Sinusitis de origen odontogénico.

Pansinusitis is defined as the inflammation of all the paranasal sinuses, on one or both sides of the nose, as a result of invasion by bacteria, viruses or fungi, which can be caused by the evolution of a lesion in the tooth with formation of periapical pus that advances into the sinus cavity. (1)  Early evaluation of patients who manifest dental invasions is essential in order to avoid clinical complications that may trigger the penetration of said cavity. In the same way, care must be taken to make an early diagnosis and the respective antibiotic coverage in conjunction with antibiotic sensitivity tests in patients with worsening of the clinical picture of odontogenic origin.La pansinusitis se define como la inflamación de todos los senos paranasales, de uno o los dos lados de la nariz, a consecuencia de la invasión por bacterias, virus u hongos, el cual puede ser causada por la evolución de una lesión en el diente con formación de pus periapical que avanza en el interior de la cavidad sinusal. (1)  La evaluación temprana de los pacientes que manifiesten invasiones dentales es esencial con el fin de evitar las complicaciones clínicas que puedan detonar en la penetración de dicha cavidad. Del mismo modo se debe estar atento a realizar un diagnóstico temprano y la respectiva cobertura antibiótica en conjunto con las pruebas de sensibilidad antibiótica en pacientes con empeoramiento del cuadro clínico de origen odontogénico

Incidencia de la educación, núcleo familiar y sociedad en el nivel de conocimiento de los métodos anticonceptivos

En este trabajo se realiza una investigación sobre la relación entre conocimientos de los métodos anticonceptivos y la función informativa que cumple el colegio, sea este de carácter público o privado. Dicho análisis contó con una población de 192 adolescentes (de 15 a 18 años de edad) de seis colegios, en los que se aplicó una encuesta auto-diligenciada en la cual se responden preguntas relacionadas a los conocimientos teóricos sobre los métodos anticonceptivos y, de igual forma, sobre variables socioeconómicas y familiares

Se tiene como objetivo demostrar la necesidad de asegurar la concurrencia de la inteligencia artificial en el derecho, en los Mecanismos Alternativos de Solución de Conflictos y, en especial, en la conciliación. Además, se demuestra cómo confluyen para generar soluciones a la congestión judicial y maximizando la aplicación del principio de acceso a la justicia, como también la necesidad de transmutar e incorporar una herramienta indispensable que coadyuve al cumplimiento de los fines esenciales del Estado

Effectiveness of a strategy that uses educational games to implement clinical practice guidelines among Spanish residents of family and community medicine (e-EDUCAGUIA project):A clinical trial by clusters

This study was funded by the Fondo de Investigaciones Sanitarias FIS Grant Number PI11/0477 ISCIII.-REDISSEC Proyecto RD12/0001/0012 AND FEDER Funding.Background: Clinical practice guidelines (CPGs) have been developed with the aim of helping health professionals, patients, and caregivers make decisions about their health care, using the best available evidence. In many cases, incorporation of these recommendations into clinical practice also implies a need for changes in routine clinical practice. Using educational games as a strategy for implementing recommendations among health professionals has been demonstrated to be effective in some studies; however, evidence is still scarce. The primary objective of this study is to assess the effectiveness of a teaching strategy for the implementation of CPGs using educational games (e-learning EDUCAGUIA) to improve knowledge and skills related to clinical decision-making by residents in family medicine. The primary objective will be evaluated at 1 and 6months after the intervention. The secondary objectives are to identify barriers and facilitators for the use of guidelines by residents of family medicine and to describe the educational strategies used by Spanish teaching units of family and community medicine to encourage implementation of CPGs. Methods/design: We propose a multicenter clinical trial with randomized allocation by clusters of family and community medicine teaching units in Spain. The sample size will be 394 residents (197 in each group), with the teaching units as the randomization unit and the residents comprising the analysis unit. For the intervention, both groups will receive an initial 1-h session on clinical practice guideline use and the usual dissemination strategy by e-mail. The intervention group (e-learning EDUCAGUIA) strategy will consist of educational games with hypothetical clinical scenarios in a virtual environment. The primary outcome will be the score obtained by the residents on evaluation questionnaires for each clinical practice guideline. Other included variables will be the sociodemographic and training variables of the residents and the teaching unit characteristics. The statistical analysis will consist of a descriptive analysis of variables and a baseline comparison of both groups. For the primary outcome analysis, an average score comparison of hypothetical scenario questionnaires between the EDUCAGUIA intervention group and the control group will be performed at 1 and 6months post-intervention, using 95% confidence intervals. A linear multilevel regression will be used to adjust the model. Discussion: The identification of effective teaching strategies will facilitate the incorporation of available knowledge into clinical practice that could eventually improve patient outcomes. The inclusion of information technologies as teaching tools permits greater learning autonomy and allows deeper instructor participation in the monitoring and supervision of residents. The long-term impact of this strategy is unknown; however, because it is aimed at professionals undergoing training and it addresses prevalent health problems, a small effect can be of great relevance. En nuestra institución, precisamente, existe un nutrido número de niños y de jóvenes talentosos en diferentes campos, a saber: La música, el teatro, la danza, el canto, los deportes, la escritura, la pintura, el diseño y, por supuesto, a nivel académico; personas sensibles al arte y a sus diferentes manifestaciones porque poseen un talento intrínseco que les permite destacarse en el medio en donde interactúan. En esta edición de “La Caldera” se pretende continuar con la labor de seguir destacando, mostrando, promoviendo algunos de los talentosos estudiantes que hacen parte de nuestra Institución. Inhibitor of apoptosis proteins, NAIP, cIAP1 and cIAP2 expression during macrophage differentiation and M1/M2 polarization

Monocytes and macrophages constitute the first line of defense of the immune system against external pathogens. Macrophages have a highly plastic phenotype depending on environmental conditions; the extremes of this phenotypic spectrum are a pro-inflammatory defensive role (M1 phenotype) and an anti-inflammatory tissue-repair one (M2 phenotype). The Inhibitor of Apoptosis (IAP) proteins have important roles in the regulation of several cellular processes, including innate and adaptive immunity. In this study we have analyzed the differential expression of the IAPs, NAIP, cIAP1 and cIAP2, during macrophage differentiation and polarization into M1 or M2. In polarized THP-1 cells and primary human macrophages, NAIP is abundantly expressed in M2 macrophages, while cIAP1 and cIAP2 show an inverse pattern of expression in polarized macrophages, with elevated expression levels of cIAP1 in M2 and cIAP2 preferentially expressed in M1.
