Cardiac Resynchronization Therapy Leads to Improvements in Handgrip Strength
Background: A reduction in skeletal muscle performance measured
by handgrip strength is common in heart failure. No trial has investigated
the role of cardiac resynchronization therapy, which leads to
improvements in cardiac performance, on the function of skeletal
muscle in patients with heart failure.
Methods: Nineteen patients were recruited, 18 male, age 69 ± 8
years, New York Heart Association class II-IV, QRS duration 173 ±
21 ms and left ventricular ejection fraction 26±8%. Handgrip strength
was measured at baseline before, and 6 and 12 months, following cardiac
resynchronization therapy. Response was assessed using quality
of life questionnaire, 6-minute walk distance, left ventricular enddiastolic
volume, and cardiopulmonary exercise testing at the same
time points.
Results: Fourteen patients were identified as responders, demonstrating
significant improvements in all four markers of response. There
was no significant difference at baseline in left or right handgrip
strength between responders and non-responders. Compared to baseline,
handgrip strength significantly increased in responders during
follow-up, left (34.4 ± 11.4 to 40.3 ± 11.3 kgf, P < 0.001) and right
(35.7 ± 12.5 to 42.2 ± 11.5 kgf, P < 0.001) at 12 months. No such
improvement was seen in non-responders.
Conclusions: This study demonstrates that positive response to cardiac
resynchronization therapy is associated with significant gains in
handgrip strength, suggesting that cardiac resynchronization therapy
may indirectly lead to secondary gains in skeletal muscle function
Measures of endothelial dysfunction predict response to cardiac resynchronisation therapy
Objectives. Cardiac resynchronisation therapy (CRT) improves morbidity and mortality in heart failure (HF). Impaired endothelial function, as measured by flow-mediated dilation (FMD) is associated with increased morbidity and mortality in HF and may help to differentiate responders from non-responders.
Methods. 19 patients were recruited, comprising 94% men, mean age 69±8â
years, New York Heart Association functional classes IIâIV, QRSd 161±21â
ms and mean left ventricular ejection fraction 26±8%. Markers of response and FMD were measured at baseline, 6 and 12â
months following CRT.
Results. 14 patients were responders to CRT. Responders had significant improvements in VO2 (12.6±1.7 to 14.7±1.5â
mL/kg/min, p<0.05), quality of life score (44.4±22.9â24.1±21.3, p<0.01), left ventricular end diastolic volume (201.5±72.5â
mLâ121.3±72.0â
mL, p<0.01) and 6-min walk distance (374.0±112.8â
m at baseline to 418.1±105.3â
m, p<0.05). Baseline FMD in responders was 2.9±1.9% and 7.4±3.73% in non-responders (p<0.05).
Conclusions. Response to CRT at 6 and 12â
months is predicted by baseline FMD. This study confirms that FMD identifies responders to CRT, due to endothelium-dependent mechanisms alone
Influence of cardiac autonomic neuropathy on cardiac repolarisation during incremental adrenaline infusion in type 1 diabetes
Aims/hypothesis
We examined the effect of a standardised sympathetic stimulus, incremental adrenaline (epinephrine) infusion on cardiac repolarisation in individuals with type 1 diabetes with normal autonomic function, subclinical autonomic neuropathy and established autonomic neuropathy.
Methods
Ten individuals with normal autonomic function and baroreceptor sensitivity tests (NAF), seven with subclinical autonomic neuropathy (SAN; normal standard autonomic function tests and abnormal baroreceptor sensitivity tests); and five with established cardiac autonomic neuropathy (CAN; abnormal standard autonomic function and baroreceptor tests) underwent an incremental adrenaline infusion. Saline (0.9% NaCl) was infused for the first hour followed by 0.01 ÎŒg kgâ1 minâ1 and 0.03 ÎŒg kgâ1 minâ1 adrenaline for the second and third hours, respectively, and 0.06 ÎŒg kgâ1 minâ1 for the final 30 min. High resolution ECG monitoring for QTc duration, ventricular repolarisation parameters (T wave amplitude, T wave area symmetry ratio) and blood sampling for potassium and catecholamines was performed every 30 min.
Results
Baseline heart rate was 68 (95% CI 60, 76) bpm for the NAF group, 73 (59, 87) bpm for the SAN group and 84 (78, 91) bpm for the CAN group. During adrenaline infusion the heart rate increased differently across the groups (pâ=â0.01). The maximum increase from baseline (95% CI) in the CAN group was 22 (13, 32) bpm compared with 11 (7, 15) bpm in the NAF and 10 (3, 18) bpm in the SAN groups. Baseline QTc was 382 (95% CI 374, 390) ms in the NAF, 378 (363, 393) ms in the SAN and 392 (367, 417) ms in the CAN groups (pâ=â0.31). QTc in all groups lengthened comparably with adrenaline infusion. The longest QTc was 444 (422, 463) ms (NAF), 422 (402, 437) ms (SAN) and 470 (402, 519) ms (CAN) (pâ=â0.09). T wave amplitude and T wave symmetry ratio decreased and the maximum decrease occurred earlier, at lower infused adrenaline concentrations in the CAN group compared with NAF and SAN groups. AUC for the symmetry ratio was different across the groups and was lowest in the CAN group (pâ=â0.04). Plasma adrenaline rose and potassium fell comparably in all groups.
Conclusions/interpretation
Participants with CAN showed abnormal repolarisation in some measures at lower adrenaline concentrations. This may be due to denervation adrenergic hypersensitivity. Such individuals may be at greater risk of cardiac arrhythmias in response to physiological sympathoadrenal challenges such as stress or hypoglycaemia
Cardiac autonomic regulation and repolarization during acute experimental hypoglycemia in Type 2 diabetes
Hypoglycemia is associated with increased cardiovascular mortality in trials of intensive therapy in type 2 diabetes (T2DM). We previously observed an increase in arrhythmias during spontaneous prolonged hypoglycemia in T2DM patients. Our aim was to examine changes in cardiac autonomic function and repolarization during sustained experimental hypoglycemia.
Twelve adults with T2DM and eleven age, BMI-matched nondiabetic controls underwent paired hyperinsulinemic clamps separated by 4 weeks. Glucose was maintained at euglycemia (6.0mmol/L) or hypoglycemia (2.5mmol/L) for one hour. Heart rate, blood pressure, heart rate variability were assessed every thirty minutes and corrected QT (QTc) and T wave morphology every 60 minutes.
Heart rate initially increased in T2DM participants but then fell towards baseline despite maintained hypoglycemia at 1 hour, accompanied by reactivation of vagal tone. In nondiabetic participants, vagal tone remained depressed during sustained hypoglycemia. Diabetic participants exhibited greater heterogeneity of repolarization during hypoglycemia as demonstrated by T wave symmetry and Principal Component Analysis (PCA) ratio compared with the nondiabetic group. Epinephrine levels during hypoglycemia were similar between groups.
Cardiac autonomic regulation during hypoglycemia appears time-dependent. T2DM individuals demonstrate greater repolarization abnormalities for a given hypoglycemic stimulus despite comparable sympathoadrenal responses. These mechanisms could contribute to arrhythmias during clinical hypoglycemic episodes
The Importance of Context in Understanding Homelessness and Mental Illness: Lessons Learned From a Research Demonstration Project
Research reports on the housing outcomes for persons who are homeless and mentally ill have focused on client characteristics, program type, and services as independent variables, with mixed results. From social work practice, evaluation theory, and public policy perspectives, context is an important variable. Yet, it has received scant research attention in studies of the outcomes of persons who are mentally ill and homeless. This article summarizes research results from a demonstration project providing outreach or linkage services to this target population, illustrating the significant impact of context variables (site and recruitment source) on client characteristics, implementation, qualitative and quantitative service assessments, and housing outcomes. Nucleation of a sodium droplet on C60
We investigate theoretically the progressive coating of C60 by several sodium
atoms. Density functional calculations using a nonlocal functional are
performed for NaC60 and Na2C60 in various configurations. These data are used
to construct an empirical atomistic model in order to treat larger sizes in a
statistical and dynamical context. Fluctuating charges are incorporated to
account for charge transfer between sodium and carbon atoms. By performing
systematic global optimization in the size range 1<=n<=30, we find that Na_nC60
is homogeneously coated at small sizes, and that a growing droplet is formed
above n=>8. The separate effects of single ionization and thermalization are
also considered, as well as the changes due to a strong external electric
field. The present results are discussed in the light of various experimental

The <i>Castalia</i> mission to Main Belt Comet 133P/Elst-Pizarro
We describe Castalia, a proposed mission to rendezvous with a Main Belt Comet (MBC), 133P/Elst-Pizarro. MBCs are a recently discovered population of apparently icy bodies within the main asteroid belt between Mars and Jupiter, which may represent the remnants of the population which supplied the early Earth with water. Castalia will perform the first exploration of this population by characterising 133P in detail, solving the puzzle of the MBCâs activity, and making the first in situ measurements of water in the asteroid belt. In many ways a successor to ESAâs highly successful Rosetta mission, Castalia will allow direct comparison between very different classes of comet, including measuring critical isotope ratios, plasma and dust properties. It will also feature the first radar system to visit a minor body, mapping the ice in the interior. Castalia was proposed, in slightly different versions, to the ESA M4 and M5 calls within the Cosmic Vision programme. Soot reference materials for instrument calibration and intercomparisons: A workshop summary with recommendations
Soot, which is produced from biomass burning and the incomplete combustion of fossil and biomass fuels, has been linked to regional and global climate change and to negative health problems. Scientists measure the properties of soot using a variety of methods in order to quantify source emissions and understand its atmospheric chemistry, reactivity under emission conditions, interaction with solar radiation, influence on clouds, and health impacts. A major obstacle currently limiting progress is the absence of established standards or reference materials for calibrating the many instruments used to measure the various properties of soot.
The current state of availability and practicability of soot standard reference materials (SRMs) was reviewed by a group of 50 international experts during a workshop in June of 2011. The workshop was convened to summarize the current knowledge on soot measurement techniques, identify the measurement uncertainties and limitations related to the lack of soot SRMs, and identify attributes of SRMs that, if developed, would reduce measurement uncertainties. The workshop established that suitable SRMs are available for calibrating some, but not all, measurement methods. The community of users of the single-particle soot-photometer (SP2), an instrument using laser-induced incandescence, identified a suitable SRM, fullerene soot, but users of instruments that measure light absorption by soot collected on filters did not. Similarly, those who use thermal optical analysis (TOA) to analyze the organic and elemental carbon components of soot were not satisfied with current SRMs. The workshop, and subsequent, interactive discussions, produced a number of recommendations for the development of new SRMs, and their implementation, that would be suitable for the different soot measurement methods

Characterization and intercomparison of aerosol absorption photometers: Result of two intercomparison workshops
Absorption photometers for real time application have been available since the 1980s, but the use of filter-based instruments to derive information on aerosol properties (absorption coefficient and black carbon, BC) is still a matter of debate. Several workshops have been conducted to investigate the performance of individual instruments over the intervening years. Two workshops with large sets of aerosol absorption photometers were conducted in 2005 and 2007. The data from these instruments were corrected using existing methods before further analysis. The inter-comparison shows a large variation between the responses to absorbing aerosol particles for different types of instruments. The unit to unit variability between instruments can be up to 30% for Particle Soot Absorption Photometers (PSAPs) and Aethalometers. Multi Angle Absorption Photometers (MAAPs) showed a variability of less than 5%. Reasons for the high variability were identified to be variations in sample flow and spot size. It was observed that different flow rates influence system performance with respect to response to absorption and instrumental noise. Measurements with non absorbing particles showed that the current corrections of a cross sensitivity to particle scattering are not sufficient. Remaining cross sensitivities were found to be a function of the total particle load on the filter. The large variation between the response to absorbing aerosol particles for different types of instruments indicates that current correction functions for absorption photometers are not adequate
