Systematic review of the use of data from national childhood obesity surveillance programmes in primary care: a conceptual synthesis
This study reviewed the use in primary care of national surveillance data for children to determine the data's potential utility to inform policy and practice decisions on how to prevent and treat childhood obesity. We reviewed the 28 countries identified by the World Obesity Federation as having high-quality comparable body mass index data for children. Literature published from any period up to December 2013 was included. Peer review literature was searched using Web of Science (Core Collection, MEDLINE). Grey literature was searched using the Internet by country name, programme name and national health and government websites. We included studies that (i) use national surveillance obesity data in primary care, or (ii) explore practitioner or parent perspectives about the use of such data. The main uses of national surveillance data in primary care were to identify and recruit obese children and their parents to participate in school and general practice-based research and/or interventions, and to inform families of children's measurements. Findings indicate a need for school staff and practitioners to receive additional training and support to sensitively communicate with families. Translation of these findings into policy and practice could help to improve current uses of national child obesity surveillance data in primary care
Variation in use of the 2-week referral pathway for suspected cancer: cross-sectional analysis
Background: A 2-Week Wait (2WW) referral pathway for earlier diagnosis of suspected cancer was introduced in England in 2000. Nevertheless, a significant proportion of patients with cancer are diagnosed by other routes (detection rate), only a small proportion of 2WW referrals have cancer (conversion rate) and there is considerable between-practice variation. Aim: This study examined use by practices of the 2WW referral in relation to all cancer diagnoses. Design and setting: A cross-sectional analysis of data extracted from the Cancer Waiting Times Database for all 2WW referrals in 2009 and for all patients receiving a first definitive treatment in the same year. Method: The age standardised referral ratio, conversion rate, and detection rate were calculated for all practices in England and the correlation coefficient for each pair of measures. The median detection rate was calculated for each decile of practices ranked by conversion rate and vice versa, performing nonparametric tests for trend in each case. Results: Data for 8049 practices, 865 494 referrals, and 224 984 cancers were analysed. There were significant correlations between referral ratio and conversion rate (inverse) and detection rate (direct). There was also a direct correlation between conversion and detection rates. There was a significant trend in conversion rate for deciles of detection rate, and vice versa, with a marked difference between the lowest and higher deciles. Conclusion: There is a consistent relationship between 2WW referral conversion rate and detection rate that can be interpreted as representing quality of clinical practice. The 2WW referral rate should not be a measure of quality of clinical care
Heat conduction in 1D lattices with on-site potential
The process of heat conduction in one-dimensional lattice with on-site
potential is studied by means of numerical simulation. Using discrete
Frenkel-Kontorova, --4 and sinh-Gordon we demonstrate that contrary to
previously expressed opinions the sole anharmonicity of the on-site potential
is insufficient to ensure the normal heat conductivity in these systems. The
character of the heat conduction is determined by the spectrum of nonlinear
excitations peculiar for every given model and therefore depends on the
concrete potential shape and temperature of the lattice. The reason is that the
peculiarities of the nonlinear excitations and their interactions prescribe the
energy scattering mechanism in each model. For models sin-Gordon and --4
phonons are scattered at thermalized lattice of topological solitons; for
sinh-Gordon and --4 - models the phonons are scattered at localized
high-frequency breathers (in the case of --4 the scattering mechanism
switches with the growth of the temperature).Comment: 26 pages, 18 figure
Sedimentary cycles in a Mesoproterozoic aeolian erg-margin succession: Mangabeira Formation, Espinhaço Supergroup, Brazil
Aeolian systems were abundant and widespread in the early Proterozoic, post-2.2 Ga. However, the majority of aeolian successions of such great age are intensely deformed and are preserved only in a fragmentary state meaning that, hitherto, few attempts have been made to apply a sequence stratigraphic approach to determine mechanisms of aeolian construction, accumulation and preservation in such systems. The Mangabeira Formation is a well preserved Mesoproterozoic erg successions covering part of the São Francisco Craton, northeastern Brazil. The lower unit of the Mangabeira Formation (~ 500 m thick) comprises aeolian deposits of dune, interdune, and sand-sheet origin, as well as some of waterlain origin. These deposits are organized into vertically stacked depositional cycles, each 6 to 20 m thick, and characterized by aeolian sandsheet and waterlain deposits succeeded by aeolian dune and interdune deposits indicative of a drying-upward trend. Aeolian cross-strata exhibit a mean dip direction to the north. Each of these cycles likely arose in response to climatic oscillation from relatively humid to arid conditions, possibly related to orbital forcing. The lower unit of the Mangabeira Formation comprises up to 14 erg sequences. The accumulation and preservation of each was determined by the relative rate of water-table rise and the availability of sand for aeolian transport, both of which changed through time, resulting in the preservation of a succession of repeated drying-upward cycles
Subleading-twist effects in single-spin asymmetries in semi-inclusive deep-inelastic scattering on a longitudinally polarized hydrogen target
Single-spin asymmetries in the semi-inclusive production of charged pions in
deep-inelastic scattering from transversely and longitudinally polarized proton
targets are combined to evaluate the subleading-twist contribution to the
longitudinal case. This contribution is significantly positive for (\pi^+)
mesons and dominates the asymmetries on a longitudinally polarized target
previously measured by \hermes. The subleading-twist contribution for (\pi^-)
mesons is found to be small
Observation of exclusive DVCS in polarized electron beam asymmetry measurements
We report the first results of the beam spin asymmetry measured in the
reaction e + p -> e + p + gamma at a beam energy of 4.25 GeV. A large asymmetry
with a sin(phi) modulation is observed, as predicted for the interference term
of Deeply Virtual Compton Scattering and the Bethe-Heitler process. The
amplitude of this modulation is alpha = 0.202 +/- 0.028. In leading-order and
leading-twist pQCD, the alpha is directly proportional to the imaginary part of
the DVCS amplitude.Comment: 6 pages, 5 figure
Complete measurement of three-body photodisintegration of 3He for photon energies between 0.35 and 1.55 GeV
The three-body photodisintegration of 3He has been measured with the CLAS
detector at Jefferson Lab, using tagged photons of energies between 0.35 GeV
and 1.55 GeV. The large acceptance of the spectrometer allowed us for the first
time to cover a wide momentum and angular range for the two outgoing protons.
Three kinematic regions dominated by either two- or three-body contributions
have been distinguished and analyzed. The measured cross sections have been
compared with results of a theoretical model, which, in certain kinematic
ranges, have been found to be in reasonable agreement with the data.Comment: 22 pages, 25 eps figures, 2 tables, submitted to PRC. Modifications:

A Kinematically Complete Measurement of the Proton Structure Function F2 in the Resonance Region and Evaluation of Its Moments
We measured the inclusive electron-proton cross section in the nucleon
resonance region (W < 2.5 GeV) at momentum transfers Q**2 below 4.5 (GeV/c)**2
with the CLAS detector. The large acceptance of CLAS allowed for the first time
the measurement of the cross section in a large, contiguous two-dimensional
range of Q**2 and x, making it possible to perform an integration of the data
at fixed Q**2 over the whole significant x-interval. From these data we
extracted the structure function F2 and, by including other world data, we
studied the Q**2 evolution of its moments, Mn(Q**2), in order to estimate
higher twist contributions. The small statistical and systematic uncertainties
of the CLAS data allow a precise extraction of the higher twists and demand
significant improvements in theoretical predictions for a meaningful comparison
with new experimental results.Comment: revtex4 18 pp., 12 figure
eta-prime photoproduction on the proton for photon energies from 1.527 to 2.227 GeV
Differential cross sections for the reaction gamma p -> eta-prime p have been
measured with the CLAS spectrometer and a tagged photon beam with energies from
1.527 to 2.227 GeV. The results reported here possess much greater accuracy
than previous measurements. Analyses of these data indicate for the first time
the coupling of the etaprime N channel to both the S_11(1535) and P_11(1710)
resonances, known to couple strongly to the eta N channel in photoproduction on
the proton, and the importance of j=3/2 resonances in the process.Comment: 6 pages, 3 figure
Measurement of the Deuteron Structure Function F2 in the Resonance Region and Evaluation of Its Moments
Inclusive electron scattering off the deuteron has been measured to extract
the deuteron structure function F2 with the CEBAF Large Acceptance Spectrometer
(CLAS) at the Thomas Jefferson National Accelerator Facility. The measurement
covers the entire resonance region from the quasi-elastic peak up to the
invariant mass of the final-state hadronic system W~2.7 GeV with four-momentum
transfers Q2 from 0.4 to 6 (GeV/c)^2. These data are complementary to previous
measurements of the proton structure function F2 and cover a similar
two-dimensional region of Q2 and Bjorken variable x. Determination of the
deuteron F2 over a large x interval including the quasi-elastic peak as a
function of Q2, together with the other world data, permit a direct evaluation
of the structure function moments for the first time. By fitting the Q2
evolution of these moments with an OPE-based twist expansion we have obtained a
separation of the leading twist and higher twist terms. The observed Q2
behaviour of the higher twist contribution suggests a partial cancellation of
different higher twists entering into the expansion with opposite signs. This
cancellation, found also in the proton moments, is a manifestation of the
"duality" phenomenon in the F2 structure function
