    A tale of three cities: persisting high HIV prevalence, risk behaviour and undiagnosed infection in community samples of men who have sex with men

    Objectives: To examine the geographical variations in HIV prevalence (diagnosed and undiagnosed), use of sexual health services, sexually transmitted infections and sexual behaviour in a community sample of men who have sex with men in three cities in England, specifically London, Brighton and Manchester.Methods: Cross-sectional surveys of men visiting gay community venues in three large cities in England. Men self-completed a questionnaire and provided an anonymous oral fluid sample for HIV antibody testing.Results: HIV prevalence ranged from 8.6% to 13.7% in the three cities. Over one-third of HIV infection remained undiagnosed in all sites despite 69% of HIV-positive men reporting attending a genitourinary medicine clinic in the last year. Similar and high levels of risk behaviour were reported in all three cities. 18% of HIV-negative men and 37% of HIV-positive men reported unprotected anal intercourse with more than one partner in the last year. 20% of negative men and 41% of positive men reported an STI in the last year.Conclusions: Across all cities, despite widespread availability of anti-retroviral treatment and national policy to promote HIV testing, many HIV infections remain undiagnosed. Data from this community sample demonstrate high levels of risk behaviour and STI incidence, especially among those who are HIV positive. Renewed efforts are needed to increase diagnosis and to reduce risk behaviour to stem the continuing transmission of HIV

    Configurations Driving NPD Performance Fit with Market Demands and Time Constraints

    The research reported in this paper is aimed at developing knowledge on organizing NPD systems to optimize their contribution to performance. To this end, a systems approach to fit is used to explain the context-structure-performance relationships for NPD performance, specifically in terms of fit with market demands of the product concept and fit with time constraints of the development process. From a sample of 164 US firms, the top 15 % performers in terms of both fit with market demands and fit with time constraints have been identified. An optimized ‘Ideal Profile’ for the organization of NPD systems, formed by a consistent pattern of: NPD Process, NPD Project Structure and Management, Innovation Climate, and NPD Goal Setting and Portfolio Management, followed from the analysis of the NPD configuration of these top performers. For the calibration sample (the other 85%) significant deviation from the ideal profile on all elements of the configuration was found, the correlations between NPD Performance Fit with Market Demands and Fit with Time Constraints and total Euclidean distance are also significant. Overall, these results provide evidence for the proposition that (1) new product success is a function of a set of NPD development system decisions and (2) to truly understand the impact of those decisions, they must be considered as a holistic system.\ud The contribution of this research is in the empirical validation of the internal consistency of an ideal organizational profile for NPD systems achieving both a high NPD performance in terms of market acceptance of their new products as well in terms of the satisfactory level of the development times of those products. By also examining ideal profiles for each of these NPD performance dimensions separately, the conflicting demands created by multiple performance metrics are highlighted as well as the organizational trade-offs necessary for optimal performance. In terms of managerial implications, this also gives direction for organizational redesign to firms either wanting to maximize their product concept (Fit with Market Demands) or development process (Fit with Time Constraints) performance

    Near-infrared integral-field spectroscopy of violent starburst environments

    Near-infrared (NIR) integral-field spectroscopy (IFS) of violent starburst environments at high spatial (and spectral) resolution has the potential to revolutionise our ideas regarding the local interactions between the newly-formed massive stars and the interstellar medium (ISM) of their host galaxies. To illustrate this point, I present NIR IFS analysis of the central starburst region of NGC 1140, obtained with CIRPASS on Gemini-South. While strong [FeII] emission is found throughout the galaxy, higher-order Brackett emission is predominantly associated with the northern starburst region. Based on the spatial distributions of the [FeII] versus Brackett line emission, I conclude that a galaxy-wide starburst was induced several x 10^7 yr ago, with more recent starburst activity concentrated around the northern starburst region.     Feminised concern or feminist Care? Reclaiming gender normativities in zero waste living

    Growing awareness of environmental issues and their relation to consumption patterns has givenrise to calls for sustainable consumption across the globe. In this article, we focus on the zerowaste lifestyle movement, which targets high-consumption households in the Global North as asite of change for phasing out waste in global supply chains. Our article is concerned with askinghow gender and household sustainability are mutually constituted in the zero waste lifestylemovement. We establish an analytical tension between understanding zero waste living as afurther intensification of feminised responsibility for people and the planet and as offering potentialfor transformational change – as feminised concern or feminist care. Through qualitative contentanalysis of the 10 most influential zero waste blogs globally, we show how the five zero wasterules of conduct – refuse, reduce, reuse, recycle, and rot – guide consumers towards everydayand situated engagements with waste. Organised by three cross-cutting themes – communingwith nature, organising time, and spending money – we present the normativities these rules callinto being for reconfiguring domestic activities such as cooking, cleaning, and grocery shopping. Inthe discussion, we draw out the implications of zero waste living’s emerging, contradictory gendernormativities, while recalling the political economy in which it is situated, namely a neoliberal,postfeminist landscape. We identify a continued feminisation of domestic responsibilities that isuncontested in zero waste living but also explore the progressive potential of waste-free livingto bring collective, naturecultural worlds into being as part of domestic environmental labour.     Clinical presentation, diagnostic findings and outcome of dogs with presumptive spinal-only meningoencephalomyelitis of unknown origin

    Objectives: To summarise clinical presentation, diagnostic findings and long-term outcome for dogs clinically diagnosed with meningoencephalomyelitis of unknown origin affecting the spinal cord alone. Methods: Medical records were reviewed for dogs diagnosed with presumptive spinal-only meningoencephalomyelitis of unknown origin between 2006 and 2015. Results: 21 dogs were included; the majority presented with an acute (43%) or chronic (52%) onset of neurological signs. Ambulatory paresis was the most common neurological presentation (67%). Neurological examination most commonly revealed a T3-L3 myelopathy, and spinal hyperaesthesia was a common finding (71%). A spinal cord lesion was visible in 90% of cases on magnetic resonance imaging. Eighteen lesions (86%) showed parenchymal contrast enhancement and 17 lesions (81%) showed contrast enhancement of overlying meninges. All dogs were treated with immunosuppressive doses of glucocorticosteroids, sometimes combined with cytosine arabinoside. At time of data capture, 10/21 dogs (48%) had died or been euthanased because of the condition. Overall median survival time was 669 days. Clinical Significance: Meningoencephalomyelitis of unknown origin should be considered in the differential diagnosis of dogs presenting with a progressive myelopathy. Magnetic resonance imaging features can possibly help to distinguish presumptive meningoencephalomyelitis of unknown origin from other more common spinal diseases. Overall, long-term survival is guarded, approximately 50% of dogs will die or be euthanased despite appropriate therapy

    CIRPASS near-infrared integral-field spectroscopy of massive star clusters in the starburst galaxy NGC 1140

    [ABRIDGED] We analyse near-infrared integral field spectroscopy of the central starburst region of NGC 1140, obtained at the Gemini-South telescope equipped with CIRPASS. Our ~1.45-1.67 um wavelength coverage includes the bright [Fe II] emission line, as well as high-order Brackett (hydrogen) lines. While strong [Fe II] emission, thought to originate in the thermal shocks associated with supernova remnants, is found throughout the galaxy, both Br 12-4 and Br 14-4 emission, and weak CO(6,3) absorption, is predominantly associated with the northern starburst region. The Brackett lines originate from recombination processes occurring on smaller scales in (young) HII regions. The time-scale associated with strong [Fe II] emission implies that most of the recent star-formation activity in NGC 1140 was induced in the past 35-55 Myr. Based on the spatial distributions of the [Fe II] versus Brackett line emission, we conclude that a galaxy-wide starburst was induced several tens of Myr ago, with more recent starburst activity concentrated around the northern starburst region. This scenario is (provisionally) confirmed by our analysis of the spectral energy distributions of the compact, young massive star clusters (YMCs) detected in new and archival broad-band HST images. The YMC ages in NGC 1140 are all <= 20 Myr, consistent with independently determined estimates of the galaxy's starburst age, while there appears to be an age difference between the northern and southern YMC complexes in the sense expected from our CIRPASS analysis. Our photometric mass estimates of the NGC 1140 YMCs, likely upper limits, are comparable to those of the highest-mass Galactic globular clusters and to spectroscopically confirmed masses of (compact) YMCs in other starburst galaxies.Comment: 16 pages LaTeX, incl. 6 postscript figures; accepted for publication in MNRA
