    Trabalho de projecto de mestrado em Medicina (Gastroenterologia), apresentado á Faculdade de Medicina da Universidade de CoimbraA encefalopatia hepática é uma das principais complicações da doença hepática crónica e pode estar presente em 50 a 70% de todos os pacientes cirróticos, incluindo aqueles com alterações demonstráveis apenas por testes psicométricos, sendo uma complicação de grande relevância clínica. Na insuficiência hepática aguda, os doentes podem sucumbir a uma morte neurológica, com edema cerebral e hipertensão intracraniana. A sobrevida em pacientes com doença hepática crónica depende da avaliação dos critérios da classificação de Child-Pugh que reconhece a importância prognóstica da encefalopatia hepática. Esta complicação da doença hepática, aguda ou crónica, resulta da diminuição da actividade hepática com incapacidade marcada de eliminação de determinadas toxinas do organismo. Hoje sabe-se que os astrócitos, nomeadamente as células de Alzheimer tipo II, tem um papel importante na fisiopatologia da encefalopatia hepática. A epidemiologia e a fisiopatologia da encefalopatia hepática permanecem ainda apenas parcialmente esclarecidas, tornando este tema fonte importante de estudos constantes. O trabalho proposto tem como objectivo uma actualizada revisão bibliográfica, centrando-se nos últimos avanços científicos sobre esta alteração neuropsiquiátrica. Com este trabalho, propõe-se estudar os diferentes métodos de diagnóstico, indicações das diferentes opções terapêuticas, comparando-as entre si, identificar o impacto económico e social da encefalopatia hepática, assim como perceber os mecanismos fisiopatológicos que contribuem para esta grave alteraçãoHepatic encephalopathy is one of the main complications of chronic liver disease and can occur in 50 to 70% of all cirrhotic patients, including those with alterations demonstrated only through psychometric tests, being a complication of great clinical relevance. In severe liver failure, patients can perish due to neurological death, with brain swelling and intracranial hypertension. Chronic liver disease patients’ survival time depends on the evaluation of Child- Pugh classification criteria that recognizes the prognostic importance of hepatic encephalopathy. This liver disease complication, chronic or severe, is a result of the reduction of liver activity with marked incapacity to eliminate certain toxins from the organism. Today it is known that astrocytes, namely Alzheimer type II cells, have an important role in hepatic encephalopathy physiopathology. The epidemiology and physiopathology of hepatic encephalopathy still remain partially clarified, becoming this subject an important source of constant studies. The main goal of this study is to make an actualized bibliographical revision, grounded on the last scientific advances on this neuropsychiatric abnormality. Through this work, one considers studying the different diagnostic methods, the different therapeutic option indications, by comparing them, identifying the economic and social impact of hepatic encephalopathy, as well as understanding the physiopathological mechanisms that contribute for this serious abnormalit

    Increased collagen synthesis rate during wound healing in muscle

    Wound healing in muscle involves the deposition of collagen, but it is not known whether this is achieved by changes in the synthesis or the degradation of collagen. We have used a reliable flooding dose method to measure collagen synthesis rate in vivo in rat abdominal muscle following a surgical incision. Collagen synthesis rate was increased by 480% and 860% on days 2 and 7 respectively after surgery in the wounded muscle compared with an undamaged area of the same muscle. Collagen content was increased by approximately 100% at both day 2 and day 7. These results demonstrate that collagen deposition during wound healing in muscle is achieved entirely by an increase in the rate of collagen synthesis

    Decadal regional air quality simulations over Europe in present climate: near surface ozone sensitivity to external meteorological forcing

    Abstract. Regional climate-air quality decadal simulations over Europe were carried out with the RegCM3/CAMx modeling system for the time slice 1991–2000, in order to study the impact of different meteorological forcing on surface ozone. The RegCM3 regional climate model was firstly constrained by the ERA40 reanalysis dataset which is considered as an experiment with perfect meteorological boundary conditions and then it was constrained by the global circulation model ECHAM5. A number of meteorological parameters were examined including the 500 mb geopotential height, solar radiation, temperature, cloud liquid water path, planetary boundary layer height and surface wind. The different RegCM meteorological forcing resulted in changes of near surface ozone over Europe ranging between ± 4 ppb for winter and summer. The area showing the greatest sensitivity in O3 during winter is central and southern Europe while in summer central north continental Europe. The different meteorological forcing impacts on the atmospheric circulation, which in turn affects cloudiness and solar radiation, temperature, wind patterns and the meteorology depended biogenic emissions. For comparison reasons, the impact of chemical boundary conditions on surface ozone was additionally examined with a series of sensitivity studies, indicating that surface ozone changes are comparable to those caused by the different meteorological forcing. These findings suggest that, when it comes to regional climate-air quality simulations, the selection of external meteorological forcing can be as important as the selection of adequate chemical lateral boundary conditions

    The success of the cyanobacterium Cylindrospermopsis raciborskii in freshwaters is enhanced by the combined effects of light intensity and temperature

    Toxic cyanobacterial blooms in freshwaters are thought to be a consequence of the combined effects of anthropogenic eutrophication and climate change. It is expected that climate change will affect water mixing regimes that alter the water transparency and ultimately the light environment for phytoplankton. Blooms of the potentially toxic cyanobacterium Cylindrospermopsis raciborskii are expanding from tropical towards temperate regions. Several hypotheses have been proposed to explain this expansion, including an increase in water temperature due to climate change and the high phenotypic plasticity of the species that allows it to exploit different light environments. We performed an analysis based on eight lakes in tropical, subtropical and temperate regions to examine the distribution and abundance of C. raciborskii in relation to water temperature and transparency. We then conducted a series of short-term factorial experiments that combined three temperatures and two light intensity levels using C. raciborskii cultures alone and in interaction with another cyanobacterium to identify its growth capacity. Our results from the field, in contrast to predictions, showed no differences in dominance (>40% to the total biovolume) of C. raciborskii between climate regions. C. raciborskii was able to dominate the phytoplankton in a wide range of light environments (euphotic zone=1.5 to 5 m, euphotic zone/mixing zone ratio 1.5). Moreover, C. raciborskii was capable of dominating the phytoplankton at low temperatures (<15°C). Our experimental results showed that C. raciborskii growing in interaction was enhanced by the increase of the temperature and light intensity. C. raciborskii growth in high light intensities and at a wide range of temperatures, suggests that any advantage that this species may derive from climate change that favors its dominance in the phytoplankton is likely due to changes in the light environment rather than changes in temperature. Predictive models that consider only temperature as a drive factor can therefore fail in predicting the expansion of this potentially toxic cyanobacterium

    Efficacy of hyaluronic acid binding assay in selecting motile spermatozoa with normal morphology at high magnification

    <p>Abstract</p> <p>Background</p> <p>The present study aimed to evaluate the efficacy of the hyaluronic acid (HA) binding assay in the selection of motile spermatozoa with normal morphology at high magnification (8400x).</p> <p>Methods</p> <p>A total of 16592 prepared spermatozoa were selected and classified into two groups: Group I, spermatozoa which presented their head attached to an HA substance (HA-bound sperm), and Group II, those spermatozoa that did not attach to the HA substance (HA-unbound sperm). HA-bound and HA-unbound spermatozoa were evaluated according to the following sperm forms: 1-Normal morphology: normal nucleus (smooth, symmetric and oval configuration, length: 4.75+/-2.8 μm and width: 3.28+/-0.20 μm, no extrusion or invagination and no vacuoles occupied more than 4% of the nuclear area) as well as acrosome, post-acrosomal lamina, neck, tail, besides not presenting a cytoplasmic droplet or cytoplasm around the head; 2-Abnormalities of nuclear form (a-Large/small; b-Wide/narrow; c-Regional disorder); 3-Abnormalities of nuclear chromatin content (a-Vacuoles: occupy >4% to 50% of the nuclear area and b-Large vacuoles: occupy >50% of the nuclear area) using a high magnification (8400x) microscopy system.</p> <p>Results</p> <p>No significant differences were obtained with respect to sperm morphological forms and the groups HA-bound and HA-unbound. 1-Normal morphology: HA-bound 2.7% and HA-unbound 2.5% (P = 0.56). 2-Abnormalities of nuclear form: a-Large/small: HA-bound 1.6% vs. HA-unbound 1.6% (P = 0.63); b-Wide/narrow: HA-bound 3.1% vs. HA-unbound 2.7% (P = 0.13); c-Regional disorders: HA-bound 4.7% vs. HA-unbound 4.4% (P = 0.34). 3. Abnormalities of nuclear chromatin content: a-Vacuoles >4% to 50%: HA-bound 72.2% vs. HA-unbound 72.5% (P = 0.74); b-Large vacuoles: HA-bound 15.7% vs. HA-unbound 16.3% (P = 0.36).</p> <p>Conclusions</p> <p>The findings suggest that HA binding assay has limited efficacy in selecting motile spermatozoa with normal morphology at high magnification.</p

    Weight Loss after Roux-en-Y Gastric Bypass in Obese Patients Heterozygous for MC4R Mutations

    BackgroundHeterozygous mutations in melanocortin-4 receptor (MC4R) are the most frequent genetic cause of obesity. Bariatric surgery is a successful treatment for severe obesity. The mechanisms of weight loss after bariatric surgery are not well understood.MethodsNinety-two patients who had Roux-en-Y gastric bypass (RYGB) surgery were screened for MC4R mutations. We compared percent excess weight loss (%EWL) in the four MC4R mutation carriers with that of two control groups: 8 matched controls and with the remaining 80 patients who underwent RYGB.ResultsFour patients were heterozygous for functionally significant MC4R mutations. In patients with MC4R mutations, the %EWL after RYGB (66% EWL) was not significantly different compared to matched controls (70% EWL) and non-matched controls (60% EWL) after 1 year of follow-up.ConclusionsThis study suggests that patients with heterozygous MC4R mutations also benefit from RYGB and that weight loss may be independent of the presence of such mutations
