    Therapeutic adherence and facing styles in adolescents with diabetes mellitus type I

    Introduction:  Diabetes mellitus, is one of the four non-transmissible chronic diseases that has become a Public health problem worldwide. Objective: To characterize the therapeutic adherence and facing styles in adolescents with diabetes mellitus type I. Methods: An experimental, cross-sectional, and correlational study was carried out with a quantitative methodological approach from November, 2020 to April, 2021, in the Comprehensive Care to Diabetic Consultation in GĂŒines, Mayabeque province. The universe was formed by 83 diabetic Type I adolescents, for the selection of the sample a non-probabilistic sampling by convenience was used, and it was formed by 30 of them. For the collecting of the information a questionnaire was used to assess therapeutic adherence and a scale of Facing Styles for Adolescents. The data processing was performed by descriptive y differential statistics. Results: There was a prevalence of 11 years and more of treatment, diabetic Type I adolescents, for a 70 % and only a 6.7 % had from one to five years of treatment. Partial adhered diabetic adolescents prevailed for a 76.7 %. The most used facing style by adolescents was the style directed to the relation with the others, in a 46.7 %, followed by the style directed to the solution of problems for a 40 %. Only 13.3% had an unproductive style. Conclusions: Diabetic adolescents adhered to treatment use different facing styles

MĂ©todos:Se realizĂł un estudio no experimental, de corte transversal, descriptivo y correlacional, con enfoque metodolĂłgico cuantitativo, en el periodo comprendido de noviembre del 2020 a abril del 2021, en la Consulta de AtenciĂłn Integral al DiabĂ©tico, del municipio GĂŒines, provincia Mayabeque. El universo estuvo constituido por 83 adolescentes diabĂ©ticos tipo I, para la selecciĂłn de la muestra se utilizĂł un muestreo no probabilĂ­stico, intencional por conveniencia, quedĂł constituida por 30 de ellos. Para la recogida de informaciĂłn se utilizĂł el Cuestionario para evaluar adherencia terapĂ©utica y la Escala de Afrontamiento para Adolescentes. El procesamiento se realizĂł mediante la estadĂ­stica descriptiva y diferencial. Resultados: ExistiĂł un predominio de los 11 años de tratamiento en adelante, en los adolescentes diabĂ©ticos tipo I, para un 70 % y solo un 6.7 % tenĂ­an de uno a cinco años. Sobresalieron los adolescentes diabĂ©ticos adheridos parciales para un 76.7 %. El estilo de afrontamiento mĂĄs utilizados por los adolescentes fue el estilo dirigido a la relaciĂłn con los demĂĄs, en un 46.7 %, seguido del estilo dirigido a la resoluciĂłn del problema para un 40 %. Solo el 13.3 % tenĂ­an un estilo improductivo. Conclusiones: Los adolescentes diabĂ©ticos adheridos al tratamiento utilizan diferentes estilos de afrontamiento

    Efficient secretion of a modified E7 protein from human papilloma virus type-16 by Lactococcus lactis

    Aims: To create and provide a strain of the food-grade bacterium Lactococcus lactis able to efficiently secrete a modified form of the E7 protein from the human papilloma virus (HPV) type-16. Methods and Results: We cloned the coding sequence of a modified E7 (E7m)from the HPV-16 in a plasmid regulated by the strong expression promoter p59. Secretion of the E7m was made by the signal peptide of the usp45 gene.The E7m was detected by Western blot in the cell-free-medium fraction, showing no degradation or aberrant forms. Conclusions: We constructed a strain of L. lactis able to secrete efficiently aHPV-16 E7 modified protein with diminished transforming activity. Significance and Impact of the Study: Human papilloma virus infection is associated with more than 99% of cervical cancers. Immunotherapy targeting E7 to treat HPV-associated cervical malignancies has been demonstrated to be highly efficient. However, native E7 maintains transforming activity. We present this new strain of a food-grade bacterium able to efficiently secrete a HPV-16 E7-modified protein with diminished transforming activity. This new strain could be used as a live vaccine to deliver E7 at a mucosal level and generate antitumour immune responses against HPV-associated tumours

    Production of biologically active human lymphotactin (XCL1) by Lactococcus lactis

    Lymphotactin-XCL1 is a chemokine produced mainly by activated CD8? T-cells and directs migration of CD4? and CD8? lymphocytes and natural killer (NK) cells. We expressed human lymphotactin (LTN) by the lactic-acid bacterium Lactococcus lactis. Biological activity of LTN was confirmed by chemo-attraction of human T-cells by chemotaxis demonstrating, for the first time, how this chemokine secreted by a food-grade prokaryote retains biological activity and chemoattracts T lymphocytes. This strain thus represents a feasible well-tolerated vector to deliver active LTN at a mucosal level

    In vascular smooth muscle cells paricalcitol prevents phosphate-induced Wnt/ÎČ-catenin activation

    The present study investigates the differential effect of two vitamin D receptor agonists, calcitriol and paricalcitol, on human aortic smooth muscle cells calcification in vitro. Human vascular smooth muscle cells were incubated in a high phosphate (HP) medium alone or supplemented with either calcitriol 10−8M (HP + CTR) or paricalcitol 3·10−8 M (HP + PC). HP medium induced calcification, which was associated with the upregulation of mRNA expression of osteogenic factors such as bone morphogenetic protein 2 (BMP2), Runx2/Cbfa1, Msx2, and osteocalcin. In these cells, activation of Wnt/ÎČ-catenin signaling was evidenced by the translocation of ÎČ-catenin into the nucleus and the increase in the expression of direct target genes as cyclin D1, axin 2, and VCAN/versican. Addition of calcitriol to HP medium (HP + CTR) further increased calcification and also enhanced the expression of osteogenic factors together with a significant elevation of nuclear ÎČ-catenin levels and the expression of cyclin D1, axin 2, and VCAN. By contrast, the addition of paricalcitol (HP + PC) not only reduced calcification but also downregulated the expression of BMP2 and other osteoblastic phenotype markers as well as the levels of nuclear ÎČ-catenin and the expression of its target genes. The role of Wnt/ÎČ-catenin on phosphate- and calcitriol-induced calcification was further demonstrated by the inhibition of calcification after addition of Dickkopf-related protein 1 (DKK-1), a specific natural antagonist of the Wnt/ÎČ-catenin signaling pathway. In conclusion, the differential effect of calcitriol and paricalcitol on vascular calcification appears to be mediated by a distinct regulation of the BMP and Wnt/ÎČ-catenin signaling pathways

    Minimal residual disease in Myeloma: Application for clinical care and new drug registration

    The development of novel agents has transformed the treatment paradigm for multiple myeloma, with minimal residual disease (MRD) negativity now achievable across the entire disease spectrum. Bone marrow–based technologies to assess MRD, including approaches using next-generation flow and next-generation sequencing, have provided real-time clinical tools for the sensitive detection and monitoring of MRD in patients with multiple myeloma. Complementary liquid biopsy–based assays are now quickly progressing with some, such as mass spectrometry methods, being very close to clinical use, while others utilizing nucleic acid–based technologies are still developing and will prove important to further our understanding of the biology of MRD. On the regulatory front, multiple retrospective individual patient and clinical trial level meta-analyses have already shown and will continue to assess the potential of MRD as a surrogate for patient outcome. Given all this progress, it is not surprising that a number of clinicians are now considering using MRD to inform real-world clinical care of patients across the spectrum from smoldering myeloma to relapsed refractory multiple myeloma, with each disease setting presenting key challenges and questions that will need to be addressed through clinical trials. The pace of advances in targeted and immune therapies in multiple myeloma is unprecedented, and novel MRD-driven biomarker strategies are essential to accelerate innovative clinical trials leading to regulatory approval of novel treatments and continued improvement in patient outcomes

    Barrier-to-Autointegration Factor Proteome Reveals Chromatin-Regulatory Partners

    Nuclear lamin filaments and associated proteins form a nucleoskeletal (“lamina”) network required for transcription, replication, chromatin organization and epigenetic regulation in metazoans. Lamina defects cause human disease (“laminopathies”) and are linked to aging. Barrier-to-autointegration factor (BAF) is a mobile and essential component of the nuclear lamina that binds directly to histones, lamins and LEM-domain proteins, including the inner nuclear membrane protein emerin, and has roles in chromatin structure, mitosis and gene regulation. To understand BAF's mechanisms of action, BAF associated proteins were affinity-purified from HeLa cell nuclear lysates using BAF-conjugated beads, and identified by tandem mass spectrometry or independently identified and quantified using the iTRAQ method. We recovered A- and B-type lamins and core histones, all known to bind BAF directly, plus four human transcription factors (Requiem, NonO, p15, LEDGF), disease-linked proteins (e.g., Huntingtin, Treacle) and several proteins and enzymes that regulate chromatin. Association with endogenous BAF was independently validated by co-immunoprecipitation from HeLa cells for seven candidates including Requiem, poly(ADP-ribose) polymerase 1 (PARP1), retinoblastoma binding protein 4 (RBBP4), damage-specific DNA binding protein 1 (DDB1) and DDB2. Interestingly, endogenous BAF and emerin each associated with DDB2 and CUL4A in a UV- and time-dependent manner, suggesting BAF and emerin have dynamic roles in genome integrity and might help couple DNA damage responses to the nuclear lamina network. We conclude this proteome is a rich source of candidate partners for BAF and potentially also A- and B-type lamins, which may reveal how chromatin regulation and genome integrity are linked to nuclear structure
