    affronĀ®, a standardised extract from saffron (Crocus sativus L.) for the treatment of youth anxiety and depressive symptoms: A randomised, double-blind, placebo-controlled study

    Background: Saffron has antidepressant and anxiolytic effects in adults with mild-to-moderate depression. However, this is the first study examining its mood-related effects in teenagers. Methods: In this 8-week, randomised, double-blind, placebo-controlled study, youth aged 12ā€“16 years, with mild-to-moderate anxiety or depressive symptoms were given tablets containing placebo or a saffron extract (affronĀ®, 14 mg b.i.d). The youth and parent versions of the Revised Child Anxiety and Depression Scale (RCADS) were used as outcome measures. Results: 80 participants were enrolled and 68 completed the study. Based on youth self-reports, affronĀ®was associated with greater improvements in overall internalising symptoms (p = 0.049), separation anxiety (p = 0.003), social phobia (p = 0.023), and depression (p = 0.016). Total internalising scores decreased by an average of 33% compared to 17% in the placebo group (p = 0.029). However, parental reports of improvements were inconsistent as mean improvements in RCADS scores were greater in the saffron group (40% vs 26%) (p = 0.026), although no other significant differences were identified. affronĀ®was well-tolerated and there was a trend of reduced headaches in participants on the active treatment. Limitations: The use of a self-report instrument, limited study duration, single treatment dose, and non-clinical sample used in this study limit the generalisability of study findings. Conclusion: The administration of a standardised saffron extract (affronĀ®) for 8 weeks improved anxiety and depressive symptoms in youth with mild-to-moderate symptoms, at least from the perspective of the adolescent. However, these beneficial effects were inconsistently corroborated by parents.This study was funded by Pharmactive Biotech Products SL.     Sleep health of young adults in Western Australia and associations with physical and mental health: A population-level cross-sectional study

    OBJECTIVES: This article aims to report on the sleep health characteristics of a population-level sample of young Australian adults and examine associations with measures of physical and mental health. METHODS: A cross-sectional study using data from the Raine Study. Data from participants (n = 1234) born into the study (Generation 2) at the 22-year follow-up were used, including data from a self-report questionnaire and polysomnography. RESULTS: The highest prevalence of suboptimal sleep health was seen on measures of sleep duration (30%), onset latency (18%), satisfaction (25%) and regularity (60%). Dissatisfaction with sleep (physical health: =0.08; mental health: =0.34) and impaired daytime alertness (physical health: =0.09; mental health: =0.08) were significantly associated with poorer physical and mental health and inadequate polysomnography-measured sleep duration was associated poorer mental health ( =0.07) (all ps \u3c 0.05). CONCLUSIONS: Satisfaction with sleep and daytime alertness, both of which are assessed via self-report, are essential aspects of sleep health for young adults. IMPLICATIONS FOR PUBLIC HEALTH: Findings could inform public health interventions, including screening guidelines, to improve the sleep health and, in turn, the physical and mental health of young adults in Australia

    Examining the Ability of an FSO Receiver to Simultaneously Communicate with Multiple Transmitters

    FSO (Free-Space Optical)-based communication systems experience difficulty with receiving and separating signals arising from multiple transmitters, a capability that would facilitate implementation of MIMO (Multiple-In, Multiple-Out) systems. Current implementations require multiple, distinct optical antennas, each tracking a single user, which proves bulky and costly, especially if the transmitters are moving and must be tracked. A fiber-bundle receiver has the potential to use multiple pathways, corresponding to the individual fibers within the receiver, to capture different combinations of the incoming optical signals. If the bundle provides linear combining of the optical signals from both the individual fibers in the bundle and amongst the incoming optical signals, signal processing could extract the individual signals from the combinations. In this paper, we experimentally investigate whether the fiber-bundle receiver possesses sufficient linearity of operation to allow the separation of two signals by simple processing algorithms, for both turbulent and non-turbulent conditions. Data from two distinct sources enters a single-bundle, single field of view receiver, and a basis signal from one transmitter provides the reference for performing simple subtraction-based extraction of the unknown signal from the other transmitter. The experimental results show that the receiver does operate linearly, and that the linear operation remains sufficiently intact in the presence of turbulence to extract a recognizable copy of one signal from the other. The ability of the fiber bundle receiver to mitigate turbulence effects appears to assist in maintaining this sufficient level of linearity

    Polyphenols are naturally occurring organic compounds found in plants. Research suggests that their intake reduces the risk of cognitive decline and related dementias. Grapes and blueberries are polyphenol-rich foods that have attracted attention for their potential cognitive-enhancing effects. Examine the effects of supplementation with a standardized and patented polyphenol-rich grape and blueberry extract (Memophenolā„¢) on cognitive function in older adults with mild cognitive impairment. Two-arm, 6 month, parallel-group, randomized, double-blind, placebo-controlled trial. One hundred and forty-three volunteers aged 60 to 80 years with mild cognitive impairment were supplemented with either 150 mg of Memophenolā„¢, twice daily or a placebo. Outcome measures included computer-based cognitive tasks, the Behavior Rating Inventory of Executive Function (BRIEF-A), the Cognitive Failures Questionnaire, and the CASP-19. Compared to the placebo, Memophenolā„¢ supplementation was associated with greater improvements in the speed of information processing ( =ā€‰0.020), visuospatial learning ( =ā€‰0.012), and the BRIEF-A global score ( =ā€‰0.046). However, there were no other statistically significant between-group differences in the performance of other assessed cognitive tests or self-report questionnaires. Memophenolā„¢ supplementation was well-tolerated with no reports of significant adverse reactions. The promising results from this trial suggest that 6-months of supplementation with Memophenolā„¢ may improve aspects of cognitive function in adults with mild cognitive impairment. Further research will be important to expand on the current findings and identify the potential mechanisms of action associated with the intake of this polyphenol-rich extract

    Regulatory objectivity in action: Mild cognitive impairment and the collective production of uncertainty

    In this paper, we investigate recent changes in the definition and approach to Alzheimerā€™s disease brought about by growing clinical, therapeutic and regulatory interest in the prodromal or preclinical aspects of this condition. In the last decade, there has been an increased interest in the biomolecular and epidemiological characterization of pre-clinical dementia. It is argued that early diagnosis of dementia, and particularly of Alzheimerā€˜s disease, will facilitate the prevention of dementing processes and lower the prevalence of the condition in the general population. The search for a diagnostic category or biomarker that would serve this purpose is an ongoing but problematic endeavour for research and clinical communities in this area. In this paper, we explore how clinical and research actors, in collaboration with regulatory institutions and pharmaceutical companies, come to frame these domains as uncertainties and how they re-deploy uncertainty in the ā€˜collective productionā€™ of new diagnostic conventions and bioclinical standards. While drawing as background on ethnographic, documentary and interview data, the paper proposes an in-depth, contextual analysis of the proceedings of an international meeting organized by the Peripheral and Central Nervous System Drug Advisory Committee of the US Food and Drug Administration to discuss whether or not a particular diagnostic convention ā€” mild cognitive impairment ā€” exists and how best it ought to be studied. Based on this analysis we argue that the deployment of uncertainty is reflexively implicated in bioclinical collectivesā€™ search for rules and conventions, and furthermore that the collective production of uncertainty is central to the ā€˜knowledge machineryā€™ of regulatory objectivity

    Cellular and Morphological Alterations in the Vastus Lateralis Muscle as the Result of ACL Injury and Reconstruction

    Background: Individuals who have had an anterior cruciate ligament (ACL) tear and reconstruction continue to experience substantial knee extensor strength loss despite months of physical therapy. Identification of the alterations in muscle morphology and cellular composition are needed to understand potential mechanisms of muscle strength loss, initially as the result of the injury and subsequently from surgery and rehabilitation. Methods: We performed diffusion tensor imaging-magnetic resonance imaging and analyzed muscle biopsies from the vastus lateralis of both the affected and unaffected limbs before surgery and again from the reconstructed limb following the completion of rehabilitation. Immunohistochemistry was done to determine fiber type and size, Pax-7-positive (satellite) cells, and extracellular matrix (via wheat germ agglutinin straining). Using the diffusion tensor imaging data, the fiber tract length, pennation angle, and muscle volume were determined, yielding the physiological cross-sectional area (PCSA). Paired t tests were used to compare the effects of the injury between injured and uninjured limbs and the effects of surgery and rehabilitation within the injured limb. Results: We found significant reductions before surgery in type-IIA muscle cross-sectional area (CSA; p = 0.03), extracellular matrix (p \u3c 0.01), satellite cells per fiber (p \u3c 0.01), pennation angle (p = 0.03), muscle volume (p = 0.02), and PCSA (p = 0.03) in the injured limb compared with the uninjured limb. Following surgery, these alterations in the injured limb persisted and the frequency of the IIA fiber type decreased significantly (p \u3c 0.01) and that of the IIA/X hybrid fiber type increased significantly (p \u3c 0.01). Conclusions: Significant and prolonged differences in muscle quality and morphology occurred after ACL injury and persisted despite reconstruction and extensive physical therapy. Clinical Relevance: These results suggest the need to develop more effective early interventions following an ACL tear to prevent deleterious alterations within the quadriceps

    Demographics of extra-articular calcaneal fractures: Including a review of the literature on treatment and outcome

    Introduction: Extra-articular calcaneal fractures represent 25-40% of all calcaneal fractures and an even higher percentage of up to 60% is seen in children. A disproportionately small part of the literature on calcaneal fractures involves the extra-articular type. The aim of this study was to investigate the incidence of extra-articular calcaneal fractures in a Level 1 trauma centre, define the distribution of the various types of fractures and compare patient demographics between extra- and intra-articular calcaneal fractures. In addition the literature was reviewed for the most common types of extra-articular calcaneal fractures with regard to incidence, treatment and clinical outcome. Methods: The radiological records between 2003 and 2005 were reviewed for intra- and extra-articular calcaneal fractures. Patient gender-distribution and age were compared. A literature search was conducted for the treatment of extra-articular calcaneal fractures. Results: In this 3-year study period a total of 49 patients with 50 extra-articular calcaneal fractures and 91 patients with 101 intra-articular fractures were identified. The median age for the first group was 32.7 years, and for the second group 40.3 years; P = 0.04. Male predominance was significantly less pronounced for extra-articular (63%) compared with intra-articular fractures (79%; P = 0.04). Conclusion: One-third of all calcaneal fractures are extra-articular. Significant differences exist between the intra- and extra-articular groups, in terms of lower age and male-female ratio. The literature study shows inconsistencies in treatment options, but most extra-articular fractures are well manageable conservatively

    Dual modification of Alzheimerā€™s disease PHF-tau protein by lysine methylation and ubiquitylation: a mass spectrometry approach

    In sporadic Alzheimerā€™s disease (AD), neurofibrillary lesion formation is preceded by extensive post-translational modification of the microtubule associated protein tau. To identify the modification signature associated with tau lesion formation at single amino acid resolution, immunopurified paired helical filaments were isolated from AD brain and subjected to nanoflow liquid chromatographyā€“tandem mass spectrometry analysis. The resulting spectra identified monomethylation of lysine residues as a new tau modification. The methyl-lysine was distributed among seven residues located in the projection and microtubule binding repeat regions of tau protein, with one site, K254, being a substrate for a competing lysine modification, ubiquitylation. To characterize methyl lysine content in intact tissue, hippocampal sections prepared from post mortem late-stage AD cases were subjected to double-label confocal fluorescence microscopy using anti-tau and anti-methyl lysine antibodies. Anti-methyl lysine immunoreactivity colocalized with 78Ā Ā±Ā 13% of neurofibrillary tangles in these specimens. Together these data provide the first evidence that tau in neurofibrillary lesions is post-translationally modified by lysine methylation
