    Formação continuada de pedagogos escolares: o significado de grupo e comunicação na prática pedagógica

    Considering that the pedagogue is the professional who articulates communication and coordinates the groups involved in the process of teaching and learning at school, this article aims to identify the articulation that the pedagogues established between the information received during the continuing education, the experiences reflections on communication and group and the practice already developed in the school space. This qualitative research presents the description of an experience of continuing education with pedagogues and interprets the phenomena in the light of hermeneutic phenomenology. For the description and interpretation of the data we used the conversation wheel of two meetings of Continuing Education and a task to be performed at school. The results indicate that the research participants, for not working most of the time from the perspective of observation and listening, also because they are not supported by the knowledge that the school should be understood as a group, through a dynamic process, which is impossible not to communicate, end up reading the school dynamics and its phenomena in a simplistic way.Al considerar que el pedagogo es el profesional que articula la comunicación y coordina los grupos involucrados en el proceso de enseñanza y aprendizaje en la escuela, este artículo tiene como objetivo identificar la articulación que los pedagogos establecieron entre la información recibida durante la formación continua, experiencias reflexivas. sobre la comunicación y el grupo y la práctica ya desarrollada en el espacio escolar. La investigación, de carácter cualitativo, presenta la descripción de una experiencia de educación continua con pedagogos e interpreta los fenómenos a la luz de la fenomenología hermenéutica. Para la descripción e interpretación de los datos, se utilizó la rueda de conversación de dos encuentros de Educación Continuada y una tarea a realizar en el colegio. Los resultados indican que los participantes de la investigación, por no trabajar, la mayor parte del tiempo, desde la perspectiva de la observación y la escucha, y también porque no se apoyan en el conocimiento de que la escuela debe entenderse como un grupo, en una dinámica y que es imposible no comunicarse, terminan leyendo la dinámica escolar y sus fenómenos, de manera simplista.Ao considerar que o pedagogo é o profissional que articula a comunicação e coordena os grupos envolvidos no processo de ensinar e aprender na escola, este artigo tem como objetivo identificar a articulação que os pedagogos estabeleceram entre as informações recebidas ao longo da formação continuada, as vivências reflexivas sobre comunicação e grupo e a prática já desenvolvida no espaço da escola. Para a descrição e interpretação dos dados utilizou-se a roda de conversa de dois encontros da Formação Continuada e uma tarefa a ser realizada na escola.     Analysis of biological diversity and miscegenation in the city of Puerto Madryn (Prov.of Chubut, Argentina)

    En el marco del estudio de la composición genética de las poblaciones cosmopolitas de Argentina, se analizó una muestra poblacional de la localidad de Puerto Madryn (PM) con la finalidad de evaluar su diversidad biológica mediante la utilización de marcadores biparentales y uniparentales y comparar los resultados con los obtenidos previamente por nuestro equipo de investigación en seis poblaciones cosmopolitas de distintas regiones de la Argentina, aunque poniendo el énfasis a las correspondientes a la Región Patagónica. Las muestras biológicas fueron tomadas con consentimiento informado a 82 dadores de sangre no emparentados que concurrieron al Banco de Sangre y al Hospital Subzonal de dicha localidad, a quienes también se les realizó una encuesta genealógica. A partir de los datos proporcionados por los marcadores autosómicos se registró una contribución europea de 67.2%, amerindia de 29.4% y africana de 3.4%. A un origen amerindio fueron adscriptos el 59.9% y 8.7% de los linajes maternos y paternos, respectivamente, revelando un desigual aporte autóctono por género. El aporte europeo se vio representado principalmente por el Hg H (19.5%) y se detectaron sólo dos linajes subsaharianos. En PM las migraciones desde el centro del país han generado un fuerte impacto, reflejado en la mayor contribución de marcadores europeos respecto de dos muestras estudiadas previamente en Chubut (Comodoro Rivadavia y Esquel). Estas diferencias al interior de una misma provincia nos advierten que no puede abordarse el análisis de la constitución genética de las poblaciones sin dar cuenta de las particularidades regionales.In the frame of the study of the genetic composition of Argentinean cosmopolitan populations, a population sample of the city of Puerto Madryn (PM) was analyzed in order to evaluate its biological diversity using biparental and uniparental markers and compare the results with those previously obtained by our research team in six cosmopolitan populations from different regions of Argentina. The emphasis was laid on the populations of the Patagonia region. Biological samples were taken with informed consent from 82 unrelated blood donors, who attended the Regional Blood Bank and Hospital of the town and also underwent a genealogical survey. From the data obtained by analyzing autosomal markers, a European contribution of 67.2%, an Amerindian one of 29.4%, and an African one of 3.4% were estimated. The 59.9% and 8.7% of the maternal and paternal lineages, respectively, were ascribed to Native American origins, revealing an unequal gender indigenous contribution. The European contribution was mainly represented by H Hg (19.5%), and only two sub-Saharan lineages were detected. In PM, migrations from the center of the country have had a strong impact, as reflected in the greater contribution of European markers, with respect to the two samples previously studied in the province of Chubut (Comodoro Rivadavia and Esquel). These differences warn us of the necessity of accounting for the regional particularities when analyzing the genetic constitution of populations.Fil: Parolin, María Laura.     Evidence-based tailored nutrition educational intervention improves adherence to dietary guidelines, anthropometric measures and serum metabolic biomarkers in early-stage breast cancer patients: A prospective interventional study

    Purpose: The impact of the adherence to dietary guidelines of early-stage breast cancer (EBC) patients on body composition changes during treatment is not entirely defined. This study aimed to evaluate the role of an evidence-based nutrition educational intervention, according to adherence to dietary guidelines, in EBC patients. Methods: This prospective study included EBC patients, candidates for neoadjuvant/adjuvant therapy. Patients received an evidence-based tailored nutrition educational intervention. The adherence to dietary guidelines, anthropometric and dietary assessments, and blood glucose and lipid profile tests were evaluated at baseline and after a 12-months nutritional intervention. Results: Two hundred and forty-three patients were enrolled. At baseline, 38.3% and 23.9% of patients were overweight and obese, weight gain ≥5% (compared to 6-months before enrollment) and central obesity were observed in 47.3% and 52.7% of patients, respectively. Adherence to dietary guidelines was low (median Med-Diet score: 6 [IQR 4-8]). After the nutritional intervention (median follow-up: 22 months [range 12-45]), adherence to dietary guidelines significantly increased (median Med-Diet score: 12 [IQR 8-13]), p < 0.0001). High adherence to dietary guidelines (defines as Med-Diet score ≥10) significantly correlated with: 1) overall weight loss ≥5% (21.8% vs. 2.5%, p = 0.003); 2) median BMI drop (from 25.6 kg/m2 to 24.4 kg/m2, p = 0.003); 3) lower prevalence of central obesity (38.2% vs. 7.2%, p = 0.01); 4) improvement in blood glucose levels and lipid profile. Conclusion: This study suggests that an evidence-based tailored nutrition educational intervention during treatment for EBC significantly increases overall adherence to dietary guidelines, and it improves both anthropometric measures and serum metabolic biomarkers in patients with high adherence

    Treatment options for multiple sclerosis (MS) have changed over the last few years, bringing about a new category of drugs with more efficient profiles. However, these drugs have come with a whole new profile of potential adverse events that neurologists have to learn well and quickly. One of the most feared complications of these MS treatments is progressive multifocal leukoencephalopathy caused by the reactivation of the John Cunningham virus (JCV). Objective: To identify the serologic profile of JCV in patients with MS. Methods: Data on serum antibodies for JCV were obtained using the enzyme-linked immunosorbent assay provided by the STRATIFY-JCV program. Results: A total of 1,501 blood tests were obtained from 1,102 patients with MS. There were 633 patients (57.1%) who were positive for antibodies for JCV and 469 patients who were negative (42.9%). Twenty-three patients became positive after initially having negative JCV antibody status. The rate of seroconversion was 18.5% over 22 months. Conclusion: The JCV serologic profile and seroconversion in Brazilian patients were similar to those described in other countries.As opções terapêuticas para esclerose múltipla (EM) modificaram-se ao longo dos últimos anos, trazendo uma nova categoria de drogas com melhor perfil de eficácia. No entanto, estas drogas vieram com um novo perfil de potenciais eventos adversos que exigem que o neurologista os reconheça bem e rapidamente. Uma das complicações mais temidas destes tratamentos para a EM é a leucoencefalopatia multifocal progressiva (LEMP), causada pela reativação do vírus John Cunningham (JCV). Objetivo: Identificar o perfil sorológico de JCV em pacientes com EM. Métodos: Dados sorológicos de JCV foram obtidos através do ensaio por enzimas imuno-adsorvidas (ELISA) fornecido pelo programa STRATIFY-JCV. Resultados: Um total de 1.501 testes sanguíneos foram obtidos de 1.102 pacientes com EM. O grupo teve 633 pacientes (57,1%) soropositivos para anticorpos anti-JCV e 469 pacientes negativos (42,9%). Vinte e três pacientes se tornaram posivitos após resultados iniciais negativos para anticorpos anti-JCV. A taxa de soroconversão foi 18,5% em 22 meses. Conclusão: O perfil sorológico do JCV e a soroconversão nos pacientes brasileiros foi semelhante àquela descrita em outros países

    Enxaqueca piora o sofrimento do paciente que tem esclerose múltipla (EM). ID-migraine é uma ferramenta útil para seleção de pacientes com enxaqueca e Migraine Disability Assessment (MIDAS) é um questionário que avalia o impacto da doença. O objetivo do presente estudo foi avaliar a presença e impacto de enxaqueca em pacientes com EM. Métodos: Pacientes diagnosticados com EM e tratados em clínicas especializadas foram convidados a responder um questionário online se também apresentassem cefaleia. Resultados: O estudo incluiu 746 participantes com cefaleia e EM que preencheram completamente as respostas. Foram 625 mulheres e 121 homens, sendo 69% dos pacientes com idade entre 20 e 40 anos. Enxaqueca foi identificada em 404 pacientes (54,1%) e moderado a grave impacto da doença foi observado em 68,3% dos casos. Conclusão: Enxaqueca é uma cefaleia primária frequente e incapacitante relatada por pacientes com EM.Migraine adds to the burden of patients suffering from multiple sclerosis (MS). Results: The study included 746 complete responses from patients with MS and headache. There were 625 women and 121 men, and 69% of all the patients were aged between 20 and 40 years. Migraine was identified in 404 patients (54.1%) and a moderate-to-high burden of disease was observed in 68.3% of the patients. Conclusion: Migraine is a frequent and disabling type of primary headache reported by patients with MS

    De novo point mutations in patients diagnosed with ataxic cerebral palsy

    Cerebral palsy is a sporadic disorder with multiple likely aetiologies, but frequently considered to be caused by birth asphyxia. Genetic investigations are rarely performed in patients with cerebral palsy and there is little proven evidence of genetic causes. As part of a large project investigating children with ataxia, we identified four patients in our cohort with a diagnosis of ataxic cerebral palsy. They were investigated using either targeted next generation sequencing or trio-based exome sequencing and were found to have mutations in three different genes, KCNC3, ITPR1 and SPTBN2. All the mutations were de novo and associated with increased paternal age. The mutations were shown to be pathogenic using a combination of bioinformatics analysis and in vitro model systems. This work is the first to report that the ataxic subtype of cerebral palsy can be caused by de novo dominant point mutations, which explains the sporadic nature of these cases. We conclude that at least some subtypes of cerebral palsy may be caused by de novo genetic mutations and patients with a clinical diagnosis of cerebral palsy should be genetically investigated before causation is ascribed to perinatal asphyxia or other aetiologies.peer-reviewe

    Consenso brasileiro para o tratamento da esclerose múltipla : Academia Brasileira de Neurologia e Comitê Brasileiro de Tratamento e Pesquisa em Esclerose Múltipla

    O crescent arsenal terapêutico na esclerose múltipla (EM) tem permitido tratamentos mais efetivos e personalizados, mas a escolha e o manejo das terapias modificadoras da doença (TMDs) tem se tornado cada vez mais complexos. Neste contexto, especialistas do Comitê Brasileiro de Tratamento e Pesquisa em Esclerose Múltipla e do Departamento Científico de Neuroimunologia da Academia Brasileira de Neurologia reuniram-se para estabelecer este Consenso Brasileiro para o Tratamento da EM, baseados no entendimento de que neurologistas devem ter a possibilidade de prescrever TMDs para EM de acordo com o que é melhor para cada paciente, com base em evidências e práticas atualizadas. Por meio deste documento, propomos recomendações práticas para o tratamento da EM, com foco principal na escolha e no manejo das TMDs, e revisamos os argumentos que embasam as estratégias de tratamento na EM.The expanding therapeutic arsenal in multiple sclerosis (MS) has allowed for more effective and personalized treatment, but the choice and management of disease-modifying therapies (DMTs) is becoming increasingly complex. In this context, experts from the Brazilian Committee on Treatment and Research in Multiple Sclerosis and the Neuroimmunology Scientific Department of the Brazilian Academy of Neurology have convened to establish this Brazilian Consensus for the Treatment of MS, based on their understanding that neurologists should be able to prescribe MS DMTs according to what is better for each patient, based on up-to-date evidence and practice. We herein propose practical recommendations for the treatment of MS, with the main focus on the choice and management of DMTs, as well as present a review of the scientific rationale supporting therapeutic strategies in MS
