    The Need for a Specific Risk Prediction System in Native Valve Infective Endocarditis Surgery

    The need for a specific risk score system for infective endocarditis (IE) surgery has been previously claimed. In a single-center pilot study, preliminary to future multicentric development and validation, bivariate and multivariate (logistic regression) analysis of early postoperative mortality predictors in 440 native valve IE patients were performed. Mathematical procedures assigned scores to the independent predictors emerged (AUC of the ROC curve: 0.88). Overall mortality was 9.1%. Six predictors were identified and assigned scores, including age (5–13 points), renal failure (5), NYHA class IV (9), critical preoperative state (11), lack of preoperative attainment of blood culture negativity (5), perivalvular involvement (5). Four risk classes were drawn ranging from “very low risk” (≀5 points, mean predicted mortality 1%), and to “very high risk” (≄20 points, 43% mortality). IE-specific risk stratification models are both needed, as disease-specific factors (e.g., cultures, abscess), beside the generic ones (e.g., age, renal impairment) affect mortality, and feasible

    Second-order L2L^2-regularity in nonlinear elliptic problems

    A second-order regularity theory is developed for solutions to a class of quasilinear elliptic equations in divergence form, including the pp-Laplace equation, with merely square-integrable right-hand side. Our results amount to the existence and square integrability of the weak derivatives of the nonlinear expression of the gradient under the divergence operator. This provides a nonlinear counterpart of the classical L2L^2-coercivity theory for linear problems, which is missing in the existing literature. Both local and global estimates are established. The latter apply to solutions to either Dirichlet or Neumann boundary value problems. Minimal regularity on the boundary of the domain is required. If the domain is convex, no regularity of its boundary is needed at all

    A public early intervention approach to first-episode psychosis: Treated incidence over 7 years in the Emilia-Romagna region

    AimTo estimate the treated incidence of individuals with first-episode psychosis (FEP) who contacted the Emilia-Romagna public mental healthcare system (Italy); to examine the variability of incidence and user characteristics across centres and years. MethodsWe computed the raw treated incidence in 2013-2019, based on FEP users aged 18-35, seen within or outside the regional program for FEP. We modelled FEP incidence across 10 catchment areas and 7 years using Bayesian Poisson and Negative Binomial Generalized Linear Models of varying complexity. We explored associations between user characteristics, study centre and year comparing variables and socioclinical clusters of subjects. ResultsThousand three hundred and eighteen individuals were treated for FEP (raw incidence: 25.3 / 100.000 inhabitant year, IQR: 15.3). A Negative Binomial location-scale model with area, population density and year as predictors found that incidence and its variability changed across centres (Bologna: 36.55; 95% CrI: 30.39-43.86; Imola: 3.07; 95% CrI: 1.61-4.99) but did not follow linear temporal trends or density. Centers were associated with different user age, gender, migrant status, occupation, living conditions and cluster distribution. Year was associated negatively with HoNOS score (R = -0.09, p < .001), duration of untreated psychosis (R = -0.12, p < .001) and referral type. ConclusionsThe Emilia-Romagna region presents a relatively high but variable incidence of FEP across areas, but not in time. More granular information on social, ethnic and cultural factors may increase the level of explanation and prediction of FEP incidence and characteristics, shedding light on social and healthcare factors influencing FEP

    In Vivo Targeting Replication Protein A for Cancer Therapy

    Replication protein A (RPA) plays essential roles in DNA replication, repair, recombination, and the DNA damage response (DDR). Retrospective analysis of lung cancer patient data demonstrates high RPA expression as a negative prognostic biomarker for overall survival in smoking-related lung cancers. Similarly, relative expression of RPA is a predictive marker for response to chemotherapy. These observations are consistent with the increase in RPA expression serving as an adaptive mechanism that allows tolerance of the genotoxic stress resulting from carcinogen exposure. We have developed second-generation RPA inhibitors (RPAis) that block the RPA-DNA interaction and optimized formulation for in vivo analyses. Data demonstrate that unlike first-generation RPAis, second-generation molecules show increased cellular permeability and induce cell death via apoptosis. Second-generation RPAis elicit single-agent in vitro anticancer activity across a broad spectrum of cancers, and the cellular response suggests existence of a threshold before chemical RPA exhaustion induces cell death. Chemical RPA inhibition potentiates the anticancer activity of a series of DDR inhibitors and traditional DNA-damaging cancer therapeutics. Consistent with chemical RPA exhaustion, we demonstrate that the effects of RPAi on replication fork dynamics are similar to other known DDR inhibitors. An optimized formulation of RPAi NERx 329 was developed that resulted in single-agent anticancer activity in two non-small cell lung cancer models. These data demonstrate a unique mechanism of action of RPAis eliciting a state of chemical RPA exhaustion and suggest they will provide an effective therapeutic option for difficult-to-treat lung cancers

    Landuse and soil degradation in the southern Maya lowlands, from Pre-Classic to Post-Classic times : The case of La Joyanca (Petén, Guatemala)

    International audienceThis work focuses on the impact of Maya agriculture on soil degradation. In site and out site studies in the area of the city of La Joyanca (NW Petén) show that "Maya clays" do not constitute a homogeneous unit, but represent a complex sedimentary record. A high resolution analysis leads us to document changes in rates and practices evolving in time in relation with major socio-political and economic changes. It is possible to highlight extensive agricultural practices between Early Pre-classical to Late Pre-classical times. Intensification occurs in relation with reduction of the fallow duration during Pre-classic to Classic periods. The consequences of these changes on soil erosion are discussed. However, it does not seem that the agronomic potential of the soils was significantly degraded before the end of the Classic period
