    Effect of statins on venous thromboembolic events: a meta-analysis of published and unpublished evidence from randomised controlled trials

    Background - It has been suggested that statins substantially reduce the risk of venous thromboembolic events. We sought to test this hypothesis by performing a meta-analysis of both published and unpublished results from randomised trials of statins. Methods and Findings - We searched MEDLINE, EMBASE, and Cochrane CENTRAL up to March 2012 for randomised controlled trials comparing statin with no statin, or comparing high dose versus standard dose statin, with 100 or more randomised participants and at least 6 months' follow-up. Investigators were contacted for unpublished information about venous thromboembolic events during follow-up. Twenty-two trials of statin versus control (105,759 participants) and seven trials of an intensive versus a standard dose statin regimen (40,594 participants) were included. In trials of statin versus control, allocation to statin therapy did not significantly reduce the risk of venous thromboembolic events (465 [0.9%] statin versus 521 [1.0%] control, odds ratio [OR] = 0.89, 95% CI 0.78–1.01, p = 0.08) with no evidence of heterogeneity between effects on deep vein thrombosis (266 versus 311, OR 0.85, 95% CI 0.72–1.01) and effects on pulmonary embolism (205 versus 222, OR 0.92, 95% CI 0.76–1.12). Exclusion of the trial result that provided the motivation for our meta-analysis (JUPITER) had little impact on the findings for venous thromboembolic events (431 [0.9%] versus 461 [1.0%], OR = 0.93 [95% CI 0.82–1.07], p = 0.32 among the other 21 trials). There was no evidence that higher dose statin therapy reduced the risk of venous thromboembolic events compared with standard dose statin therapy (198 [1.0%] versus 202 [1.0%], OR = 0.98, 95% CI 0.80–1.20, p = 0.87). Risk of bias overall was small but a certain degree of effect underestimation due to random error cannot be ruled out. Please see later in the article for the Editors' Summary. Conclusions - The findings from this meta-analysis do not support the previous suggestion of a large protective effect of statins (or higher dose statins) on venous thromboembolic events. However, a more moderate reduction in risk up to about one-fifth cannot be ruled out

    Bessere Verlaufsdokumentation bei Patienten mit lumboradikulÀren Schmerzsyndromen: Der modifizierte NASS-Fragebogen

    Zusammenfassung: Hintergrund: Zweck dieser Studie war es, ein zuverlĂ€ssiges und einfach anzuwendendes Instrument zur Erfassung des Krankheitsverlaufs und des Therapieerfolgs bei RĂŒckenschmerzpatienten mit lumboradikulĂ€ren Syndromen vorzustellen. Methodik: Es wurden Daten von Patienten, welche sich einer Mikrodiskektomie wegen lumboradikulĂ€rem Syndrom unterzogen, in die Studie eingeschlossen und mittels des 17-teiligen NASS-Fragebogens (North American Spine Society) in Form eines Interviews vor und nach dem operativen therapeutischen Eingriff befragt. Außerdem wurden die demographischen Daten und KomorbiditĂ€ten erhoben. Über die Auswertung der EffektstĂ€rke bzw. Standardized Response Mean konnten die Fragen mit der stĂ€rksten VerĂ€nderung (vorher/nachher) fĂŒr das Behandlungsergebnis ausgewĂ€hlt werden. Ergebnisse: Es konnten die DatensĂ€tze von 139Patienten ausgewertet werden. Aus den 3Dimensionen Schmerz, neurologische Symptome und BeeintrĂ€chtigung im Alltag wurden jeweils diejenigen Fragen mit der höchsten VerĂ€nderung ausgewĂ€hlt (hohe E.S. bzw. S.R.M.). In Abstimmung mit der klinischen Relevanz ergab sich schließlich die Auswahl von 8Fragen als konzentrierte Kurzform des NASS-Fragebogens. Schlussfolgerung: In dem von uns vorgestellten Instrument eines kurzen und aussagekrĂ€ftigen Fragebogens sehen wir eine nĂŒtzliche und in der Klinik einfach einzusetzende Möglichkeit zur verbesserten Dokumentation bei Patienten mit lumboradikulĂ€ren Syndromen, insbesondere auch zur Effektmessung therapeutischer Interventionen im Sinne einer patientenorientierten ErgĂ€nzung der klinischen Befunderhebung und Diagnostik. Dieses neue Instrument könnte helfen, die QualitĂ€tssicherung bei der konservativen und interventionellen Schmerzbehandlung der Patienten mit lumboradikulĂ€ren Schmerzsyndromen zu verbesser

    Demonstration of Quadrature Squeezed Surface-Plasmons in a Gold Waveguide

    We report on the efficient generation, propagation, and re-emission of squeezed long-range surface-plasmon polaritons (SPPs) in a gold waveguide. Squeezed light is used to excite the non-classical SPPs and the re-emitted quantum state is fully quantum characterized by complete tomographic reconstruction of the density matrix. We find that the plasmon-assisted transmission of non-classical light in metallic waveguides can be described by a Hamiltonian analogue to a beam splitter. This result is explained theoretically

    Solid-state laser system for laser cooling of Sodium

    We demonstrate a frequency-stabilized, all-solid laser source at 589 nm with up to 800 mW output power. The laser relies on sum-frequency generation from two laser sources at 1064 nm and 1319 nm through a PPKTP crystal in a doubly-resonant cavity. We obtain conversion efficiency as high as 2 W/W^2 after optimization of the cavity parameters. The output wavelength is tunable over 60 GHz, which is sufficient to lock on the Sodium D2 line. The robustness, beam quality, spectral narrowness and tunability of our source make it an alternative to dye lasers for atomic physics experiments with Sodium atoms

    Biological measurement beyond the quantum limit

    Quantum noise places a fundamental limit on the per photon sensitivity attainable in optical measurements. This limit is of particular importance in biological measurements, where the optical power must be constrained to avoid damage to the specimen. By using non-classically correlated light, we demonstrated that the quantum limit can be surpassed in biological measurements. Quantum enhanced microrheology was performed within yeast cells by tracking naturally occurring lipid granules with sensitivity 2.4 dB beyond the quantum noise limit. The viscoelastic properties of the cytoplasm could thereby be determined with a 64% improved measurement rate. This demonstration paves the way to apply quantum resources broadly in a biological context

    The number of transmission channels through a single-molecule junction

    We calculate transmission eigenvalue distributions for Pt-benzene-Pt and Pt-butadiene-Pt junctions using realistic state-of-the-art many-body techniques. An effective field theory of interacting π\pi-electrons is used to include screening and van der Waals interactions with the metal electrodes. We find that the number of dominant transmission channels in a molecular junction is equal to the degeneracy of the molecular orbital closest to the metal Fermi level.Comment: 9 pages, 8 figure

    A sex-chromosome mutation in Silene latifolia

    Silene latifolia is dioecious, yet rare hermaphrodites have been found, and such natural mutants can provide valuable insight into genetic mechanisms. Here, we describe a hermaphrodite-inducing mutation that is almost certainly localized to the gynoecium-suppression region of the Y chromosome in S. latifolia. The mutant Y chromosome was passed through the megaspore, and the presence of two X chromosomes was not necessary for seed development in the parent. This result supports a lack of degeneration of the Y chromosome in S. latifolia, consistent with the relatively recent formation of the sex chromosomes in this species. When crossed to wild-type plants, hermaphrodites performed poorly as females, producing low seed numbers. When hermaphrodites were pollen donors, the sex ratio of offspring they produced through crosses was biased towards females. This suggests that hermaphroditic S. latifolia would fail to thrive and potentially explains the rarity of hermaphrodites in natural populations of S. latifolia. These results indicate that the Y chromosome in Silene latifolia remains very similar to the X, perhaps mostly differing in the primary sex determination regions

    Quantum coherent control of highly multipartite continuous-variable entangled states by tailoring parametric interactions

    The generation of continuous-variable multipartite entangled states is important for several protocols of quantum information processing and communication, such as one-way quantum computation or controlled dense coding. In this article we theoretically show that multimode optical parametric oscillators can produce a great variety of such states by an appropriate control of the parametric interaction, what we accomplish by tailoring either the spatio-temporal shape of the pump, or the geometry of the nonlinear medium. Specific examples involving currently available optical parametric oscillators are given, hence showing that our ideas are within reach of present technology.Comment: 14 pages, 5 figure

    Diversity, migration routes, and worldwide population genetic structure of Lecanosticta acicola, the causal agent of brown spot needle blight

    Lecanosticta acicola is a pine needle pathogen causing brown spot needle blight that results in premature needle shedding with considerable damage described in North America, Europe, and Asia. Microsatellite and mating type markers were used to study the population genetics, migration history, and reproduction mode of the pathogen, based on a collection of 650 isolates from 27 countries and 26 hosts across the range of L. acicola. The presence of L. acicola in Georgia was confirmed in this study. Migration analyses indicate there have been several introduction events from North America into Europe. However, some of the source populations still appear to remain unknown. The populations in Croatia and western Asia appear to originate from genetically similar populations in North America. Intercontinental movement of the pathogen was reflected in an identical haplotype occurring on two continents, in North America (Canada) and Europe (Germany). Several shared haplotypes between European populations further suggests more local pathogen movement between countries. Moreover, migration analyses indicate that the populations in northern Europe originate from more established populations in central Europe. Overall, the highest genetic diversity was observed in south-eastern USA. In Europe, the highest diversity was observed in France, where the presence of both known pathogen lineages was recorded. Less than half of the observed populations contained mating types in equal proportions. Although there is evidence of some sexual reproduction taking place, the pathogen spreads predominantly asexually and through anthropogenic activity
