    Predicting the impact of intervention strategies for sleeping sickness in two high-endemicity health zones of the Democratic Republic of Congo

    Two goals have been set for Gambian human African trypanosomiasis (HAT), the first is to achieve elimination as a public health problem in 90% of foci by 2020, and the second is to achieve zero transmission globally by 2030. It remains unclear if certain HAT hotspots could achieve elimination as a public health problem by 2020 and, of greater concern, it appears that current interventions to control HAT in these areas may not be sufficient to achieve zero transmission by 2030. A mathematical model of disease dynamics was used to assess the potential impact of changing the intervention strategy in two high-endemicity health zones of Kwilu province, Democratic Republic of Congo. Six key strategies and twelve variations were considered which covered a range of recruitment strategies for screening and vector control. It was found that effectiveness of HAT screening could be improved by increasing effort to recruit high-risk groups for screening. Furthermore, seven proposed strategies which included vector control were predicted to be sufficient to achieve an incidence of less than 1 reported case per 10,000 people by 2020 in the study region. All vector control strategies simulated reduced transmission enough to meet the 2030 goal, even if vector control was only moderately effective (60% tsetse population reduction). At this level of control the full elimination threshold was expected to be met within six years following the start of the change in strategy and over 6000 additional cases would be averted between 2017 and 2030 compared to current screening alone. It is recommended that a two-pronged strategy including both enhanced active screening and tsetse control is implemented in this region and in other persistent HAT foci to ensure the success of the control programme and meet the 2030 elimination goal for HAT

    A media advocacy intervention linking health disparities and food insecurity

    Media advocacy is a well-established strategy for transmitting health messages to the public. This paper discusses a media advocacy intervention that raised issues about how the public interprets messages about the negative effects of poverty on population health. In conjunction with the publication of a manuscript illustrating how income-related food insecurity leads to disparities related to the consumption of a popular food product across Canada (namely, Kraft Dinner®), we launched a media intervention intended to appeal to radio, television, print and Internet journalists. All the media coverage conveyed our intended message that food insecurity is a serious population health problem, confirming that message framing, personal narratives and visual imagery are important in persuading media outlets to carry stories about poverty as a determinant of population health. Among politicians and members of the public (through on-line discussions), the coverage provoked on-message as well as off-message reactions. Population health researchers and health promotion practitioners should anticipate mixed reactions to media advocacy interventions, particularly in light of new Internet technologies. Opposition to media stories regarding the socio-economic determinants of population health can provide new insights into how we might overcome challenges in translating evidence into preventive interventions

    Comparison of Muscle Activity During a Ring Muscle Up and a Bar Muscle Up

    International Journal of Exercise Science 16(1): 1451-1460, 2023. The muscle up (MU) is a variation of a common gymnastics movement that combines a pull up and a dip. It can be performed on a bar (BMU) or a set of rings (RMU). The difference in upper extremity muscle recruitment (MR) between BMU and RMU has not been evaluated. Therefore, the purpose of this study was to compare the MR of select muscles during BMU and RMU. Ten active males (27.6 ± 7.9 years) performed 5 repetitions of BMU and RMU in randomized order. Muscle recruitment of the upper (UT) and lower trapezius (LT), serratus anterior (SA), pectoralis major (PM), latissimus dorsi (LD), triceps brachii (TB), biceps brachii (BB), and forearm flexors (FF) was assessed using electromyography. A 2 X 2 ANOVA (ring vs bar, pull phase vs push phase) with repeated measures was performed for each muscle. Least significant differences post hoc tests were performed when a significant interaction effect occurred. The RMU significantly elicited more muscle activation in the UT (p = 0.007), BB (p = 0.001), and FF (p = 0.001) during the pull phase. The RMU also significantly elicited more muscle activation in the TB (p = 0.025) and BB (p = 0.001) during the push phase. These results suggest that the instability of the RMU primarily increases the required recruitment of the upper limbs but does not significantly change the recruitment of the shoulder stabilizers. Appropriate upper limb development is needed to perform the RMU and the BMU may be a better technique to learn first due to its lower difficulty

Because Communities Can Often Integrate Plant-Based Technologies at Minimal Cost and with Low Infrastructure Needs, the Use of These Technologies Can Be Applied Broadly to Minimize Potential Contaminant Exposure and Improve Environmental Quality. These Natural Treatment Systems Also Provide Valuable Ecosystem Services to Communities and Society. in the Future, Integrating and Coordinating Phytotechnology Activities with Public Health Research Will Allow Technology Development Focused on Prevention of Environmental Exposures to Toxic Compounds.     Reducing Superfluous Opioid Prescribing Practices After Brain Surgery: It Is Time to Talk About Drugs

    BACKGROUND: Opioids are prescribed routinely after cranial surgery despite a paucity of evidence regarding the optimal quantity needed. Overprescribing may adversely contribute to opioid abuse, chronic use, and diversion. OBJECTIVE: To evaluate the effectiveness of a system-wide campaign to reduce opioid prescribing excess while maintaining adequate analgesia. METHODS: A retrospective cohort study of patients undergoing a craniotomy for tumor resection with home disposition before and after a 2-mo educational intervention was completed. The educational initiative was composed of directed didactic seminars targeting senior staff, residents, and advanced practice providers. Opioid prescribing patterns were then assessed for patients discharged before and after the intervention period. RESULTS: A total of 203 patients were discharged home following a craniotomy for tumor resection during the study period: 98 who underwent surgery prior to the educational interventions compared to 105 patients treated post-intervention. Following a 2-mo educational period, the quantity of opioids prescribed decreased by 52% (median morphine milligram equivalent per day [interquartile range], 32.1 [16.1, 64.3] vs 15.4 [0, 32.9], P \u3c .001). Refill requests also decreased by 56% (17% vs 8%, P = .027) despite both groups having similar baseline characteristics. There was no increase in pain scores at outpatient follow-up (1.23 vs 0.85, P = .105). CONCLUSION: A dramatic reduction in opioids prescribed was achieved without affecting refill requests, patient satisfaction, or perceived analgesia. The use of targeted didactic education to safely improve opioid prescribing following intracranial surgery uniquely highlights the ability of simple, evidence-based interventions to impact clinical decision making, lessen potential patient harm, and address national public health concerns

    Genetic dysmyelination alters the molecular architecture of the nodal region

    We have examined the molecular organization of axons in the spinal cords of myelin-deficient (md) rats, which have profound CNS dysmyelination associated with oligodendrocyte cell death. Although myelin sheaths are rare, most large axons are at least partially surrounded by oligodendrocyte processes. At postnatal day 7 (P7), almost all node-like clusters of voltagegated Na � channels and ankyrin G are adjacent to axonal segments ensheathed by oligodendrocytes, but at P21, many node-like clusters are found in axonal segments that lack oligodendrocyte ensheathment. In P21 wild-type (WT) rats, the voltage-gated Na � channels Na v1.2, Na v1.6, and Na v1.8, are found in different subpopulations of myelinated axons, and md rats have a similar distribution. The known molecular components of paranodes—contactin, Caspr, and neurofascin 155— are not clustered in md spinal cords, and no septate-like junction

    What Can We Learn About Effective Mathematics Teaching?:A Framework for Estimating Causal Effects using Longitudinal Survey Data.

    This study investigates the impact of teacher characteristics and instructional strategies on the mathematics achievement of students in kindergarten and first grade and tackles the question of how best to use longitudinal survey data to elicit causal inference in the face of potential threats to validity due to nonrandom assignment to treatment. We develop a step-by-step approach to selecting a modeling and estimation strategy and find that teacher certification and courses in methods of teaching mathematics have a slightly negative effect on student achievement in kindergarten, whereas postgraduate education has a positive effect in first grade. Various teaching modalities, such as working with counting manipulatives, using math worksheets, and completing problems on the chalkboard, have positive effects on achievement in kindergarten, and pedagogical practices relating to explaining problem solving and working on problems from textbooks have positive effects on achievement in first grade. We show that the conclusions drawn depend on the estimation and modeling choices made and that several prior studies of teacher effects using longitudinal survey data likely neglected important features needed to establish causal inference

    Processing of a Multiple Membrane Spanning Epstein-Barr Virus Protein for Cd8+T Cell Recognition Reveals a Proteasome-Dependent, Transporter Associated with Antigen Processing–Independent Pathway

    Epstein-Barr virus (EBV) latent membrane protein (LMP)2 is a multiple membrane spanning molecule which lacks ectodomains projecting into the lumen of the endoplasmic reticulum (ER). Human CD8+ cytotoxic T lymphocytes (CTL)s recognize a number of epitopes within LMP2. Assays with epitope-specific CTLs in two different cell backgrounds lacking the transporter associated with antigen processing (TAP) consistently show that some, but not all, LMP2 epitopes are presented in a TAP-independent manner. However, unlike published examples of TAP-independent processing from endogenously expressed antigens, presentation of TAP-independent LMP2 epitopes was abrogated by inhibition of proteasomal activity. We found a clear correlation between hydrophobicity of the LMP2 epitope sequence and TAP independence, and experiments with vaccinia minigene constructs expressing cytosolic epitope peptides confirmed that these more hydrophobic peptides were selectively able to access the HLA class I pathway in TAP-negative cells. Furthermore, the TAP-independent phenotype of particular epitope sequences did not require membrane location of the source antigen since (i) TAP-independent LMP2 epitopes inserted into an EBV nuclear antigen and (ii) hydrophobic epitope sequences native to EBV nuclear antigens were both presented in TAP-negative cells.
