    FT-ICR-MS approach to monitor asparagine deamidation and its isomers products in collagen from ancient bones

    This project investigates the use of ultrahigh resolution mass spectrometry along with fragmentation techniques such as electron capture dissociation (ECD) and collisionally activated dissociation (CAD) to study deamidation of ancient bone, by using potential deamidation markers present in bovine collagen standards. With the application of these techniques, several marker peptides present in the digested protein standard of bovine collagen were successfully assigned. The sequences of these peptides correlated well with the reported sequences for bovine collagen in the literature. FT-ICR-MS was used to monitor deamidation of collagen by following a shift of +0.948 Da in the spectrum, resulting in a mass difference of 19 mDa from the 13C of the non-deamidated form and the 12C of the deamidated form, which can be difficult to assign due to overlap with the 13C isotopic distribution in peptides. The rate constants for the deamidation reaction were calculated, and the extent of deamidation before sample handling was determined. The methodology developed was then applied to collagen extracted from real bone samples, both modern and ancient, proving to be a useful method for monitoring asparagine deamidation before sample preparation. Differentiation of the isomers products of deamidation (aspartic and isoaspartic acid) were successfully assigned (where possible) using the diagnostic ions originated from their ECD spectra

    Hyperoxemia and excess oxygen use in early acute respiratory distress syndrome : Insights from the LUNG SAFE study

We examined this issue in patients with ARDS enrolled in the Large observational study to UNderstand the Global impact of Severe Acute respiratory FailurE (LUNG SAFE) study. Methods: In this secondary analysis of the LUNG SAFE study, we wished to determine the prevalence and the outcomes associated with hyperoxemia on day 1, sustained hyperoxemia, and excessive oxygen use in patients with early ARDS. Patients who fulfilled criteria of ARDS on day 1 and day 2 of acute hypoxemic respiratory failure were categorized based on the presence of hyperoxemia (PaO2 > 100 mmHg) on day 1, sustained (i.e., present on day 1 and day 2) hyperoxemia, or excessive oxygen use (FIO2 ≥ 0.60 during hyperoxemia). Results: Of 2005 patients that met the inclusion criteria, 131 (6.5%) were hypoxemic (PaO2 < 55 mmHg), 607 (30%) had hyperoxemia on day 1, and 250 (12%) had sustained hyperoxemia. Excess FIO2 use occurred in 400 (66%) out of 607 patients with hyperoxemia. Excess FIO2 use decreased from day 1 to day 2 of ARDS, with most hyperoxemic patients on day 2 receiving relatively low FIO2. Multivariate analyses found no independent relationship between day 1 hyperoxemia, sustained hyperoxemia, or excess FIO2 use and adverse clinical outcomes. Mortality was 42% in patients with excess FIO2 use, compared to 39% in a propensity-matched sample of normoxemic (PaO2 55-100 mmHg) patients (P = 0.47). Conclusions: Hyperoxemia and excess oxygen use are both prevalent in early ARDS but are most often non-sustained. No relationship was found between hyperoxemia or excessive oxygen use and patient outcome in this cohort.     Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Determination of pharmaceuticals in fish and wastewater using isotope dilution high performance liquid chromatography-tandem mass spectrometry.

    Includes bibliographical references (p. 83-88).A method has been developed for the trace analysis of 15 pharmaceuticals and employs liquid chromatography/tandem mass spectrometry (LC-ESI-MS/MS). Unlike many previous methods, which suffer from matrix suppression and recovery loss, the present method utilizes isotope dilution for each compound to correct for these phenomena. Matrix spike recoveries for all compounds were between 87 and 113% for wastewater effluent, 98 and 144% for fish fillet tissue, and 81% and 105% for fish liver tissue. Method Detection limits (MDLs) in wastewater, fish fillet, and liver tissue were ≤21, ≤13, and ≤41 ng/g, respectively for most analytes. Additionally, this method was directly compared with matrix matched calibration to evaluate potential to compensate for matrix effects. Matrix matched was not suitable to account for matrix effects in samples with highly loaded matrix such as fish liver, though this method offered greater sensitive compared to isotope dilution in the detection of some compounds such as norfluoxetine, sertraline, and fluoxetine.by Pilar Perez Hurtado.M.S

    Collagen is the major component of skin, tendons, ligaments, teeth, and bones, it provides the framework that holds most multicellular animals together, and collagen type I constitutes the major fibrillar collagen of bone. Because of the complexity of collagen’s structure, the study of post-translational modifications such as deamidation for this protein is challenging. Although there is no evidence of this protein being used for age assessment, it has been shown that deamidation of collagen is remarkably increased in old bones from mammals. Nonspectrometric methodologies have been used for the determination of the extent of deamidation as a measure of the amount of amide nitrogen released in ammonia as well as constant rates for deamidation of asparagine in collagen. In general, these methodologies required more sample and separation processes. To understand if collagen plays a significant role in the aging process of fossil materials, a simpler and more accurate method is needed to determine the extent of deamidation at the whole protein level. The present work shows a method to determine the extent of deamidation in collagen using Fourier transform ion cyclotron resonance-mass spectrometry (FTICR-MS) along with collisionally activated dissociation (CAD) and electron capture dissociation (ECD).
