    ¿Resistencia en el acné? Un metaanálisis a propósito de la controversia

    ResumenAntecedentesEl acné es una de las afecciones dermatológicas con mayor incidencia a nivel mundial; su origen es multifactorial y, por lo tanto, su tratamiento puede ser complejo. Propionibacterium acnes tiene un papel primordial en la inflamación de esta dermatosis y para su tratamiento se usan antibióticos tópicos; entre los principales se encuentran eritromicina y clindamicina, en los que se ha documentado una amplia resistencia bacteriana, lo que genera controversia respecto a su uso. Por este motivo se presenta un metaanálisis de las publicaciones de los últimos 10 años para confirmar esta hipótesis.Material y métodosEn la literatura de los últimos 10 años se buscaron artículos sobre resultados de cultivos con antibiograma de pacientes con acné. Se realizaron búsquedas tipo MeSH con los términos «acne vulgaris», «Propionibacterium acnes», «topical administration», «treatment», «erythromycin», «clindamycin», «nadifloxacin», «antibacterial agent», «bacterial drug resistance», en los metabuscadores PubMed, Ovid, EBSCO, Cochrane, ScienceDirect y ClinicalKey.ResultadosSe encontraron 13 artículos que cumplieron con los criterios de inclusión. La razón de momios promedio (OR 1.24, IC 95%) de los artículos demostró una ligera tendencia hacia la resistencia de Propionibacterium acnes.ConclusionesSe confirma el aumento en la resistencia bacteriana de Propionibacterium acnés a eritromicina y clindamicina tópica, por lo que recomendamos el uso de estos antibióticos combinados en casos selectos por periodos cortos y en combinación con peróxido de benzoilo, para obtener el mejor resultado clínico en los pacientes con Acne vulgaris.AbstractBackgroundAcne is one of the dermatological pathologies with the highest incidence around the world. It is a multifactorial disease and its treatment can be complex. Propionibacterium acnes play a key role in the inflammation of this dermatosis. Topical antibiotics, including mainly erythromycin and clindamycin, have been used, but there is controversy over their use due to the widely documented bacterial resistance. For this reason a meta-analysis of the publications over the past 10 years is presented in order to confirm this hypothesis.Material and methodsA search was made of the publications over the past 10 years that included the results of antibiogams of patients with acne. MeSH type searches were performed with the terms “acne vulgaris”, “Propionibacterium acnes”, “topical administration”, “treatment”, “erythromycin”, “clindamycin”, “nadifloxacin”, “antibacterial agent”, “bacterial drug resistance” in PubMed, Ovid, EBSCO, Cochrane, ScienceDirect and ClinicalKey meta-searches.ResultsA total of 13 articles were found that met the inclusion criteria. The mean odds ratio (OR 1.24, 95% CI) of the articles showed a slight tendency toward resistance of Propionibacterium acnes.ConclusionsAn increase in bacterial resistance to topical erythromycin and clindamycin can be confirmed, thus the use of these antibiotics is recommended in selective cases for short periods, and in combination with benzoyl peroxide for the best clinical outcome in patients with acne vulgaris

Accumulating evidence links diabetes with severity of AP, suggesting that endogenous insulin may be protective. We investigated this putative protective effect of insulin during cellular and in vivo models of AP in diabetic mice (Ins2Akita) and Pancreatic Acinar cell-specific Conditional Insulin Receptor Knock Out mice (PACIRKO). Caerulein and palmitoleic acid (POA)/ethanol-induced pancreatitis was more severe in both Ins2Akita and PACIRKO vs control mice, suggesting that endogenous insulin directly protects acinar cells in vivo. In isolated pancreatic acinar cells, insulin induced Akt-mediated phosphorylation of 6-phosphofructo-2-kinase/fructose-2,6-biphosphatase 2 (PFKFB2) which upregulated glycolysis thereby preventing POA-induced ATP depletion, inhibition of the ATP-dependent plasma membrane Ca2+ ATPase (PMCA) and cytotoxic Ca2+ overload. These data provide the first mechanistic link between diabetes and severity of AP and suggest that phosphorylation of PFKFB2 may represent a potential therapeutic strategy for treatment of AP

    A numerical model analysis of the tidal flows in the Bay of Algeciras, Strait of Gibraltar

    A numerical model has been applied to study the tidal flows of the Bay of Algeciras in the eastern part of the Strait of Gibraltar, focusing on the M-2 semidiurnal constituent. The numerical model was satisfactorily validated against a comprehensive set of observations collected in the bay in the year 2011 and the model outputs were used for a detailed analysis of the local tidal circulation. The M-2 net (vertically integrated) transport across the mouth of the bay has an amplitude of 2.7 x 10(-3) Sv, while that of the sea surface signal is of similar to 30 cm and is in quadrature with this flow. However, the vertically integrated flow is the result of a pronounced baroclinic structure consisting of an upper (S 37.5) layers, whose associated transports are one order of magnitude higher. This reveals a noticeable internal tide that is characterized by an inward (to the head of the bay) propagation and a likely quarter-wave resonance. During the rising tide, Atlantic water from the strait comes in and produces the thickening of the upper (Atlantic) layer in the bay, while Mediterranean water of the lower layer is pushed out to join the Mediterranean water stream that is flowing to the west along the Strait of Gibraltar. During the falling tide, Atlantic water flows out of the bay and incorporates to the eastward flow in the strait. In this tidal phase, Mediterranean water flows into the bay. Therefore, Atlantic and Mediterranean waters accumulate in the bay during the rising and falling tide, respectively. This pattern is opposite to that observed in the strait, where the Mediterranean layer thickens during the rising tide and becomes thinner during the falling tide. This suggests that the internal tide in the bay is basically determined by the baroclinic forcing at its mouth imposed by the baroclinic tide of the Strait of Gibralta

    Lipasa pancreática es una enzima clave en el metabolismo de lípidos. Los flavonoides son compuestos bioactivos de gran relevancia debido a sus interacciones con enzimas digestivas. Se evaluó la actividad de lipasa pancreática en presencia de flavonoides. Mediante espectroscopía UVVisible se determinó que el mejor inhibidor fue quercetina, seguido de rutina > luteolina > catequina > hesperetina, con valores de IC50 de 10.30, 13.50, 14.70, 28.50 y 30.50 μM, respectivamente. Todos los flavonoides mostraron una inhibición mixta, excepto catequina que mostró una inhibición acompetitiva. La capacidad inhibitoria de los flavonoides se relacionó con propiedades estructurales compartidas entre los distintos flavonoides, como la hidroxilación en las posiciones C5, C7 (anillo A), C2’ y C3’ (anillo B), y el doble enlace entre C2 y C3 (anillo C). Los resultados de inhibición coincidieron con el análisis de la fluorescencia extrínseca. Los estudios de docking molecular indicaron que la interacción entre lipasa pancreática y los flavonoides fue principalmente mediante interacciones hidrofóbicas (pi-stacking). Las interacciones de todos los flavonoides, excepto rutina, se dieron en el mismo sitio (subsitio 1) de la enzima. La insaturación entre C2 y C3 fue determinante para el acomodo de los flavonoides con la enzima, principalmente por interacciones de pi-stacking.ABSTRACTPancreatic lipase is a key enzyme in lipid metabolism. Flavonoids are bioactive compounds obtained from vegetables with big relevance, due to their intrinsic interaction with digestive enzymes. Pancreatic lipase activity was evaluated in the presence of flavonoids, through UV-Vis spectroscopy. All tested flavonoids showed a mixed-type inhibition, except catechin, which showed a uncompetitive inhibition. The best inhibitor was quercetin followed by rutin > luteolin > catechin > hesperetin, with IC50 values of 10.30, 13.50, 14.70, 28.50 and 30.50 μM, respectively. The flavonoids inhibitory capacity was related to structural properties shared between the different flavonoids, such as the hydroxylation at C5, C7 (ring A), C2’ and C3’ (ring B), and the double bond between C2 and C3 (ring C). The inhibition results are in agreement with the extrinsic fluorescence analysis. Molecular docking studies indicated that the interaction between pancreatic lipase and flavonoids was mainly through hydrophobic interactions (pi-stacking). The interactions of all flavonoids, except rutin, occurred at the same enzyme site (subsite 1). Instauration between C2 and C3 was decisive for the arrangement of flavonoids with the enzyme, mainly due to pi-stacking interactions

    El presente proyecto surge de la necesidad de dar continuidad a un trabajo que ya se había iniciado en el año de 1992, el cual ha tenido como objetivos centrales: - Contribuir al cambio social construyendo con el nuevo maestro su papel frente al saber pedagógico. - Reflexionar sobre la educación ambiental como planteamiento integrante e integrador. - Cualificar los procesos de lectura y escritura de los niños del Colegio Fe y Alegría El Limonar. - Establecer relaciones entre la lectura, la escritura y la educación ambiental Este proyecto se desarrolló básicamente en tres niveles; primero se desarrolló el diagnóstico pedagógico de la institución, luego se construyeron y se ejecutaron los proyectos de aula; por último se elaboraron las memorias del trabajo. El trabajo expuesto a continuación se convierte en herramienta de apoyo y en derrotero para futuros proyectos pedagógicos, donde a partir de la lectura y la escritura y la educación ambiental se generen nuevos procesos educativos que contribuyan a mejorar la calidad de la educación

    The involvement of thaumatin-like proteins in plant food cross-reactivity: a multicenter study using a specific protein microarray.

    Cross-reactivity of plant foods is an important phenomenon in allergy, with geographical variations with respect to the number and prevalence of the allergens involved in this process, whose complexity requires detailed studies. We have addressed the role of thaumatin-like proteins (TLPs) in cross-reactivity between fruit and pollen allergies. A representative panel of 16 purified TLPs was printed onto an allergen microarray. The proteins selected belonged to the sources most frequently associated with peach allergy in representative regions of Spain. Sera from two groups of well characterized patients, one with allergy to Rosaceae fruit (FAG) and another against pollens but tolerant to food-plant allergens (PAG), were obtained from seven geographical areas with different environmental pollen profiles. Cross-reactivity between members of this family was demonstrated by inhibition assays. Only 6 out of 16 purified TLPs showed noticeable allergenic activity in the studied populations. Pru p 2.0201, the peach TLP (41%), chestnut TLP (24%) and plane pollen TLP (22%) proved to be allergens of probable relevance to fruit allergy, being mainly associated with pollen sensitization, and strongly linked to specific geographical areas such as Barcelona, Bilbao, the Canary Islands and Madrid. The patients exhibited mayor que50% positive response to Pru p 2.0201 and to chestnut TLP in these specific areas. Therefore, their recognition patterns were associated with the geographical area, suggesting a role for pollen in the sensitization of these allergens. Finally, the co-sensitizations of patients considering pairs of TLP allergens were analyzed by using the co-sensitization graph associated with an allergen microarray immunoassay. Our data indicate that TLPs are significant allergens in plant food allergy and should be considered when diagnosing and treating pollen-food allergy

    In vitro Inhibition of Pancreatic Lipase by Polyphenols: A Kinetic, Fluorescence Spectroscopy and Molecular Docking Study

    Svrha je ovog istraživanja bila ispitati molekulsko uklapanje i inhibicijski učinak četiri fenolna spoja pronađena u ljutim papričicama, i to: kavene kiseline, p-kumarne kiseline, kvercetina i kapsaicina, na aktivnost lipaze izolirane iz svinjske gušterače. Najjači inhibicijski učinak imao je kvercetin (IC50=(6.1±2.4) μM), zatim p-kumarna (170.2±20.6) μM) i kavena kiselina (401.5±32.1) μM), dok su kapsaicin i ekstrakt ljute papričice imali iznimno slab učinak. Svi polifenolni spojevi imali su inhibicijski učinak miješanog tipa. Mjerenjem fluorescencije utvrđeno je da su polifenolni spojevi ugasili prirođenu fluorescenciju lipaze izolirane iz gušterače, i to pomoću statičkog mehanizma. Sekvencija Stern-Volmerove konstante bila je: kvercetin, kavena kiselina, te p-kumarna kiselina. Rezultati ispitivanja molekulskih uklapanja pokazali su da se kavena kiselina, kvercetin i p-kumarna kiselina vežu blizu, za razliku od kapsaicina koji se veže daleko od aktivnog mjesta. Vodikove veze i hidrofobne pi-interakcije glavni su načini međusobnog povezivanja polifenolnih spojeva u lipazi izoliranoj iz gušterače.The inhibitory activity and binding characteristics of caffeic acid, p-coumaric acid, quercetin and capsaicin, four phenolic compounds found in hot pepper, against porcine pancreatic lipase activity were studied and compared to hot pepper extract. Quercetin was the strongest inhibitor (IC50=(6.1±2.4) μM), followed by p-coumaric acid ((170.2±20.6) μM) and caffeic acid ((401.5±32.1) μM), while capsaicin and a hot pepper extract had very low inhibitory activity. All polyphenolic compounds showed a mixed-type inhibition. Fluorescence spectroscopy studies showed that polyphenolic compounds had the ability to quench the intrinsic fluorescence of pancreatic lipase by a static mechanism. The sequence of Stern-Volmer constant was quercetin, followed by caffeic and p-coumaric acids. Molecular docking studies showed that caffeic acid, quercetin and p-coumaric acid bound near the active site, while capsaicin bound far away from the active site.     Shallow magma convection evidenced by excess degassing and thermal radiation during the dome-forming Sabancaya eruption (2012–2020)

    We used a large set of satellite- (visible, infrared, and radar images from Planetscope, MODIS, VIIRS, Sentinel2, Landsat 8, and Sentinel 1) and ground-based data (optical images, SO2 flux, shallow seismicity) to describe and characterize the activity of the Sabancaya volcano during the unrest and eruption phases that occurred between 2012 and 2020. The unrest phase (2012–2016) was characterized by increasing gas and thermal flux, sourced by a convective magma column rising along with the remnants of a buried plug still permeable to fluid flow. Conversely, a new conduit, adjacent to the previous one, fed the eruptive phase (2016–2020) which was instead characterized by a discontinuous extrusive activity, with phases of dome growth (at rates from 0.04 to 0.75 m3 s−1) and collapse. The extrusive activity was accompanied by fluctuating thermal anomalies (0.5–25 MW), by irregular SO2 degassing (700–7000 tons day−1), and by variable explosive activity (4–100 events d−1) producing repeated vulcanian ash plumes (500–5000 m above the crater). Magma budget calculation during the eruptive phase indicates a large excess of degassing, with the volume of degassed magma (0.25–1.28 km3) much higher than the volume of erupted magma (< 0.01 km3). Similarly, the thermal energy radiated by the eruption was much higher than that sourced by the dome itself, an unbalance that, by analogy with the degassing, we define as “excess thermal radiation”. Both of these unbalances are consistent with the presence of shallow magma convection that fed the extrusive and explosive activity of the Sabancaya dome

    Changes in bread consumption and 4-year changes in adiposity in Spanish subjects at high cardiovascular risk

    The effects of bread consumption change over time on anthropometric measures have been scarcely studied. We analysed 2213 participants at high risk for CVD from the PREvención con DIeta MEDiterránea (PREDIMED) trial to assess the association between changes in the consumption of bread and weight and waist circumference gain over time. Dietary habits were assessed with validated FFQ at baseline and repeatedly every year during 4 years of follow-up. Using multivariate models to adjust for covariates, long-term weight and waist circumference changes according to quartiles of change in energy-adjusted white and whole-grain bread consumption were calculated. The present results showed that over 4 years, participants in the highest quartile of change in white bread intake gained 0·76 kg more than those in the lowest quartile (P for trend = 0·003) and 1·28 cm more than those in the lowest quartile (P for trend 2 kg) and gaining waist circumference (>2 cm) during follow-up was not associated with increase in bread consumption, but participants in the highest quartile of changes in white bread intake had a reduction of 33 % in the odds of losing weight (>2 kg) and a reduction of 36 % in the odds of losing waist circumference (>2 cm). The present results suggest that reducing white bread, but not whole-grain bread consumption, within a Mediterranean-style food pattern setting is associated with lower gains in weight and abdominal fat
