    Different patterns of pelvic ureteral endometriosis. What is the best treatment? Results of a retrospective analysis

    Objective. Endometriosis is an estrogendependent disease. The incidence of urinary tract endometriosis (UE) increased during the last few years and, nowadays, it ranges from 0.3 to 12% of all women affected by the disease. The ureter is the second most common site affected. The ureteral endometriosis is classified in extrinsic and intrinsic. The aim of this study is to individuate the best treatments for each subset of ureteral endometriosis. Materials and Methods. 32 patients diagnosed with surgically treated UE were retrospectively reviewed. The patients were divided into 3 subsets (intrinsic UE, extrinsic UE with and without obstruction). The patients with intrinsic UE (n = 10) were treated with laser endoureterotomy. The patients with extrinsic UE (n = 22) were divided in two subsets with (n = 16) and without (n = 6) hydronephrosis. All the patients underwent ureteral stenting, and resection and reimplantation was performed in the first group, and when the mass was > 2.5 cm (n = 3) Boari flap was performed. Laparoscopic ureterolysis (shaving) was performed in the second group. Results. In the extrinsic subset of UE, we obtained an high therapeutic success (84%). Conversely, in the intrinsic subset there was a recurrence rate of the disease in 6/10 of the patients (60%). Conclusions. Ureterolysis seems to be a good treatment in extrinsic UE without obstruction. Resection and reimplantation allows excellent results in the extrinsic UE with obstruction. In the intrinsic subset, the endoureterotomy approach is inadequate

    Pericardiectomy for constrictive pericarditis: a risk factor analysis for early and late failure

    Predictors of early and late failure of pericardiectomy for constrictive pericarditis (CP) have not been established. Early and late outcomes of a cumulative series of 81 (mean age 60\ua0years; mean EuroSCORE II, 3.3%) consecutive patients from three European cardiac surgery centers were reviewed. Predictors of a combined endpoint comprising in-hospital death or major complications (including multiple transfusion) were identified with binary logistic regression. Non-parametric estimates of survival were obtained with the Kaplan\u2013Meier method. Predictors of poor late outcomes were established using Cox proportional hazard regression. There were 4 (4.9%) in-hospital deaths. Preoperative central venous pressure > 15\ua0mmHg (p = 0.005) and the use of cardiopulmonary bypass (p = 0.016) were independent predictors of complicated in-hospital course, which occurred in 29 (35.8%) patients. During follow-up (median, 5.4\ua0years), preoperative renal impairment was a predictor of all-cause death (p = 0.0041), cardiac death (p = 0.0008), as well as hospital readmission due to congestive heart failure (p = 0.0037); while partial pericardiectomy predicted all-cause death (p = 0.028) and concomitant cardiac operation predicted cardiac death (p = 0.026), postoperative central venous pressure < 10\ua0mmHg was associated with a low risk both of all-cause and cardiac death (p < 0.0001 for both). Ten-year adjusted survival free of all-cause death, cardiac death, and hospital readmission were 76.9%, 94.7%, and 90.6%, respectively. In high-risk patients with CP, performing pericardiectomy before severe constriction develops and avoiding cardiopulmonary bypass (when possible) could contribute to improving immediate outcomes post-surgery. Complete removal of cardiac constriction could enhance long-term outcomes

    Chaotic Scattering in Heavy--Ion Reactions

    We discuss the relevance of chaotic scattering in heavy--ion reactions at energies around the Coulomb barrier. A model in two and three dimensions which takes into account rotational degrees of freedom is discussed both classically and quantum-mechanically.     Beam Test Performance and Simulation of Prototypes for the ALICE Silicon Pixel Detector

    The silicon pixel detector (SPD) of the ALICE experiment in preparation at the Large Hadron Collider (LHC) at CERN is designed to provide the precise vertex reconstruction needed for measuring heavy flavor production in heavy ion collisions at very high energies and high multiplicity. The SPD forms the innermost part of the Inner Tracking System (ITS) which also includes silicon drift and silicon strip detectors. Single assembly prototypes of the ALICE SPD have been tested at the CERN SPS using high energy proton/pion beams in 2002 and 2003. We report on the experimental determination of the spatial precision. We also report on the first combined beam test with prototypes of the other ITS silicon detector technologies at the CERN SPS in November 2004.     Strangeness enhancements at central rapidity in 40 A GeV/c Pb-Pb collisions

    Results are presented on neutral kaon, hyperon and antihyperon production in Pb-Pb and p-Be interactions at 40 GeV/c per nucleon. The enhancement pattern follows the same hierarchy as seen in the higher energy data - the enhancement increases with the strangeness content of the hyperons and with the centrality of collision. The centrality dependence of the Pb-Pb yields and enhancements is steeper at 40 than at 158 A GeV/c. The energy dependence of strangeness enhancements at mid-rapidity is discussed.Comment: 15 pages, 10 figures and 3 tables.     Hot gas in Mach cones around Virgo Cluster spiral galaxies

    The detailed comparison between observations and simulations of ram pressure stripped spiral galaxies in the Virgo cluster has led to a three dimensional view of the galaxy orbits within the hot intracluster medium. The 3D velocities and Mach numbers derived from simulations can be used to derive simple Mach cone geometries for Virgo spiral galaxies. We search for indications of hot gas within Mach cones in X-ray observations of selected Virgo Cluster spiral galaxies (NGC 4569, NGC 4388, and NGC 4501). We find extraplanar diffuse X-ray emission in all galaxies. Based on the 3D velocity vectors from dynamical modelling a simple Mach cone is fitted to the triangular shape of NGC 4569's diffuse X-ray emission. Assuming that all extraplanar diffuse X-ray emission has to be located inside the Mach cone, we also fit Mach cones to NGC 4388's and NGC 4501's extraplanar X-ray emission. For NGC 4569 it is hard to reconcile the derived Mach cone opening angle with a Mach number based on the sound speed alone. Instead, a Mach number involving the Alfv\'enic speed seems to be more appropriate, yielding a magnetic field strength of 3\sim 3-6 μ\muG for a intracluster medium density of n104n \sim 10^{-4} cm3^{-3}. Whereas the temperature of the hot component of NGC 4569's X-ray halo (0.5 keV) is at the high end but typical for a galactic outflow, the temperature of the hot gas tails of NGC 4388 and NGC 4501 are significantly hotter (0.7-0.9 keV). In NGC 4569 we find direct evidence for a Mach cone which is filled with hot gas from a galactic superwind. We suggest that the high gas temperatures in the X-ray tails of NGC 4388 and NGC 4501 are due to the mixing of the stripped ISM into the hot intracluster medium of the Virgo cluster.Comment: 11 pages, 10 figures, 5 tables.     Performance of ALICE pixel prototypes in high energy beams

    The two innermost layers of the ALICE inner tracking system are instrumented with silicon pixel detectors. Single chip assembly prototypes of the ALICE pixels have been tested in high energy particle beams at the CERN SPS. Detection efficiency and spatial precision have been studied as a function of the threshold and the track incidence angle.     Artificial Neural Network Inference (ANNI): A Study on Gene-Gene Interaction for Biomarkers in Childhood Sarcomas

    Objective: To model the potential interaction between previously identified biomarkers in children sarcomas using artificial neural network inference (ANNI). Method: To concisely demonstrate the biological interactions between correlated genes in an interaction network map, only 2 types of sarcomas in the children small round blue cell tumors (SRBCTs) dataset are discussed in this paper. A backpropagation neural network was used to model the potential interaction between genes. The prediction weights and signal directions were used to model the strengths of the interaction signals and the direction of the interaction link between genes. The ANN model was validated using Monte Carlo cross-validation to minimize the risk of over-fitting and to optimize generalization ability of the model. Results: Strong connection links on certain genes (TNNT1 and FNDC5 in rhabdomyosarcoma (RMS); FCGRT and OLFM1 in Ewing’s sarcoma (EWS)) suggested their potency as central hubs in the interconnection of genes with different functionalities. The results showed that the RMS patients in this dataset are likely to be congenital and at low risk of cardiomyopathy development. The EWS patients are likely to be complicated by EWS-FLI fusion and deficiency in various signaling pathways, including Wnt, Fas/Rho and intracellular oxygen. Conclusions: The ANN network inference approach and the examination of identified genes in the published literature within the context of the disease highlights the substantial influence of certain genes in sarcomas

    Rapidity distributions around mid-rapidity of strange particles in Pb-Pb collisions at 158 AA GeV/c

    The production at central rapidity of K0s, Lambda, Xi and Omega particles in Pb-Pb collisions at 158 A GeV/c has been measured by the NA57 experiment over a centrality range corresponding to the most central 53% of the inelastic Pb-Pb cross section. In this paper we present the rapidity distribution of each particle in the central rapidity unit as a function of the event centrality. The distributions are analyzed based on hydrodynamical models of the collisions.Comment: 15 pages, 10 figure

    KOSMOS and COSMOS: New facility instruments for the NOAO 4-meter telescopes

    We describe the design, construction and measured performance of the Kitt Peak Ohio State Multi-Object Spectrograph (KOSMOS) for the 4-m Mayall telescope and the Cerro Tololo Ohio State Multi-Object Spectrograph (COSMOS) for the 4-m Blanco telescope. These nearly identical imaging spectrographs are modified versions of the OSMOS instrument; they provide a pair of new, high-efficiency instruments to the NOAO user community. KOSMOS and COSMOS may be used for imaging, long-slit, and multi-slit spectroscopy over a 100 square arcminute field of view with a pixel scale of 0.29 arcseconds. Each contains two VPH grisms that provide R~2500 with a one arcsecond slit and their wavelengths of peak diffraction efficiency are approximately 510nm and 750nm. Both may also be used with either a thin, blue-optimized CCD from e2v or a thick, fully depleted, red-optimized CCD from LBNL. These instruments were developed in response to the ReSTAR process.
