    Stability control of nonlinear micromechanical resonators under simultaneous primary and superharmonic resonances

    Fast effects of a slow excitation on the main resonance of a nonlinear micromechanical resonator are analytically and experimentally investigated. We show, in particular, how the bifurcation topology of an undesirable unstable behavior is modified when the resonator is simultaneously actuated at its primary and superharmonic resonances. A stabilization mechanism is proposed and demonstrated by increasing the superharmonic excitation

    Extravasation of contrast medium during CT examination: An observational casecontrol study

    Introduction: extravasation is an adverse reaction to intravenous injection of contrast medium (CM) during CT examination. The objectives ofthis study are to determine the frequency, management and outcomes of extravasations and to assess risk factors for extravasation. Methods:every incident of extravasation which occurred between March 2012 and March 31, 2013 was recorded in an extravasation form. Ethics Committeeapproval was obtained and the patients gave their consent to participate in the study. Data collected in the form included patients' age, sex,  comorbidities, symptoms, CM used, injection mode, site and rate,  extravasated volume, location of extravasation, severity of injury,  treatment and patient outcome. Each case was matched with 4 controls of the same age ± 5 years and the same gender when possible.Results: extravasation occurred in 18 (7 women, 11 men) out of 2,000 injections of CM (0.9%) with a median age of 53 (10-78) years. Automated injection was performed in all cases with a mean rate of 1.7ml/s. Large  extravasated volumes (B 50ml) were more observed in patients  undergoing CT angiography (28.6% vs. 6.6%, although not significant P=0.112). Multivariate analysis revealed a significant association between patients with cardiac diseases and extravasation (OR: 7.3, 95% CI (1.09-49.05), P=0.04) whereas the injection rate is a protective factor from extravasation (P=0.002).Conclusion: extravasation of CM results in mild to moderate adverse effects in all cases. Our study suggests that patients with cardiac disease are more predisposed to contrast extravasation than others. Further and larger studies are needed to confirm this trend

    Functional ultrastructure and cytochemistry of vitellogenesis and mature vitellocytes of the digenean Cainocreadium labracis (Dujardin, 1845), parasite of Dicentrarchus labrax (L., 1758)

    Vitellogenesis and vitellocytes of Cainocreadium labracis were studied by transmission electron microscopy (TEM) and TEM cytochemistry. Four developmental stages were distinguished during vitellogenesis: (I) stem cell of high nucleo-cytoplasmic ratio; (II) early differentiation with chief activity focused on the beginning of protein synthesis and shell globule formation; (III) advanced differentiation with rapid intensification of protein synthesis, progressive fusion of single shell globules into large globule clusters, and formation of unsaturated lipid droplets surrounded by β-glycogen particles; and (IV) mature vitellocyte. Early vitellogenesis with vitellocyte maturation consists of: (1) increase in cell volume; (2) increased development of large, parallel cisternae of GER with production of proteinaceous granules; (3) development of small Golgi complexes that package granules; and (4) within vacuoles, progressive enlargement of proteinaceous granules into shell globule clusters formed during vitellogenesis. Three types of inclusions accumulate in large amounts in mature vitelline cells: (1) shell globule clusters, importantcomponentintheformationofeggshell;(2)numerousunsaturatedlipiddroplets.Thoughfewer,therearealsodiphasic droplets consisting of saturated and unsaturated lipids in the same droplet, and (3) a relatively small amount of β-glycogen particles, usually surround a few groups of lipid droplets. The β-glycogen and lipid droplets are nutritive reserves for embryogenesis. General pattern and functional ultrastructure of vitellogenesis greatly resemble those observed in some lower cestodes, such as bothriocephalideans and diphyllobothrideans. Variations and differences in the amount of lipids and of glycogen during vitellogenesis in lower cestodes and other trematodes are compared and discussed

    From MEMS to NEMS: closed-loop actuation of resonant beams beyond the critical Duffing amplitude

    International audienceBecause of its moderate cost in terms of electronics, resonant sensing has become commonplace in the context of MEMS and NEMS devices. It is usual to drive such resonators below the critical open-loop Duffing amplitude, above which the oscillations become unstable. However, when scaling sensors down to NEMS, nonlinearities may occur at very low amplitudes, making oscillations very difficult to detect. This paper describes a very general way to compute the critical amplitude in open-loop operation for beam resonators, before it focuses on closed-loop Duffing-type resonators. The major contribution of this paper is the use of describing function analysis validated by numerical simulations to show that it is possible to obtain stable oscillations with amplitudes much larger than the critical Duffing amplitude. As a practical consequence, the measured currents are significantly increased and the constraints on the sensing electronics can be relaxed

    Tumeur maligne des gaines nerveuses périphériques révélant une neurofibromatose type 1

    La Neurofibromatose type 1 (NF1) ou maladie de Von Recklinghausen est une affection à expression clinique variable. La transformation maligne est rare et redoutable. Le but de ce travail est d’étudier les critères diagnostiques ainsi que la prise en charge thérapeutique de cette transformation maligne. Observation : A.B, âgé de 16 ans, a consulté pour une tuméfaction basicervicale antérieure d’apparition récente, ayant augmenté progressivement de taille sans signes de compression ni de dysthyroïdie. L’examen clinique a trouvé une masse dure, bien limitée, indolore, de 4 cm de grand axe siégeant en regard du manubrium sternal, adhérente au plan profond et débordant sur le creux sus sternal. A cette masse s’associaient des taches cutanées café au lait disséminées sur tout le corps, des nodules de Lish iriens et des neurofibromes occipitaux et parotidiens. Après une exérèse de la masse par voie cervicale, l’examen histologique a conclu à une tumeur maligne de la gaine nerveuse. Le malade a eu dans un 2ème temps, une exérèse plus large emportant le manubrium sternal et les tissus adjacents  avec une reconstruction par des fragments de côtes mais avec des limites chirurgicales envahies. L’évolution a été marquée par une poursuite évolutive précoce imposant une exérèse chirurgicale plus large suivie d’une chimiothérapie et d’une radiothérapie externe (RT). Le malade était cependant toujours en poursuite évolutive et le décès est survenu dans un contexte d'envahissement médiastinal.Mots-clés : Neurofibromatose type 1, Sarcome, tissus mous, gaines nerveuses périphériques, Chirurgie

    Appendagite épiploïque primitive: à propos de cinq cas

    La torsion de frange épiploïque (ou appendagite) est une pathologie rare qui survient principalement chez les adultes entre 20 et 50 ans.L'incidence de cette pathologie n'est pas réellement connue et elle varie de 2 à 7% chez les patients hospitalisés pour suspicion d'appendicite oude sigmoïdite. Nous rapportons cinq cas d'appendagites dont nous précisons les particularités cliniques, radiologiques et thérapeutiques. Il s'agit de 5 patients dont l'âge moyen est de 34.6 ans (24-55). Le sexe ratio est de 1.5. Le principal motif de consultation était un syndrome douloureux de l'abdomen principalement au niveau de la fosse iliaque droite. L'examen abdominal montrait toujours une sensibilité localisée. La fièvre était présente chez 3 patients. Le bilan biologique révèle un syndrome inflammatoire biologique chez trois patients. Les examens complémentaires radiologiques en particulier échographie abdominale et TDM abdominale ont éliminé formellement une urgence chirurgicale et ont évoqué le diagnostic d'appendagite dans trois cas. Trois patients ont bénéficié d'une coelioscopie diagnostique confirmant le diagnostic  d'appendagite. L'évolution était favorable chez tous les patients. Les appendagites épiploïques primitives sont des étiologies rares et  sous-estimées de syndrome abdominal aigu. Le diagnostic peut être affirmé par imagerie notamment avec le scanner hélicoïdal injecté,  permettant d'instaurer ainsi un traitement médical premier et d'éviter un traitement chirurgical et des hospitalisations excessives

    Prevalence of Mood Disorders and Associated Factors at the Time of the COVID-19 Pandemic: Potocol for a Community Survey in La Manouba Governorate, Tunisia

    Aims: The present survey aims to assess the overall mood disorder prevalence and identify associated socio-demographic and clinical factors in a Tunisian community sample, with special attention to the COVID-19 pandemic. Background: Mood disorders are one of the leading causes of all non-fatal burdens of disease, with depression being at the top of the list. The COVID-19 pandemic may have increased the prevalence of mood disorders, especially in Low and Middle-income countries (LMICs) and in vulnerable populations. Objective: 1/ Assess point and lifetime prevalence of depressive and bipolar disorders as well as subthreshold bipolarity in a representative population sample of La Manouba governorate and assess treatment patterns for these disorders; 2/Study socio-demographic and clinical correlates of mood disorders 3/ Assess the association between mood disorders and quality of life 4/ Study the impact of the COVID-pandemic on the prevalence of mood disorders 5/ Assess coping mechanisms to the COVID-pandemic and whether these mechanisms moderate the appearance of mood disorders or symptoms since the beginning of the pandemic Methods: This is a household cross-sectional observational survey to be conducted in La Manouba Governorate in a sample of 4540 randomly selected individuals aged ≥ 15 years. Data collection will be carried out by trained interviewers with clinical experience, through face-to-face interviews and the use of the computer assisted personal interviewing approach (CAPI). The following assessment tools are administered: Results: Structured clinical Interview for DSM IV-TR (Mood disorder section and Screening questions on Anxiety), Mood Disorder Questionnaire (MDQ), Suicide Behaviors Questionnaire-Revised (SBQ), 12-item Short Form Survey (SF-12), the Brief-COPE, and a questionnaire about a headache. In addition, socio-demographic and clinical data will be collected. Conclusion: This will be one of the very few household surveys in a general population sample to assess mental health problems and COVID-19-related variables since the beginning of the pandemic. Through this research, we aim to obtain an epidemiological profile of mood disorders in Tunisia and an estimation of the impact of the COVID-19 pandemic on their prevalence. Results should contribute to improving mental health care in Tunisia

    Formability analysis of pre-strained AA5754-O sheet metal using Yld96 plasticity theory: Role of amount and direction of uni-axial pre-strain

    Automotive industries are very much interested in formability of different pre-strained aluminum alloy sheets in the context of multistage stamping to fabricate complex components. In the present work, different uni-axial pre-strains of 6.4% and 12.2% were induced in AA5754-O aluminum alloy both along rolling direction (RD) and transverse direction (TD). The true stress-strain response, limiting dome height (LDH) and strain based forming limit diagram (ε-FLD) of as received and all pre-strained materials were evaluated experimentally. The anisotropy constitutive material model was developed using the Yld96 plasticity theory in-conjunction with the Hollomon isotropic hardening law to predict the yield strength evolution of the pre-strained materials. Also, it was found that the limiting strains in ε-FLD shifted significantly depending on the amount and direction of uni-axial pre-strain. Hence, the limiting strains of the as-received materials were transposed into stress space to estimate the stress based forming limit diagram (σ-FLD) using the anisotropy constitutive material model. Further, the dynamic shifts of ε-FLDs of four different pre-strained materials were predicted by successfully decoupling the σ-FLD of as-received materials within root mean square error of 0.008. Finite element models of both uni-axial pre-straining and subsequent LDH tests were developed, and the forming behavior of the pre-strained materials were predicted implementing the Yld96 plasticity model and estimated σ-FLD. It was found that LDH was significantly influenced by the amount of pre-strain, and the maximum thinning location shifted close to pole in the case of 12.2% pre-strained materials. However, the effect of uni-axial pre-strain direction on both LDH and maximum thinning location in AA5754-O material was very negligible
