Mapping the Landscape of Behavioral Theories: Systematic Literature Review
The term “behavioral” has become a hot topic in recent years in various disciplines; however, there is yet limited understanding of what theories can be considered behavioral theories and what fields of research they can be applied to. Through a cross-disciplinary literature review, this article identifies sixty-two behavioral theories from 963 search results, mapping them in a diagram of four groups (factors, strategies, learning and conditioning, and modeling), and points to five discussion points: understanding of terms, classification, guidance on the use of appropriate theories, inclusion in data-driven research and agent-based modeling, and dialogue between theory-driven and data-driven approaches.ESRC, CHR
Consentimento informado: reflexão sob o olhar da bioética
Prevê-se que os profissionais de saúde façam o melhor para o doente, com o melhor
conhecimento que têm, e dentro das boas práticas.
Na relação profissional-doente, da qual deve fazer parte a confiança, ocorre um certo
desnivelamento. Por um lado o enfermeiro garante a qualidade dos cuidados e
actuação em função do bem do outro, por outro ao doente cabe a confiança no
profissional, seus conhecimentos e acção.
A bioética ajuda a balancear este desnível e proteger a relação de eventuais abusos,
com ajuda de alguns princípios, nomeadamente o da autonomia. Como corolário deste
princípio surge o consentimento informado. O CI dá primazia à decisão consciente do
doente, face ao que lhe diz respeito. Mas se por um lado, favorece um aumento de
conhecimento, diminuindo o risco de uma medicina paternalista, por outro pode gerar
desconforto na equipa de saúde. O CI deve ser olhado como ponto de equilíbrio, numa
relação de respeito, ao invés de causador de medos recíprocos.
Assim a relação tende a aproximar-se do ideal, capacitando o doente da tomada de
decisões e consequente actuação de acordo com a sua autonomia e
autodeterminação, em simultâneo com uma atitude profissional, segura e firmada na
boa fé. Caminha-se para a aliança terapêutica, na qual estão impressas a confiança e
segurança recíprocas.
A legitimidade que se exige ao CI tem obrigatoriamente a informação cedida àquele
que toma a decisão. Cabe ao profissional o dever de informar e de forma
compreensível. Só com informação compreendida pelo receptor, todos os outros
direitos se preenchem de sentido, na medida em que a livre escolha, participação e
cidadania são plenamente exercidos.
Informar é o meio mais eficaz para «empoderar» cada pessoa, dando-lhe uma parte
significativa dos meios necessários à concretização dos projectos pessoais de saúde e
de vida. E o enfermeiro é o fio de prumo na balança desta relação, que se deseja
equilibrada
Increased expression of miR-187 in human islets from individuals with type 2 diabetes is associated with reduced glucose-stimulated insulin secretion

MicroRNAs (miRNAs) are post-transcriptional regulators of gene expression and therefore alterations in miRNA levels may be involved in the deterioration of beta cell function. Methods: Global TaqMan arrays and individual TaqMan assays were used to measure islet miRNA expression in discovery (n = 20) and replication (n = 20) cohorts from individuals with and without type 2 diabetes. The role of specific dysregulated miRNAs in regulating insulin secretion, content and apoptosis was subsequently investigated in primary rat islets and INS-1 cells. Identification of miRNA targets was assessed using luciferase assays and by measuring mRNA levels. Results: In the discovery and replication cohorts miR-187 expression was found to be significantly increased in islets from individuals with type 2 diabetes compared with matched controls. An inverse correlation between miR-187 levels and glucose-stimulated insulin secretion (GSIS) was observed in islets from normoglycaemic donors. This correlation paralleled findings in primary rat islets and INS-1 cells where overexpression of miR-187 markedly decreased GSIS without affecting insulin content or apoptotic index. Finally, the gene encoding homeodomain-interacting protein kinase-3 (HIPK3), a known regulator of insulin secretion, was identified as a direct target of miR-187 and displayed reduced expression in islets from individuals with type 2 diabetes. Conclusions/interpretation: Our findings suggest a role for miR-187 in the blunting of insulin secretion, potentially involving regulation of HIPK3, which occurs during the pathogenesis of type 2 diabetes. © 2013 The Author(s).This work was supported by the Wellcome Trust (project

Testamento vital: onde se situam os enfermeiros? Integração de uma nova realidade nos cuidados de enfermagem ao doente e família
O enfermeiro é o profissional de saúde que tem como objectivo prestar cuidados de
Enfermagem ao ser humano, são ou doente, ao longo de todo o ciclo vital. (Decreto-Lei
n.º 161/96 de 4 de Setembro, alterado pelo Decreto-Lei n.º 104/98 de 21 de Abril.

A presença de um doente com testamento vital internado no serviço de cardiologia,
cujo conhecimento por parte da equipa multidisciplinar foi tardio, suscitou várias
questões tendo despoletado a reflexão sobre esta temática.
O doente tem o dever de fornecer aos profissionais as informações necessárias para a
obtenção de um adequado diagnóstico e tratamento. Quando tal não ocorre ou não é
possível, cabe também ao enfermeiro uma adequada colheita de dados para a
prestação de cuidados de qualidade.
Actualmente a consulta do Registo Nacional do Testamento Vital (RENTEV) é da
responsabilidade do médico que “assegura da existência do documento de Directivas
Antecipadas da Vontade e/ou procuração de cuidados de saúde” aí registados. (Lei Nº

A ausência desta informação por parte do enfermeiro poderá trazer constrangimentos
à sua intervenção dado que, desconhecendo-se a vontade do doente poderá facilitar a
realização de procedimentos invasivos.
O testamento vital é já uma realidade mas, perante tais obstáculos que podem pôr em
causa a vontade expressa do doente e os cuidados de excelência, como poderemos
colmatar tais lacunas?
Cremos ser importante constar da avaliação inicial e processo de cuidados a colheita
de informação sobre a existência ou não de testamento vital, que assegure o consequente conhecimento do seu conteúdo, independentemente do que vigora na lei
sobre o acesso administrativo dos enfermeiros ao RENTEV.
Entende-se que esta não é matéria terminada e que o quadro legal que a suporta
poderá necessitar de aperfeiçoamento e compatibilização com a realidade que o
futuro desvendar.

Higiene da mãos: novas estratégias diferentes resultados?
A higiene das mãos é uma das medidas mais simples e efectiva na redução das
infecções associadas aos cuidados de saúde. (Direcção Geral da Saúde, circular
normativa nº 13/DQS/DSD – Orientação de Boa Prática para a Higiene das Mãos nas
Unidades de Saúde).
O Hospital Fernando Fonseca E.P.E. (HFF) aderiu à campanha de Nacional de Higiene
das Mãos e passados cinco anos a avaliação do HFF, de acordo com a escala da
Direcção Geral da Saúde é de muito satisfatório. Mas será que podemos melhorar de
maneira a conseguirmos aumentar a taxa de adesão à higiene das mãos pelos
profissionais de saúde?
No Serviço de Cardiologia (Enfermaria e UCI) verifica-se que a taxa de adesão dos
profissionais é menor antes do contacto com o doente. Apesar da existência de
Solução Anti-séptica de Base Alcoólica no serviço, de cartazes alusivos e de formação
periódica, que estratégias poderão mais ser implementadas para melhorar a adesão
dos profissionais de saúde?
No início de Março foi distribuído pelo serviço (unidades dos doentes e outros locais)
pequenos cartazes (stoppers) a relembrar os profissionais de saúde para a higiene das
mãos.
A eficácia desta estratégia vai ser analisada através de alguns indicadores preconizados
pela OMS.
Daremos destaque ao consumo de SABA, comparando o seu consumo neste mês com
os anteriores e, a taxa de adesão à higienização das mãos, através da observação-
informação.
A análise preliminar revela-nos um aumento do consumo de SABA o que nos poderá
sugerir o sucesso da colocação de stoppers em locais estratégicos do serviço. Os resultados que daqui surgirem serão alvo de análise e discussão entre os
profissionais de saúde de forma a envolver a equipa multidisciplinar na adopção de
novas estratégias que visem uma melhor adesão à higiene das mãos.
Reconhecemos que a eficácia desta medida é de curta duração pelo que de futuro
visamos a implementação de outras medidas que melhorem igualmente a taxa de
adesão dos profissionais à higiene das mãos.

Order release in a workload controlled flow-shop with sequence-dependent set-up times
In this paper, we report a simulation study on the role of sequence-dependent
set-up times in decision making at the order release level of a workload controlled
make-to-order flow-shop. The study evaluates the potential for set-ups savings,
dependent on the level of workload in the shop, for two alternative strategies,
namely considering set-up times centrally, within the release decision or locally,
within the dispatching decision. These strategies are compared and assessed on
the basis of two main performance measures namely time in system and standard
deviation of the job lateness. Results indicate that the local strategy, which has
been traditionally adopted in practice and in most of the studies dealing with
sequence-dependent set-up times, does not always give the best results.
The release frequency and the shop workload appear critical to the selection of

Extensive Karyotype Reorganization in the Fish Gymnotus arapaima (Gymnotiformes, Gymnotidae) Highlighted by Zoo-FISH Analysis.
The genus Gymnotus (Gymnotiformes) contains over 40 species of freshwater electric fishes exhibiting a wide distribution throughout Central and South America, and being particularly prevalent in the Amazon basin. Cytogenetics has been an important tool in the cytotaxonomy and elucidation of evolutionary processes in this genus, including the unraveling the variety of diploid chromosome number (2n = from 34 to 54), the high karyotype diversity among species with a shared diploid number, different sex chromosome systems, and variation in the distribution of several Repetitive DNAs and colocation and association between those sequences. Recently whole chromosome painting (WCP) has been used for tracking the chromosomal evolution of the genus, showing highly reorganized karyotypes and the conserved synteny of the NOR bearing par within the clade G. carapo. In this study, painting probes derived from the chromosomes of G. carapo (GCA, 2n = 42, 30 m/sm + 12 st/a) were hybridized to the mitotic metaphases of G. arapaima (GAR, 2n = 44, 24 m/sm + 20 st/a). Our results uncovered chromosomal rearrangements and a high number of repetitive DNA regions. From the 12 chromosome pairs of G. carapo that can be individually differentiated (GCA1-3, 6, 7, 9, 14, 16, and 18-21), six pairs (GCA 1, 9, 14, 18, 20, 21) show conserved homology with GAR, five pairs (GCA 1, 9, 14, 20, 21) are also shared with cryptic species G. carapo 2n = 40 (34 m/sm + 6 st/a) and only the NOR bearing pair (GCA 20) is shared with G. capanema (GCP 2n = 34, 20 m/sm + 14 st/a). The remaining chromosomes are reorganized in the karyotype of GAR. Despite the close phylogenetic relationships of these species, our chromosome painting studies demonstrate an extensive reorganization of their karyotypes
The Roosevelt – Rondon expedition marmoset (Mico marcai) : unveiling the conservation status of a data deficient species
The Roosevelt-Rondon Expedition marmoset, Mico marcai, was collected in 1914 and to date, all information on this species comes from three skins brought back by the Expedition and two additional skins collected in the 1990s. It is no surprise then that M.marcai has been classified as Data Deficient (DD). Given that Mico marcai’s suspected range sits on the path of the advancing Brazilian “Arc-of-Deforestation”, it is urgent that relevant data be collected to assess this taxon. Here we present the first comprehensive field data on the distribution, population size and threats on M. marcai with the goal of removing the species from the DD category. From 2012 to 2015, we surveyed for the species in 11 localities, in and around the Marmelos-Aripuanã interfluve, and estimated density using distance sampling on 10 transects. We also used spatial predictive modelling to project the amount of habitat that will be lost within its range in 18 years under different deforestation scenarios. We found marmosets in 14 localities and calculated its Extent of Occurrence to be 31,073 km2. We walked 271 km and detected 30 marmoset groups, allowing us to estimate their density to be 8.31 individuals/km2 and a total population of 258,217.71 individuals. By a “Business as usual” scenario, 20,181 km2 of habitat will be lost in three marmoset generations (~18 years), compromising 33% of the species’ range. Accordingly, M. marcai should be classified as globally Vulnerable under category A3c. Following our study, we propose the Amazonian marmosets, genus Mico, should undergo similar re-assessment as their ranges all fall in the path of the Arc-of-Deforestation.

Cancer and thrombosis: Managing the risks and approaches to thromboprophylaxis
Patients with cancer are at increased risk of venous thromboembolism (VTE) compared with patients without cancer. This results from both the prothrombotic effects of the cancer itself and iatrogenic factors, such as chemotherapy, radiotherapy, indwelling central venous devices and surgery, that further increase the risk of VTE. Although cancer-associated thrombosis remains an important cause of morbidity and mortality, it is often underdiagnosed and undertreated. However, evidence is accumulating to support the use of low-molecular-weight heparins (LMWHs) in the secondary prevention of VTE in patients with cancer. Not only have LMWHs been shown to be at least as effective as coumarin derivatives in this setting, but they have a lower incidence of complications, including bleeding, and are not associated with the practical problems of warfarin therapy. Furthermore, a growing number of studies indicate that LMWHs may improve survival among patients with cancer due to a possible antitumor effect. Current evidence suggests that LMWHs should increasingly be considered for the long-term management of VTE in patients with cancer
