Influencia del Anticiclón del Atlántico Norte en la pluviosidad de la brisa marina en Carolina del Norte, Estados Unidos
The sea-breeze (SB) is an important source of summertime precipitation in North Carolina (NC, southeast United States). However, not all SB events produce precipitation. A climatology of wet and dry SB events in NC is used to investigate the conditions that are conducive to precipitation associated with the sea breeze. Radar imagery was used to detect 88 SB events that occurred along the NC coast between May-September of 2009-2012. The majority (85%) of SB events occurred during offshore flow (53%) or during flow that was parallel to the coast (22%). SB events were separated into dry (53%) and wet (47%) events and differences in the dynamic and thermodynamic parameters of the environment in which they formed were analyzed. Significant differences in dynamic and thermodynamic conditions were found. SB dry events occurred under stronger winds (6.00 ± 2.36 ms-1) than SB wet events (4.02 ± 2.16 ms-1). Moreover, during SB wet events larger values of convective available potential energy and lower values of convective inhibition were present, conditions that favor precipitation. Overall, the SB wet events accounted for 20-30% of the May-September precipitation along the NC coastal region. The position of the North Atlantic Subtropical High (NASH) controls both moisture availability and winds along the NC coast, thus providing a synoptic-scale control mechanism for SB precipitation. In particular, it was shown that when the NASH western ridge is located along the southeast coast of the United States, it causes a moist southwesterly flow along the NC coast that may favor the occurrence of SB wet events.La brisa marina (BM) es una importante fuente de precipitación de verano en Carolina del Norte (NC en su sigla en inglés), sudeste de Estados Unidos. Se utilizaron imágenes de radar para detectar 88 eventos de BM ocurridos a lo largo de la costa NC entre mayo y septiembre de 2009 a 2012. La mayoría (85%) de los eventos de BM ocurrieron durante períodos de viento hacia el mar (53%) o viento paralelo a la costa (22%). Los eventos BM se separaron en eventos secos (53%) y lluviosos (47%) y se analizaron las diferencias en los parámetros dinámicos y termodinámicos del entorno en el que se formaron. Se encontraron diferencias significativas en las condiciones dinámicas y termodinámicas. Eventos de BM secos ocurrieron bajo vientos más fuertes (6,00 ± 2,36 ms-1) que los eventos de BM lluviosos (4,02 ± 2,16 ms-1). Las BM lluviosas ocurrieron bajo valores de energía potencial convectiva disponible más altos y valores del parámetro de inhibición convectiva más bajos, condiciones que favorecen la lluvia. En general, los eventos de BM lluviosos representaron el 20-30% de la precipitación a lo largo de la región costera de NC de mayo a septiembre. La posición de la Alta Subtropical del Atlántico Norte (ASAN) controla la disponibilidad de humedad y los vientos a lo largo de la costa de NC, proporcionando así un mecanismo de control de escala sinóptica para la precipitación de la BM. Characteristics, Determinants, and Clinical Relevance of CD4 T Cell Recovery to <500 Cells/µL in HIV Type 1—Infected Individuals Receiving Potent Antiretroviral Therapy
Background. The CD4 T cell count recovery in human immunodeficiency virus type 1 (HIV-1)—infected individuals receiving potent antiretroviral therapy (ART) shows high variability. We studied the determinants and the clinical relevance of incomplete CD4 T cell restoration. Methods. Longitudinal CD4 T cell count was analyzed in 293 participants of the Swiss HIV Cohort Study who had had a plasma HIV-1 RNA load .05). Older age (adjusted odds ratio [aOR], 1.71 per 10-year increase; 95% confidence interval [CI], 1.21-2.43), lower baseline CD4 T cell count (aOR, 0.37 per 100-cell increase; 95% CI, 0.28-0.49), and longer duration of HIV infection (aOR, 2.39 per 10-year increase; 95% CI, 1.19-4.81) were significantly associated with a CD4 T cell count <500 cells/µL at 5 years. The median increases in CD4 T cell count after 3-6 months of ART were smaller in incomplete responders (P < .001) and predicted, in conjunction with baseline CD4 T cell count and age, incomplete response with 80% sensitivity and 72% specificity. Conclusion. Individuals with incomplete CD4 T cell recovery to <500 cells/µL had more advanced HIV-1 infection at baseline. CD4 T cell changes during the first 3-6 months of ART already reflect the capacity of the immune system to replenish depleted CD4 T lymphocyte
Discontinuation of primary prophylaxis against Pneumocystis carinii pneumonia in HIV-1-infected adults treated with combination antiretroviral therapy. Swiss HIV Cohort Study
BACKGROUND: It is unclear whether primary prophylaxis against Pneumocystis carinii pneumonia can be discontinued in patients infected with the human immunodeficiency virus (HIV) who are successfully treated with combination antiretroviral therapy. We prospectively studied the safety of stopping prophylaxis among patients in the Swiss HIV Cohort Study. METHODS: Patients were eligible for our study if their CD4 counts had increased to at least 200 cells per cubic millimeter and 14 percent of total lymphocytes while they were receiving combination antiretroviral therapy, with these levels sustained for at least 12 weeks. Prophylaxis was stopped at study entry, and patients were examined every three months thereafter. The development of P. carinii pneumonia was the primary end point, and the development of toxoplasmic encephalitis the secondary end point. RESULTS: Of the 262 patients included in our analysis, 121 (46.2 percent) were positive for IgG antibodies to Toxoplasma gondii at base line. The median CD4 count at study entry was 325 per cubic millimeter (range, 210 to 806); the median nadir CD4 count was 110 per cubic millimeter (range, 0 to 240). During a median follow-up of 11.3 months (range, 3.0 to 18.8), prophylaxis was resumed in nine patients, and two patients died. There were no cases of P. carinii pneumonia or toxoplasmic encephalitis. The one-sided upper 99 percent confidence limit for the incidence of P. carinii pneumonia was 1.9 cases per 100 patient-years (based on 238 patient-years of follow-up). The corresponding figure for toxoplasmic encephalitis was 4.2 per 100 patient-years (based on 110 patient-years of follow-up). CONCLUSIONS: Stopping primary prophylaxis against P. carinii pneumonia appears to be safe in HIV-infected patients who are receiving combination antiretroviral treatment and who have had a sustained increase in their CD4 counts to at least 200 cells per cubic millimeter and to at least 14 percent of total lymphocytes

Evaluation of the model representation of the evolution of convective systems using satellite observations of outgoing longwave radiation
We introduce a technique for assessing the diurnal development of convective storm systems based on outgoing longwave radiation fields. Using the size distribution of the storms measured from a series of images, we generate an array in the lengthscale-time domain based on the standard
score statistic. It demonstrates succinctly the size evolution of storms as well as the dissipation kinematics. It also provides evidence related to the temperature evolution of the cloud tops. We apply this approach to a test case comparing observations made by the Geostationary Earth Radiation Budget instrument to output from the Met Office Unified Model run at two resolutions. The 12km resolution model produces peak convective activity on all lengthscales significantly earlier in the day than shown by the observations and no evidence for storms growing in size. The 4km resolution model shows realistic timing and growth evolution although the dissipation mechanism still differs from the observed data
Comparison of serum lipoprotein(a) distribution and its correlates among black and white populations.
BACKGROUND. Epidemiological data on serum lipoprotein(a) (Lp(a)), a presumably strong risk factor for coronary artery disease in White populations, has mostly been derived, in Black populations, from small samples. This study compares the distribution and the determinants of serum Lp(a) in Blacks and in Whites using large representative samples and the same methods in both populations. METHODS. The distribution and the correlates of serum Lp(a) were investigated in population-based samples of 701 Blacks in the Seychelles and 634 Whites in Switzerland, aged 25-64 years. Serum Lp(a) was quantified using a commercial immunoradiometric assay. RESULTS. The distribution of serum Lp(a) was similarly skewed in both ethnic groups, but median Lp(a) concentration was about twofold higher in Blacks (210 mg/l) compared to Whites (100 mg/l). The proportions of individuals with elevated serum Lp(a) (> 300 mg/l) was about 50% higher in Blacks (37.5%) than in Whites (25.2%). In both ethnic groups, serum Lp(a) was found to correlate with total cholesterol, LDL-cholesterol and apoprotein B but not with HDL-cholesterol, alcohol intake, smoking, and body mass index. The variance in serum Lp(a) concentration explained by any combination of these factors was smaller than 5.3% in the two populations. CONCLUSIONS. The measured factors did not explain the higher levels of serum Lp(a) found in Blacks compared to Whites. These findings are consistent with the hypothesis that genetic factors account for much of the variation of serum Lp(a) in both populations
Tests of the Equivalence Principle with Neutral Kaons
We test the Principle of Equivalence for particles and antiparticles, using
CPLEAR data on tagged K0 and K0bar decays into pi^+ pi^-. For the first time,
we search for possible annual, monthly and diurnal modulations of the
observables |eta_{+-}| and phi_{+-}, that could be correlated with variations
in astrophysical potentials. Within the accuracy of CPLEAR, the measured values
of |eta_{+-}| and phi_{+-} are found not to be correlated with changes of the
gravitational potential. We analyze data assuming effective scalar, vector and
tensor interactions, and we conclude that the Principle of Equivalence between
particles and antiparticles holds to a level of 6.5, 4.3 and 1.8 x 10^{-9},
respectively, for scalar, vector and tensor potentials originating from the Sun
with a range much greater than the distance Earth-Sun. We also study
energy-dependent effects that might arise from vector or tensor interactions.
Finally, we compile upper limits on the gravitational coupling difference
between K0 and K0bar as a function of the scalar, vector and tensor interaction
