    Assessment of Exposure to Alcohol Vapor from Alcohol-Based Hand Rubs

    This study assessed the inhaled dose of alcohol during hand disinfection. Experiments were conducted with two types of hand rub using two hand disinfection procedures. Air samples were collected every 10 s from the breathing zone, by bubbling through a mixture of K2Cr2O7 and H2SO4. The reduction of dichromate ions in the presence of alcohols was followed by UV-vis spectrophotometry. The difference in intensity of the dichromate absorption peak was used to quantify the alcohol concentration expressed in ethanol equivalent. During hygienic hand disinfection, the mean ethanol equivalent concentrations peaked at around 20–30 s for both hand rubs (14.3 ± 1.4 mg/L for hand rub 1 and 13.2 ± 0.7 mg/L for hand rub 2). During surgical hand disinfection, two peaks were found at the same time (40 and 80 s) for both hand rubs. The highest mean concentrations were 20.2 ± 0.9 mg/L for hand rub 1 and 18.1 ± 0.9 mg/L for hand rub 2. For hand rub 1, the total absorbed doses, calculated from ethanol with an inhalation flow of 24 L/min and an absorption rate of 62%, were 46.5 mg after one hygienic hand disinfection and 203.9 mg after one surgical hand disinfection. Although the use of ABHRs leads to the absorption of very low doses, sudden, repeated inhalation of high alcohol concentrations raises the question of possible adverse health effects

    Solution behavior, circular dichroism and 220 MHz PMR studies of the bovine myelin basic protein

    Bovine myelin basic protein has been investigated with regard to its solution behavior, circular dichroism and 220 MHz PMR spectral properties.At pH 4.8 [gamma]/2 = 0.1 acetate buffer, light scattering yielded a Mr of 17 700 and a virial coefficient of 1.0[middle dot]10-4 mol[middle dot]ml/g2. Above pH 7.0 the protein was found to aggregate to higher mol. wt species.Sedimentation experiments at pH 4.8 yielded s[deg]20,w of 1.27 S at [gamma]/2 = 0.1 and 1.46 S at [gamma]/2 = 0.35. The diffusion coefficient determined from ultracentrifugal experiments was 7.25[middle dot]10-7 cm2/s at [gamma]/2 = 0.1 and 0.35. The value of [function of (italic small f)]/[function of (italic small f)]0 from diffusion at pH 4.8 and [gamma]/2 = 0.35 was 1.64, corresponding to an axial ratio of 11 to 1. The radius of gyration was calculated as 4.28 nm and the root mean square end to end distance was 10.5 nm. At pH 9.0, [gamma]/2 = 0.1, s[deg]20,w was 1.71 S and D[deg]20,w was estimated at 7.4[middle dot]10-7 cm2/s. The behavior at pH 9.0 reverted to the behavior at pH 4.8 when the pH was readjusted. The E1cm1% = 5.64 at 276.4 nm and 225 at 196 nm.Titration of the protein with trifluoroethanol elicited three distinct regions of conformational stability having increasing helical content as the mol fraction of trifluoroethanol increased.The results of the present study have permitted some comparison of analogous properties and conformational behavior with the basic membrane protein cytochrome c.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/21990/1/0000400.pd

    CCC meets ICU: Redefining the role of critical care of cancer patients

    <p>Abstract</p> <p>Background</p> <p>Currently the majority of cancer patients are considered ineligible for intensive care treatment and oncologists are struggling to get their patients admitted to intensive care units. Critical care and oncology are frequently two separate worlds that communicate rarely and thus do not share novel developments in their fields. However, cancer medicine is rapidly improving and cancer is eventually becoming a chronic disease. Oncology is therefore characterized by a growing number of older and medically unfit patients that receive numerous novel drug classes with unexpected side effects.</p> <p>Discussion</p> <p>All of these changes will generate more medically challenging patients in acute distress that need to be considered for intensive care. An intense exchange between intensivists, oncologists, psychologists and palliative care specialists is warranted to communicate the developments in each field in order to improve triage and patient treatment. Here, we argue that "critical care of cancer patients" needs to be recognized as a medical subspecialty and that there is an urgent need to develop it systematically.</p> <p>Conclusion</p> <p>As prognosis of cancer improves, novel therapeutic concepts are being introduced and more and more older cancer patients receive full treatment the number of acutely ill patients is growing significantly. This development a major challenge to current concepts of intensive care and it needs to be redefined who of these patients should be treated, for how long and how intensively.</p

    Dualities of dementia illness narratives and their role in a narrative economy

    YesThe concept of 'narrative economies' has recently been proposed as a set of exchange relationships that, through biography and story-telling, facilitate access to resources and act as a source of value. We utilise this concept to inform our analysis of 18 qualitative interviews with five people with dementia and four informal carers. Our participants are members of a pre-existing group of dementia advocates, representing the voices of those living with the condition. There are a growing number of people in the early stages of dementia - like our participants - being called upon to account for their experience, as a means of developing a politicised 'collective illness identity'. These interviews present an opportunity to study a group of people who are actively involved in speaking as, and for, people with dementia. Four themes emerged from the data: becoming a voice of or for people with dementia; biographical reinforcement; responsibilisation; and resistance. These themes illustrate the ways in which people with dementia participate in their own identity construction and, as representatives of those living with dementia, they also illustrate the ways in which illness narratives produce material and symbolic value
