    On the Cultivation of Their Community of Practice: A Case Study of EFL Malaysian Pre-Service Teachers

    This study examined the experiences of five EFL student teachers/pre-service teachers (PSTs) who participated in a Community of Practice (CoP) during their simulated teaching course and the practicum stage or teaching practice (TP), at The University of Malaya, a public Malaysian university. The experiences and tensions they encountered through this stage were discussed in the light of cultivating their CoP over five stages. Joining the CoP, increasing participation and negotiation of one\u27s tacit knowledge and assumptions were found to be productive at their learning to teach stage. However, some tensions, such as English proficiency level, self-confidence and agency, power relationships and worries of assessment persisted until the end of their practice. The results highlighted the significance of collaboration, reflection and social interactions with other CoP members as key to PSTs\u27 learning

In many countries, people have had to self-isolate and to be confined to their homes for several weeks to months to prevent the spread of the virus. Social distancing measures have had both negative and positive impacts on various aspects of economies, lifestyles, education, transportation, food supply, health, social life, and mental wellbeing. On other hands, due to reduced population movements and the decline in human activities, gas emissions decreased and the ozone layer improved; this had a positive impact on Earth’s weather and environment. Overall, the COVID-19 pandemic has negative effects on human activities and positive impacts on nature.     Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Evaluation of appendicitis risk prediction models in adults with suspected appendicitis

    Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent). Conclusion Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy.     Discrimination of Samar and Talh honey produced in the Gulf Cooperation Council (GCC) region using multivariate data analysis

    Lately, ensuring clear discrimination of the authenticity of natural honey is a concern not only to consumers but also to producers, traders and industries. The intrinsically simple nature of the honey composition, the distinguished health benefits as well as the high price make adulteration and falsification of the honey very common and the detection of authenticity very difficult. Samar honey (n = 59) and Talh honey (n = 64) produced from two species of Acacia trees (Acacia tortilis and Acacia gerrardii Benth, respectively) in different countries, including the United Arab Emirates, Saudi Arabia, Oman, and Yemen, has been studied by applying Multivariate Data Analysis. The discrimination is based on the official chemical quality parameters that inform about the nectary sources, including glucose, fructose, sucrose content, total reducing sugar, moisture content, acidity and diastase activity. Results show that the total reducing sugar, glucose and fructose were the most important positive loading descriptors that influence the quality of Samar and Talh honey. In addition, most of the Talh honey samples clustered at the top of the hierarchy, while Samar honey samples clustered at the bottom. The Multivariate Data Analysis indicates that the acidity and diastase activity are the most effective characteristics influencing the floral and geographical discrimination of both types of honey. This is the first study in the GCC area to discriminate between Samar and Talh honey using Multivariate Data Analysis by applying the principal component analysis and hierarchical cluster analysis. The Multivariate Data Analysis can be a helpful method to differentiate between Talh and Samar honey

    Bone Mineral Density in Egyptian Children with Familial Mediterranean Fever

    Background: Familial Mediterranean fever (FMF) has episodic or subclinical inflammation that may lead to a decrease in bone mineral density (BMD). The objective of this study was to assess BMD in Egyptian children with FMF on genetic basis. Methods: A cross sectional study included 45 FMF patients and 25 control children of both sexes in the age range between 3-16 years old. The patients were reclassified into two groups, namely group I(A) with 23 cases using colchicine for 1 month or less, and group I(B) with 22 cases using colchicine for more than 6 months. For both the patients and control groups, MEFV mutations were defined using molecular genetics technique and BMD was measured by DXA at the proximal femur and lumbar spines. Results: Four frequent gene mutations were found in the patient group E148Q (35.6%), V726A (33.3%), M680I (28.9%), and M694V (2.2%). There were also four heterozygous gene mutations in 40% of the control children. Patients receiving colchicine treatment for less than 1 month had highly significant lower values of BMD at the femur and lumbar spines than the control children (P=0.007, P<0.001). Patients receiving colchicine treatment for more than 6 months had improved values of BMD at femur compared with the control, but there were still significant differences between them in lumbar spine (P=0.036). There were insignificant effect of gene mutation type on BMD and the risk of osteopenia among the patients. Conclusion: FMF had a significant effect on BMD. However, regular use of colchicine treatment improves this effect mainly at the femur

    Chemical Profiling and Nutritional Evaluation of Bee Pollen, Bee Bread, and Royal Jelly and Their Role in Functional Fermented Dairy Products

    Honeybee products, as multicomponent substances, have been a focus of great interest. The present work aimed to perform the nutritional and chemical profiling and biochemical characterization of bee pollen (BP), bee bread (BB), and royal jelly (RJ) and study their applications in the fortification of functional fermented dairy products. Their effects on starter cultures and the physicochemical and sensorial quality of products were monitored. A molecular networking analysis identified a total of 46 compounds in the three bee products that could be potential medicines, including flavonoids, fatty acids, and peptides. BB showed the highest protein and sugar contents (22.57 and 26.78 g/100 g), which cover 45.14 and 53.56% of their daily values (DVs), with considerable amounts of the essential amino acids threonine and lysine (59.50 and 42.03%). BP, BB, and RJ can be considered sources of iron, as 100 g can cover 141, 198.5, and 94.94% of DV%, respectively. BP was revealed to have the highest phenolic and flavonoid contents (105.68 and 43.91 mu g/g) and showed a synergetic effect when mixed with RJ, resulting in increased antioxidant activity, while BB showed a synergetic effect when mixed with RJ in terms of both antioxidant and proteolytic powers (IC50 7.54, 11.55, 12.15, 12.50, and 12.65 cP compared to the control (10.55 cP)), reflecting their organoleptic properties and highlighting these health-oriented products as promising natural products for human health care

This study aimed to evaluate the anti-HIV activities and cytotoxic effects of extracts and chemical constituents isolated from In our previous study we demonstrated that the hexane, CH Cl , ethyl acetate and methanol soluble fractions of a 1:1 CH Cl /MeOH crude extracts of the leaves and stem bark of exhibited potent anti-HIV activities against HIV-1 with IC values ranging from 0.02–8.1 μg/mL and cytotoxicity effects against MT-4 cells ranging from IC = 0.58–174 μg/mL Hence, hexane soluble extract of 1:1 CH Cl /MeOH crude extract of the leaves of that was more potent against HIV-1 at IC = 0.02 μg/mL was subjected to column chromatography leading to the isolation of 2-methoxy benzyl benzoate (1), lupenone (2), lupeol acetate (3), betulin (4), lupeol (5), sitosterol (6) and stigmasterol (7). Lupenone (2), lupeol acetate (3) and betulin (4) exhibited anti-HIV-1 inhibition at IC = 4.7 nM, 4.3 and 4.5 μg/mL respectively.
