are threatened by inundation from sea level rise, storm surge, wetland degradation, land subsidence, and hydrological flooding. Communication among scientists, stakeholders, policy makers and minority and poor residents must improve. We must predict processes spanning the ecological, physical, social, and health sciences. Communities need to address linkages of (1) human and socioeconomic vulnerabilities; (2) public health and safety; (3) economic concerns; (4) land loss; (5) wetland threats; and (6) coastal inundation. Essential capabilities must include a network to assemble and distribute data and model code to assess risk and its causes, support adaptive management, and improve the resiliency of communities. Better communication of information and understanding among residents and officials is essential. Here we review recent background literature on these matters and offer recommendations for integrating natural and social sciences. We advocate for a cyber-network of scientists, modelers, engineers, educators, and stakeholders from academia, federal state and local agencies, non-governmental organizations, residents, and the private sector. Our vision is to enhance future resilience of LECZ communities by offering approaches to mitigate hazards to human health, safety and welfare and reduce impacts to coastal residents and industries

    Monitoring the Depth of Anaesthesia

    One of the current challenges in medicine is monitoring the patients’ depth of general anaesthesia (DGA). Accurate assessment of the depth of anaesthesia contributes to tailoring drug administration to the individual patient, thus preventing awareness or excessive anaesthetic depth and improving patients’ outcomes. In the past decade, there has been a significant increase in the number of studies on the development, comparison and validation of commercial devices that estimate the DGA by analyzing electrical activity of the brain (i.e., evoked potentials or brain waves). In this paper we review the most frequently used sensors and mathematical methods for monitoring the DGA, their validation in clinical practice and discuss the central question of whether these approaches can, compared to other conventional methods, reduce the risk of patient awareness during surgical procedures

    Incidence of epidural haematoma and neurological injury in cardiovascular patients with epidural analgesia/anaesthesia: systematic review and meta-analysis

    BACKGROUND: Epidural anaesthesia is used extensively for cardiothoracic and vascular surgery in some centres, but not in others, with argument over the safety of the technique in patients who are usually extensively anticoagulated before, during, and after surgery. The principle concern is bleeding in the epidural space, leading to transient or persistent neurological problems. METHODS: We performed an extensive systematic review to find published cohorts of use of epidural catheters during vascular, cardiac, and thoracic surgery, using electronic searching, hand searching, and reference lists of retrieved articles. RESULTS: Twelve studies included 14,105 patients, of whom 5,026 (36%) had vascular surgery, 4,971 (35%) cardiac surgery, and 4,108 (29%) thoracic surgery. There were no cases of epidural haematoma, giving maximum risks following epidural anaesthesia in cardiac, thoracic, and vascular surgery of 1 in 1,700, 1 in 1,400 and 1 in 1,700 respectively. In all these surgery types combined the maximum expected rate would be 1 in 4,700. In all these patients combined there were eight cases of transient neurological injury, a rate of 1 in 1,700 (95% confidence interval 1 in 3,300 to 1 in 850). There were no cases of persistent neurological injury (maximum expected rate 1 in 4,600). CONCLUSION: These estimates for cardiothoracic epidural anaesthesia should be the worst case. Limitations are inadequate denominators for different types of surgery in anticoagulated cardiothoracic or vascular patients more at risk of bleeding

    Building Resilience through Collaborative Management of Coastal Protection and Restoration Planning in Plaquemines Parish, Louisiana, USA

    This paper presents the results of a collaborative planning process to develop an integrated coastal restoration plan for Plaquemines Parish, Louisiana that recognizes the knowledge, experience, and priorities of residents and local stakeholders. To achieve this goal, the research team convened a broad group of stakeholders who live and work in Plaquemines Parish, including representatives of the seafood, navigation, and oil and gas industries, as well as residents, landowners, and those who are indigenous to the region, all of whom rely upon the ecosystem services provided by the wetlands, bays, and waterways for sustenance and wellbeing. Using a combination of local knowledge mapping and participatory modeling, the group worked with scientists to develop a restoration plan consisting of a suite of interlinked natural and nature-based solutions. The approach was intentionally interactive and iterative, creating a venue for open dialogue between residents, scientists, and resource users where no one source of knowledge was given primacy over another. Residents were able to contribute information regarding coastal restoration planning within their own communities, and a consensus plan for prioritizing restoration efforts in clusters was submitted for consideration as part of the State of Louisiana’s Coastal Master Plan process. Providing local stakeholders with direct access to scientists allowed their local knowledge to be translated into data products that could be more readily ingested into numerical models and other scientific planning tools
