    Full-thickness bilateral rotator cuff tears as a result of a bench-pressing accident: case report and literature review of treatment of bilateral rotator cuff tears

    Rotator cuff injuries are frequent pathologies presenting to orthopedic surgeons. These injuries especially occur among older adults due to intrinsic or extrinsic degeneration. They can however present in young athletes, but as result of different etiologies. Overhead athletes may incur rotator cuff injuries due to repetitive trauma. Bilateral simultaneous traumatic shoulder dislocations have been reported in the literature following acute trauma or weight-lifting activity, but bilateral traumatic rotator cuff tears following bench pressing is an unusual presentation in a young individual. To our knowledge, there has been no previous report describing this injury. This article presents a case of a young male athlete who had bilateral rotator cuff tears after a barbell bench press. Both shoulders were treated operatively in a sequential manner, three months apart, and the patient regained excellent functional status 24 months postoperatively

    High frequency of carbapenem-resistant Acinetobacter baumannii in patients with diabetes mellitus in Saudi Arabia

    Carbapenem-resistant Acinetobacter baumannii is becoming increasingly prevalent in patients with diabetes mellitus in the Middle East. We examined the relationship of these bacteria and their resistance mechanisms to the diabetic disease status of patients in Saudi Arabia. Susceptibilities of 271 isolates to carbapenems, tigecycline and colistin were determined, followed by detection of carbapenemase genes. A blaVIM gene was detected in ~95 % of isolates; blaOXA-23 and blaOXA-40 genes were also prevalent. Diabetic patients were significantly more likely to carry carbapenem-resistant isolates. Carbapenem-resistant A. baumannii is a serious problem in diabetic patients, and molecular detection of resistance mechanisms in these isolates is required

    The Hyades Kinematical Structure with Gaia Era

    268-273In this work, we have improved the Hyades members with crossmatch between Hipparcos and the recent Gaia EDR3 source, the obtained members with highly probable are about 186 candidates. Considering the classical convergent point and depending on proper motions and radial velocities, we have computed the apex position A, D 93. 36 0. 046, 7. 43 0. 713 which is in line with others. The internal structural parameters of the Hyades open cluster are demonstrated here with space spatial velocities; i.e., , V, V; km s (-5.97±0.41, 45.54±6.75, 5.52±0.43) and , V, W ; km s (-42.11±6.50, -19.09±4.37, -1.32±0.44) and on basis of matrix elements μ, the Velocity Ellipsoid Parameters were achieved, e.g., λ, λ, λ; km s 2137.36 23.12, 6.06 0.41, 2.53 0.63 and σ, σ, σ; km s 46.23 6.80, 2.47 0.64, 1.59 0.80. For the observational quantities, we have deduced a correlation coefficient of about 0.83 for the kinematical property of proper motions on both sides μ cos δ , μ; mas yr and the physical property with the angular distances λ from the vertex, and those prove that the attributes are completely related linearly

    Post-COVID-19 fatigue and health-related quality of life in Saudi Arabia: a population-based study

    BackgroundDespite substantial literature on symptoms and long-term health implications associated with COVID-19; prevalence and determinants of post-acute COVID-19 fatigue (PCF) remain largely elusive and understudied, with scant research documenting health-related quality of life (HRQoL). Hence, prevalence of PCF and its associated factors, and HRQoL among those who have survived Covid-19 within the general population of Saudi Arabia (KSA) is the subject under examination in this research.MethodsThis cross-sectional study was conducted on 2063 individuals, selected from the KSA’s general population, using a non-probability sampling approach. An online survey was used to employ a self-administered questionnaire to the participants, which included socio-demographic information, the patient’s COVID-19 infection history, 12-item Short Form Health Survey (SF-12) to assess quality of life, and Chalder Fatigue Scale (CFS) (CFQ 11) to evaluate the extent and severity of fatigue. Data were analyzed using SPSS version 26. A p < 0.05 was considered to be strong evidence against the null hypothesis.ResultsThe median age of participants was 34 (IQR = 22) years, with females comprising the majority (66.2%). According to the SF-12 questionnaire, 91.2% of patients experienced physical conditions, and 77% experienced depression. The prevalence of PCF was 52% on CFQ 11 scale. Female gender, higher levels of education, a pre-existing history of chronic disease, as well as the manifestations of shortness of breath and confusion during acute COVID-19 infection, were identified as independent predictors of fatigue.ConclusionTo facilitate timely and effective intervention for post-acute COVID-19 fatigue, it is essential to continuously monitor the individuals who have recovered from acute COVID-19 infection. Also, it is critical to raise health-education among these patients to improve their quality of life. Future research is required to determine whether COVID-19 survivors would experience fatigue for an extended duration and the impact of existing interventions on its prevalence and severity

    Oral manifestations of systemic disease

    While the majority of disorders of the mouth are centred upon the direct action of plaque, the oral tissues can be subject to change or damage as a consequence of disease that predominantly affects other body systems. Such oral manifestations of systemic disease can be highly variable in both frequency and presentation. As lifespan increases and medical care becomes ever more complex and effective it is likely that the numbers of individuals with oral manifestations of systemic disease will continue to rise. The present article provides a succinct review of oral manifestations of systemic disease. In view of this article being part of a wider BDJ themed issue on the subject of oral medicine, this review focuses upon oral mucosal and salivary gland disorders that may arise as a consequence of systemic disease

    Pioneer School Effectiveness and Improvement in Saudi Arabia: the Case of the Secondary Educational Institutions

    This study, Pioneer Schools Effectiveness and Improvement in Saudi Arabia: The Case of the Secondary Educational Institutions, is the first systematic investigation of school effectiveness (SE) and school improvement (SI) in relation to the Kingdom’s ambitious Pioneer Secondary Schools Programme (PSP) first introduced in 2000. It selected all eight boys’ Pioneer Schools in the Educational District of Al Madinah Al Munawwarah, as a case study to determine, a decade after its inception, how four key groups now understand and describe the attributes of an effective school: principals, teachers, students and parents. Its unique approach is to utilise a mixed research method by combining both quantitative statistical analysis and qualitative approaches, and using hermeneutics in the latter in order to triangulate the findings. The study departed from the once traditional approach which relied heavily on quantified test results or achievement scores to determine SE and methods of SI. Instead, this study posed three fundamental research questions ad generated lists of identifiable indices of priorities and outlooks of the four respondent groups in relation to SESI for the schools in question. The findings of this study consistently show that, from the perspective of those surveyed and interviewed, much more has to be done in pioneer schools in Saudi Arabia before they can be considered truly effective schools in international terms. Moreover, the discussion of the data generated draws the further conclusion that international educational research on SESI issues points to a much more involved and sophisticated process than is suggested by the priorities and outlook of the respondents of this study in the Kingdom of Saudi Arabia

    Unusual presentation of medullary thyroid carcinoma: Regional and distant metastases in the background of benign non-neoplastic Multi-Nodular Goiter

    Background: Medullary Thyroid Carcinoma (MTC) is a malignancy that primarily arises from the C-cells of the thyroid gland with a tendency to regional and distant metastases. Methods: We report a case of a 34-year-old female with MTC where there was no evidence of primary foci of the disease within the thyroid gland, while there was a widespread metastasis in the cervical lymph nodes, lungs, abdomen, and vertebrae. Results: The patient was diagnosed with metastatic sporadic MTC based on the pre-operative work up. She underwent completion thyroidectomy with bilateral neck dissection levels II-VI. The post-operative histopathology report confirmed the diagnosis. Conclusion: The case report shows an unusual presentation of MTC. It demonstrates that there is an entity of metastatic sporadic MTC, confirmed by an abnormal calcitonin level, with unknown primary
